Dr. David Balle from Grosse Pointe Dermatology and Cosmetic Center gives his professional advice on caring for your skin during the winter season. Smith, a leading attorney in the city and a patron of the arts. 15530 Windmill Pointe is an immense 14,547 sq ft stately Colonial Revival mansion situated on 1.9 acres on the shores of Lake St. Clair. It’s important to take care of our teeth and gums throughout our lives, not only for a healthy mouth but also for a healthy body. As we age, medical conditions, medications, and mobility issues affect our oral health. Here, Dr. Horacio Falcón of Falcon Dental Group in Harper Woods provides us with expert advice on retaining the teeth we have and options to replace those lost.

Another thing we see is poor oral hygiene due to medical conditions, especially Parkinson’s and things like that, because it's harder for them to brush. We have to accommodate that and teach them how to maintain oral hygiene, which may mean using a different type of toothbrush.

WHAT ARE THE RECOMMENDATIONS ON HOW OFTEN WE SHOULD VISIT A DENTIST’S OFFICE FOR CLEANINGS, EXAMS, AND X-RAYS?

Brush at least twice a day, using a soft brush and not putting too much pressure on the bristles. Electric toothbrushes make it much easier to remove debris and do a better cleaning overall. Next, be sure to floss, and then use a fluoride antiseptic rinse that does not contain alcohol, which can make your dry mouth even worse. After rinsing, do not eat or drink anything for at least 30 minutes so it has time to repair enamel and kill bacteria.

If you’re healthy, don't get many cavities, and don't take medications, you can see a dentist twice a year and get x-rays every other year. If you have gum disease, are prone to cavities, and/or take a lot of medications, then the suggestion is every three to four months for cleanings. After three months, the good bacteria in your mouth turn into bad bacteria, and that's when it starts doing damage. X-rays would then be done every year.

ARE THERE ANY PARTICULAR ORAL HEALTH ISSUES SEEN IN THE SENIOR POPULATION MORE SO THAN IN OTHER POPULATIONS? Most elderly people take medications, which can dry out the mouth, and having a dry mouth can mean you’re more likely to get cavities. Saliva helps clean the debris and also helps to buffer the acidity and the bacteria in the mouth.

WHAT CAN SENIORS DO AT HOME TO ENSURE GOOD ORAL HYGIENE?

ARE THERE ANY ORAL CARE PRODUCTS SPECIFICALLY DESIGNED FOR THE ELDERLY POPULATION? There are dry-mouth kinds of toothpaste, and for patients who have a lot of cavities, we can give them a prescription. If they cannot grasp a toothbrush well, the main thing is to make the handle of the brush as big as they can so they can grab it. I tell patients to put tape around it, so they have a better grasp. If they can’t floss with regular floss, they should use flossers that can be placed more easily between the teeth. Interdental brushes are brushes that look like Christmas trees. Don't push them in; just let them flow in, and if they don't fit, don't force them.

HOW CAN ORAL HEALTH ISSUES AFFECT THE REST OF OUR BODIES? Many studies have shown that poor oral health and Alzheimer’s disease are connected. Another big study found an increase in heart disease from poor oral hygiene because plaque in the mouth accumulates into the bloodstream. Diabetes will also affect your gums, which will continue to affect your diabetes.

WHAT ARE SOME SOLUTIONS FOR THOSE WHO MAY HAVE WAITED TOO LONG TO HAVE THEIR NATURAL TEETH REPAIRED? The first is called an implant bridge, which is putting in multiple implants and then connecting them, so it doesn't

come out. It also maintains the bone. The next one in line would be an implant-retained denture removed at night. The last one is just a regular removable denture that patients use an adhesive to keep in. These can be very uncomfortable and are not great for chewing food well.

IN ADDITION TO GUM AND TOOTH HEALTH, WHAT OTHER ISSUES DO DENTISTS AND HYGIENISTS SCREEN FOR WHEN EXAMINING OUR MOUTHS? We do oral cancer screenings. In the U.S., about 50,000 people are diagnosed with oral cancer every year, and about 9,700 of those die. We also check the bone and the jaw. If you can't chew well, then you can develop digestive issues. We also do head and neck checks, and another main thing we do, especially in my office, is take every patient’s blood pressure. Winter Skincare Professional Advice from Dr. David Balle

While it’s important to take care of our skin all year round, certain issues can arise or get worse when the weather turns cold. Some can be allayed with high-quality over-the-counter products, whereas others may require a dermatologist visit. “In winter months, we tend to see more issues with dry and itchy skin,” says Dr. David Balle, a board-certified dermatologist with Grosse Pointe Dermatology & Cosmetic Center. “It's a time of year when some things, like eczema and psoriasis, tend to act up.” Fortunately, there are quite a few things we can do at home to alleviate uncomfortable symptoms. Here, he gives us some helpful tips and product recommendations. “When showering or bathing, use a mild, gentle cleanser,” he says. “Harsher soaps may dry or strip the skin of its natural oils.” He recommends Dove® for Sensitive Skin as well as CeraVe® brand cleansers. “Secondly, when taking a shower or a bath, don't linger because the longer we're exposed to water, the drier our skin becomes in the winter.” It is also critical to use a moisturizer after each bath and shower. “What I usually recommend and what I use myself are CeraVe moisturizers,” says Dr. Balle. He explains that adhering to a skin regimen becomes even more important as we age. “Studies have shown that we produce less moisture in our skin than we did at a younger age, so moisturize daily for preventative maintenance.” Grosse Pointe Dermatology offers its own moisturizing cream. “We make a super-rich moisturizing formula that’s often very helpful for areas with thicker, drier skin with thicker scales, like hands and feet,” he says. “That's something unique we offer our patients, and they love it.”

For the older population, Dr. Balle also recommends cutting down on the number of showers and baths we take. Instead of every day, every other day may suffice, he says to only use cleansers on necessary areas. Using our hands to clean ourselves is also gentler than using a washcloth or loofah. “When we're younger, I think using a washcloth is a great idea, especially for the facial area where we produce more oil,” he says. “But, we tend to produce less oil and naturally have less moisture in our skin as we age.” He says using a hyaluronic acid serum is also helpful. “By 45 to 50 years of age, it’s thought that we’ve probably lost almost 50% of the hyaluronic acids that we naturally have in our skin,” says Dr. Balle. “Hyaluronic acids are molecules and agents holding moisture and water in our skin. So as time goes on, not only do we produce less moisture, but we hold onto it less effectively.” If you follow all the tips above but are still itchy, it might be time to see a dermatologist. “A lot of times when people have itchy skin, they may need more help,” he says. “Chronic dry skin may eventually turn into a mild form of eczema, and they may need prescription level medication or cream to manage that.” Severely dry, itchy skin, says Dr. Balle, can also be a sign of something more serious such as a thyroid or autoimmune disorder, or sometimes even cancer, which is another reason to visit a dermatologist. “Dr. Google never went to medical school,” he jokes. “When we see things online, we don't know where that information comes from. Anybody can call themselves a skin care specialist, and many do. IEP COMES TO GROSSE POINTE

When faced with an urgent medical issue, our first call may be to our primary doctor. However, it can be difficult to get a same-day appointment. Or, if it’s after hours or over the weekend, that may not even be an option. The thought of going to a hospital and waiting for hours to be seen for something that may not be an emergency can be daunting. Thankfully, there’s another alternative. IEP (Independent Emergency Physicians) Urgent Care, which opened in Grosse Pointe in June of 2021, has board-certified emergency medical doctors, nurses, physician’s assistants, and other highly-qualified staff to handle a multitude of medical needs. “We feel we’re a very important niche for our availability,” says Dr. Steven McGraw, a partner with Dr. Daniel LaLonde of the Grosse Pointe facility. “We built access to emergency care one would typically find in emergency departments, but you don't have to be in an emergency department if you're not having a life-threatening complaint. Sometimes, being in the

emergency department is both more costly and less efficient.” Urgent care facilities are popping up nationwide, and IEP, founded in 1997, now boasts eight centers in Michigan. All can treat issues such as ear infections, cold and flu symptoms, lower back and mild abdominal pains. They have a full lab on-site and routinely test for COVID, RSV, strep throat, and many other illnesses. They’re also well-equipped with x-ray machines and can treat most broken bones. Of course, urgent care centers are not a replacement for emergency departments, especially for people having severe chest pains or other serious symptoms such as blurred vision and dizziness. However, if you were to show up with these complaints, the staff would surely get you where you need to go. “We don't just send you on your way,” says Dr. LaLonde. “We are happy to make those phone calls and expedite your care to the appropriate center. We can get you door to door very quickly.” On the other hand, if you come in with mild discomfort in your chest or other vague symptoms and don’t think you need to go to the emergency department, IEP can help make that determination. “A qualified advanced practice professional is going to not only do a physical exam but perform an EKG, evaluate you, and sort of stratify you in a set of risk components,” says Dr. McGraw. “If you would best be served based on their findings in the emergency department, we will expedite your care to the emergency department nearby.” In addition to treating illness and injuries, IEP offers non-urgent services such as sports physicals. “We did about 200 free sports physicals for


the Grosse Pointe community, for kids entering middle school and high school,” says Susan Durgan, the director of marketing and communication for IEP. “I'm so proud of having an urgent care in our backyard in Grosse Pointe where I live, and Steve lives,” says Dr. LaLonde. “These are my neighbors and friends going to our Grosse Pointe Urgent Care center, so it's very personal to me that they have a good experience. If they don't, I want to know about it, and we'll certainly fix anything that doesn't in any way satisfy.” Some people may be hesitant to go to an urgent care center because they’re afraid that most won’t accept their health insurance plans, especially if they’re on Medicaid. “When we set up these urgent care facilities, and because of Dr. McGraw, Dr. LaLonde, and our emergency physician owners, it was really important to make our urgent cares accessible to everyone so that high-quality care would be available to all,” says Susan. THE DRAW OF STUNNING SCENERY AND MAJESTIC NATURAL THERMAL WATERS

Just over an hour’s drive from Phoenix is a literal oasis in the desert. Stunning scenery, ample activities, and mouthwatering dining options are part of the package—but perhaps the biggest draw is the resort’s ancient natural hot springs and their mineral-rich waters, revered for centuries for their purported healing power. The geothermal features act like natural hot tubs, and have drawn everyone from Native American tribes who journeyed to the springs for medicinal purposes, to frontier ranchers and explorers, and later, to celebrities and dignitaries. “The waters at Castle Hot Springs paint the land with a spectrum of hues reflective of the minerals running through them,” says General Manager Kevin Maguire. “Lithium bears a deep purple shade, while iron evokes rusty red and oxidized copper merges into blues and greens. plus general aches and pains were alleviated by soaking in the three pools cascading into various temperatures—all initiating from the springs.” First built in 1896, the historic Castle Hot Springs destination, which sits on 1,100 acres in the Sonoran Desert and is nestled against the majestic Bradshaw Mountains, underwent reconstruction and development in 2019, though always honoring the legacy of the property. Dedicated to wellness, it provides the perfect destination for guests to recharge while connecting with nature—its very design creates a feeling of being far from the hustle and bustle of everyday life; of stepping back in time to when time moved more slowly and moments were appreciated. There are just 34 rooms at the resort, and the accommodations were built to immerse guests with the environment at every opportunity. The Sky View Cabins are positioned for stargazing, while the Spring Bungalows feature private fireplaces, as well as outdoor tubs. And yes, the tubs source mineral water directly from the springs. Since the resort employs a “digital detox” approach—where guests are encouraged to unplug from the stresses of daily life and connect with nature and recharge—none of the rooms’ furnishings include televisions, but instead, shelves of books. In addition to swimming and soaking in the thermal springs, there are a variety of activities on property, including horseback riding, hiking,

and training in archery amongst the resort’s surrounding rugged terrain; pickle and bocce ball on the great lawn; yoga and meditation sessions; and Arizona’s first-ever Via Ferrata cable climbing course. Keeping to the theme of wellness, the ingredients for the food served at the resort’s Harvest restaurant are grown on its 3-acre farm. A team of agronomists cultivate and harvest more than 150 crops each season. Guests can even tour the farm, often tasting crops along the way that may later end up on their plate. There’s also a flower garden where flowers are grown for the floral arrangements, spa products, and even culinary arts.

“‘Flavor Farming’-driven culinary and agricultural teams at Castle Hot Springs emphasize growing and utilizing crops for flavor rather than size and yield,” explains Lead Agronomist Ian Beger. “The agricultural team, comprised of Arizona natives, grows more than 500 varieties of rare and indigenous fruits, vegetables, herbs, and florals (and more than 30 types of heirloom tomatoes alone) annually on the resort’s thriving 1-acre organic farm and greenhouse, all of which supply the resort’s dining capabilities.” Every detail of the destination has been carefully designed and executed, leaving guests to do nothing but relax, recharge, and to leave inspired. 1 1/4 cups quick oats 1 cup white whole wheat flour 1 1/2 teaspoons baking powder 1 1/2 teaspoons cinnamon 1/2 teaspoon kosher salt 1/4 cup unsweetened applesauce 4 tablespoons unsalted butter melted and cooled 1 large egg 1 teaspoon pure vanilla extract 1/2 cup honey 1/3 cup dark chocolate chips 1/4 cup raisins 1/4 cup well chopped raw walnuts or pecans (in this recipe, we usually go for pecans, but both are delicious) or an additional 2 tablespoons chocolate chips and 2 tablespoons raisins

DIRECTIONS In a large bowl, whisk together the oats, flour, baking powder, cinnamon, and salt. In a medium bowl, combine the applesauce, butter, egg, vanilla, and honey. Whisk until blended. Pour the liquid mixture into the flour mixture and stir just until combined. The dough will be wet and sticky. Fold in the chocolate chips, raisins, and walnuts. Place in the refrigerator and let chill for at least 30 minutes or up to 3 days. When ready to bake, place a rack in the center of your oven and preheat the oven to 350 degrees. Line a large-rimmed

baking sheet with parchment paper or a silicone baking sheet. Remove the dough from the refrigerator (if it is very stiff, you may need to let it sit out for 5 to 10 minutes). With a cookie scoop or spoon, drop the dough into 1 1/2-inch balls and arrange on the baking sheet, leaving 1 inch of space around each. With your fingers, gently flatten each cookie to be about 3/4-inch thick. Bake until the cookies are golden and firm around the edges and set on top, about 9 to 10 minutes. Place the baking sheet on a wire rack and let the cookies cool on the baking sheet for 2 minutes, then transfer the cookies to the rack to cool completely.
