EDITORIAL
At this time of the year it is usual to take stock not only of goods available, but also of the work which lies ahead.
During 1948 the new Health Act came into being, and many persons thought that voluntary organizations, whose members had been very active with ambulance services, assistance at hospitals and the like, were no longer needed. Even some memb ers of our own Organization were under this impression. That this is not so has been proved during the first six months of the Act; and the call for voluntary effo rt has not been lessened in any way, in fact one might almost state that the calls upon our personnel have been great er than ever before. Admittedly, some alterations in arrangements were made, and to some of our members this meant the setting up of a new organization, as far as the ambulance service was concerned, in which they had little or no share. In other directions, however, our personnel were asked to undertake the whole of the ambulance service for the local authority. The call for voluntary assistance in hospitals, either as nursing personnel or in other capacities, is still very urgent. Time and time again we read of hospitals or wards in hospitals being closed owing to lack of staff. H ere is one very exce llent opportunity of service which more membErs can undertake during the coming year.
The voluntary spirit in this country is not dead, but it must be admitted that that spirit of helping others for the sake of b.elping others is not so prevalent as before the war. There is too much selfishness about, too much « 'What do I get out of it ? " outlook; and too little « Service to Mankind".
In this respect we can do much by interesting the younger generation in « Service for Others". In a report received this month we were reminded that 4I years ago Sir Robert Baden-Powell when opening the n ew h eadquarters of the Radcliffe Ambulance Division of the Brigade, foreshadowed the Boy Scout Movement « based on the precepts of the Order" in the following words:
" If we could induce the rising generati on to take in the clements of chivalry and imbibe its ideals, they would grow up with th em and develop much more than we could ever do in trying to induce the present generation to take them up." It would appear that even in thos e days it was to the younger generation that appeals were made; and so to-day the same fact is very apparent-that it is the younger generation on whom we have to rely.
The result of our efforts may not be apparent immediately, but slowly and surely the voluntary spirit of « Service to Others" will again be an outstanding principle of this country.

DELEGATION TO BRITISH ZONE OF GERMANY
A second visit to the British Zone o£ Germany was paid by 1\IajorGeneral J. E. T. Younger (Secretary of the Order) and ]\1r. O. Horne ssistant Secretary of the Association) in November last to follow up their initial visit of April, 194 "when the institution of classes in first aid to members in the British Zone was discussed. Briefly, it can be reported that:
(I) A good beginning has been made in forming Association Classes in the British Zone. So far 30 classes have been formed, and more are in process of formation.
(2) The existing organization for running classes in first aid and kindred subjects is working smoothly.
(3) The extension of first aid classes to B.A.O.R. military personnel and Boy Scouts in Germany is gratifying.
(+) The 'Velfare and ]\ledical Directorates of the Control Commission are giving admirable support to the project.
(5) Since the fust visit the opinion in favour of extending instruction in first aid and kindred subjects to German nationals has developed.
A number of classes were visIted, and at the Training School for German male and female police at Ludendorf Barracks, Dusseldorf, Colonel M. S. O'Rorke (Inspector-General of Public afety) receiyed General Younger and ?\1r. Horne to whom an address of welcome was given bv the German Commander and the Deputy l'Ilinister for Internal Affairs. The winning team of the German Police Competition in Land North Rhine 'Vestphalia gave an excellent demonstration of first aid, General Younger, speaking in German to the students of the Training School, mentioned the long tradition of service to humanity which lay behind the present-day work of the Order and tho Association, and congratulated the winning team on its efficient display. He then handed to the winners the Cup presented by the Order and Association. This may be the beginning of a considerable extension of the knowledge of first aid in Germany; 25,000 German Police have already been trained in Land North Rhine 'Yestphalia, and it is hoped t.o 8
introduce this training to the police in the other" Lander".
Miss Ensor, Chief 'Velfaro Officer, B.A.O.R., was visited at her Headquarters, and the 'Welfare Department at certain Army and Air Force Hospitals were inspected. General Younger and :\1r. Horne cannot speak too hilShly of the splendid work :Miss Ensor (who is a mcmbel of the Order and of the Brigade) and her ' Velfare Officers are doing in the B.A.O.R. The delegation pays tribute to the help and kindness received from ::\lajor-General Sir Ed"ward Phillips (Director of Military Services), from other officers and officials throughout the tour, who made the visit so pleasant and so valuable.

ST. JOHN EXHIBITION
A very satisfactory financial result was the outcome of the St. J ohn Exhibition and Bazaar held in the Abbey Hall, Plymouth. The opening ceremony was performed by Commodore S. ]\1. Raw, R.N., who deputised for Vice-Admiral R. S. G. rIicholson (Admiral Superintendent of H.M. Dockyards, Devonport). Among those present were the Lord Mayor and Lady Mayoress of Plymouth, County Commissioner C. S. C. Prance, Asst. Commissioner Col. G. Thomson, and County Supt. 1rs. Balsdon, together with members of the County Staff, and Mrs. A. Camden who explained the reason for the Exhibition. Each day members of the Brigade acted as stallholders and stewards; the organizer being County Cadet Officer G. S. Thompson.
At Reading the Exhibition and Fair was opened by Mrs. Attlee on December 16 in the Town Hall. 1\1rs. Attlee was received by the Mayor of Reading, who presented the Mayoress, County Commissioner Poole, County Supt. the Hon. Mrs. Leslie Gamage, irs. A.
Colonel Sir James Sleeman (Chief Commissioner, Brigade Overseas) left London on December 30, to visit the Hong Kong District of the Brigade, where he will carry out a tour of inspection and hold an Investiture on behaH of the Grand Prior. From Hong Kong Sir James will visit Singapore and Malaya.
SOUTH AFRICA.
According to the annual report for 194 7, the outstanding event of the year was, of course, the Royal Visit and the organization of First Aid Posts, Dressing Stations, and Patrol Duties at each of the centres visited, as well as intermediate points along the line of route travelled. Their Majesties frequently commented upon the work that was being done by members, and there were many instances of their graciousness in speaking to St John members and in expressing their appreciation of Brigade services.
The members of the East Rand Corps, for the third year in succession, answered a call to assist at the Brakpan-Benoni Hospital during an outbr.eak of enteric which lasted several weeks. At the F.A. Camden, and others. The Coun ty Commissioner presided at the opening, and welcomed Mrs. Attlee, who ir declaring the Exhibition and Fair open paid tribui e to the work carried out by members of the St. John Ambulance Brigade. Grand Prior' Cadet Sergt. Agnes Charge (Windsor) presented Mrs. Attlee with a bouquet, and was herself presented with the Grand Prior's Badge by ]\1rs. Attlee. Irs. Gamage voiced the thanks of those present to Irs. Attlee, who made a tour o£ the stands, and paid much interest to the Exhibits displayed. Those present at the opening included members of the County Staff, Mrs. Beckwith-Smith, the Mavor of 'Vokingham, the. Mayors and- Mayoresses of Maidenhead, the Royal Borough o£ '''indsor, the County l\Iedical Officer of Health, the Reading Acting Iedical Ofiicer of Health, the Chief Constable of Berkshire and of Reading, and other local dignatories. Mrs. A. Camden gave a short talk on the exhibition, explaining the reasons for its journeys round the country.
Post, l\Iilner Park, Transvaal, which was opened early in the year, 15,000 patients have been treated. In the Cape Peninsula, the l\Iaternity Hospital Transport has been kept busy, and 464 patients have been taken to their homes by Nursing members. At a camp in Durban where there were 800 Scouts, a hospital tent of 24 beds was staffed for ten days. In all Districts members continue to give their services to hospitals, clinics, care of cripples at sports meetings, cinemas, etc. Although the membership in the Adult Divisions shows a decline, that of the Cadet Nursing continues to show an increase. Experiments arc being tried with Student Divisions for the older girls in order to maintain their interest. All the Cadets attend Brigade functions with great enthusiasm, and are proving themselves extremely useful in assisting the Adult Divisions.
9 The development of the training amongst the non-Europeans, the enrolment of trained personnel into organized units, both Adult and Cadet, has made steady progress. It is difficult to keep pace wi th the demand for training; those
engaged in it are handling greater numbers that would seem humanly possible, and are overcoming handicaps that would daunt many of us. They have to face the difficulties of transport and escort, lack of accommodation, theft of equipment, and in one location, the only lighting is that of pressure lamps and lanterns. IO praise is too high for the European members who go out night after night regardless of personal danger, to work among the non-Europeans who are so eager to learn all they can of the work of St. John. The reports of the activities of these members among their own people are very interesting, and clearly show that their knowledge of first aid and home nursing is of very great value to their community.
In response to representations made, the Government of the Union has been successful in obtaining the approval of His Majesty the King to the award of a South African " ' ar Medal to those who were not eligible for other war decorations, but who rendered valuable service towards the war effort in the Union.
MALTA DISTRICT.
The annual inspection of the :J1alta District was carried out by His Excellency the Governor accompanied by the Hon. Dr. Boffa (President), Countess }1ountbatten of Burma (Superintendent-inChief, Brigade at Home and Patroness of the District), and CommissJOner Lt.-Col. ]. V. Abela. After the inspection the Governor presented Service m edals and Bars and 'Varrants of Appointments to numerous officers of the Brigade. Addressing the parade, His Excellency recalled the valuable part St. John

Ambulance had played during the war, and reminded the members that their peace time job was none the less important. He hoped more people w.ould take.an interest in the work of the Bngade, wh Ich had made great progress. His Excellency concluded by congratulating the on their smart turn-out, and wIshed them success in the future.
Amongst the distinguished guests present were Admiral and Lady Power, His Honour Sir David Campbell, Mrs. P. Boffa, Rear Admiral and Mrs. Clarke, Air Yice-l\Iarshal and 1\1rs. Steele, Mrs. Revell-Smith, Sir George and Lady Borg, the Hon. Dr. Schembri-Adam, and RearAdmiral Lord Mountbatten.
The fifth Medical Comforts Depot was recently opened at Qormi, and will be under the direction of Lady Div. Supt. ::'\1rs. Muscat.
JAMAICA.
The first Investiture in Jamaica of the Order of St. John took place in the ballroom of King's House. The impressive ceremony was witnessed by many representatives of the Brigade and other visitors.
His Excellency the Governor entered the ballroom, wearing ceremonial dress with decorations, and his aide-de -camp presented the postulates by name. Those invested included Lady Huggins (as Commander Sister), Commissioner Dr. E. Hoerman (Commander Brother).
Those present included Brigadier and Mrs. E. K. Page, Police Commissioner W A. Calver, Inspector Clive Smith, Mr. R. H. Fletcher, r.S.O., Dr. Lightbourne, Mr. Sidney Chang, and Captain G. Fletcher.

A test tube containing blood from volumes of gases in terms of cubic which the oxygen has been removed centimetres and litres, but I propose is not bright scarlet. It has a to use instead the pint as a means of dark bluish tint, but if we shake measuring, because we are probably this blood with air it very rapidly more familiar with this volume . You may find to edge in this way, they would stretch it difficult to understand how a large five or six times round the world. volume, such as a gallon of oxygen, can In order that the cells of the organs pass through the walls of the air sacs throughout the body should receive and blood vessels in the lungs in one oxygen, blood vessels carrying these minute. This is probably easier to red b lood cells are distributed in great understand when we know that the richness to these organs, as can be well walls of the air sacs, if they could be shown in a microscopic picture. spread out to form one continuous What is the quantity of oxygen used sheet, would cover an area about the by the body? It is usual to state size of a tennis court .

How is our breathing controlled?
\Ve have just seen that we do not always use the same quantity of oxygen, nor do we always breathe same quantity of air. The quantIty we breathe is regulated. Our breathing is automatic. It goes on without our having to think about it. It goes on while we are asleep. This automatic control is brought about by the activity of a nerve centre in the brain, which we can call the it breathing centre". This centre is connected by nerves to the diaphragm, and at regular messages travel from the centre to the diaphragm, making it contract. The activity of this centre can be altered. vVhen we work, breathing is increased. Wben we rest it is decreased. 'When we work, not only is the breathing increased, but we use more oxygen and we produce more carbonic acid gas. The carbonic acid gas is carried into the blood, and got rid of through the lungs in breathing. The breathing centre is made to work more actively by the carbonic acid gas as it incr eases in the blood. \iVhen the centre works harder it gets rid of the carbonic acid gas more rapidly, so that this breathing centre acts as a sort of it governor" and controls the breathing in accordance with the needs of the body.
\Ve can alter the carbonic acid gas in the blood at will. We can decrease it by overbreathing. The atmospheric air contains only very little carbonic acid gas, so that if we breathe more deeply we wash out the carbonic acid gas from our lungs and cause it to come more rapidly out of the blood. If we have breathed deeply for a short time then we find that as a result of the carbonic acid gas of the blood being decreased we have no need to breathe for a little time. \Ve can cause the carbonic acid gas of the blood to be increased by holding our breath. As it increases, the breathing centre is made more active and we can hold our breath only for a short period of time until the messages from the centre become too strong and overcome our effort to hold our breath. We can increase the carbonic acid gas in our blood by breathing the same air over and over again, and as the carbonic acid gas in the blood increases, the quantity of air breathed becomes larger and larger.
CARBOX POISONING
After-damp in mines and coal gas which we use in our homes, contain the gas carbon monoxide. !his gas combines with the hremoglobm of the blood just as oxygen does, it is so easily separated from It. ThIS stronger combination with hremoglobin is such that if we wanted equal quantities of oxygen and carbon monoxide to combine with the blood we would have to present to the blood a gas mixture in which there was about 300 parts of oxygen for everyone of carbon monoxide. Carbon monoxide kills because it prevents the hremoglobin from being available for the carrying of oxygen To recover a person who has been poisoned with carbon monoxide, it is of some advantage to use oxygen instead of atmospheric air, and to add to the oxygen some carbonic acid gas. The presence of the carbonic acid gas assists in the separation of the oxygen from hremoglobin when it has been carried to the tissues, and also helps to make the breathing centre work.

LEDER TO THE EDITOR
Dear Sir,
COMPETITION JUDGES
Articles are frequently published from Doctor- Judges, and often speeches are made at competitions by the judges, giving advice to competitors. They are very welcome as they help to correct our mistakes. Would it be presumptious if I, as a competitor, criticized some of our judges?
For the past two years all examinations and competitions have been judged on the 39th Edition of the Textbook plus the Supplement, yet we still find competitions based on the Textbook alone! I have in front of me as I write this letter two marking sheets given this year awarding marks for the use of a St. John tourniquet. Last year I saw marking sheets with marks for treating a fractured spine in the face downwards position, a varicose vein bandaged below and above the wound, and marks awarded for removing clothing in the treatment of a scald. Surely the can expect to be judged accordmg to the latest instructions ?
Glaring inconsistencies are common. In the" Dewar", Police, and Inter-Railway Finals all inj uries are faked and symptoms 12
simulated wherever possilJle. If t.he t.est. calls for a compound fracture with the bone protruding, then the bone does protrude and nothing is left to the imagination. Should the competitor inquire "does the bone protrude? he deserves to lose marles for asking irrevalent questions. Competitors welcome the modem trend to depict injuries; but what happens in the local competition? A dozen chairs represents a crowd, the patient is a person without any knowledge of the signs and symptoms of his supposed injury and there is no faking of injuries. I appreciate that better facilities are available for the" Finals ", but let us have more " do" and less "say" competitions. Better co-operation between the judge and competition secretary would eliminate some of the troubles. Standard methods and standard judging are all that competitors ask for.
As my competing days are not yet over, permit me to hide behind my nom-deplume.
it CHESHIRE CAT."
(Editor's Note.-Competitions are one of the most excellent methods of training, and it is necessary that the tests be staged as near realistically as possible. The fact that marking sheets are incorrect is, in my opinion, because the Competition Secretary has not taken the trouble to check over the itemized sheets before the competition. On the other hand, there are some judges who do not send in their tests until the morning of the competition and are surprised when they find that certain material required is not to hand. The judge and the competition secretary must work closely together. It is not always realized that arranging competitions is a big job, if it is to be done well. In the big " Finals" months of preparation are given to the tests. It is no secret to say that the "Finals" for 1949 are already being prepared to ensure that every detail receives attention.
The Brigade Surgeons are fully alive to the importance of competitions; and at their recent Conference it was suggested that a panel of doctors be available to whom tests could be submitted before being put into operation.
Although facilities for staging local events are not comparable with that of the National Finals, much can be done by the enthusiastic secretary. The local Amateur Players are usually very willing to supply patients who will act a part, and quite a number of" props" can be obtained from the same source.
It is not always the judges' fault that tests are wrong; it is not always the secretary's fault; but better co-operation between both judge and secretary will do much to overcome local obstacles.)
