Delivering UK first in renal robotic surgery

In December 2023, the UK’s �irst long distance proctoring surgical procedure was carried out at Russells Hall Hospital by two consultants who were 170 miles apart!

Dudley consultant urologist Mr Syed Shahzad performed a complex robot-assisted partial nephrectomy during surgery while consultant urologist Mr Mark Rochester remotely entered the operating theatre via the Intuitive Hub from almost 200 miles away in Norfolk. During the surgery, Mr Rochester, from Norfolk and Norwich University Hospital, had full access to the operation in real time and the surgeons communicated through a headset without any time-delay. Mr Syed Shahzad said: “Robotic-assisted renal surgery is a great

success story for Black Country collaboration and this is a proud day for The Dudley Group. The use of remote technology will support surgeons during complex procedures which could lead to better patient care and provide a better patient experience.” This complex robotic-assisted operation includes removal of tumour from the kidney as opposed to removing the whole kidney, which adds to patients’ health and longevity. This allows preservation of kidneys and avoids the need for dialysis. Chief executive Diane Wake said: “Robotics availability at The Dudley Group will help reduce waiting times for renal cancer surgery with enhanced capacity and improved surgical outcomes.” Mr Rochester said: “As an experienced trainer, I have helped many surgeons to safely set up robot-as-

sisted surgical programmes for kidney cancer. Usually this requires me to travel to be in the theatre with the surgeon but with this new technology, it is possible for me to see the same view as the operating surgeon and speak to them in real time from my desk. It is great to be able to support other surgeons with advice and guidance using this new platform and to share the experience we have built up at Norfolk and Norwich University Hospital.” The Black Country Integrated Care System is leading in terms of expansion in robotic-assisted surgery, and this is a great achievement of Black Country collaboration and exemplary initiative to improve cancer care within the region. The induction and implementation of a robot-assisted renal programme can be a challenging and

complex task. Remote proctoring supports this implementation by allowing trainee surgeons to learn from specialised urologist surgeons, without the need for them to be in the same room. Intuitive Hub allows surgeons to collaborate virtually with each other, transmitting and receiving real-time audio and video. Remote surgeons can have a view of the operating theatre as well as the endoscopic view of the surgery itself. Through this technology, surgeons can communicate and share their perspectives without the need for travel during complex procedures. It also enables peer-to-peer learning without the need to travel, making training and upskilling in robot-assisted surgery more accessible.

Welcome to the latest edition of Your Trust. Our previous edition celebrated 75 years of the NHS, and it is safe to say the way healthcare is delivered has come a long way since the NHS was founded in 1948. As a Trust, we are proud to be delivering excellent and innovative care to the people of Dudley. In this edition, we will be highlighting some brilliant examples of innovation, such as robotic-assisted renal surgery. In December, the UK’s �irst long distance proctoring surgical procedure was carried out at Russells Hall Hospital, with one of our consultant urologists performing surgery with the assistance of a surgeon 200 miles away in Norfolk! Robot-assisted surgery is also in place for our knee and hip replacement surgeries. These cutting-edge treatments will help us reduce waiting times and improve outcomes for you, our patients, and I am delighted that these procedures will be more accessible for those in Dudley and the wider Black Country region. I am also thrilled to share our recent ‘Good’ rating from the Care Quality Commission (CQC) for our children and young people services. This is a huge achievement for our Trust and is largely down to the commitment of our staff delivering these services. They have been committed to ensuring that improvements have been made, and I am so pleased that their hard work has been re�lected by this new CQC rating, which demonstrates our drive to provide the very best services for children and young people.

Use of robotic arm transforms joint surgery

The Stryker Mako robot

A new advanced robotic arm, which will transform knee and hip replacement surgeries, is now in place at Russells Hall Hospital. The Stryker Mako robot allows for more accurate positioning of the new joint than traditional surgeries, leading to improved long-term outcomes for patients, such as reduced pain, better mobility and faster recovery times. The robot-assisted procedure also increases the longevity of the implant, making it less likely that patients will need further surgery in the future. A pre-operative CT scan of the patient’s hip or knee is taken in order to create a 3D model of the joint. With the help of a Mako product specialist, surgeons use the 3D model to create a detailed implant plan to determine the exact amount of bone excision to be carried out by the robotic arm in the operation. The Mako robot also gives real-time feedback during the procedure, enabling surgeons to assess the tension of the joint and �ine-tune soft tissue balancing whilst they are operating, which is essential for optimal function of the joint.

State-of-the-art

We are currently one of a small number of NHS trusts with this state-of-the-art technology. Mr Sohail Quraishi, consultant orthopaedic surgeon, said: “The aim of surgery is to reduce pain and restore normal function to the joint. With the Mako robot, we can greatly improve the alignment of the implant, reducing the possibility of the patient experiencing pain after surgery and restoring their anatomy fully so that they can go on to enjoy physical activities after they recover.” The Mako robot was installed as part of a procurement partnership with Stryker, at no cost to the Trust. The Mako robot was �irst used in December 2023, with several more procedures carried out since. Diane Wake, chief executive, said: “It is fantastic to be able to offer such an innovative treatment to our patients. The Mako robot will help us transform the lives of those with even the most complex cases of arthritis and I am very proud that we can deliver that care here in Dudley.”

Hard work pays off after service praised

In November, our Trust received a new rating of ‘Good’ for its services for children and young people from the Care Quality Commission (CQC).

Following an inspection of urgent and emergency services and services for children and young people at Russells Hall Hospital in May and June 2023, the CQC shared their new rating con�irming that the services for children and young people had moved from a rating of ‘Requires Improvement’ to ‘Good’.

Feedback

When sharing their feedback, the CQC said that staff treated children and young people with compassion and kindness, respecting their privacy and dignity. The service planned care to meet the needs of local people and took account of the individual needs of the children and young people in their care. Furthermore, the leaders ran the service well, supporting their staff to develop their skills and it was clear staff had solid training in under-

standing how to protect children and young people, managing their safety well. Diane Wake said: “We are really pleased that the Care Quality Commission has rated our children and young people service as ‘Good’ following an inspection in May and

June, moving up from the previous rating of ‘Requires Improvement’. “This achievement is down to the commitment and determination of staff to improve care for young people and children using the service and we are pleased that the inspectors highlighted widespread

improvement in the quality of care provided.” The CQC programme aims to provide an up-to-date view of the quality of hospital care across the country and a better understanding of what is working well to support learning at a local and national level.

Winter is always a challenging time for the NHS and requires extensive planning with our partners across health and social care services, we have also seen the added pressure of sustained industrial action across the NHS. I’d like to thank our staff and those of our partner organisations, who have worked tirelessly, supporting their colleagues and continuing to provide safe care for the people of Dudley. I would also like to remind you that there are many ways to get

The critical care team and the end-of-life rapid response team received the Gold Standards Framework (GSF) award. The Trust also welcomed healthcare professionals from Singapore to share learning

Leading way for end-of-life care Two clinical teams at Russells Hall Hospital have become the first in the country to reach the gold standard for end-oflife care for their speciality in healthcare.

The critical care team is the first intensive care unit to receive the Gold Standards Framework (GSF) accreditation while the end-of-life rapid response team is the first district nursing team to achieve this nationally. The GSF ensures people live well

before they die and die well in the place and manner of their choosing. This makes such a difference to both patients and their families during a very emotional time. Palliative medicine consultant Dr Joanne Bowen said: “The work of staff across the Trust to care for those at the end of their life has been remarkable to watch over the past year. “There has been a significant culture change that has ensured departments are understanding the

importance of end-of-life care for both patients and their families.” Three other wards have also attained their GSF re-accreditation and there will be other areas of the Trust working towards their’s in the coming year.

Standard

The GSF is an evidence-based approach to optimise care for all patients approaching the end of life, delivered by generalist frontline care providers.

The aim is to enable a gold standard of care for everyone, with any condition, in any setting, given by any care provider, at any time in a person’s last years of life. In November, a team of healthcare professionals from Singapore’s ‘Healthcare Manpower Development Programme (HMDP)’ visited the Trust to gain experience and learn from our successful implementation of the GSF. They were welcomed by chief operating officer Karen Kelly and depu-

ty chief nurse Jo Wakeman and then, following a short presentation and round table discussion, visited the Forget-me-not Unit and Critical Care to meet staff, learn from their experiences and understand the impact of the Gold Standards Framework. The feedback from the Singapore team was excellent and they were amazed at the achievements and impact on patient care. It was an honour to host the Singapore team, enable shared learning and build relationships to share best practice.

Funding boost for expansion The Trust has been successful in securing a £16.9m redevelopment bid for our Emergency Department (ED), which will transform our resus facilities. The new state-of-the-art facility will provide an enhanced resuscitation space that will revolutionise the care given at the Trust’s emergency department. Trust chief executive Diane Wake said: “This will have huge benefits not only for our patients and the Black Country community, but also for our staff by improving the facilities.” The resuscitation design includes additional resuscitation facilities with a dedicated paediatric area and isolation rooms to provide appropriate safe care in the event of future pandemics or outbreaks. The department will also have state-of-the-art technology and equipment, including a digital medicine system to improve the storing, usage and prescribing of drugs to patients. Dr Ahmed Ismail, consultant and clinical director of urgent & acute services, said: “It’s going to have a huge impact, with advanced equipment optimised to enhance the way we treat our patients – our Trust truly needs and deserves this!” The build will begin this year, with the target completion date of Spring 2025.

Cutting-edge treatment to improve glaucoma care Selective Laser Trabeculoplasty is now available as one of many treatments for patients with glaucoma at the Trust.

The new laser machine was unveiled by Trust chair Sir David Nicholson and chief of surgery Mr Babar Elahi during World Sight Day in October 2023 and patients have already begun to receive this new treatment.

Mr Elahi said: “This recommended shift in clinical practice not only aims to reduce the dependency on eye drops for patients but also promises to enhance their quality of life by providing a wider array of treatment options. “It marks a significant stride towards better managing and treating glaucoma for our valued patients. “We look forward to providing this cutting-edge treatment and improving the lives of those affected by glaucoma in our community.” Trust chair Sir David Nicholson said: “Here in Dudley we deliver excellent healthcare, and this new glaucoma treatment shows that we

are focusing on innovative solutions to help our patients both now and into the future.” Glaucoma is a lifelong and potentially sight-threatening condition and around 10 per cent of individuals in the UK who experience vision

Health and Care Excellence (NICE) on glaucoma management, patients newly diagnosed with glaucoma and ocular hypertension (OHT) will now be offered Selective Laser Trabeculoplasty as the first-line treatment.

Cardiology wins research award The Trust is delighted to announce that our cardiology research team has won Research Contribution of the Year at the Clinical Research Network West Midlands Awards 2023.

The team, consisting of consultant cardiologists Dr Joe Martins, Dr Craig Barr and Dr Georgios Leventogiannis, research nurses Alexandra Florou and Alexandros Margaritis and clinical research associate Eoughin Perry, were nominated by Claire Phillips, research and development manager, for “providing an environment where research, innovation and the adoption of new technology �lourish”. The cardiology research team has delivered several clinical trials, such as the REACH-HFpEF Trial, which assesses the effectiveness of home-based cardiac rehabilitation and supports self-management for people with heart failure and their caregivers. Research trials are fantastic as they give patients in Dudley early access to modern, cutting-edge treatments. Consultant cardiologist Joe Martins said: “We are all delighted and very proud to win this award. It is testament to the hard work from

fessor Matthew Brookes. The CRN West Midlands works to support researchers with the infrastructure they need to carry out their research. Diane Wake said: “This is a great achievement and I am so pleased that they have been recognised. Their clinical research allows our patients in Dudley to have access to the latest high-quality, innovative care and will help us better care for our patients in the future.”

In November, schoolchildren from across the Black Country were given the chance to go behind the scenes at Russells Hall Hospital, getting �irst-hand experience of the day-to-day work NHS staff do. Over 80 pupils from six schools were taken on the Behind-the-Scenes interactive tour, visiting various clinical and non-clinical areas of the hospital, including the maternity, imaging and therapy departments. The schoolchildren from years 9, 10 and 11 at Leasowes High School, Kingswinford Academy, The Wordsley School, Redhill School, West Bromwich Collegiate Academy and Windsor High School and Sixth Form were also given the opportunity to get hands-on practical experience. Students who visited the Pharmacy Department, which was upgraded last year following a £1.2 million investment, learnt how to complete prescription checks, as well as being shown how medication is dispensed by the brand-new pharmacy robot. Pupils were also shown basic life support skills using a simulation manikin at The Ron Grimley Undergraduate Simulation Centre and practised using ECG equipment on a dummy at the Action Heart Centre.

Events and partnerships helping Glitter Ball 2023

The Dudley Group NHS Charity hosted our iconic annual Glitter Ball for a third year running at the Copthorne Hotel in Dudley and raised a whopping £16,500. The event was kindly sponsored by Mitie, Summit Healthcare, How to �ind a Care Home and Black Country & Marches Institute of Technology. Companies such as Ansons Solicitors, HSBC, West Bromwich Building Society and Waldrons Solicitors were in attendance. The funds raised from the event supported the charity’s patient and staff wellbeing fund. Guests were treated to a wonderful evening and some of the speakers included Dr Richard Alleyne, who spoke about his work in palliative care and the vital role the Trust Charity plays in supporting patients in end-of-life care at Dudley. In addition, guests also heard from Billy Spakemon, a well-known local Black Country busker, who has raised over £43,000 for the charity to date. There were some really passionate speakers, with compelling stories that I myself personally related to, particularly around palliative care.

Young patients can experience worry, uncertainty and anxiety before their surgery. It is dif�icult for them to understand what is going to happen before, during and after their

surgery, and the cars provide a perfect distraction tool. Our celebration of achievements A focus on Equality and Wellbeing

The equalities and wellbeing team would like to reflect on their achievements last year, most of which have only been possible because of our staff networks and our wellbeing champions, who have worked alongside the team to improve the working lives of their colleagues. Many of the Key Performance Indicators (KPIs) have seen improvements, such as improving the Trust’s disability declaration rates to six per cent, increasing our ethnically diverse workforce to over 26 per cent and maintaining our gold TIDE mark. The equality and wellbeing team have all been working to increase visibility by attending staff meetings, highlighting the Trust wellbeing offers and delivering training sessions such as Allyship and Manager’s Essentials. Throughout 2024, they will focus on embedding their campaigns and projects and will continue to work through the actions set out in their Equality, Diversity, and Inclusion Journey and their new Wellbeing Journey launching soon. Highlights from the end of 2023 include: ■ Celebrated Black History Month in October and Disability History Month in November/December. ■ Recruited over 100 Wellbeing Champions ■ Launched a neurodiverse toolkit.

October 2023 the Trust launched its anti-racism statement and resources. They plan to further strengthen their anti-racism work by focusing on a policy statement, reporting mechanisms and ensuring there is clear guidance for colleagues, patients and visitors to follow to stamp out racism.

LGBTQ+ Staff Network

The networks celebrated Black History Month and Disability History Month and members are working to support all Trust staff

■ Launch of a centralised process for reasonable adjustments. ■ Launched the Trust’s anti-racism statement. ■ Implemented cycle one of the Equality Delivery System (EDS), evaluating patient experience, workforce wellbeing and inclusive leadership. ■ Undertook the NHS Rainbow Badge Phase II assessment to support actions to improve the experience of our LGBTQ+ community. ■ Worked towards thrive at work accreditation and the delivery of two mental health first aid courses.

The Trust has four established staff networks, with two more join-

ing them soon – a carers network and an armed forces network. The networks have focused on growing their membership over the past year to strengthen their voices.

Disability and Long-term Conditions Staff Network

The Disability and Long-term Conditions Staff Network has co-produced a supportive neurodiverse toolkit which launched in Disability History Month in November 2023. The network has also continued to support staff who have a disability or long-term condition. Staff can now contact the equalities & wellbeing team, who provide support with applications to the Access to Work program through the government. The

team will then order any equipment or software to ensure adjustments are made as quickly and efficiently as possible. Future plans include embedding the centralised process for reasonable adjustments, further improving disability declaration rates and generally advocating to improve staff experience for those with a disability or long-term condition.

EmbRACE Staff Network

The EmbRACE network and its members have been supporting the work of our equalities midwife in maternity services, driving improvements in learning and development content, and raising awareness of lived experience to support the Trust to improve patient experience. During Black History Month in

The network played an instrumental part in the Trust obtaining an initial stage of the Rainbow Badge Phase 2 accreditation. The Rainbow Badge assessment was thorough and has enabled an action plan to be formed to support the Trust to improve our offering for LGBTQ+ staff, patients, and their allies. This year will see the network prioritise creating a safe space for LGBTQ+ members to talk about issues, and signposting and offering support and will support the Trust in delivering actions from the Rainbow Badge Assessment.

Women’s Staff Network

The Women’s Network has supported a women’s medics careers fair and held sessions to encourage, support, and start conversations around issues around reproductive health. Message from Alex Giles, public elected governor: Stourbridge and lead governor, The Dudley Group Council of Governors.

I have been a public governor for the Stourbridge constituency for the last two years and the lead governor for just over a year. The governor role has provided me with opportunities to go behind the scenes and interact with our staff and patients to identify areas of development and see all the excellent work done daily. As governors, we ensure that the Trust constantly strives to achieve one of its core values – to provide the best patient care. In the last few months, I have been proud to support the governors with the various Trust-led activities, like the Patient-Led Assessment of the Care Environment, and Quality & Safety reviews. A few of our governors also attended events hosted by Dudley Integrated Health and Care NHS Trust, including their Annual General Meeting and the Black African Caribbean Health event. The governors are always keen to support Trust charity events. These experiences help the governors to interact with patients and staff members to share their views as part of the Annual Planning Workshop. All governors want to ensure that the Trust is managed well to provide a better staff and patient experience.

Governors take to the airways In December, two of our governors, Dr Mohit Mandiratta and Alex Giles, joined Brian Dakin (aka Billy Spakemon) on Black Country Xtra to promote our foundation trust membership programme and talk about the governor’s role. They spoke passionately about their roles as appointed governor and public governor and how, as a council of governors, they collectively work to ensure that the Trust delivers on its vision to provide excellent healthcare and improved health for all. Project offers a snapshot of 24 hours behind the scenes The Dudley Group NHS Foundation Trust came together with health organisations from around the Black Country to give a rare “behind the scenes” look into life in the NHS via social media.

In Dudley, we focused on our hospital avoidance and our community services who do such a fantastic job every day preventing hospital admissions by reaching people in their homes and seeing patients in community settings. District nurse Laura Hague told us: “I really enjoy the job that I do. I love taking care of patients in their own homes, in their own comfort. Hospital avoidance is a massive part

Shaping the future of our workforce

The ‘Make it Happen’ campaign aims to listen to staff to understand how it feels to work at The Dudley Group and to take on board their views and make steps to improve the staff experience for everyone

The Dudley Group NHS Foundation Trust has launched a ‘Make it Happen’ campaign, which stems from a desire to improve how it feels to work at The Dudley Group and what it is like to be a part of the Trust.

It has become an umbrella for staff engagement and cultural aspects of work, which focus on Dudley being a brilliant place to work and thrive.

One speci�ic area of focus is embedding our approach to listening to staff and identifying actions, ensuring these are completed and feeding back. They way we do this is by dropping into departments across the Trust to talk and engage with our people. This gives them the opportunity to share their thoughts and opinions directly and have their voices heard. The campaign plans to run these

walkarounds quarterly throughout the year, with each round having a different theme. Our �irst round focused on work culture; what do we do well, what we want to see and what needs to be better. We received over 900 pieces of feedback from staff across the Trust, which was a huge achievement for the campaign.
