Sales Presentation Binder Elderplan is an HMO plan with Medicare and Medicaid contracts and has a coordination of benefits agreement with the New York State Department of Health. Enrollment in Elderplan depends on contract renewal. Anyone entitled to Medicare Part A and B may apply. Enrolled members must continue to pay their Medicare Part B premium if not otherwise paid by Medicaid or a third party. Elderplan renews its contract with the Centers for Medicare & Medicaid Services (CMS) annually. The formulary, provider network and pharmacy network, may change at any time. You will receive notice when necessary. Benefits, premiums and/or co-payments /co-insurance may change on January 1st of each year. This information is not a complete description of benefits.
