The 12th Annual Women’s Health & Fitness Expo is more exciting than ever this year. Designed to provide a wealth of information that promotes healthier living, this event offers everything from tips on how to prepare healthy and delicious meals to information about fitness, recreational activities and wellness seminars. Losing weight and keeping it off is not easy, but it is possible and sometimes even enjoyable. When I was stuffi ng my face with potato chips, or sitting down gingerly in chairs that might not hold my weight, I never in a million years imagined that I could lose half my weight and live the rest of my life as an active mom to seven beautiful children. Instead, I imagined myself sitting in a chair while the rest of my family enjoyed hikes, bike rides, running in the yard, and swimming. Instead of envisioning myself running down the road with the wind in my hair, I imagined myself getting my shopping done on a grocery store scooter, oblivious to onlookers as I drove through the aisles picking out cookies, candy, and chips. My fi rst step out from the oppression of obesity involved realizing that I would never get closer to any dream by just sitting down and waiting for something to happen magically. Your fi rst step may be reading this book, but your journey to lose weight and stay at a healthy weight for the rest of your life will involve many steps. You can start by answering the questions I pose in the upcoming chapters, making a commitment to healthy eating, learning to eat the right portion sizes, and starting an exercise program that fits within your lifestyle and physical capabilities. The plan I offer isn’t full of silver bullets and false hope. It’s a practical and realistic plan grounded in common sense and sound medical information. I know that it can be scary to try to lose weight, as you might have failed before. But I assure you that you will never regret making the smart choice between a healthy and an unhealthy lifestyle. Never once when I was losing weight did I regret turning down a brownie or having a salad instead of a fatty hamburger. And never once was I sorry that I exercised. As you begin your journey for the fi rst or fiftieth time, remember that this can be your fi nal weight-loss attempt. Once you successfully lose weight using Fit to the Finish and keep it off, you never have to go down the weight-loss road again. Instead, you can go through the rest of your life focusing on keeping your family relationships healthy, achieving your professional goals, and enjoying the satisfaction of knowing that you’ve already done the work you needed to do to keep your weight at a healthy level. The fact is, the only way you can be at your personal best is when your brain is at its personal best. Your mind is responsible for every thought, emotion, dream and idea you have. It interprets each sensation and orchestrates all of our movements. It allows us to learn and to remember and ultimately determines our ability to lead a productive and meaningful life. In other words, our brain is the essence of our being. This is why investing in your brain is the most lifechanging action you can take. When your brain at its best your ability to make wise decisions, to think creatively and to problem solve will dramatically improve. You will notice a new sense of inner joy and calm that are the natural result of a healthy, balanced brain. An additional benefit is that you will also look better. It’s about spending time with friends and family, and learning or experiencing something new. That’s right. The physical health of our brain is greatly influenced by the lifestyle choices we make. You should also pay attention to how you nourish

and replenish your brain. As a neurologist who cares for patients with Alzheimer’s disease, I tell family members concerned about prevention to be as physically active as possible and to eat a nutritionally rich and balanced diet. (I recommend skipping the supplements. They will never replace fresh, unprocessed foods and those rich in antioxidants like Omega-3s.) One of the most remarkable breakthroughs in neuroscience has been discovering our brain’s ability to change and redesign itself. We used to think that we were born with all the brain cells we’d ever have and that our brains would never change or gradually devolve. We now know, however, that our brain is able to make new neurons and new connections throughout our lives. The scientific word for this is neuroplasticity. The good news is that by tapping into our brain’s ability to regenerate itself, we can rewire and strengthen our brain. In other words, we are the architects of our own minds. By developing new interests and engaging in activities that stir our passion we can keep our minds, young and healthy. When we are passionate about what we are doing it enhances our brain’s ability to learn and remember. By enriching your mind each day with new experiences and information you are building up what is called a “cognitive reserve.” It’s like putting money in the bank – the more information you have stored over time, the more resilient your brain will become. So the message for this week is simple. Start tapping into the power of your brain to change by being open to new experiences and striving to learn something new every day. In doing so, you will begin forge new neural pathways and make new connections. Actually, you’ve already started. The simple act of reading this blog has physically changed your brain. I also hope you will start taking good care for your most precious organ and provide it with a nurturing environment so you can enjoy the full potential of your mind.

Intuitive eating At any point in time, one in three women and one in five men in the United States are on some kind of diet. Most dieters opt for traditional programs that count nutrients or servings of food or actually specify which foods to eat. Those diets produce short-term weight loss, but two or three years later, 95 percent of traditional dieters regain the weight. But the diet industry manages to hold on because hope springs eternal in the human breast. Research shows that starvation, whether from natural causes or intentional dieting, increases the risk of overeating and binge eating disorder (binge eating disorder is a distinct entity and not the same as overeating.) On-again, off-again dieters regain lost weight by over-eating in-between periods of restrictive dieting. The human body is simply programmed to respond to starvation by hoarding food when it becomes available. On-again, off-again dieters develop a “dieting mindset.” They lose touch with their thresholds for taste and fullness. For example, when normal eaters eat sweets or a meal, they cross over a threshold and lose their desire for more sweets or food. Chronic dieters, on the other hand, learn to ignore those signals. They decide when, what, and how much to eat based on whether they are on or off a diet. That leaves them susceptible to eating in response to external cues, like TV commercials and food pushers, and to non-food cues such as boredom and unpleasant feelings.

Intuitive eating, also called the non-diet approach, conscious eating, breaking free, and other layman terms derived from book titles, shifts the paradigm from weight loss to wellness, from food fear to food pleasure, from body dissatisfaction to body trust. The approach was popularized in the late 1970s by mental health and nutrition professionals working independently in the eating disorders field, all with the common belief that diets don’t work. Intuitive eating teaches self-regulation in response to the body’s internal signals of hunger and satiety. The journey is one of self-discovery, not a test of will. Guide books exist to provide structured training in the non-diet approach. Authors Geneen Roth, Evelyn Tribole, Karen R. Koenig, Michelle May, and others listed in the Gürze Books Catalog of Eating Disorder Resources teach how to break free of restrictive dieting and relearn how to eat normally, which is not the same as healthy eating. When lifelong dieters relearn to make food choices based on body signals (with a reasonable understanding of good nutrition), intake seems to gravitate to the amount of food needed to maintain a healthy weight. Over time, when weight gain from the diet-binge cycle ends, with a reasonable intake and moderate activity, the body drifts towards a natural weight and the need for dieting ends. For very overweight individuals who need a radical short term approach, the switch to intuitive eating should be made to guarantee long-term maintenance.

3 Small Steps for Starting (or Resuming) a Successful exercise Program
Thinking of starting (or starting back up) an exercise routine? Good for you! Before jumping in, here are three steps you can take to make sure you continue on your fitness journey beyond the end of the month (or week): Take one small step towards your goal each day. One of the biggest mistakes new exercisers make is going from zero to sixty with their fitness routines, which just sets you up for a major disaster from burn out or overtraining. For example, if you haven’t been running regularly, today isn’t the day to go out and go for five miles. Instead, lace up your sneakers and go for a fifteen-minute walk. Walking is a great way to start a fitness routine – its free, easy, doesn’t require equipment and everyone knows how to do it. Gradually increase the duration and intensity of your workouts over time in order to make them manageable over the long term. Gather a group for support. Studies show that when you surround yourself with people who have similar goals, you’ll be more likely to achieve your own. So tell your friends about your intentions to start exercising, and see if any of them will join you. Go for walks with your girlfriends instead of meeting for a meal or ask your husband to join you at them gym. The more support you receive from those around you, the easier it will be to stick to your routine. Reward yourself along the way. The hardest part about sticking with an exercise program is that you won’t see results right away. A lot of the progress you’ll be making as you become more fit will be internal, and may not show as quickly as you’d like on the scale. Avoid frustration by rewarding yourself for mini-goals along the way. If your goal is to lose 20 pounds, you may not get to the fi nish line for several months. Why not set a mini-goal instead of exercising for at least 20 minutes, three days a week this month? Once you’ve reached that milestone, reward yourself with a new pair of sneakers, a new workout music mix or outfit to help you stay inspired and motivated to keep going back for more. And fi nally, remember, Rome wasn’t built in a day, and your fitness program isn’t going to transform you overnight either. Be patient, persistent and progress gradually for long-term, sustainable success. And of course, be sure to check with your doctor before beginning any exercise program. Little changes really can change everything – so don’t wait, get started today!

Smart at Heart
The following passage highlights the spirit of the HAPPY Heart Study. This study is the basis of the book Smart at Heart which brings together the importance of self care, friendship and taking time to build a happier healthier heart.
In order to be a participant in the HAPPY Heart study, participants have to have at least two major risk factors for cardiovascular disease: high blood pressure, abnormal cholesterol levels, diabetes, obesity, cigarette smoking, a sedentary lifestyle, and a genetic history of cardiac issues in the family. Over 80 percent of the sixty-five women in the program have at least three risk factors: the most common are obesity, low HDL (or the good kind) cholesterol levels, and a sedentary lifestyle. Unfortunately, the women are in good company. The American Heart Association (AHA) issued new guidelines for the prevention of cardiovascular disease in women in early 2011, and the statistics cited in the introduction are troubling– to say the least. Two in three women over the age of thirty are either overweight or obese. More than twelve million American women have diabetes, a disease that is so tightly linked to cardiovascular disease that doctors often treat the two conditions simultaneously. Many physicians now refer to diabetes and obesity as “diabesity,” because of their frequent co-existence. High blood pressure is on the rise, especially among African-American women; an overwhelming 44 percent of that population has high blood pressure. Although the seemingly most popular two American pastimes—fast food restaurants and extended periods of screen time—might lead you to think otherwise, the epidemic of cardiovascular disease is not limited to the United States. “Heart disease is the leading cause of death in women in every major developed country and most emerging economies,” the 2011 AHA proclaims . Given that heart disease is the number one killer of women—in the States in 2006, over 430,000 women died
from cardiovascular disease, compared to about 270,000 from all forms of cancer—the public awareness is still disturbingly low. In the AHA guidelines ,researchers found that only 53 percent of women polled said that the first thing they would do if they suspected they were having a heart attack would be to call 911. That lack of awareness, combined with the rise of obesity, is contributing to a trend that shouldn’t be happening in the twenty-first century: death rates from cardiovascular disease for women under the age of 54 are, amazingly, rising. For the first time in forty years, the number of US women between ages thirty-five to fifty-four who die from heart-related issues is actually increasing. Like many women across the world, the threat of a cardiac event stares down the HAPPY Heart participants daily. “My father died of arterial sclerosis at sixty-two,” says Christie, sixty years old, one of the participants who heard about the program from me when she came to my office with heart palpitations, “And my mother was a diabetic who had a triple bypass and a pacemaker. Seven of her siblings died of heart problems before they took her at age eighty. Those thoughts just live in the back of my head.” Although it may seem like cardiovascular disease is best treated by doctors in white coats, medical care is just one piece of the puzzle. Sure, doctors can monitor your cholesterol and your glucose levels, your weight and your blood pressure, but your cardiovascular--and overall--health depends on so much more than simple measurements. I firmly believe—and science has proven—that getting smart at heart is about evaluating your whole life, from your relationships to your environment to your mental state. “Medications alone aren’t enough. Surgical procedures aren’t enough. Stents aren’t enough,” says Kate Traynor, a colleague of mine and the program director at the Cardiovascular Disease Prevention Center at Massachusetts General Hospital in Boston. “How many times have you heard that somebody needs to go back for another stent? Where the rubber hits the road—and what will keep you and your heart healthy—are lifestyle changes.”

American Cancer Society Cancer Prevention Study (CPS-3) There are over 11 million cancer survivors alive today, thanks to advancements in screening tools, treatment medications, and better knowledge about prevention. Award Winning Energy Dance Company of Kingston, NY performs explosive Hip-Hop dance, combined with Latin, Caribbean, and Jazz-Funk movements. Their works are choreographed by C.C.E.’s Artistic Director, Mr. Bryant ‘Drew’ Andrews, with the assistance of Miss Lisa Brown. Energy has swept the valley with its fresh and entertaining Hip-Hop, Reggae and Latin dance styles. They have won fi rst place awards granted by the renowned Apollo Theater in New York, the “Wild out Wednesday” segment of B.E.T., and they have competed in The Arnold Schwarzenegger ‘Classic’competition in Columbus, Ohio. They have gone on to perform in the Debbie Allen Dance Company Annual Dance Intensive, in Culver City California where they were privileged to dance alongside well-known choreographers for various well-known artists. Energy was also the opening act for ‘America’s Best Dance Crew’ and was named Albany’s Number One Dance Crew. In addition to these wonderful achievements, they were invited to perform in a Music Festival held in Bad Arolsen, Germany in 2010. They were asked to return this year to perform (2012), due to the overwhelming reception they received when they last visited. POOK, the Percussion Orchestra of Kingston, is celebrating its 15th year in 2012. This popular inter-generational ensemble has performed throughout the region and inspired hundreds of young people with a love of music and rhythm. Possessing up-to-date photographs and detailed information about a child can prove to be important proactive measures that can greatly assist local law enforcement officials to quickly respond to a child’s disappearance. Using equipment that contains the latest digital fi ngerprinting technology and high resolution photography capabilities, the Town of Ulster Police Department will produce SAFE CHILD cards for parents and guardians. The cards contain a child’s name, biographical information (date of birth, gender, height, weight, hair color, eye color, etc.), and a fi ngerprint image of both index fi ngers. Cards are made in less than two minutes and can be easily carried in a wallet or pocketbook.

If you are a smoker, have high blood pressure, diabetes or high cholesterol, and are older than the age of 60, you are at risk for Peripheral Arterial Disease (PAD), also known as atherosclerosis, poor circulation or hardening of the arteries. It’s hard to know if you have it, because symptoms often don’t become unbearable until the later stages; in its later stages, it can cause severe circulation problems and pain in the legs and feet, leading to non-healing wounds and ulcers, gangrene, and even amputation. Additional consequences of PAD are an increased risk for developing coronary artery disease and cerebrovascular disease, which can lead to a heart attack or stroke. About eight million people in the United States suffer from some form of PAD. To get diagnosed, your primary physician can order a common, non-invasive test called the ankle-brachial index (ABI). It measures the difference in your ankles’ blood pressure, and compares it to that of your arms while at rest and after exercise. Further tests like ultrasound, CT scanning and Magnetic Resonance Angiography can guide your treatment plan. A wide variety of non-surgical and minimally invasive solutions are offered by the board-certified vascular surgeons at the Center of Vascular Excellence at HealthAlliance of the Hudson Valley’s Benedictine Hospital campus. Some minimally-invasive options include angiography, stent placement, clot-removal treatments, and atherectomy. If you are at risk for PAD, it’s not too late. Lifestyle changes can greatly reduce your chances of further disease progression. According to the Centers for Disease Control and Prevention, smoking increases the risk of PAD by two to six times. Stop smoking, receive treatment for diabetes, high blood pressure and cholesterol and increase exercise and physical activities.
