WHO WE ARE Chai Care knows that there is an increasing number of older adults in our community and that family dynamics are changing. As a result, the need for an estate planning team to include professional care managers is growing. Chai Care’s professional team is trained in gerontology, social work, nursing, health care and counseling. Our professionals will make clientspecific recommendations and will become your client’s extended family, helping to assure advocacy, referral and support.
HOW WE HELP Our care management professionals: • Minimize medical and long-term care expenses • Foster continuity of care • Minimize recurrent acute episodes of illness • Reduce medical complications, hospitalizations and unnecessary procedures • Increase access to appropriate community resources • Intervene during times of crisis and emergency situations • Help smooth transitions between care settings
WHO IS ELIGIBLE Chai Care is a program of the Jewish Federation of Broward County. Clients who give a minimum $100,000 irrevocable endowment to one of the Jewish Community Foundation’s Vision Funds may be eligible for Chai Care, starting at the age of 65.
As more frail older adults require health care and social services, their caregivers struggle to navigate these systems, perform direct care and manage emotional stress. Our Chai Care Professionals are a resource for families, especially those separated by distance.

The Florida Chapter of the Aging Life Care Association invested in a groundbreaking research project to gauge the perceived value of Aging Life Care Professional services by both the care recipient (client) and the responsible parties (individuals who authorize and pay for the Aging Life Care services). The project was complete in two phases during 2010-2016. Below are some of their findings.
