PUTNAM HOSPITAL CENTER

Putnam Hospital Center (PHC) opened in 1964 and is a 164-bed non-profit acute care hospital offering high quality inpatient and outpatient medical, surgical, psychiatric, obstetrical/gynecological and pediatric care as well as 24-hour emergency services. The wide array of services include the Birthing Center, the Orthopaedic Institute at Putnam Hospital Center, Wagner Cancer Pavilion with radiation therapy and genetics counseling, The Spine Institute at Putnam Hospital Center, Center for Vascular and Endovascular Therapy, Center for Thoracic Surgery, The Center for Wound Healing with hyperbaric oxygen therapy, intensive care, ambulatory surgery, imaging/ radiological services, blood management program, respiratory therapy, infusion center, hand and plastic surgery, robotic surgery, pain management, pulmonary medicine, nutrition services, urology, gastroenterology, ophthalmology, neurology, cardiology, sleep medicine, weight loss surgery program, rehabilitation services, and stroke program.

VASSAR BROTHERS MEDICAL CENTER

Vassar Brothers Medical Center (VBMC) is a 365-bed facility that has served New York’s Mid-Hudson Valley since 1887. Located in Poughkeepsie, VBMC has established centers of excellence in cardiac services, cancer care and women and children’s health services. As a regional medical center, Vassar houses the area’s first and only cardiothoracic surgery center between Westchester and Albany and the only Level III Neonatal Intensive Care Unit (NICU) in the region for premature, underweight and critically ill infants. Innovative procedures and services have been brought to the VBMC campus, including robotic orthopedic surgery, interventional neuroradiology and thoracic surgical oncology, negating the need to travel for this care. VBMC plans to build a 696,000-square-foot, seven-level inpatient pavilion with 264 private medical/ surgical patient rooms and 30 critical care rooms that will solidify its place as the destination of choice for patients in the region. Groundbreaking is scheduled for May 2016 and the first patient is expected to be cared for in the building in January of 2019. 2

Inpatient Services Many of the services provided by Health Quest Medical Practice are performed only in the hospital. The following information highlights some of these services. NEONATOLOGISTS The Mid-Hudson Valley’s only Level III Neonatal Intensive Care Unit (NICU), located at Vassar Brothers Medical Center, offers a host of state-of-the-art technologies and trained personnel to ensure the best outcome for extremely premature newborns and other infants with special medical needs. Neonatology is a subspecialty of pediatrics that specializes in the care of newborns. When a baby is prematurely born, or is an at-term newborn with critical medical issues, a neonatologist joins the team of hospital caregivers and they are available around the clock to provide the latest in diagnosis and treatment. Neonatologists work with other NICU staff to ensure the highest level of sophisticated care for the young patient’s lungs and airways, cardiovascular system, brain and nervous system, skin, nutritional needs, and overall health.

SURGICAL PHYSICIAN ASSISTANTS Each of our hospitals is committed to providing the best care possible for inpatients. Among the services that allow us to do so is our staff of Surgical Physician Assistants. Surgical Physician Assistants are specially trained in operative procedures and assist the surgeon in the operating room. In addition, Surgical PAs in our hospitals are on duty day and night, every day of the year, to monitor patients after surgery and ensure continuity of care. Every patient’s primary care physician, and other caregivers in and outside the hospital, work with the PAs as a patient-oriented team to deliver quality, cost-effective care for any issue that may arise.

HOSPITALISTS

Hospitalists, available at all three hospitals, are fully trained and board-certified medical doctors who specialize in the care of patients admitted to the hospital. With the focus of their practice on inpatient care, Hospitalists are attentive to the medical needs of each patient on a daily basis. If you are admitted for a stay in the hospital, your primary care physician will be in frequent contact with the Hospitalist, as will any other specialists you may be seeing. In addition to overseeing and evaluating your medical treatment, Hospitalists are available to answer questions, and will write prescriptions for any necessary medications upon discharge. Because Hospitalists do not see patients outside the hospital, your primary care provider once again is your care-managing doctor after discharge. If you do not have a primary care physician, a physician directory can be found online at HealthQuest.org to help select a provider.

INTENSIVISTS

Vassar Brothers Medical Center is proud to bring an additional group of medical specialists—Intensivists—to its team of inpatient caregivers for hospitalized patients. Intensivists are physicians with advanced training in the care of critically ill patients. Intensivists apply their knowledge and special skills to the needs of patients in the intensive care unit, as well as in other areas of the hospital as needed. Intensivists work closely with other doctors, nurses, pharmacists, case managers, nutritionists, and other caregivers to ensure the best critical care. With an around-the-clock presence at the hospital, Intensivists are always available to attend patients admitted from the emergency room, patients already in intensive care, or any other inpatient needing new or additional critical support. At Health Quest’s Division of Behavioral Health, patients will find access to an integrated, comprehensive range of recognized treatment approaches. Our team of experts will develop a personalized individual treatment plan which might include individual and group therapy, art therapy, medication management, mental health education, and psychotherapy. Team leader Dr. Yelena Yermak served as a chief clinical fellow in consultation psychiatry at Harvard Medical School’s Brigham and Women’s Hospital in Boston after completing her MD at Albany Medical College and postdoctoral residency at Harvard’s Longwood Residency Training Program. Dr. Yermak has been locally recognized as one of the “Top Docs” of the Hudson Valley for 2014 in Hudson Valley Magazine. Dr. John J. Wang completed his psychiatric residency at Mount Sinai School of Medicine and served as a clinical assistant professor 6

Dr. Hillary Harrison earned her MD from Northwestern University, and completed her residency at Hofstra North Shore LIJ School of Medicine at Zucker Hillside Hospital in Glen Oaks. Dr. Marylinn Markarian provides psychiatric evaluation and treatment services in our beautiful Livingston Street Poughkeepsie location, steps from Vassar Brothers Medical Center. She also provides psychiatric consultation at VBMC. Dr. Markarian is board-certified, completing her residency training in Psychiatry at NYU Medical Center, and a Fellowship in Addiction Psychiatry at Northwestern. sing a collaborative approach in a multidisciplinary breast center facility with evidence-based medicine is just the beginning of the story at the Dyson Center for Cancer Care. Our team of caring clinicians dedicated themselves to providing state-of-the-art care tailored to each individual patient and creating a personal treatment plan within the context of a supportive, focused, and healing-centered environment. Dr. Angela J. Keleher, Director of Breast Surgery, leads the development and advances of The Dyson Breast Center. In 2013, Dr. Keleher was one of only fourteen US surgeons to be named a Breast Cancer Champion by Yoplait® and the American Cancer Society. She has been locally recognized as one of the “Top Docs” of the Hudson Valley for 4 years in a row in Hudson Valley Magazine. Dr. Keleher is a Fellow of the American College of Surgeons, Board Certified by the American Board of Surgery and practiced in Pittsburgh before coming to Dyson Center for Cancer Care in 2007. Along with her partner Dr. Gregory Zanieski, they both surgically treat all breast diseases and specialize in cancer care. Dr. Gregory Zanieski has leading roles in clinical research and is the principal investigator in many National Breast Cancer trials at Vassar Brothers Medical Center. He has a special interest in taking care of locally advanced breast cancer patients. Dr. Zanieski completed his surgical oncology fellowship at

the University of Pittsburgh Medical Center. He is a Fellow of the American College of Surgeons and Board Certified by the American Board of Surgery. He joined the Surgical Breast Team in 2010. In addition to Dr. Keleher and Dr. Zanieski, there are three advanced practitioners in the Breast Practice. Sara Winterleitner, NP is a Board Certified Adult Nurse Practitioner specializing in Oncology who joined the practice in 2008. Sara is certified in Breast Cancer Care from the Oncology Nursing Society. She has been practicing in the Hudson Valley for over 25 years. She leads the High Risk Breast Center and has an active role in program development. She also participates with volunteering to support cancer patients in the community. Physician Assistant Alexa Kyle is a vital member of the Breast Surgery team. Alexa graduated from Philadelphia University’s Physician Assistant program in 2010 and is certified by the NCCPA. She is a certified Breast Patient Navigator. Alexa’s special interests include office procedures, high risk patients, and benign breast diseases. She joined the breast team in 2012. The newest addition to the Breast Surgery Division is Lisa Hartman, who joined the team in 2014. She is a Board Certified Registered Nurse Practitioner. Her special interests include continuum of care for the patient. She is involved from initial visit through their surgery and transitioning the patient to the survivorship program with long term follow up. r. Joseph Christiana always knew he wanted to excel in the office, the hospital, the lab, and the consulting room alike. Dr. Christiana earned his MD from New York Medical College and completed an internship and residency in internal medicine and a fellowship in cardiovascular disease at Westchester Medical Center. Dr. Ali Hammoud earned his MD at NYU. He then completed a residency in internal medicine at NYU, Bellevue Hospital and a fellowship in cardiovascular disease at Westchester Medical Center. Sabrina earned a BSN at Binghamton University, an MSN in nursing from Mount Saint Mary College, and completed internships in a wide variety of settings. Dr. Zakow earned his MD from New York University School of Medicine and completed both his Residency and a Fellowship in Cardiothoracic Surgery from NYU Medical Center/Bellvue Hospital Center. He has been with the Cardiothoracic Surgery Team since its inception at Vassar Brothers in 2000. Dr. Shahani joined the Cardiothoracic Surgery Team in 2006 and completed his Residency and Fellowship in Cardiothoracic Surgery at Mount Sinai Hospital in New York. He is a passionate patient advocate and was voted “Physician of the Year” at Mount Sinai Hospital in 2002. Complementing the thoracic team is the newest addition Cliff P. Connery, MD, FACS. Dr. Connery has been using the da Vinci® robotic surgical system since 2002 and is recognized for his skill in treating lung and esophageal cancers, mediastinal tumors, thymomas and patients with myasthenia gravis. pened in 2012, the Center for Healthy Aging is a comprehensive resource designed to meet the needs of older adults. A complete geriatric assessment including common elder care concerns such as falls, skin care, memory loss and management of multiple prescriptions is the first step in providing custom-tailored care in both inpatient and outpatient treatment. Among other services, the Center provides social work counseling for patients and caregivers experiencing anxiety, depression, bereavement, difficult life transitions, and other emotional concerns. Early in her career, Dr. Friedman, medical director of the Center for Healthy Aging, realized she had a special affinity for experienced citizens. Beyond simply finding their conversation delightful, she began to recognize gaps in the way their health care was being provided—needs that she was sure could be met in a better way. Dr. Friedman completed her BA in Psychology at Atlanta’s Emory University and went on to earn her MD at the University of Pittsburgh Medical School. She is a member of the American Board of Family Physicians and the American Geriatric Society. Dr. Friedman completed her residency and a fellowship in geriatrics at Thomas Jefferson University in Philadelphia. The Center for Health Aging also includes Allison Gould, LCSW-R. Allison is a licensed clinical social worker with a master’s degree in social work from New York University in New York City. She also has a postgraduate certificate in advanced clinical practice, a certificate in gerontology and extensive training in family therapy. Prior to her social work training, Allison received her undergraduate degree in sociology from Emory University in Atlanta, GA. For over a decade, she has provided social work services to individuals and families across the lifespan and is passionate about helping people live healthful and fulfilling lives. The division employs a multidisciplinary approach to treat pathologic conditions of the lung, esophagus, and chest wall, as well as tumors of the mediastinum and diseases of the diaphragm and pericardium. With more than twenty years of experience, Dr. Connery is highly skilled in the use of a 3-D high-definition vision system designed to increase precision and control in complex, minimally invasive surgeries. He also serves as the Medical Director for the Dyson Center for Cancer Care at Vassar Brothers Medical Center . Dr. Connery performs traditional open surgeries, thoracoscopies, lobectomies, Da Vinci robotic-assisted thoracic surgery, and other procedures. Dr. Connery also leads a multifaceted lung cancer screening program to detect lung cancer in earlier stages with advanced diagnostics. With a wide range of surgical expertise, his focus includes gastrointestinal , colorectal, esophageal and liver cancer, melanoma, rare and recurrent tumors, and sarcoma tumors, among others. Dr. Nitzkorski has performed the region’s first single incision laparoscopic colectomy as well as the first isolated limb infusion treatment for advanced melanoma. Dr. Nitzkorski is a Fellow of the American College of Surgeons and completed his MD at the University at Buffalo School of Medicine and Biomedical Sciences. He completed his surgery residency at St. Luke’s-Roosevelt Hospital in New York, where he served as chief resident.
