88 Zadní přístup indikujeme: • při neúspěšné zavřené repozici jednostranné nebo oboustranné luxace dolní krční páteře • při útlaku nervových struktur zezadu (lamina, klouby) • při vysoce nestabilních zlomeninách C/Th přechodu • při úrazové nestabilitě u pacienta se spondylitis ankylosans (Morbus Bechtěrev) • při vícečetných zlomeninách obratlových těl U těchto poranění s výhodou doporučujeme ke zvýšení stability nahradit zadní přístup halo fixačním aparátem na dobu 6–8 týdnů. Výhody zadního přístupu: • jednoduchý a bezpečný přístup k páteři • vysoká biomechanická stabilita háčkových dlah • • • •

Nevýhody zadního přístupu: traumatizace svalů krční páteře horší hojení rány větší krevní ztráty horší funkční výsledky omezením funkce původně zdravých sousedních segmentů, doprovázené větší bolestivostí krční páteře

Více komplikací je popisováno při použití zadního přístupu(16,33,44,58,100). Arand(12) zaznamenal horší funkční výsledky po zadní osteosyntéze. Ebraheim(37) ve své práci uvedl větší bolesti krční páteře po zadním přístupu.

Operační postup: zadní přístup k subaxiální páteři Poloha pacienta při intubaci je supinační. Pak doporučujeme naložit na hlavu pacienta halo a otočit pacienta do pozice pronační. Pomocí Mayfieldovy svorky přitáhneme halo k operačnímu stolu. Pozici krční páteře sledujeme pod skiaskopickou kontrolou v bočné projekci. Horní končetiny jsou náplastí nebo tahem za horní končetiny vedeny podél těla pacienta. Po dezinfekci a zarouškování operačního pole vedeme řez nad palpovatelnými obratlovými trny ve střední čáře. Ostře postupujeme k střední fascii m. trapezius. Pomocí raspatoria a elektrokauteru skeletizujeme krční páteř subperiostálně laterálně až ke kloubům. Průběžně stavíme krvácení bipolární nebo monopolární koagulací. Při preparaci můžeme střídat strany a hemostázu dělat dočasnou tamponádou. Nakládáme rozvěrač. Vstupní body pro šrouby v jednotlivých segmentech jsou identické od C3 po C6 (obr. 4.1.25.). O správnosti výšky zvoleného vstupu pro daný obratel se přesvědčíme v bočné projekci pomocí skiaskopie. Pro vstup do kosti ve zvoleném bodě použijeme šídlo nebo kostní frézku jako prevenci proti sklouznutí vrtáku nežádoucím směrem. Vrtákem postupujeme divergentně se

Poranění páteře sklonem 25° (obr. 4.1.26a). V bočné projekci směřujeme kraniálně 30° a postup vrtáku kontrolujeme pomocí skiaskopie (obr. 4.1.26b). V současnosti jsou k dispozici polyaxiální šrouby, které není nutné zavádět bikortikálně, proto obyčejně stačí vrtat do 14–16 mm. Před spojením šroubů s tyčí je nutné dekortikovat zadní struktury krční páteře pomocí kostní frézky a přiložit kostní štěpy (obr. 4.1.27a,b). K transpedikulární aplikaci šroubů do obratle C7 platí jiná pravidla pro vstup i sklon (obr. 4.1.28a–d). Při poranění C/Th přechodu je někdy nutné ukotvení šroubů do horní hrudní páteře, proto připojujeme i vstupní body a směr konvergence pro horní hrudní pedikly (obr. 4.1.29a,b,c). Vstup do pediklu vytvoříme K-drátem, šídlem nebo kostní frézkou a dál postupujeme tupou sondou nebo vrtákem 3,2 mm. Pátradlem se přesvědčíme o intaktnosti stěn kostního kanálu. Všude do stran i vpředu musíme cítit kostní kontakt. Změříme délku šroubu pomocí kalibrované sondy. Hloubku zavedení sondy sledujeme na skiaskopii v bočné projekci. Pak pod skia kontrolou zavádíme šroub potřebné délky. Po dokončení instrumentace následuje výplach, kontrola krvácení, založení Redonova drénu a sutura operační rány po vrstvách. Pooperační režim se neliší od předního přístupu.
