onica Ledesma and her son, Hector, were simply looking for a family doctor when they came to Legacy’s Santa Clara clinic in 2016. Little did they suspect it would be the beginning of a turbulent two-year journey featuring extended hospital stays, insurance issues, and the loss of one of their medical specialists. Legacy quickly became much more than a place to simply see the doctor, as the entire Santa Clara team rallied to get Hector the life-saving treatment he needed. Hector was born with a birth defect that caused his intestines to develop outside his body. Despite reconstructive surgery, his body couldn’t absorb nutrients. Years of IV nutrition damaged Hector’s liver and, at five years old, he received a liver and small intestine double transplant in California. “He was a whole new person,” says Monica. “It literally gave him life.” The procedure also came with a lifetime of anti-rejection drugs, constant monitoring of his health, and countless medical tests. Despite all of this, Hector was healthy when the family decided to move to Houston in 2015, and they’d already found a transplant specialist to continue his care. But, two short years later, things changed. Hector aged out of Medicaid insurance when he turned 18, and was then denied Social Security benefits. His health took a turn for the worse, and he spent six weeks in the hospital as medical bills continued to mount. Then, his transplant doctor suddenly died. “Things just got worse and worse,” says Monica. “Hector absolutely needed this treatment, and we needed help to continue his care. Legacy found us the lifeline we were searching for.” Dr. Sarah Miller, Hector’s provider at Legacy, had already formed a special relationship with the family. “They were some of my first patients when I arrived at Legacy two years ago,” says Dr. Miller. “When things started to snowball, everyone on Hector’s team said we were going to do whatever it takes.” Legacy Care Team Assistant Daisy Saucedo became the point person on insurance, making calls and setting
appointments so the family could obtain coverage. She coordinated communication, and connected the family with social workers and financial assistance. “Many of our patients don’t have insurance and can’t access resources because they don’t know where to turn,” says Daisy. ”I know how to make those connections, and it feels great to be able to help people like the Ledesma family.” Daisy also worked with Dr. Miller, connecting with Hector’s original California transplant team to continue his treatment. She called his physicians to coordinate lab tests and results, allowing them to collaborate as they finetuned Hector’s medications. “For months, we were a crosscountry care team,” says Dr. Miller. “Being able to help Hector through this period in his life has been wonderful, and bridging the gap between their lack of insurance and uncertain health care to their present, more manageable situation has been so rewarding.” 2018 has started out much brighter for Hector. He now has insurance through the Affordable Care Act marketplace, and he’s just started seeing a new transplant specialist. But he’s not leaving Legacy. Because Dr. Miller specializes in pediatrics and internal medicine, Hector will remain her patient as he transitions to adulthood. “She knows so much about me and my background. She’s more than a doctor. She’s part of the family, now,” says Hector. It’s a family that’s chosen Legacy as their medical home. Monica and Hector’s stepfather, Josue, come to Legacy for their own health care, in part because of the convenient, integrated primary and behavioral health services. “The services we need are all in the Legacy network, and all of our records are in one place. We don’t have to worry about going to several different offices,” says Monica. “The providers here connect with you with grace and kindness. You’re not a number. You’re a name.” We look forward to reaching more medical milestones with Hector. Building relationships with all of our patients builds healthy families and futures. It’s just one more way Legacy is driving healthy change in our communities.
Remembering Dr. Mathilde Krim, AIDS Research Pioneer & Activist When Dr. Mathilde Krim died earlier this year, she left a legacy as a passionate crusader against the AIDS epidemic. Dr. Krim, a geneticist and cancer researcher, dedicated herself to educating and increasing global awareness about AIDS, as well as reducing the stigma against people living with HIV. As the founding chairperson of the American Foundation for AIDS Research (amfAR), Dr. Krim was instrumental in establishing Houston’s clinical research trials in the late 1980s. Back then, Legacy was known as the Montrose Clinic. “We were doing prevention, testing, education, case management, and palliative care,” says Legacy CEO Katy Caldwell. “There were no drugs back then. If one of our patients tested positive, we had to help them cope with the mental aspects of their diagnosis, and then figure out how they could move forward.” No funding or coordinated support was coming from government officials, so the clinic launched the Houston Clinical Research Network (HCRN). “Our employees helped conduct the trials,” says Caldwell. “We had nurses on staff and a medical director. It was about bringing the community into the research process.” Chris Jimmerson was one of the employees who took a chance and called Dr. Krim for help. “And, lo and behold, she called us back and helped us get in the very first clinical trial at HCRN,” he says. “Pharmaceutical companies were used to reaching out to larger institutions to conduct trials and didn’t know about us. Dr. Krim put us in contact with a drug company that wanted to conduct a study. The company even provided funding for the project.” Houston was one of the first amFAR-funded clinical research sites in the country, and Dr. Krim would visit during twice-yearly meetings. “She was a very caring woman and a researcher herself. Righting this wrong became her passion.” “This was a time when the community was still taking care of itself and raising money, facing great hostility from state and local officials,” says Sue Lovell, the first female president of the Gay Political (now GLBT) Caucus. She remembers when Robert Bernstein, the Texas Health Commissioner at the time, proposed a quarantine in 1986 for “incorrigible” people with AIDS who practiced risky behavior. Dr. Krim flew to Austin from her home in New York to testify against the statute. Lovell and activist Glen Maxey (who later became a State Representative) met her at the airport. “We sat down at the airport bar to walk her through the background of the bill and what might happen,” says Lovell. “And she was incredible in front of the panel, and stood her ground. If they expected a demur woman, they were surprised. She explained to them in medical terms what was happening and why quarantine wouldn’t work.” Days after Krim testified, the proposal was withdrawn. “I told her, ‘It was because of you, Dr. Krim, talking and giving them a scientific basis to make this decision.’ She said, ‘Well, it was an education for me, too,’” Lovell recalls. Legacy’s Lynne Tyer was the Director of HIV Services and Education for the Red Cross at the time, and would often attend roundtable lunches featuring Dr. Krim. “She had a consistent, clear message: that researchers could unlock the mysteries of HIV and save lives,” remembers Tyer. “She was tired of young, vital people dying, having no hope of surviving with HIV or being able to take any medication to make it better.” And, Dr. Krim’s connections with the entertainment industry brought star-studded attention to Houston. “Here’s this woman from amfAR, who hangs out with the likes of Liz Taylor and Elton John, working to make a difference in the lives of people with HIV. It brought recognition and raised awareness at a time when people didn’t want to talk about it,” says Tyer. “It was a grassroots commitment to taking care of your brother and your sister. It was pure and powerful, and I was so proud to be a part of that.” Dr. Mathilde Krim’s 91 years were filled with pioneering activism and a drive to make a difference that has impacted countless lives. We are grateful she stood with Houston and Legacy in the battle against HIV.

The signs advertise Legacy’s complete health care services for the entire family – from pregnancy to pediatrics, adult medicine, and most recently, geriatrics and endocrinology.

Legacy Montrose The newly-renovated first floor at Legacy Montrose now offers a volunteer-staffed information desk and check-in stations for all patient appointments. The space freed-up on the clinic’s upper levels will be reconfigured with additional exam rooms and treatment areas to meet patient demand.

Emily recruited young volunteers to host a celebration with our pediatric patients on February 3. Way to go, Emily!
Gary Hammett, International Marketing & Operations, Apache Corporation Long-time Legacy supporter Gary Hammett hosted a book drive at his Apache Corporation, collecting over 1,000 books for Little Readers.
