This form is to be completed by an authorised Hustler dealer only. Upon diagnosing a machine fault that may be eligible for factory warranty cover, please complete the following details and submit to Hustler for approval BEFORE any repair is attempted. 1. Complete all fields below in full. 2. Attach relevant photos of the fault — these greatly assist in diagnosing the issue. 3. Attach the machine’s Maintenance Schedule (found in the instruction manual). 4. A Hustler Customer Service representative will acknowledge receipt of your claim and may follow up with further questions. 5. If the claim is approved, a claim number will be issued as a reference for repair costs. The dealer must invoice these costs back to Hustler upon completion of the repair. 6. Note: All repairs are reimbursed in accordance with our Standard Repair Times (SRTs) guide. Any variance from the SRTs must be justified and approved prior to proceeding.
