Clare Garden Suites service plans are coordinated by a dedicated and highly trained multidisciplinary team of professionals working together to meet your needs and goals while supporting overall wellness. We recognize that aging presents unique challenges, so we’ll meet you where you are with A Life Engaged programming that brings social, spiritual, and recreational experiences to our residents, creating meaningful, joy‐filled opportunities and a true sense of belonging.

*2nd Occupant monthly rent will be reduced to $500 if no services are provided. We offer 4 levels of care between our two dis nct memory care neighborhoods, Garden Suites (levels 1‐4) and Clare Suites (level 5‐6) that ensure your loved one is properly cared for when living with memory loss, regardless of the severity. Our levels of care focus on personalized resident needs and the type of assistance with Ac vi es of Daily Living (ADL's) required. ADL's include but are not limited to bathing, toile ng, dressing, personal hygiene, ability to transfer and mobility. Prior to admission, a thorough assessment by our team will determine which level of care best suits your loved one.
Included in all levels: 24 hour certified nurse aid supervision, social and recreational programming, social services and pastoral care, medication administration, laundry service, and one bath with supervision by staff.
Level 1—Base Monthly Month Resident functions independently but desires a structured environment. Resident may require minimal supervision with their ADL’s.
Level 2 Resident requires minimal physical assistance, benefitting from oversight, encouragement, and verbal with their ADL’s. Resident has mild cognitive impairment and requires occasional redirecting.

Level 3 Resident requires physical assistance with their ADL’s. Resident needs assist of 1 to transfer, and may be incontinent and needs assistance with toileting. Resident may wander and require a wander guard and near constant redirecting.
