OPINION

By Juan Carlos Caramés Aranda

Gastro-esophageal reflux disease in children

G

astro-esophageal reflux is a disease characterized by the passage of stomach contents into the esophagus (the return of food or contents of the stomach to the throat, nose or even vomit), is an abnormal condition of the digestive tract. It occurs frequently in infants and children. Vomiting is the most frequent symptom of gastroesophageal reflux in infants up to 67 percent and then decreases to affect only 5-10 percent of infants at 12 months of age. The difference between those infants who have physiological reflux from the one who is already pathological, is the presentation of reflux, considering as the illness when there is resistance to food, pain when passing it, irritability, anemia, occasional vomiting with bleeding, or failure to develop. Atypical symptoms may occur where they involve the respiratory tract such as: a chronic cough, repetitive infections, recurrent pneumonia, even putting the child’s life at risk, causing bronchoaspiration. The prognosis of infants with Gastroesophageal Reflux Disease (GERD) is excellent, they mostly disappear, persisting in less than 5 percent of pediatric patients. Symptoms of GERD in preschool children include intermittent vomiting, abdominal pain, waking up at night without any cause, resistance to food, and diseases related to the respiratory system, as well as allergies. Older children and adolescents have similar symptoms to adults such as heartburn, epigastric pain (pain in the stomach), chest pain, often resembling heart pain, night pain, difficulty passing food and returning the contents of the stomach with ease. In some cases, they may not present the above symptoms described only the occasional sensation of shortness of breath, dizziness, fatigue, even suffocation. Erosive esophagitis can cause a chronic loss of blood and as a result an anemic picture.

Diagnostic methods: There are several useful diagnostic methods, little relationship is to be known between the symptoms and the state of the severity of damage in the gastric and esophageal mucosa. There are no universally accepted symptoms that can predict the exact diagnosis, in order to decide the most appropriate treatment. • The upper gastrointestinal series: It is useful for detecting anatomical abnormalities in children, but it is not specific to diagnose GERD. • Esophageal pH monitoring: Esophagitis is present in approximately 50 percent of children with abnormal results of pH monitoring. Abnormal esophageal pH monitoring correlates with the presence of endoscopic and histological esophagitis in children. • Esophagoscopy with biopsy: it is an accurate method to diagnose reflux esophagitis in infants and children, also high endoscopy is useful to assess other causes of abdominal pain and vomiting in pediatric patients. Treatment Options: Included are changes in lifestyle and pharmacological treatment, and in some cases may have to do surgery. The change in lifestyle included in infants are, changing the formula, feeding techniques, and of the most importance the time elapsed between feeding and lying down, it is necessary to keep the infant with the head elevated and recharge it on the shoulder of the adult. at least 20-30 minutes and then place it in a position (left side and slightly raised head) that does not represent a risk in case of reflux, thus avoiding the risk of Sudden Infant Death Syndrome. In the adolescent the indications are similar that in the adults, like the change in their alimentary regime, avoid the overweight is basic, to avoid chocolate and spiced foods. The medical treatment will depend on each particular case, aimed at improving the state of health, avoiding gastric acidity, and vomiting.

Enfermedad por Reflujo Gastro-Esofágico en niños

E

BICHECTOMY A new face in 20 minutes Thanks to the extraction of Bichat bags that are located on the inside of the cheeks and containing fat, you can refine and stylize facial features.

By Pamela Escobar The Bichat Balls or Bags were first discovered in 1732 but it was not until 1802 that the French biologist, Xavier Bichat described his real anatomy and classified them as a purely “bag” of fat. Currently remove them from the cheeks is an aesthetic procedure called Bichectomy that achieves weight loss in a very fast and simple way. The oral and maxillo-facial surgeon Raúl Antonio González Villanueva describes it as an intro-oral surgical procedure. It is done by means of local anesthesia and consists of removing the fat that is on the cheeks; the intervention takes only 20 minutes. “The recovery is painless and takes five to seven days to return to the daily routine; requires some postoperative care to avoid any oral infection, you should not eat anything hot or spicy foods” warned the surgeon. He added that you can not eat pork or marine products. It is also recommended not to expose yourself to the sun, as well as a deep and thorough oral hygiene before and after surgery. “There is only a very small wound that is the incision where the bags of fat are removed and during the 24 hours after the procedure a special bandage is used to endorse the skin to the face, as it swells the face, but inflammation takes a month to disappear completely”, said González Villanueva. The maxillo-facial specialist said that there are people who exercise and lose weight, but do lose weight on the face because there is no exercise to redu-

8

ce the cheek and for this reason we recommended bichectomy, he said, is like a liposuction, but on the face. He explained that the amount of fat that is extracted from the bags of Bichat is six to eight grams and only 20 percent can be accumulated again if the patient gains weight, but will never return to the state in which the procedure was initially performed. RECOMMENDATIONS

To remove the bags of Bichat, the doctor recommended that if the patient su-

ffers from diabetes or hypertension, these diseases are under control to avoid any risk during the intervention. “The ideal people for such treatment are those with a very rounded or square face and who want to make it more sharp and stylized. It is an aesthetic procedure, to improve the appearance since it does not serve to cure any disease”, commented the also member of the College of Dental Surgeons of Reynosa. He said it is a little known and unusual treatment, within the preferences of people who want to change their appearance.

It can be done by any certified plastic surgeon or a maxillofacial surgeon and costs around $ 300 dollars. “Bichectomy has very good results, fully recommended for people who take care of their health and appearance; since it will improve its facial appearance to stylize the shape of the face and its anatomy”, explained González Villanueva. You can visit Dr. Raúl Antonio González Villanueva in his clinic located just one block from the main square in Reynosa, on Calle Porfirio Díaz, number 965 and make your appointment at 922-1292 and 956-225-2672, this last if calling from the United States.

An option to TONSILLECTOMY

frequent

infections

With the extirpation of the tonsils, you do not get fat and do not get expose to diseases, on the contrary, it ends with abscesses, tumors, and infections. By BEATRIZ FLORES Tonsillectomy is a procedure in which the otolaryngologist surgically removes the palatine tonsils or commonly called tonsils. Otorhinolaryngologist José Luis Salinas González, explained that the palatine tonsils or tonsils are part of a lymphoid tissue that is around the nose and mouth and is the most visible, but there is also a lymphoid tissue or an amygdala that is below the tongue and is called lingual tonsil. He added that there are also tissues in the back of the nose, in the nasopharynx and that is called adenoids and there is another that is around the Eustachian tube, a conduit that connects the ear with the nasopharynx ventilating the ear and sometimes it also has a surrounding lymphoid tissue called the tubal amygdala. “Together, all the lymphoid tissue form something called the Wualdeyer ring, but normally in surgeries, when it is done in children we remove the palatine tonsils and scrape or try to remove as much as possible of the adenoid tissue of the nasopharynx”, he explained. IN WHAT CASES?

The doctor mentioned that this procedure can be performed at any age, preferably after 2 years, although it is more common between 2 and 7, since the palatine amygdala has the function of maturing the defense cells and subsequently to that maturation its function ends. “It becomes the remnant lymphoid tissue and can be removed without any subsequent health problems,” said the vice president of the College of Otolaryngologists and Surgeons of Cabeza y Cuello of Reynosa, A.C. The specialist commented that in the case of adults there is a greater risk of bleeding after surgery, as well as greater pain. “The amygdala is covered by three muscles: imagine that the amygdala is between two curtains of muscles, one is the palatoglossus and the other is the palatopharyngeal and inside of them or on the floor of that bed is the upper constrictor of the pharynx;

10

when taking off this, in the children the tissue is more elastic and allows us to handle it without so much pain, but in adults it is more fibrous and harder and the surgery is more painful”, he said. He said that around this surgery circulate many beliefs and false claims, for example, that by removing the tonsils the patient will be exposed and without defenses against diseases, in addition to gaining weight. “It has been said that after surgery the patient is exposed or that he can become infected more and that is a lie, the tonsils are not a filter, it is lymphoid tissue that is put there, but they are not a filter, it is not a net for the passage of air”, he said. Salinas Gonzalez said that this procedure is indicated in patients who have more than six infections in a year or more than four in two consecutive years. Also that the patient has obstructive sleep apnea, due to the large size of the amygdala and the suspicion of a neoplasm or a tumor. “When a tumor is suspected, the amygdala must be completely removed; for the tonsil tissue there is no biopsy for tumor, you can not remove a part, the minimum biopsy in a tumor is to remove the entire amygdala”, he said. Another indication is that the patient has

or has had a peritonsillar abscess, that is, the presence of pus inside the amygdala. The otorhinolaryngologist stated that surgery is also indicated when the patient reports halitosis or bad breath, as it is usually caused by small accumulations or food remains that remain in the cracks or crypts of the amygdala and is called inclusion cysts. RETURN HOME ON THE SAME DAY

The tonsillectomy is performed with general anesthesia, a tube is placed from the mouth to the trachea for the patient to breathe and put on a mouthpiece that allows the doctor to see the area to operate. It lasts 10 to 15 minutes with each amygdala, it is an outpatient procedure and after a 3 or 4 hour surgery the patient goes home. AFTER SURGERY

Gonzalez Salinas said that the most common complication that should be avoided is bleeding. “It is very important that patients understand that they can not eat anything hard or toasted, or fried, all the ice should be scraped or in snow ball because it can hurt since the muscle will be exposed, the muscle will be alive and any Slab, frying or torti-

lla edge can tear and start to bleed”, he said. He added that sometimes the bleeding can be so abundant that the life of the patient is in danger and sometimes must return to the operating room to perform sutures, after surgery. Among the recommendations is to have rest and not be exposed to the sun, in addition to a treatment that includes an antibiotic and an analgesic to prevent an infectious process. The patient is also given indications about toothbrushing care as it can hurt the surgical area. “When we remove adenoids - which is the tissue behind the nose - they are also washed with saline solutions”, said the doctor. The indicated diet is a soft diet: porridges, mashed potatoes, jellies, soups and ice cream, yogurt and it is recommended to eat constantly, as this does not harden the scab on the amygdala bed and makes it less painful. He recommended, above all to parents, not to be carried away by false information as it is a safe and simple procedure. The otolaryngologist José Luis Salinas González, is in the medical tower of the Christus Muguerza Hospital, in Reynosa, in office 216 and the telephone number is 899-221-0300.

AMIGDALECTOMÍA

La opción

a las infecciones

frecuentes

12

No... detecting cancer,

does not cause it

Of all the information that appears daily in social networks, not everything is reliable, because much of it is false... however, these issue is more delicate when talking about health. By Beatriz Flores Social networks have become a powerful tool to disseminate information on various topics, however, the contents presented are not always reliable or truthful. There are many items addressed on the Internet, from beauty, fashion trends, food and health, the latter being those that can generate more confusion among followers. Recently, information posted on Facebook about mammography and it was stated that this study had more negative than positive consequences, even assuring that it causes cancer, instead of preventing it. Radio-oncologist Marco Antonio Corona Díaz, who is part of the Oncology team, spoke about some of these claims. Mammography is an x-ray for the breast, which is done in order to detect cancer. “It is a study that we call screening which allows us to visualize the mammary gland through X-rays”, explained the doctor. He said that previously it was performed on patients older than 60 years, but due to the evolution of breast cancer, which is the most common in the world and that appears in younger women now it is indicated starting at 40 years. “It is also indicated when there is a risk factor, such as a family history of breast cancer and for patients of ages before 40, other studies such as breast ultrasound may be performed”, he explained. The graduate of the University of Guadalajara said that prior to the realization of the procedure we explain to the patient what will the study consist of. “It is a relatively annoying study, the patient puts on the dressing gown and removes the underwear, the equipment is called mammography and the breast is placed between two plates that are going to compress a little; generally two projections of each one are made”, expressed the mem-

14

Marco Antonio Corona Díaz

ber of the Mexican Society of Radiotherapy. The study lasts approximately 15 or 20 minutes. WHAT IS CLEAR .....

The doctor Marco Antonio Corona Díaz responded to each of the statements spread through Facebook. Affirmation: “Mammography is a dangerous and harmful study that causes breast cancer because the breasts are crushed with a machine and subjected to radiation”. Answer: The specialist commented that the belief that breast cancer is caused by crushing or a blow is completely false, but that it is a multifactorial disease that is very complex genetically. “As for radiation, it is true that there are diseases or tumors that can be generated or be more prone to radiation, however, the dose that a mammogram emits is really very low”, he said. He explained that for reasons of radiological safety, it is allowed that people who are not in regular contact with sources that emit radiation can receive a maximum exposure dose of five millisieverts per year, which is the unit with which it is measured. “Mammography generates a dose of .4

millisieverts of what is allowed per year, so cancer risk does not exist and the benefit is much higher since it can detect cancer at an early stage”, he said. Corona Díaz said that the personnel exposed to radiation handle a device called dosimeter, which is in charge of monitoring the total radiation dose that is being acquired and the quantification is check out every year. Affirmation: “The touches made by women encouraged by prevention campaigns cause inflammation of the lymphatic tissue”. Answer: “It is recommended that all women perform self-examination at least once a month, generally when the technique is not known or they do not know how to perform it can be a bit uncomfortable or annoying”, he said. He commented that some patients may present a complex of diseases called fibrocystic mastopathy that can be fibrosis or cysts, which are benign and are usually inflamed by a hormonal effect. “Sometimes people can have any of these entities and if they touch when it is inflamed

it can bother them, but as such that this will generate any complication, no”, he added. Affirmation: “Ultrasound or mammary radiography is less risky than a mammogram”. Answer: The radiologist said that the study will be determined based on the density of the breast and the amount of fibroglandular tissue that is available. “A 20 years old patient does not have the same characteristics as a patient that is 40 or 50 years; an ultrasound in older people will not be able to determine the same characteristics of the breast as a mammography and vice versa”, said the expert. The doctor recommended people to stay informed through recognized websites, such as the Health department, the National Cancer Institute or the Breast Cancer Foundation and in the United States, you can choose from the American Foundation of Cancer. “Unfortunately there is too much bad information, the vast majority is false and we must rely on pages that are supported by a well-established medical association”, he said.

No... detectar

Excesses cause

cirrhosis The immoderate intake of alcohol, bad eating habits and abuse of medicines are the main causes for this disease.

to statistics 50 percent can recover, and the rest dies. “Many times these people begin to have digestive problems. When the liver is not functioning well, its venous blood pressure decreases, then in the esophagus we have a vein and since that blood is not coming completely, these veins begin to break, causing bleeding in the upper digestive tract. “That’s why many patients who have cirrhosis start with vomiting and bleeding from the digestive tract. Or they begin to make black stools. Most likely, those patients with well-marked cirrhosis will die sooner or later. If they are treated, they may resist for a while longer”, he explains.

By José Manuel Meza When a prognosis of cirrhosis is discussed in a doctor’s office or an emergency room, patients should take it very seriously, because it is the result of chronic liver damage caused by a prolonged liver disease. “It is basically the inflammation of this organ, which is usually associated with a poor diet, the consumption of intoxicating beverages, drugs and in some cases environmental diseases such as viruses and bacteria”, said Dr. Juan Miguel Segura, from the Christus Muguerza Hospital. In relation to the foods that can cause cirrhosis, the general practitioner from the Universidad Mexico-Americana del Norte (UMAN), and who made his internship in the Military Hospital of the Fourth Region of Monterrey, pointed out that they are all those that are elevated in fats “Hamburgers, pizzas, tacos, generate cholesterol and triglycerides increase, but here I want to clarify a myth, because the egg is not a direct causality. “That results in a general malaise of the organism. Although the liver is a very noble organ, comes the time when steatosis or a fat liver is produced”, he said. BECAUSE OF TO MUCH DRINKING

Meanwhile, when it comes to alcohol cirrhosis, the interviewee commented that there is no specific amount for the disease to occur, but that depends on the sensitivity of the liver. “There are people who take more than 10 years, but others with six months are enough to start with cirrhosis and many people die”, he said. He stated that some symptoms that a patient with cirrhosis can present are

18

fevers, yellow eyes, hiccups and other signs have to do with spots on the skin. “Many of these skin diseases are reflected because the liver is inflamed, although there are those who have pain in the back”, Segura added. However, he added that those who suffered from hepatitis can also develop cirrhosis. “But there it would be because of a virus or bacterium infection. Tests must be done to rule out that it has been transmitted directly from another person or caused by sexual transmission. “We classify this disease as type A, B, and C. They are flying on the air. The first almost always resolutive and has treatment, is removed. The B and C, no. Those two last ones do have treatments, but they are prolonged, because we would be talking about cancer”, he warned.

AND FROM MEDICINES?

In a third place appears the cirrhosis caused by drugs, which according to the doctor, however simple they may be, can cause liver deficiency. “Medication as simple as acetaminophen can cause cirrhosis. There are people who self-medicate for a whole year. I have had the experience of people who take migraine treatments, stomach pain, every day and do not have a medical follow-up. “And they are overloading the liver function, intoxicating it and that causes cirrhosis”, he said. Depending on the stage of the disease, Dr. Segura considered that it will be the percentage of people who will be able to be cured. “Fortunately the liver is very noble. Cirrhosis is like a cancer, but according

He explained that cirrhosis has an effect on other organs and directly affects the circulatory system, causing patients to go to the hospital and start vomiting blood. “They are not candidates for surgery, so they try to control it. They undergo tests, an endoscopy; of hemoglobin and an assessment. If the person is conscious, a transfusion is done. “Normally those who suffer from cirrhosis if they are treated on time do well, if they follow the indications with a good diet as well. But some relapse to medications, alcohol and their bad eating habits, because the liver does not have a good relationship with sugar and fat”, he said. To give you an idea, he said that the liver is a bag that is carried in the upper right part of the stomach and normally weighs about 500 to 750 grams. A transplant is an option for a person with cirrhosis to recover, although he recognized that in Mexico there is no culture of donation. However, the doctor mentioned that it is better to prevent and avoid excesses.

Excesos provocan

cirrosis

O comienzan a hacer heces fecales negras.

FLATFOOT Can be corrected

in time Orthopedic doctor, Michael Medina Ramírez, said that this condition affects two out of ten people. By José Manuel Meza There is a deformation characterized by the disappearance of the foot bridge, causing those who suffer it to support the whole foot plant on the ground to walk. This malformation is known as flat feet and sometimes, if left unattended, can lead to problems in performing various activities and discomfort. According to Michael Medina Ramírez, traumatologist from the National Autonomous University of Mexico (UNAM), it is important for society to know what can be done in these cases and what are the treatments to follow. “It is important because, as the person grows, he will have pain in his feet and fatigue. We see situations in which they can not do some exercise or sport without having discomfort in their feet, so it is important to pay attention”, he said. The interviewee explained that flat feet are advisable to treat it at an early age. According to the doctor the origin of this deformation is genetic but also affects the type of footwear that someone uses in their first years of life, regardless of the sex of whoever suffers from it. “All of us at birth have flat feet, but during development, the normal anatomical structure will form, however, a combination of genetics, the environment and footwear can condition the appearance or disappearance of flat feet”, said Medina Ramírez. TAKING DOWN THE MYTHS

For the specialist of the UNAM, although not having a definite arch is the cause of discomfort in the adult stages of a human being, the physical damages are at the local level. This means that it does not damage or wear

20

Michael Medina Ramírez

away other areas of the skeleton. “It does not actually cause any other problem, it affects the foot. In some cases, it could lead to poor patient posture when walking, but this deformity is usually focused. “What can cause is pain or limitation to perform some activity or walk long distances”, he added. Medina Ramirez acknowledged that although this is common for both men and women, the latter can better cope with the issue of flat feet. “It has been proven that flat feet in women is more bearable in adulthood, because the use of hi heel shoes comes to mitigate the pain, unlike men, who in most cases use low shoes. “The thing is that it can increase more discomfort in the man”, he said. However, the doctor said that flat feet can be corrected. He said that the ideal ages to

do so are between 3 and 6 years. “The treatment is based on insoles, orthopedic shoes and a periodic review to see the progress until corrected”, he said. DO NOT LEAVE UNATTENDED

The waiting time for a patient to improve his flat foot problem can be months or even a couple of years, so Dr. Michael Medina Ramirez emphasized that he should be given the attention he deserves. “Obviously, of the group of people with flat feet, maybe some of them will end up with surgical treatment, because the treatment did not correct the problem, although the majority can be solved without the surgery”, he said. However, in adulthood, the interviewee indicated that if the flat foot does not cause any kind of discomfort, it is not necessary to apply other treatments. “We must take the importance of not ne-

glecting the child who has this problem, because if it is not handled at this time, during adulthood, surely will feel the discomfort. Children do not complain, but adults do suffer and feel pain”, he said. Likewise, Medina Ramirez acknowledged that preventive health care will always be a better option than seeking to repair advanced diseases. “If you know that your child has flat feet, you should go to an orthopedist, or even the pediatrician can detect the problem and refer you to a specialist”, he said. Thus the feet, which are by nature one of the human body parts most punished by weight, also need attention and care.

PIE PLANO

Se puede corregir

a tiempo

What you should

fibroids

know about

They begin to appear with menstruation and although many myths revolve around them, the truth is that 99.9 percent are benign tumors that, however, can hinder a woman’s pregnancy. PEDRO ORTIZ MONTERREY Uterine fibroids are the most common condition in women who are of childbearing age, especially in those over 40, who are more likely to develop tumors that can reach the size of a watermelon. These tumors begin to appear with menstruation and although many myths revolve around them, the truth is that 99.9 percent are benign tumors that, however, can hinder a woman’s pregnancy. It is estimated that four million 500 thousand Mexicans women have fibroids, this means that one out of every five women of childbearing age can develop them and after 40 years it is common for six out of 10 to have them. Pablo Díaz Espíndola, a gynecologist specialist in reproductive medicine and microsurgery, said that the tumor does not have a precise origin or an identified cause, it simply develops when women begin to menstruate. “It is a very common condition that occurs in the womb for unknown reasons, they can be inside, on or outside the female reproductive system and depending on the size or discomfort that it causes, a surgical intervention may or may not be necessary”, he explained. However, he emphasized that having such tumors do not always mean that women will be left without the womb, because that is one of the main myths about fibroids. “Having myomas does not mean removing the womb or operating, it is only treated when there are three important issues: pain, bleeding or if the woman wants to get pregnant. “They can be so small that they can not be seen with the naked eye, but unfortunately they cause pain, bleeding, anemia and prevent females from getting pregnant, everything varies according to size and location”, the specialist explained. Tumors can grow from one to 50 centimeters, depending on the size they cause a deformity in the womb to the extent of making a woman look pregnant. Despite the effects they can cause on those affected, the problem does not endanger the life of those who suffer from it, but it

does generate a lot of discomforts. “The most serious is bleeding, anemia, pain, discomfort, feeling of heaviness and infertility. It does not allow a comfortable sexual life and the day to day becomes tedious, especially if it generates anemia, women are weak and constantly tired. “Fortunately to treat the disease does not necessarily means to have an operation, you have to be very careful because many times the specialist sees myomas and wants to remove the womb”, he said. Díaz Espíndola said that the above is the most serious mistake that can be made by specialists in the care of women, especially if they have not had children or want to get pregnant. “Many times women go to their annual study and specialists detect fibroids that do not cause any discomfort to the patient, they do not need to be removed, let alone remove the womb. “Today, the treatment of myomas is done with oral medications, surgeries where only the myoma is removed and only in the most extreme cases is the womb extracted”, he said. The oral treatment lasts up to six months and reduces the size of the myoma up to 50 percent, but mainly removes bleeding and pain. Extracting the womb is a last measure,

and is done after the woman already had the children she wanted to have and the tumor generates more problems than benefits. The costs vary, an oral treatment can cost

between 3 thousand 500 and 5 thousand pesos per month, the cost of the surgery will depend on the size, quantity, and position to establish the operation. “To identify this problem is very simple. When a woman goes to her general review, with an ultrasound we can know if she has fibroids, what size they are and if they have any symptoms. “They should have their gynecologist as a reference doctor, like a family doctor, for him to recommend their annual reviews and be aware of the woman”, he suggested. Patients who undergo treatments can continue with their daily activities without any problem, instead if patients undergo surgery to remove fibroids, within a week they are recovered, while the removal of the womb may take up to a month for recovery. For more information, you can consult the page www.miomascreesquesabes. mx where patients can look for general information, how tumors look and more. “Patients should not let time pass, they are important people in the life of a family as in working life, what we want is that they feel full, complete and that they do their activities well. “An annual review or every six months is basic and primordial in all women”, he said.

Lo que debes saber

miomas

OPINION By Jesus Vidaurri Leal

What is dry eye?

T

he eyes need tears to stay healthy and without discomfort. If the eyes do not produce enough tears, it has what is known as a dry eye. Dry eye also occurs when it does not produce the right kind of tears. HOW DO TEARS WORK?

When the eyelids closes and open, it spreads a sheet of tears over the eye. This keeps the surface of the eye soft and smooth. The tear film is important for good vision. WHAT ARE THE SYMPTOMS OF DRY EYE?

You feel itching and burning in your eyes. Has a feeling of something scratching or grit inside the eye. There are lines of mucus inside or around the eyes. Your eyes are red or irritated, especially when you are in a place with wind or near cigarette smoke. Using contact lenses creates pain. It has many tears (the eyes produce more tears in response to the irritation caused by the dry eye). WHAT CAUSES DRY EYE?

People tend to produce fewer tears as they get older. Both men and women can suffer from dry eyes. However, it is more common in women, particularly

those who have already gone through menopause. OTHER CAUSES FOR DRY EYE ARE:

Some diseases, such as rheumatoid arthritis, Sjögren’s syndrome, thyroid diseases and lupus. Blepharitis (when the eyelids become inflamed or red). Entropion (the eyelids fold inwards); ectropion (the eyelids fold outwards). Be in places with smoke, wind or a very dry climate. Looking at the screen of a computer for a long time. Wear contact lenses for a long time. Undergo refractive eye surgery. The use of some medications HOW IS DRY EYE TREATED?

The ophthalmologist can treat dry eye problems by recommending: Drops, artificial tears or ointments for eyes (previously prescribed). Warm compresses over the eyes. Massage on the eyelids. Certain eyelid cleaners. Keep the tears (the ophthalmologist may recommend blocking the tear ducts, which causes the natural tears to remain in the eyes for a longer time). Among other recommendations.

¿Qué es el ojo seco?

L

Physical pain against

emotional pain

Attempting against your own body are practices carried out mainly by adolescents who go through difficult situations known as self-harm that provide momentary relief in the face of adversity. By Beatriz Flores Considered by many as a fashion or something to attract attention, self-injury or self-injury are the physical actions that a person performs against himself. The psychologist Claudia Olivares Hernández, explained that there can be many causes that originate it. “It usually occurs more in adolescents and less in adults, there may be many things that are happening, such as problems in the family such as divorce, separation, that is suffering violence or may also be because of bullying in school or some sexual abuse”, said the professional. “Adolescents are ceasing to be children, they are going through many changes, physical and mental, they are neither children nor adults and for them, the change is painful and abrupt”, she added. He indicated that in these cases people do not express what they are suffering and cau-

36

se damage when they feel anxiety, emotional pain or sadness and find relief in doing so. Cutting the skin is the most common practice, although they can also hit the walls, bite, tear the hair, among other things. A LOT OF ATTENTION

Although those who self-harm regularly hide their wounds, there are some indicators that parents and people close to them should pay attention to. Being most of the time sad, not wanting to go out, cry a lot or react violently are symptoms or manifestations that something is not right with the person. “You have to approach to investigate the cause, know what happens if the teenager does not like to talk or does not have confidence, because there is no communication or problems with the parents, find a way to find information, go to school to talk with the teachers, with their friends or with relatives they trust”, she said.

Claudia Olivares Hernández.

Definitely, professional help is necessary and the psychologist will carry out an investigation to understand and discover what is causing this behavior. “When the young person or the person says that if they are hurting themselves, they should attend a therapy that focuses on emotional control and social skills therapy, for example, that they learn to express what is happening to them because they are not communicating, face their problems and also a psychoanalytic therapy, where they will show little by little what is happening to them”, said Olivares Hernández. But not only will it attend therapy who self-aggregate, it is also recommended that the family do it to

help it to a greater extent. The Master’s Degree in Education Management said that there may be six or more therapies, depending on the severity of the situation or the causes that are causing this behavior. The psychologist recommended parents to get involved with their children, their friends, and activities, that is, to know the different environments in which they develop daily. He explained that sometimes it is necessary to provide pharmacological treatment to those who manifest episodes of anxiety that they can not control, which will be provided by a psychiatrist who will be supported by the psychologist who takes the case.

Dolor físico contra

dolor emocional

CARDIOVASCULAR DISEASES

It also

attacks ladies As Mexico is second place in adult obesity in the world, the risk of suffering from some type of cardiovascular disease is greater. Today, it is estimated that in this country the number of deaths from cardiovascular disease is 292 per 100 thousand inhabitants. According to Dr. Miguel Quiñones, Master from the American College of Cardiology and member of the Faculty of Cardiology at the Houston Methodist Hospital, “cardiovascular disease is the leading cause of death in women and the relationship between the number of deaths from this cause and the ratio between breast cancer is 30 to 1”. Until a few years ago it was known that hormones were a protective shield for the female population, however, bad habits coupled with carelessness and increased body fat make the female population vulnerable, Quinones explained. “The hormonal factor is very important because, in the reproduction stage, hormones are a protection for women against coronary disease, but as age increases, many women begin to eat more, therefore they gain weight and some stop exercising, they become sedentary and the risk factors

terms of blood pressure , blood glucose, ideal weight, cholesterol levels and do everything possible to keep these numbers at healthy levels”, he said. BE ALERT

begin to gain relevance. “Some risk factors that are not being controlled well are smoking, high blood pressure, high cholesterol, and genetics, since it has been proven that, in a direct line, if a cardiovascular disease was suffered, it could be inherited. Diabetes is very common in women and being a comorbidity of obesity, makes people more likely to suffer from cardiovascular disease”, said the specialist. According to Dr. Quiñones, in general, before 55 years of age,

women are less prone to heart attack or serious cardiovascular problems, however, reaching the age of 65, it equals the incidence rate in men, and after 70 years the probability of suffering a cardiovascular event increases above men. “It is very important that women increase their health care after menopause, that is to say, that they eat well, do not gain weight and know their numbers well, which means that they must know the levels they handle in

Because cardiovascular disease is usually silent, it is necessary to consider some symptoms that can help to save a life. “The symptoms of a serious problem are multiple, but there are a number of classic symptoms such as oppression on the chest that can radiate into the arm, shortness of breath and anxiety, and in women, these symptoms can occur, but there is also a greater frequency of atypical symptoms, so it is important that if they begin to appear for the first time, immediately go to the doctor to confirm or rule out the disease”, said the specialist in cardiology. Some of the symptoms are excessive tiredness, difficulty breathing due to physical activity and anxiety. “By knowing the levels of their health, women will be more careful, coupled with the need to change harmful habits such as smoking, weight gain, and sedentary lifestyle”, he concluded. ENFERMEDADES CARDIOVASCULARES

Fuente: EL UNIVERSAL Diciembre 2017 | FRONTERA DOCTORS HEALTH MAGAZINE

Procreation with medical help

Infertility is a problem that usually affects the female population, but also males; However, beyond the congenital causality, there is a significant percentage of sterility caused by the use of contraceptives. “Already after 50 years that can be a cause of malformations or congenital diseases due to alterations in sperm”, he warned. However, the doctor also mentioned the use of narcotics as a matter of risk for the product. “There are patients who say they use marijuana and ask how much they are going to cause malformations in their baby. We know that this drug causes men not to have a good erection time, not so much in malformations, but when we talk about other types of hallucinogens, of course, they can affect ovulation or sperm structure”, he remarked.

By JOSÉ MANUEL MEZA Nuevo Progreso, Tam. A significant percentage of patients who enter the Abba Clinic (located in this city) have fertility problems and this is more common than people assume. Before there was a sector of the female population that was going through a pregnancy early, but now the woman seeks to succeed professionally, study and address other aspects of his life before reaching this step, said Ciro Luis Trejo Fuentes, who He is a doctor from the Anahuac University, specialized in the General Hospital of Mexico and the Valencian Institute of Infertility. “Now we see people who are older looking for a pregnancy, and many times it costs them more work. This is influenced by many factors such as age, environmental issues and food with many chemicals that administer everything”, he said. Trejo Fuentes, who also holds a master’s degree in hospital management and finance, added that the indiscriminate use of contraceptives is one of the main causes why a woman’s reproductive capacities are affected. “The fact that they start a sexual life so young and the use of hormones for many years, can eventually condition ovulation and find a baby, in many cases, is complicated. “As these contraceptives are antiovulatory, the ovaries do not fulfill their function and this conditions that they do not get preg-

nant”, she said. INFERTILITY IN FIGURES

Dr. Trejo Fuentes presented some percentages of the majority of people who come to the Abba Clinic and are struggling to be able to procreate. “I would say that due to congenital problems sterility affects 4 or 5 percent of the population in general, but because of environmental factors, diet and use of hormones, it can definitely reach up to 30 percent”, he said. However, age is another factor that must

be considered, because it stated that there are women who want their first child at 43 or 45 years, something that in itself is already risky due to the appearance of genetic diseases and the fact that They have a hard time getting pregnant. Trejo Fuentes also emphasized that much is said about sterility in women, but men can also be the main affected. “Definitely the female fertility cycle is shorter. In a woman, at 38 or 40 years this factor decreases a lot, while in men up to 45 or 50 years can be fertile.

Those who want to bring a baby to the world and have the financial means, generally exhaust all possibilities, such as going to a fertility clinic. “Fortunately there are methods from the most basic, which is to induce ovulation so that they get pregnant through sexual intercourse. Later comes an artificial insemination, an in vitro fertilization; when they are women older than 40 years and we see that their ovarian reserve is very low, there is the donation of ovules or a sperm bank”, he explained. - Can these methods be done right here? “Not all. Here we really prepare the patients and go to a fertility clinic, since you have to incubate the embryos, once fertilized”, he added. Trejo Fuentes explained that on the suc-

cess of these procedures to an in vitro fertilization on average gives a 70 percent chance of pregnancy, while in an artificial insemination, 40 percent. Although there are patients who could not procreate family at an early age, that does not mean that after 40 they can not, although the risks increase. “We have now managed to get people up to 50 years pregnant. Obviously, preparing the uterus of the patient because they are women who no longer menstruate and with ovules donated by women of 25 years. “Obviously these ovules bring the genetic factors of the donor. What is done is that if the patient is white with clear eyes, try to find a donor with similar physical characteristics and this can be a family member, in this way has less rejection. Or if the future parents are Hispanic, they look for a donor with similar characteristics”, said the fertility specialist. SECURITY AND TRUST

vIt is well known that Nuevo Progreso is known as the Medical Tourism Capital of Mexico. People from many parts of the United States and Canada come to this site to take care of their health and in the case of fertility, it is not the exception, since here patients can find the most reasonable specialties, security, and prices. “Definitely here the cost is much lower than in other countries. We are talking that it is 70 percent cheaper and it is fully guaranteed. These methods of conservation of sperm ovules are universal. “The same thing we do here is done in the American Union, in Mexico City or in Germany. The ovules are received, they are left in quarantine to see that they do not develop any type of disease and the donors are tested for hepatitis, HIV and various types, to check that they are with all the tranquility and security that they do not they carry no complications”, he said. Trejo Fuentes also explained that advances in science allow certain genetic modifications to embryos. “Yes it can be done and this is called preimplantation diagnosis, which began to

56

be carried out with some diseases related to sex because there are some that only give them to girls or boys. Considering that they put pure male embryos or vice versa. “But by doing this they also study the embryos to know they do not have Down syndrome, Turner’s, or some other genetic problems that are discarded. “Here we prepare the patients, because this requires an incubation period of five or six days, depending on the quality of the embryos”, explained the doctor. He indicated that the Abba Clinic has a bank with sperm for babies to have anatomical compositions of European origin. “From Swedes and Spaniards, it’s what I work with, and there’s a record of the father’s donor, if he’s blond, if he’s tall, if he’s light-eyed, what IQ is, and obviously this all influences the genes of the embryo. “Maybe the patient is 1.60 and the donor is 1.90, surely he will have genes from a higher person”, commented the interviewee. He stressed that something very important to consider is to be very careful about the use of contraceptives because that can prevent someone from getting pregnant when they want to. “Really if they are going to use a method, they rely on a doctor because there are times that is what they see in magazines, or what comes out in commercials, or what pharmacies recommended. “There are cases of women aged 17 and 18 who get injections with doses of very high hormones that can cause ovulation problems and use them without any consideration. But also these contraceptives can cause other problems of tumors in the breast and cancer as well”, he said. At the Abba Clinic, in addition to helping people to procreate a baby, they emphasize being careful with certain habits that are harmful to health. “It is very dangerous and if they have varicose veins it can give them a cerebral infarction, cardiac or very severe pulmonary affections. They are putting their lives at risk, perhaps not in the short term, but in 10 years they will have serious problems”, said Trejo Fuentes.

Influenza can trigger other diseases in adults 4A cold caused by influenza in adults can trigger other diseases, such as a heart attack, stroke or disability, even months after recovering, said the specialist in infectious diseases, William Schaffner. This is due to a systematic inflammatory response that occurs when fever and pain occur. “Influenza can leave a person who is functional in society and leave him very sick, but after recovering it may never return to the same level of functioning as before”, Schaffner said.

Bicycle seats affect women 4Researchers at Yale University discovered that women who ride a bicycle have less sensitivity in the genital area. The specialists mentioned that this is because in the seats of bicycles the weight of the body can come to compress the nerves and blood vessels of the perineum area, necessary for the proper functioning of the sexual organs, due to its design. Symptoms include numbness and loss of sensation in the genital area. The study was carried out on women who traveled at least 16 kilometers a week by bicycle.
