BUSTING WIDE-AWAKE SURGERY MYTHS with Dr. William Melton
Orthopaedic Surgeon - Hughston Clinic Orthopaedics
D
r. William Melton, a surgeon at Hughston Clinic Orthopaedics, specializes in both surgical and nonsurgical treatment of fractures, nerve and tendon injuries, arthritis, and more affecting the hands, wrists, and elbows. Read on as Dr. Melton busts some common myths about wide-awake surgery as an alternative to surgery under general anesthesia. Myth 1: There is still discomfort during wide-awake surgery despite the numbing procedures. MOSTLY FALSE Dr. Melton: There can be occasional discomfort associated with pressure, although there should be no sharp pain. It is similar to a dentist using a drill in the mouth: the pressure from the drill can be felt, but there is no sharp pain from the drill. Myth 2: Patients experience increased anxiety during wide-awake surgery. FALSE Dr. Melton: The operating room staff and I try to make the experience as easy as possible. Most of these procedures are relatively quick, and we are able to talk through any concerns the patient has. The benefit of being wide awake is that it allows for conversation at the time of surgery. If there is still ongoing anxiety, or the patient has concerns, anti-anxiety medicine can be given. Myth 3: There is more pain after wideawake surgery. FALSE Dr. Melton: The surgery is no different, except for the anesthesia. I typically use the
same numbing medicine whether the patient is awake or asleep. The benefit of being awake is, again, open dialogue and conversation during surgery, when I can coach the patient and answer any questions they may have about the surgery or postoperative care. This may lead to less pain due to less confusion or uncertainty. Myth 4: It is harder to qualify for a wide-awake surgery than for surgery using general anesthesia. FALSE Dr. Melton: It is much easier to qualify for wide-awake surgery. General anesthesia has risks, and there is often a required evaluation by medical doctors preoperatively. This is not the case for wide-awake surgery. I do not routinely obtain a medical evaluation preoperatively for wide awake surgery. Myth 5: There are more risks involved in wide-awake surgery. FALSE Dr. Melton: There are actually fewer risks involved in wide-awake surgery due to the absence of general anesthesia or sedation. For example, I still use the same technique for a carpal tunnel release or trigger finger release, whether the patient is asleep or awake. The risk with surgery is the same, but the risk of anesthesia is not.

As the publisher of Lebanon City Lifestyle, my heart remains firmly rooted right here in our hometown. Our mission has always been to love on Lebanon—to celebrate our people, highlight our small businesses, and tell stories that reflect the spirit of this community. Over the past few years, that passion and consistency have been recognized and led to an incredible honor: we were asked to take over Gallatin City Lifestyle starting in January 2026. While we are thrilled to welcome Gallatin into our family, Lebanon will always be our foundation and homebase. In fact, we’ve grown our team intentionally to ensure we can continue serving this city at the level it deserves—more boots on the ground, more community connection, and more storytelling that reflects our shared values and passions. Our vision moving forward is simple: Lebanon remains our heartbeat. As we expand, our commitment to lifting up the people, places, and stories of this community only grows stronger.
Following years in the restaurant industry, I planted roots in Gallatin, opening a place I believed would anchor me there. But God had other plans—plans that would lead me to Lebanon and back to where my heart had always been: behind a pen and keyboard, telling the stories of local life and the incredible people who shape the culture of a community. Putting the heart of Lebanon’s leaders into words has been an honor over the last three years. This town welcomed me with kindness, allowing me to write about small farms, passionate educators, risk takers, underdogs, and the people who feed us. It’s given me a face in a downtown I might have otherwise passed through unnoticed. And now, in a beautifully unexpected turn of events, I’m returning to Gallatin as well—as we assume Gallatin City Lifestyle alongside Lebanon City Lifestyle, and I step into the role of editor for both. The entrepreneurial spirit thrives here, allowing me to work with countless people who pour so much into making our communities flourish. Managing The Heart of Lebanon Collection subscription box, creating our newsletters and providing project support are just a few of the things I get to do, alongside daily operations. I’m excited to connect with more people in Gallatin, a place I enjoy visiting, especially for time on the water. Drake White remembers the moment everything went silent. After taking the stage, about three songs in, he noticed his body was not feeling normal. “The cotton candy sky became the ground and the grass became the sky. I was like, man, I'm having a stroke." When White suffered a hemorrhagic stroke in 2019 due to a ruptured arteriovenous malformation (AVM)—a tangle of arteries and veins in his brain—he had to relearn everything— walking, talking, even the simplest tasks. That experience inspired him to dedicate his life to helping others navigate the challenges he faced, both physically and emotionally. Panic could have swallowed him whole. Instead, he fixed his mind on breath. On presence. On the name he repeated like a lifeline: Yahweh, Yahweh. “As long as I could breathe, I just made that sound,” he recalls, “I saw angels, anchors of peace. I knew I was protected.” His wife Alex was seven hours away in Nashville. The call came and she drove through the night, reaching his bedside just

as he woke. Her hand slipped into his left—only he couldn’t move it. Couldn’t move his leg. Couldn’t recognize the body he suddenly felt trapped inside. “That was one of the lowest lows,” he says, “Realizing I’m not going to Australia. I’m not touring with Zac Brown. I can’t even move.” Depression didn’t creep in; it crashed. The future blurred into a long, uncertain road no doctor could promise he’d ever fully walk. But somewhere inside that darkness, White made a vow. A quiet deal with God. “If you let me hold my wife’s left hand again with my left hand,” he prayed, “and give me the ability to have kids, to keep going—I’ll make this about other people. I’ll show them the light that you are. I’ll show them how I got through this.” Six years later—with walls dented from thrown phones, with hard-fought victories hard-won, with days he admits were filled with both despairing thoughts and others blazing with gratitude—he’s kept that promise.
White’s recovery wasn’t just physical. It was a mental marathon. A spiritual wrestling match. A test of identity, marriage, endurance, and faith. And through it, he learned that the real battlefield is often invisible: the struggle happening inside the mind. Out of that realization came Benefit for the Brain, which White describes as a night dedicated to the battle between our ears, a nonprofit built to support not only survivors of neurological trauma but the caregivers who often shoulder more than anyone sees. The mission goes beyond raising funds; it’s about providing education, advocacy, and community for people dealing with traumatic brain injuries, mental health struggles, and the complex healthcare systems surrounding them. White emphasizes practical tools for survivors and caregivers alike—how to manage treatment, navigate insurance hurdles, and find hope amid uncertainty. His story—once defined by a single terrifying moment—is now a testament to what happens when vulnerability meets purpose. “I just want people to know I get it,” he says, “It’s hard. I’ve been there. Healing Through Music Finding Peace and Purpose with Karla Davis Soulful singer-songwriter Karla Davis has found her voice on some of the nation’s most iconic stages, from NBC’s The Voice to Late Night with Jimmy Fallon, The Grand Ole Opry, and the Ryman Auditorium. Known for her “country blue-eyed soul” sound, Karla draws inspiration from legends like Roberta Flack, James Taylor, and Bonnie Raitt. Her recently released cover of Bruce Springsteen’s Dancing in the Dark is available on all platforms, reflecting both her versatility and deep emotional connection to music. Through her performances, Karla seeks to create experiences that uplift and inspire. She blends storytelling, spirituality, and song to reach audiences on a deep level. “I hope they will feel inspired… to hold onto hope and lean into Jesus,” she says. “I’m convinced that I’ve been given new life after doing the same! If it weren’t for music, I’m not sure where I would be today.” Karla also emphasizes the profound connection between music and mental health. Creating and sharing music has been her personal therapy, a tool to process emotions, and a way to navigate life’s challenges. “When I’m sad, I write. When I’m overjoyed, I write. When I’m scared, I pray and write,” she says. “Sometimes these magical moments come from putting pen to paper and trying to create something. I don’t know where I would be without music and what it has done for me in my life.”
While her career has taken her to Nashville and beyond, Karla remains drawn to Lebanon, Tennessee, where she has found community, inspiration, and a sense of home. “The souls of this town are beautiful,” she says. “From the first day I arrived, I was met by the most incredible people who helped me move into my very first apartment. There was a sense of community from day one, and that felt so special.” Looking ahead, Karla is excited to expand her ministry through music with a new gospel record featuring some of her favorite hymns. She also treasures being part of initiatives like the Well Woman Collective, where music, wellness, and women’s empowerment intersect. “I’m proud to be a small part of a group of women uplifting others,” she says. For Karla, music is more than a career—it’s a vehicle for healing, hope, and connection. Lebanon families caring for loved ones with Alzheimer’s or dementia now have a new source of support with the arrival of Abe’s Garden. Recognizing the emotional and physical demands of caregiving, the organization has expanded its trusted Nashville approach to Middle Tennessee, offering programs designed to ease caregiver stress while promoting purposeful living for participants. The Lebanon location grew out of conversations with local leaders, including Kristen Harris and Mayor Rick Bell, who identified a growing need for high-quality memory care services in the community.

At the heart of Abe’s Garden Lebanon is The Club, a day program for adults in the early to mid-stages of dementia. Unlike traditional adult day programs, The Club follows the Hearthstone Institute’s I’m Still Here® philosophy, a research-based, non-pharmacologic approach that prioritizes engagement over medication. The program combines meaningful activities, specialized communication techniques, and enriched environments to give participants opportunities for joy, connection, and purpose. Daily programming is designed to encourage participation even when apathy or hesitation arises. Music, art, philanthropy, sports, and exercise allow participants to engage in activities that match their interests. Group readings feature opinion-based questions, so everyone can join the conversation without fear of doing it wrong. Drum circles, movement exercises, and playful competitions such as Who Did It Better provide social roles that reinforce confidence and connection, making every day fulfilling. Understanding the toll caregiving can take, Abe’s Garden extends support beyond The Club through Abe’s Garden at Home, offering personalized in-home care. Families receive assistance tailored to the needs and preferences of their loved ones, helping with daily tasks and providing meaningful engagement. Potatoes 1. Preheat oven to 450°F. 2. Wash and dice potatoes into medium fry-style pieces. 3. Toss with olive oil, salt, and pepper; spread on a sheet pan. 4. Roast 10–12 minutes, toss, then roast another 10–12 minutes until crisp and golden. Beef 1. Heat a large skillet over medium-high. Add 2 tsp olive oil and the diced onion. 2. Cook onion, past translucent, to deeply caramelized browned to bring out sweetness, adding a splash of water if needed. 3. Push onion aside, add ground beef to the middle – cook until browned. Season with salt and pepper.
Cabbage 1. Heat 1 tsp oil over medium. 2. Add cabbage, garlic powder, and salt. 3. Sauté 3–4 minutes, adding a splash of water to steam if needed. Make Ahead Mac Sauce 1/3 cup plain nonfat Greek yogurt 1 Tbsp no-sugar-added ketchup 1 Tbsp sweet relish ½ tsp vinegar ½ tsp onion powder 1. Stir all ingredients together until smooth. 2. Portion into small containers. Serve Layer potatoes, beef, cabbage, cheese, and pickles in a bowl. Top with Mac Sauce and enjoy.
