It’s both greeting and good-bye in today’s post-COVID culture. And yet, it’s no easy task. We must simultaneously juggle all the intricacies of life – work, home, family, finances – and still make the time to eat, sleep and exercise adequately. We do so to maintain or improve our physiques and attain a state of wellness or well-being that encompasses both mind and body. We do it to feel better and to enjoy the way we look and move in our own skin. It’s a lot. But what if wellness just seems like work because we’ve made it so? I wonder whether the secrets to health lie buried in everything we’ve forgotten since high school. One look at the determination and joy on the faces of Loudoun’s most successful athletes speaks volumes. In this wellness issue, we’ve assembled suggestions and examples of how to organize the essentials and plan for the future. Here are ways to connect mind, body and spirit and, through the use of technology, even do so in less time. Here are innovators trying to heal the very health care systems that make us so stressed when they fail us, and even tips on making our homes more like sanctuaries where we can enjoy peace and the company of those we love. Yet, I’m convinced we’re missing something vital here – something we each need to rediscover every waking day inside ourselves. We all have the power to make the choices that either enhance or detract from our wellbeing nearly every minute of every day. We just have to want it bad enough. “You’ve always had the power, my dear, you just had to learn it for yourself!” ~ Frank Baum, The Wizard of Oz

Turn some day into right now "Lisa, thank you so much for the opportunity to share with you our dream. When my husband and I began to entertain the thought of looking for a new home, we never thought that building our dream home would be the route we would decide to take. We had our eye on a particular neighborhood for some time, but all of the homes that popped up for sale had either been too small for our growing family, or way out of our price range. We wanted a special place, built from our hearts, for our daughters. Lisa was there every step of the way, through countless home tours, fielding many hysterical buyer calls, and later, countless builder meetings where her extensive knowledge shined. She was part therapist, part realtor, and every bit friend. This dream home is no longer just a dream, it’s our reality, thanks to Lisa. If these walls could talk, they’d play a soundtrack of tiny feet running up and down the hallways, late night giggles between sisters in their beds, and thousands upon thousands of apologies and I love you’s. If I could bottle up our joy and gratefulness, I would. Lisa absolutely nailed every detail of my Miracle on 31st Street home, on a few acres of beautiful horse land in wine country." - Tricia C.

AN EYE ON NUTRITION

O UR TOWN | HEALTH + W E LLNE S S
TIPS FOR KEEPING YOUR TEETH AND BODY HEALTHY.
Dr. Shohreh Sharif, D.D.S., FAAPD, FICD
Nutrition, coupled with diet, and exercise are keys to lifelong health. What we eat and drink greatly affects our mouths and entire bodies. The United States Surgeon General has stated that without attention to oral health you can’t be your healthiest self! We need to have good nutrition and a healthy diet to be able to have our best gums, teeth, and smiles. In today’s world, food is not just a source of nutrition – it is a part of our culture and society. It is important that we do our best to maintain a healthy diet. This means a diet that includes all nutrients in the right amounts. A healthy diet is based on variety, balance, and moderation. Nutrients have three basic functions in the body: • provide energy • regulate functions • contribute to its building structure Very few foods consist of just a single nutrient. In fact, one of the only ones is table sugar. It is exclusively a carbohydrate. Most are a combination of nutrients. Milk, as an example, contains carbohydrates, proteins, fats, water, vitamins, and minerals.
These nutrients are classified in six major categories: • carbohydrates (sugars, starches, and fibers) • proteins (animal or vegetable) • fats (animal or vegetable • vegetable liquid is preferred) • vitamins • minerals • water It is important to understand the difference between a calorie and a nutrient as well. Calories are measures of energy. Carbohydrates and proteins provide about the same amount of calories, gram for gram, whereas fats provide double the amount. Energy is vital to have, but excess energy is stored in the body as fat. Nutrients serve auxiliary functions, fats provide insulation, and proteins give the building block for tissues and muscles. My Plate is a program celebrating its 10-year anniversary that is helpful in making every bite count. Its personalized program allows you to track and better understand if what you are putting in your body is what you want to be putting in your body. Children and teenagers are constantly growing so their diet is even more important in ensuring healthy teeth and bones. A major risk for children is constant sugar exposure in the mouth, leaving teeth susceptible to breakdown and then to cavities. Sippy cups are a leading cause of constant contact of the teeth with sugars. In older children and teenagers sugary beverages such as juices, sodas, and sports drinks bring about these risks. Drinking plenty of water is always the best choice. Maintain a variety in food selection. Whole grains, fruits, vegetables, and variable protein sources comprise a balanced meal. Always keep in mind how important what we put in our bodies is for our oral and overall health. Restricting sweets to an occasional dessert helps keep sugar levels in control. Reducing snacking between meals also is a great change to make. Taking care of our body with good diet and nutrition will give us the best chances to have our most beautiful smile! SHE GAVE ME MY LIFE BACK Local clinic has a modern, medical solution to treat your Peripheral Neuropathy and is seeing incredible results!
“It started out as a normal outing. My wife and I had to run to the grocery store. We were driving down the road casually going over our shopping list when the car ahead of me started to slow down anticipating the yellow light. I started to move my foot from the gas to the brake as any normal person would do when they see brake lights in front of them but I couldn’t. I couldn’t feel my foot. The car kept moving forward and I just couldn’t get my foot on the brake. And CRUNCH! I finally came to a stop when I hit the car in front of me.”
This tragic story was shared with us by Dan S. an Ashburn resident who has peripheral neuropathy. And while no one was hurt in this accident, Dan S. had suffered almost everyday of his life with tingling and burning in his feet until numbness set it and he could no longer feel even the brake pedal beneath his foot. “The first stage is pain.” shares Rachal Lohr, Acupuncturist of FIREFLY Acupuncture & Wellness. “You feel burning, tingling, sharp pains, or you feel like you’re walking on tacks or marbles. This pain eventually subsides and the numbness sets in. Unfortunately the numbness brings with it a whole other host of problems.”
This was the case with Dan. “I said I wasn’t going to drive again. What if that had been a pedestrian?” It is terribly common that peripheral neuropathy and its debilitating symptoms interfere with a person’s ability to live their life. Dan was now reliant on his wife to drive him around, even the simple pleasure of cruising down to play golf or taking her out to dinner was outside his capabilities. And even more common, Dan’s general practitioner and several specialists told him there was nothing they could do other than prescribe him pills that would ease the pain of his neuropathy. That’s where Rachal Lohr and her staff at FIREFLY come in. “About 75% of our current patients come to us suffering from the same condition as Mr. Dan,” tells Rachal.
“They’re in constant pain from neuropathy and it prevents them from not only living their lives but more importantly, it prevents them from enjoying it. Depending on the severity of their nerve damage, we typically see tremendous progress in 3-4 months of treatment. I like to say we’re in the business of making your golden years golden.”
Visit www.FIREFLYAcuAndWellness.com to learn more and to take advantage of their New Patient Offer!
“I can’t lie,” confides Dan. “I was skeptical at first. The folks down at my pain center told me there was nothing that could be done and then there’s a doctor right here in Loudoun who tells me she can help. Turns out she was right! About three months after treatment I was able to confidently drive myself to my appointments! My wife and I celebrated by buying ourselves a new car! It’s hard to put into words how incredible this is, quite frankly [Rachal] gave me my life back.”
While FIREFLY specializes in acupuncture and it’s definitely part of their protocols in treating neuropathy, the real secret is in a more modern medical solution called ATP Resonance BioTherapy™. “This technology was originally developed by NASA to expedite healing and recovery” shares Terri, a Senior Patient Care Coordinator at the clinic. “It’s like watering a plant. ATP Resonance BioTherapy™ stimulates the blood vessels to grow back around the peripheral nerve and provide them the proper nutrients to heal and repair.” You can learn more about FIREFLY By visiting FIREFLYAcuAndWellness.com. If you’re ready to schedule a consultation call (703)263-2142 and do so quickly. FIREFLY is a very intimate clinic and the staff takes pride in their ability to take their time with each patient so they are very limited in their ability to take on new patients.

TRAINING 'BATTLE BUDDIES' FOR FORMER SOLDIERS
Meet Ashley, Service Dog in Training (SDiT). Ashley is a chocolate Labrador Retriever in the last of her two years of training with Veterans Moving Forward, a non-profit headquartered in Dulles, Virginia. The organization provides trained service dogs to veterans dealing with physical and/or mental health challenges at no cost to the veteran or their family. As a reminder of those who paid the ultimate price for their country, SDiT Ashley is named in honor and memory of Army First Lieutenant Ashley White. 1st Lt. White was killed during combat operations in Kandahar Province, Afghanistan on October 22, 2011 when the assault force she was supporting triggered an improvised explosive device. Ashley was posthumously awarded the Bronze Star, the Purple Heart, and the Combat Action Badge.

Veterans Moving Forward acquires puppies from selected local breeders with whom it has developed very special relationships over the years, and matches its trained animals with vets largely on the basis of temperament. They are then trained by Master Trainer Katie Poulson; two dozen already are serving in the field. When trained, SDiT Ashley will become a special “Battle Buddy” for a deserving veteran to provide support and assistance including obedience training, retrieving medicines for the veteran, and learning how to ease their companion’s anxiety when they encounter a mental challenge. Ashley is in the process of being placed with her veteran, living in Montana, a process which takes 8 months. Veterans Moving Forward relies entirely on donations to acquire, raise and train its dogs for deployment as canine companions. They have a critical need for volunteers to support their efforts in fundraising, marketing, puppy sitting, puppy raising and puppy walkers. Hours are flexible and can be on an as-needed basis. Also needed are administrative, IT or general office volunteers. Veterans or those with experience working with veterans and of course experienced dog trainers and handlers are also welcome! Donations, and particularly monthly commitments, can have a lasting impact on the lives of veterans across the country. Veterans Moving Forward, 44225 Mercure Circle Ste 130 Dulles, Virginia, 20166. A Basement Remodeled for Family Fun THIS AWARD WINNING PROJECT BY MICHAEL NASH DESIGN BUILD KEEPS THE FAMILY HAPPY AT HOME
ARTICLE BY HANN LIVINGSTON Owners of a single family home in Vienna tired of their basic builder-grade, unfinished basement. The couple had a desire for a much-needed entertaining space, an in-home office, an au-pair suite, a renovated bathroom, a large scale bar/kitchenette area, and a dry Sauna. They brought in Michael Nash Design Build to create their dream environment. “Working with unfinished space is always fun for us,” says Sonny Nazemian, master designer/remodeler at Michael Nash Design Build. “We basically have a blank slate to create exactly what the homeowner has envisioned.”
The bar is perfect for entertaining.

The new open atmosphere.
That being said, to make adequate floor space and an open living area, the team had to relocate the furnace and hot water heater to a new utility room and add beams to eliminate steel support structures. Then it was off to the races. The main living area is the highlight of the space. Large-scale porcelain tile was laid throughout and a large kitchenette/bar area was placed in one corner that features an espresso machine, a 36” refrigerator, microwave, sink and dishwasher. A bar-stool seating area under three beautiful antique pendant lights looks toward glass-fronted cabinetry that displays liquor for entertainment. Another highlight feature is the see through fireplace mounted in the stone-veneer wall separating the bar area from the sitting area where the entire family and guests can watch the large screen
television or play arcade games and foosball. The children call it their new “airy and fun space.” “We decided to open up the single doorway to the outside back patio with a double French door,” says Sonny. “This coupled with the indoor direct and indirect lighting and the natural light colors we chose for the floor and golden honey ledgestone accents on the walls and kitchen backsplash creates an extremely bright area that doesn’t seem at all like a basement.” The space to the left of the staircase coming from main level was turned into an office, and a new bedroom and bath was added as an au pair suite. The bathroom features a walk-in shower with a pebble floor and a stone and dark-cafe vanity. One of the most unique features of this remodel is the custom-made sauna that was fit into the wall behind the bathroom, so the homeowners can relax after a hard work day.

Supercharge Your Workout Reclaim Your L ife
Daphne Mustafa Channels Her Inner Warrior in a WB20 EMS Suit
ARTICLE BY MELINDA GIPSON I PHOTOGRAPHY BY MARTY SHOUP

H OW TO | FITN ESS
WB20 Intensifies Your Workout, Tones and Strengthens Your Body
The red lightning bolt that splits the logo for WB20 is a fitting emblem for this upstart workout studio in Ashburn. Zap! It first symbolizes how personal trainers leverage electromagnetic muscle stimulation (EMS) to supercharge your muscles, delivering the equivalent of 90 minutes of exercise in just 20 minutes. Then, Kapow!, it’s a diagram for how quickly the family-owned business took off, well, like lightning... in a pandemic, no less. Daphne Mustafa, co-owner of the family-owned business incorporated as Be Fit Technologies, LLC, says it was exactly the right time for Whole Body Twenty (WB20), “because people understand that fitness is not a luxury anymore. It’s a necessity and needs to be a part of everyday life.” Getting to the gym is hard. It’s not just the 90 minutes of exercise on site, but the travel and changing time that can rob your day of at least two and a half hours. Daphne knows this from experience because her husband and partner in the business, Bernard Mustafa, is a self-professed “gym rat.” So yes, for some people, grueling hours in a weight room is nirvana. But if what you want are the results minus the time sink, WB20 is your gateway to feeling and looking better.
“What we offer is a 20-minute workout, twice a week, that delivers the same results. We can do it in your home or you can come to the studio. Either way you’re saving at least an hour and a half,” Daphne says. Many customers in a 10- to 15-mile radius of the studio already take advantage of home-based sessions for a number of reasons. Whether in home or in-studio, the workout is supremely suited to help keep in shape or restore fitness following injury. Daphne’s stepson Bernard (named for his father), says that following his motorcycle accident, EMS-based training allowed him to strengthen his muscles during rehab when traditional strength training would have put too much pressure on his back and limbs. The elder Bernard, a veteran, echoes, “When you get older, your joints don’t quite work the way they used to. I have a shoulder I had operated on and bad knees. I gravitated towards EMS technology in part because it helped me to overcome issues with my knees and my shoulder injury.” “There are no magic bullets,” he adds. “But if you’re determined to do to improve your health and wellness, this is the efficient and exciting way to do it.” CONTINUED > JANUARY 2 0 2 2 | C I TYL I FE STYL E . • WB20 offers a variety of affordable packages of once or twice weekly workouts that fit your lifestyle. • When you arrive, you first step on a Fit3D scanner, which provides an extremely (depressingly) accurate picture of your physique, height, weight, body shape rating, measurements, balance, body fat percentage, posture and fitness rating. Each of these measurements are also shown as where you are relative to lean, ideal, average or “needs improvement” ratings (derived from other Fit3D users of your gender and age) so you can gage just how far you have to go in your fitness journey. • After you and your personal trainer discuss your goals for your workouts, you change into a base-layer consisting of a black t-shirt and shorts. The stretchy, woven fabric covers all the muscle groups you’ll be working and aids in connectivity of the electrodes. • While you’re putting this on, your trainer is lightly spraying your EMS suit with water, also to help the electric impulses reach deep muscle groups. He or she then helps strap it on so it’s snug but not too tight. • The trainer then puts a drop or two of peppermint essential oil on your hands and asks you to take a few deep breaths inhaling the scent. It’s not only relaxing, it serves to take away any lingering jitters about the slight prickling sensations to come. • After a 5-minute warm-up, you then engage in the same series of squats, lifts, curls and push-ups familiar to anyone who has ever worked out, but in short intervals of around 10 repetitions, tailored to your goals and abilities and enhanced by your personal trainer with whatever degree of stimulation is comfortable. While the workout doesn’t seem that intense, afterwards you may experience the same kind of rubbery-legged sensation you might feel following more strenuous muscle exhaustion. • Exercise is followed by a period of low stimulation relaxation that feels as good as a massage, except that it doesn’t reach those tense neck muscles. In short... ahhh! Worth the whole trip. You should drink plenty of water following the workout and prepare for some stiffness a couple of days afterwards. It’s one way you know it’s working!
Daphne and Family Accept Award for Health & Wellness Business of the Year
So, how does it work? It’s all about the suit. Electrodes in the EMS suit, sized to your frame and then shaped to your body with Velcro flaps, mimic impulses that your central nervous system uses to contract your muscles. The intensity of the stimulation is controlled and directed by a personal trainer using a tablet and bluetooth to produce a customized workout. Because the impulses activate 95% of your muscles, this accelerates your metabolism, helping to burn more fat. Leesburg Lifestyle publisher, Hann Livingston, learned this first hand when he dropped 20 pounds and added muscle with just six weeks of twice weekly sessions. Testimonials like his and clients whose back and joint pain vanished or whose stomach and ab muscles tightened and toned drove rave reviews and rapid adoption. Daphne credited “word of mouth” as the primary reason for the business’ rapid growth in her speech accepting the Loudoun Chamber of Commerce’s Health & Wellness Business of the Year honors in October.
Of course, there’s more to their success in a crowded field than a great product. The elder Bernard built and then sold a successful Microsoft accounting technology government contracting business before looking into technology that could help him stay fit despite the aches and pains of age and his war-fighting injuries. He discovered that EMS technology for fitness training is common in Europe and even used by athletes like Usain Bolt. Using the same skillset that helped him build and sell Pleasant Valley Business Solutions (PVBS) to XTIVIA Inc., he combed the continent for the best suppliers of the technology and training. Daphne’s background in health care and administration made her a natural to lead the business. The value proposition, that WB20 can design an effective workout for people of any age or level of fitness – especially for those with special needs and limited time – is certainly compelling. But it’s also the couple’s track-record of community service that caused so many in the area to embrace their new business. Bernard senior’s years of community service on various committees and specifically his volunteer work with veterans earned him an Outstanding Volunteer Award in 2020. He’s been board chairman for the Loudoun County Economic Development Authority for nearly a decade, helping other local entrepreneurs build successful businesses. Daphne’s work earned her a presidential commendation for her community service during COVID. The secret sauce seems to be that building WB20 together as a family makes everyone else feel like family too. What’s it like launching another career with your family when you might as easily retire? “It’s gratifying I can tell you that,” says Daphne. “It’s great to work so closely with the people I love. And after two years of doing this, living together, working together and spending virtually all our time together because of COVID, we still love each other! We’re growing a lot together and learning a lot because this is all new for each of us.” Website: https://www.wb20.io/. FINALLY! CLUB PILATES OPENS IN LEESBURG ARTICLE AND PHOTOGRAPHY BY MELINDA GIPSON
Nearly four years into his rollout of studios in Ashburn and Potomac Falls, and after capturing a “Best Health & Wellness Business” award from the Loudoun Chamber of Commerce, Ryan Ask’s Club Pilates has at last opened its doors in Leesburg at the Marketplace in Potomac Station. The studio is welcoming newcomers with a free, 30-minute intro class and a 20% discount for the first three months on all its classes. We’re huge fans of Ryan, not only because he’s supported our publication from day one, but because his studios’ workouts serve to fill a critical niche in Loudoun County’s fitness environment with their focus on balancing the mind, body and spirit. The studio’s main apparatus is a “Reformer,” designed to provide resistance training and support while you lie, kneel, stand or sit on the sliding bench to perform various exercises. TRX® straps, Pilates rings, bar, balance balls and bands combine to provide a full range of stretching, cardio and strengthening workouts. In anticipation of the new studio location, we stopped into the Ashburn studio to tap trainer Carlota Tulloch’s expertise in distinguishing among the eight different classes, each with levels 1, 1.5, 2 and 2.5. The classes are small and the exercises – some 500 of them – may each be modified to each person’s ability and needs. There’s just so much to choose from that we thought a short primer would be useful. In short, a picture’s worth 1,000 words, so with the help of Rex Larrabee, the Ashburn sales manager and “model,” we put together this brief tutorial.

Rex Demonstrates an Intro Class Exercise
Excerpt from the CP Reformer Flow Class

F E AT U R E | FITNESS
First, just a word about Carlota (https://bit.ly/CCTulloch). After 30 years in hospitality, she injured her back snowmobiling a few years ago. Her doctor advised low-impact exercise like swimming or Pilates and because she found swimming boring, she started practicing Pilates. Within six months, she says her back pain was gone and she was hooked. The training even helped her strengthen her knees so she was able to return to running without pain. After belonging for two years, she decided to quit the corporate world and become an instructor. It usually takes a year to become an instructor, but because she was able to do the training during COVID, it took her only four months. To illustrate the intro class, which offers an overview of all the various class formats, Rex places a rubber ball between his knees to create thigh muscle tension as he straightens then bends his legs. In an excerpt from the Club Pilates (CP) Reformer Flow class, Rex demonstrated a chest expansion exercise sitting on a box on the reformer. As his arms press back against the tension, he squeezes his shoulder blades, keeping his shoulders back, chest “proud.” In a second exercise, he leans back with one leg in a tabletop position and does bicep curls to increase arm strength.

CP Center Class Offers a New Blend of Barre

CP Cardio Sculpt makes use of the Reformer “jump board” to do low impact jumping, straight legged and with alternating legs up. To illustrate the CP Center and Balance class, Rex next stands on a Bosu ball at an adjacent bar, lifting one leg back and extended while squeezing the glutes. Center + Balance puts the emphasis on both posture and strength, so it is also illustrated here with weight-enhanced bicep curls on a Bosu ball. CP Control makes the most use of barre-based moves in a workout that primarily tones legs and glutes. Rex illustrates by performing squats on his toes using the barre for balance.

F E AT U R E | FITNESS
The CP Suspend class makes use of TRX® straps, so Rex does a series of squats using the straps for balance. The straps also are useful for stretching out tight back muscles. And finally, CP FIT is a blend of cardio and Pilates-based strength training. The CP Restore class is best alternated with other workouts to stretch and lengthen tight muscles. One sample exercise involves kneeling with hands above the head on the Bosu ball, allowing the back and neck to stretch along its contours. Classic muscle rollers are used for deep tissue massage and muscle recovery.

HOW A COMMITMENT TO PATIENT CARE DRIVES MOSAIC THEORY MD TO HEAL THE SYSTEM

Drs. Anusha Khan and Sheri Kochendoerfer, MD, with their 'Community Wall'

If you’ve found yourself frustrated with today’s health care system, imagine how the doctors feel! Primary care physicians in particular are drawn to healing people as a calling. That is certainly true of both Anusha Khan, MD and Sheri Kochendoerfer, MD, MT-ASCP, co-founders of Mosaic Theory MD, an innovative new medical practice in Sterling. It is readily apparent that what both women enjoy most in life is taking care of people – helping them get and stay well. Equally evident is their antipathy toward today’s traditional health care system, which they say minimizes patient care and leaves patients holding the bill. As with many innovators, Mosaic Theory’s founders solidified their determination to change their approach to their profession based on personal experience. When their medical training took them to the island of Dominica, they were plunged into a situation where they could work directly to heal people who were desperate for medical assistance in an environment that was completely devoid of bureaucratic red tape and health insurance “hoops.” The freedom each experienced in working directly with patients to diagnose their ailments and make medical decisions without outside influences, was life-changing. Both say that the autonomy they experienced in providing personalized, relationship-based medical care to their Caribbean patients was exactly what led them to become doctors in the first place. Imagine their delight when they discovered that other doctors, similarly motivated, are leading an innovative, grassroots movement called Direct Primary Care. Numbering more than 1,000 nationwide, DPC practices are designed to refocus on comprehensive patient care vs. a pay-for-services model rationed by health insurance. Most belong to the Direct Primary Care Alliance (https://dpcalliance.org/), which organizes an annual conference and helps to educate the public about the differences between Direct Primary Care and the model to which most people have become accustomed. “We didn’t come up with the DPC model all on our own; it was created by doctors so they can focus on patients and give them more time,” Dr. Khan said. As she defines Direct Primary Care, “We’re essentially giving concierge level care but making it affordable.” Instead of signing up to a complex and limiting health insurance plan, Mosaic Theory patients sign up for a monthly “membership” in the practice. For something on the order of $149/month, patients can count on same- or next-day doctor appointments of 30-90 minutes during which time they command the full attention of their physician. In addition to the usual “sick” visit, the doctors offer physician supervised weight management programs, Regenerative Medicine injections for joint pain, aesthetic procedures to treat acne and other skin issues, medical marijuana, hormone therapy, and IV vitamin therapy, all designed to help the body heal itself. “Anyone who wants a true doctor-patient relationship where we can focus on preventative care as well as dealing with

any acute problems, and do that affordably,” is welcome, Dr. Khan says, but our advice would be to act without delay. The partners can provide their patients with a higher level of attention because each physician is voluntarily capping her practice at 500-700 patients each, depending on the acuity of their health concerns. Should the practice grow to outstrip these self-imposed limits, they’ll have to hire more doctors who share their philosophy. Already, they’ve grown so quickly since opening their doors last year that they’re bringing on a nutritionist. Advances in medical technology have made it possible for a myriad of labs, ultrasound and other diagnostic procedures to be done in-house as part of each patient’s membership. Outsourced services like X-rays, mammograms and hospital-based surgeries may either be paid for out-of-pocket or be covered along with major accidents by a less expensive health insurance plan with a high deductible. If hospital treatment is called for, both doctors are committed to accompany each of their patients every step of the way, monitoring and explaining any and all procedures. Not being bogged down in a bureaucracy where multiple levels of approvals would be required to purchase the latest technology means that the partner can discuss investments in technology that best suit the needs of their patients. Their EKG monitor is
42
FAM ILY CA R E | HEALTH + W E LLNE S S
keyboard sized, using Bluetooth technology instead of adhesives, and their ultrasound is literally called a “butterfly” because it’s not much bigger than a human hand. That’s important because more often than not a diagnosis can be delivered and a treatment plan discussed all in the same visit! What is most compelling about such a system is the belief both Drs. Khan and Kochendoerfer share that being able to spend more time with their patients and being attentive to their overall wellbeing helps immeasurably in diagnosing illnesses and prescribing preventative care. “We have all this training, all of this expertise, but in a traditional practice, we wouldn’t be able to use our full skill set just because of time. Often, I would be forced to send patients off to a specialist because I didn’t have time to take care of them,” Dr. Khan says. What makes it all work is training, transparency and making care more accessible. Both Drs. Khan and Kochendoerfer are internists, and so are attuned to how all the body’s organs work together when functioning properly, as well as what could be wrong when something is amiss. Both have previous careers as skilled diagnostic lab scientists. Dr. Kochendoerfer’s background includes a focus on mental health and how patients’ psychological health may impact their physical well-being.

Physical ailments are approached using all the usual diagnostic tools, but are bolstered by listening to patients the doctors have come to know well. Transparency comes by way of documenting what each procedure and treatment costs, in contrast to tiered packages offered by today’s health insurance plans. Dr. Khan explains, “You may say you have Cadillac-level insurance, but how good is it if you can’t see a doctor when you need to or spend more than 5-7 minutes with them?” she asks. “You may be paying $500/month for insurance to cover your lab work that actually costs $5. Instead, many procedures are included in our membership, and our patients pay the wholesale cost of outgoing labs and imaging. If you have insurance and want to use it, great – you can use it for those ancillary services, but the meat and potatoes of this practice is the doctor-patient relationship. Regardless of your coverage status, you have a concierge-level doctor here that is affordable, and that’s why you’d choose us.” “We make patients our real focus and take out the middlemen and all the red tape and nebulous prices in the system,” Dr. Khan adds. “We are transparent about what things cost and we are as inclusive as possible so that, other than the membership fee, you don’t have to pay out of pocket. If you do have to pay, it’s literally just a few dollars at a time rather than hundreds of thousands of dollars in surprise bills.” Accessibility is guaranteed by membership, regardless of a patient’s health insurance status. To be responsive to patients’ urgent need, each doctor has seen patients after hours, and coordinated visits to the ER if they’re absolutely necessary. “That’s what we mean by making care accessible, no matter the time or place,” said Dr. Khan. Because both her and her partner’s patients can always reach them by text, phone or email, her daily calendar is only booked with patients who need to see her in person. That makes for much more effective care than seeing 30 patients a day. “When you get home after a day like that, you have to ask yourself, ‘did I really help 30 patients, or did I just put them on a conveyor belt?” says Dr. Khan. Before establishing Mosaic Theory, “it felt like that episode of ‘I Love Lucy’ where’s she’s forced to put pieces of chocolate in boxes faster and faster until she admits defeat. It feels like the current insurance-dependent system will speed up faster and faster until it crashes. At that point we will be in the perfect position to build it all back up the right way.” Regardless, she adds, “I’d rather provide excellent care to 500 people than poor care to 3,000.” For example, one morning a Mosaic Theory patient in assisted living called to report he had been injured. Dr. Kochendoerfer was able to go examine him at the
44
FAM ILY CA R E | HEALTH + W E LLNE S S
patient’s facility, get imaging and medications ordered and coordinate and spend time with family to keep them in the loop on the patient’s care. “In a different system, I would be lucky if I could see them in a couple of weeks or might say, ‘if it’s that bad, go to the emergency room.’ That’s why we love this practice. It lets us be the kind of doctors that we want to be and that we’re capable of being,” Dr. Kochendoerfer said. Internet accessibility to telemedicine also allowed the doctors to help one of their patients who had contracted COVID while studying in Prague. The patient had recovered to the point where she was symptom free, but was barred from travel because she couldn’t prove that she was no longer contagious. “Because I had an existing relationship with the patient and I know what her baseline is, I was able to clear her and fill out all the appropriate forms to bring her safely back into the country,” Dr. Khan said, “And that is priceless.” In the early days of the pandemic, the doctors conducted rapid PCR COVID tests as a service to the community, and gained many of their first members as a result of just being able to address the myriad questions people weren’t getting answered by their former physicians. Looking back at why the partners chose to launch their practice during a health crisis, neither can point to any single thing that caused them to put Direct Primary Care principles to work in their own practice. There wasn’t one straw that broke the camel’s back; Dr. Khan says it was more like “death by 1,000 cuts,” that led her and her partner to create Mosaic Theory. But they could well be at the forefront of a much bigger exodus. “Doctors are human too, and we went into medicine because we wanted to use the gifts God has given us to help people. The current system would rather we be glorified data entry clerks, clicking on screens and filling out paperwork. That’s not patient care.” “For us, it’s all about value and sustainability. Within the DPC model, we are able to provide tremendous value in care as well as price transparency. The basis of our model is to make care as affordable and accessible as possible. It’s about making care, patient-centered again.” When not caring for patients, the doctors may be found giving free blood pressure tests at your local farmer’s market, helping to harvest at JK Community Farm, or offering discounted pregnancy care at a local charity pregnancy center. Eventually the partners hope to be able to donate 10% of their practice free of charge. 1990's: White Zinfandel
Out With the Old and In With the New! A WIne Resolution for the New Year ARTICLE BY JIM MAGNER
Horton's Wine Library
46
H AP PY H OU R | FOOD + BE VE RAGE
1980's:
The Mountain Winery, formerly the Paul Masson Mountain Winery, is a winery in Saratoga, California, USA. It was founded by Paul Masson.
Our association with fermented grape juice stretches so far back into pre-history that our brightest paleo-archeologists can only guess at where and when humans first started making it on purpose. Even so, there is always a new trend, a new taste, or a new method to keep the wine industry fresh. A few years ago, big, bold reds with high tannins and strong alcohol contents were all the rage, but not anymore. Now it’s all about lighter wines with fruitier flavors and less alcohol. Out with the old, and in with the new! Or, does it just seem that way because people like me who talk and write about wine need to have something new to discuss? Does wine really change? For a little new year’s fun, I decided to go back in time to look at some wines of yesteryear that were near and dear to the hipster parvenus of their day and see how they hold up, now that they have been relegated to the “scoffed and sneered at” bin. By the decades, I revisited my own wine timeline to see if I still enjoy the stuff that impressed me back when I was first discovering the beverage. 1970’s: Blue Nun. Blue Nun burst onto the U.S. market in the early 1970’s and became a huge hit. It was exotic (i.e. German, not French or Italian), easy drinking and cool. It was what people who knew wine ordered at fancy restaurants. Today, if you were to show up at a dinner party with a bottle of B.N., the host might giggle and hope that you are being ironic. But how is it, really? Actually, it is still very drinkable. It’s not very cool or fashionable, and it’s not going to win any blind taste tests. After opening a bottle, I sat down to watch a hockey game. Then I went back for a second glass. I’ve hated wines that were a lot more expensive. Verdict: Not terrible.
1980’s: Orson Wells sat in front of a fire in a tuxedo and told us that Paul Masson would “sell no wine before it’s time,” and we are still waiting for him to sell us that one instead of whatever this other stuff is. 1990’s: White Zinfandel; or “White Zin” was in with those sporting argyle sweater vests and driving a Peugeot in the early 1990’s. Zinfandel was everywhere playing the role of California’s answer to Bordeaux. It was all any wine snobs could talk about and de rigueur for ladies at high class garden parties everywhere. How does it hold up? In a word: “Blech!” I hated it in the 1990s and I hate it now. It is sugary and flavorless, entirely lacking in substance. 2000’s: Upon settling down in Virginia, I usually drank beer and bourbon before I’d reach for a glass of wine. Then my father gave me a book about Norton, a wine grape that had been developed right here in the Commonwealth in the 1800’s and had saved the worldwide wine industry by being resistant to phylloxera. Horton, a winery near Charlottesville, rediscovered the grape and was making wine with it. I tried it and, at just $11 a bottle, it was the first wine that I really liked. I credit Horton Norton and its back-story for launching my lifelong interest in wine, not Châteauneuf-du-Pape or Alsace Reisling or even Grand Cru Champagne. Having spent the last couple of decades exploring great wines from around the globe, I wondered: how does this humble Virginia varietal hold up today? It doesn’t have a lot of depth or character, but it tastes good and you can drink it, and isn’t that all you really need out of a bottle of wine that costs less than lunch at a fast-food restaurant? Coppola Claret
“My advice for this next decade of wonderful wine experiences may surprise you. Resist peer pressure. Neither be put off of a wine that you enjoy simply because it has gone out of fashion, nor pretend to like something hip and new that you don’t care for simply for appearances.” 2010’s: The previous decade marked the opening of Francis Ford Coppola Winery and the beginning of mass-marketed reds under the Coppola label. You can send me all of the hate mail you want, but I never liked California Cabernet Sauvignons. Coppola Claret is a California Cab Savbased red blend that is mass produced. It should be terrible, taste jammy and be cloyingly fruity. But it is none of those things and, when I first tried it, I was attracted to its funky gold netting around the bottle. I liked it and I still like it. It is, in fact, rich and smooth and as much like a reasonably priced French Bordeaux as you will find from California.

2020’s: Wine preferences, like music preferences, have a way of marking your frame of mind and memorable events over the years. My advice for this next decade of wonderful wine experiences may surprise you. Resist peer pressure. Neither be put off of a wine that you enjoy simply because it has gone out of fashion, nor pretend to like something hip and new that you don’t care for simply for appearances. There is not enough time in one person’s life to waste drinking unpleasant wine just to please others.

Making Your Home More Mindful ARTICLE BY DR. SUZANNE NIXON
With the fast-moving pace of the holidays behind us, our minds and bodies naturally crave down time and our souls long for a place to just be. That place is your home. Homes are places of belonging and connection where safety and love reside, where peace can be found, a sense of relaxation restored and where nourishment abounds. The pandemic taught us just how vital the structure and energy of our homes are in shaping and fortifying our well-being. This awareness should turn our attention towards creating a more Zen-like, mindful living environment. Zen is a Buddhist word that simply defined means meditation. Meditation is the practice of focused concentration based in moment-to-moment awareness and, according to tradition, a pathway towards goodness, harmony and ultimately, enlightenment. Zen philosophy honors the sacredness and interconnection of all living things and holds reverence for nature. When humans flow in harmony with nature, every aspect of our lives, our health, our relationships and our homes will prosper with goodness and well-being. Creating a Zen-like home environment shouldn’t be daunting. The process is as important as the product. Begin by implementing the main principles of mindfulness: • Set your intentions and stay aware of what you want. • Engage in the process moment to moment. • Make decisions thoughtfully and deliberately. • Be mindful of your budget and the time it takes to recreate. • Respectfully honor all family opinions and voices.

To simplify the process in creating a calmer, more peaceful, nourishing home, use these foundational tenants as design guideposts: Ambience, Spaciousness, Flow and Nature. Zen is about being and flowing, not thinking and doing. Ambience comes with a keen awareness of lighting, color, sound, smell and simple beauty. Allow natural lighting to seep in through the windows, infusing serenity and beauty throughout a room. Peaceful paint shades include blue, violet, green, gray, tan and white. Furniture in these hues is warm and welcoming. Be mindful of sound. The constant ringing and dinging of cell phones, loud music and the high volume and harsh tone of speech can stimulate and aggravate the brain and nervous system. Cultivate a positive, peaceful mood state by establishing boundaries for sound, cell phones, music and in conversation. Spaciousness is a feeling of openness and freedom. It invites breath and movement. Notice your experience as you stand at your doorway or sit in your family room. Do you feel open and free, or closed and restricted? Decluttering and organizing your space are the first steps towards creating a feeling of spaciousness. A Zen-like home is abundant with nature. The greenery of plants, small potted trees, smooth tumbled stones, coarse rocks, fresh flowers and water fountains bring life energy into your home. Nature is a healing force. Connecting with, and being in nature, refreshes your mind, restores your body and soothes your soul. The energy, vibration and pulse of your home is ever changing. You can shift the energy and create a mindful, meditative home to nourish you, your family and friends for a lifetime.

For many, the new year is a time of renewal and reinvention, which makes it the perfect time to optimize your personal finances for 2022 and beyond. What financial, business, or life priorities do you need to address in the new year? Here are some suggestions: Review your financial plan. Ensure that it is still reflective of your personal goals. A financial plan is a living document, and as such, it should be revisited and adjusted regularly. Having a plan in place will make it easier for you to track your progress and will help you set specific short- and long-term goals with clear strategies on how to achieve those goals. Don’t have a financial plan yet? The team at Kendall Capital can help with that! Create a spending plan. Notice I used the term “spending plan” —not “budget.” Budgets can feel restrictive, complicated, and rigid which is why people dread it, and then struggle with sticking to that process. Instead, create a spending plan to use as a roadmap for how you’ll spend your money, focusing on paying yourself first. Treat contributions to your savings goals and retirement accounts like any other household bill. Once you’ve worked out how much of your income you’ll use to “pay yourself” and how much to direct to non-discretionary expenses like housing and utilities, you can feel empowered to freely spend the rest on whatever you like! Double check your contribution to your retirement plan at work. Periodically, the IRS increases the maximum allowable annual contribution to employer retirement plans, so New Year’s is a great time to double check that you’re making the most of your plan. For 2022, the 401(k)-contribution limit has been increased to $20,500, though the catch-up contribution for those over 50 remains the same for this year, at $6,500. Did you forget to fund your IRA or HSA last year? You have until April 15, 2022 to make a “prior year” contribution to a Traditional IRA, Roth IRA, or Health Savings Account (HSA) for 2021. You can stash away
up to $6,000 in an IRA for 2021, $7,000 for those over 50. The 2021 HSA contribution limit is $3,600 for single coverage or $7,200 for family coverage, plus an extra $1,000 if you’re over 55. Spend down your Flexible Spending Account if your employer offers a grace period to use funds from the 2021 plan year. Typically that expires on March 15th. Check out the list of eligible medical expenses at: https://fsastore.com/fsa-eligibility-list.aspx Check on your credit report. You can get a free copy of your credit report from www.annualcreditreport. com and regularly monitor new activity and alerts on your credit through Credit Karma. They also provide helpful tips for building credit, but they’re funded by affiliate links for credit card companies, so be wary! Review your insurance policies to make sure you aren’t over- or under-insured. Check all your policies—homeowner’s, auto, disability, life, and longterm care insurance. Are the limits adequate? Should the deductibles be raised? Is there a less expensive policy with similar coverage? Are you taking advantage of all the discounts offered to you by your insurance providers? Websites such as www.insurance. com and www.policygenius.com provide quotes from several insurers so you can compare and find the best coverage for the best price. Hire a fee-only fiduciary advisor. If you are struggling with questions regarding your current or future financial situation, you are not alone. Having a trustworthy professional in your corner to help guide you as you navigate financial decisions can help you reduce stress, build confidence, and feel empowered to spend your money on the things that truly enrich your life. At Kendall Capital, we help Middle-Class Millionaires® put their money to work for them instead of the other way around. Give us a call today and let us help you make 2022 the beginning of an even brighter future for you and yours.

Mekhla Salimani and her work
Mekhla Salimani: Gifted, and Giving ARTICLE AND PHOTOGRAPHY BY MELINDA GIPSON
Though she’s been painting her whole life, it was the pandemic that took Mekhla Salimani’s artwork to the next level. “My kids are a little grown so I started giving more time to what makes me happy and I think I needed it,” she told us. “Since April of last year, I’ve painted every single day.” One of 33 artists whose work graced the Third Annual Loudoun Cares Art Auction in October, Mekhla was one of handful of newcomers to the event, and the only one for whom it marked a debut. Besides the bold brushstrokes in pure colors, what distinguishes her work is that she gives it all away!

“My thought process there was, I have a full-time job. I don’t want anything else where money becomes a thing that I have to think about or stress about how much I’m going to sell. So, the plan was, I’ll sell it and give what I make to charity. I think that’s what works for me.” Currently, her proceeds go to support the Tin Cup Fund in her Loudoun neighborhood. An auction organizer saw her hand-painted cards at a farmer’s market and invited her to enter the show. For work, she’s a program manager and coach, so perhaps that helps her both see and paint her subjects with such clarity. Look for more of her work in the Ashburn Public Library. “Painting gives me joy, so I’m just following my path, and I’ll take whatever comes,” she said.

When it comes to buying and selling homes virtually, “Rely on Pam”! Woodland $1,000,000 $1,053,500 105% After retiring further south, we decided to sell our River Creek home after leaving it on the
rental market for over ten years. Selling an older home in a high-end neighborhood, with a tenant in place, with $150k of needed updates, long-distance and during COVID-19 pandemic—this could have been the ultimate real-estate horror story. Instead, it was one
of the easiest, smoothest house sales I’ve ever experienced. Pam handled everything and quickly sold the house for more than the asking price. Pam is the most professional and capable agent that I’ve ever worked with. —Former River Creek Residents
