Annual Impact Report :: July 2024 - June 2025

Dear Friends of LifeStream:

I’m proud to reflect on LifeStream’s 50 years of service and to share our vision for the future. As we celebrate our milestone anniversary, our commitment to improving the quality of life for older adults, people with disabilities, and caregivers across our 12-county service area remains as strong as ever. Programs we offer include congregate and home-delivered meals, transportation, non-Medicaid case management for individuals age 60 and over, information and assistance for any age/need, Medicaid waiver case management for individuals ages 59 and younger, wellness classes, community events, and caregiver support. We are continuously working to adapt and ensure that each of our programs meets the needs of our community members.

As we look to the future, we look forward to continuing to expand our caregiver support services as well as our Dementia Outreach work. These programs support not only those who live with dementia daily, but also their care team members who work hard alongside to keep them active, healthy, and safe. Offering information, education, and community through screenings, support groups, and area partner resources is a critical part of making East Central Indiana an age-friendly community.

I want to thank each of you for your continued support of LifeStream. Whether you are a volunteer, community partner, or supporter, your involvement helps us fulfill our mission. We are grateful to have your support as we embark on the next 50 years of service. This annual report highlights our recent involvements and provides updates on the programs and services that make a difference in the lives of so many throughout East Central Indiana.

Caregiver Initiatives

National Caregiver Month

November is National Caregiver Month – In 2024, we recognized caregivers and their important role by hosting a caregiver workshop with comedian Sky Sands. Throughout the day, caregivers enjoyed speaking with community resources, health screenings, taking part in wellness activities, and potting plants to take home with them.

LifeStream Services Becomes a Dementia Screening Location

In October 2024, LifeStream Services became certified as a dementia screening location through the Alzheimer’s Foundation of America. Dementia Outreach Specialists are certified to perform memory screenings across our 12-county service area. The Dementia Outreach Specialists work with community partners and health networks to provide evidence-based trainings and development, community education, and partnerships to help improve the quality of life, care, and services for persons living with dementia.

Caregiver Kits

LifeStream has expanded its supply of Caregiver partner locations. The goal of these kits is to help caregivers get connected with resources to help maintain a good quality of life, as well as increase interaction between the caregiver and person with care needs. Caregiver kits have different themes, and are geared towards individuals living with Dementia, Parkinson’s, and in kinship care and their caregiver. These kits consist of topic-specific books and resources for the caregiver, as well as activities for the caregiver and person they care for to enjoy together. Caregivers will be given a keepsake packet of resources and handouts to keep upon returning the kit, courtesy of LifeStream Services.

Community Initiatives

Annual Golf Outing 2024

The Annual Golf Outing took place in August 2024 at the Players Club in Yorktown. Individual Support Services was the presenting sponsor for the day. We had more than 120 golfers join us along with 9 businesses who were cart and hole sponsors, and several volunteers who all contributed to the success of the event. American Pest Professionals took home the John R. Craddock Cup for the win!

Evidence-Based Wellness

LifeStream offers many evidence-based wellness courses including A Matter of Balance, Geri-Fit, Healthy Eating Every Day, Enhance Fitness, Stepping-Up-Your-Nutrition, Bingocize, and our newest course, Drums Alive. LifeStream Wellness completed 341 evidence-based wellness courses between July 2024–June 2025; this includes A Matter of Balance classes taught by our partners at Purdue Extension and the Muncie YMCA, where they offer Enhance Fitness classes.

LifeStream Volunteers

661 volunteers provided 23,655 hours of service equal in value to $764,311

Volunteers are a vital part of the impact non-profit organizations have, including LifeStream Services. Volunteers are an important part of many LifeStream programs—they assist in delivering meals, serving at our senior café sites, providing companionship, and many other important tasks. One way we help our communities show appreciation for volunteers is through the LifeStream Golden Hoosier Award. Local Golden Hoosiers Lonnie Snow (Wayne County) and Randy Davis (Delaware County) were both nominated to the State for the Golden Hoosier Award.

Community Initiatives

Celebrating 50 Years of Nutrition at LifeStream Café sites

Thank you to everyone who joined us at our local meal sites to celebrate National Nutrition Month and LifeStream’s 50th anniversary all throughout March! Everyone had a great time and left with goody bags, information about LifeStream’s programs, and full bellies.

Staying Golden at Healthy Aging Expo

We really brought out the “Golden Year” theme at our annual Healthy Aging Expo at Kulhman center in Richmond. The annual Healthy Aging Expo is a free event for the community to learn more about programs, services, and activities available to older adults while socializing, playing games, taking part in wellness demostrations, and enjoying a boxed lunch. With more than 320 participants in 2025, this event proves to be a valuable source of information in the Wayne County area and beyond.

LifeStream’s 20th Annual Aging Well Conference & 50th Anniversary Celebration!

In addition to our Annual Aging Well Conference, LifeStream also hosted an anniversary celebration and awards program honoring some of those who have helped advance LifeStream’s mission to “provide the right information, resources, and support for improving quality of life and maintaining independence for older adults, those with disabilities, and caregivers.”

ADRC Initiatives

As a way to help our Aging and Disability Resource Center (ADRC) teams continue to stay up-todate on available information and local provider resources, our Training team hosted a provider fair, inviting LifeStream area providers to showcase materials that gave a better understanding of how they could be of assistance to the communities we serve. The event had more than 50

Navigating Change with Continued Learning

Each year, our team navigates various resource updates and policy changes to continue providing the most accurate and helpful information to those in our community, with this year being no expception. As always, our staff rose with the tide to face these changes head-on. Our Aging & Resource Disability Center (ADRC) took part in more than 1,000 hours of training with our Information and Assistance team answering more than 8,500 calls. LifeStream’s Information & Assistance Department (I &A) is the first line of communication with the caller and our agency, and also serves as the line to schedule appointments with a SHIP counselor for Medicare Open Enrollment.

LifeStream Takes on TRAM Operation

In January 2025, LifeStream Services assumed operations of Transportation for Rural Areas of Madison County (TRAM). TRAM, a service of Madison County Council of Governments (MCCOG), provides demand-response (reservationbased) transportation to the general public in Madison County.

LifeStream Transportation is supported in part by INDOT, Jay County Commissioners, and Randolph County Commissioners.
