it is more... comfortable
Not all women seek breast enlargement. Some want them to decrease, because they have problems with fatigue, back discomfort or simply do not feel comfortable with their appearance. By José Manuel Meza It is no secret to anyone that one of the aesthetic procedures that are most in demand in plastic surgery clinics is breast augmentation; however, there are also cases of people who do not want big breasts and on the contrary, want to reduce them. “There are patients who, due to motherhood and breastfeeding, have noticed a growth in their breasts, but there are also those who are genetic. In both cases, the weight eventually creates certain problems in terms of shoulder pain, because the bra leaves marks on their back. “That’s one of the reasons why patients come to get a breast reduction. While many seek the increase, some women prefer a small breast,” said Ernesto Castellanos Muñoz. The plastic surgeon commented that there are busts which can exceed up to one kilogram, which can be annoying. “In my personal experience, I have come to remove volumes of up to 800 grams. It is a world of weight for the patient. “But most of the average is 200 to 250 grams which are removed per bust for this type of surgery,” he said. The vast majority, said the doctor, has an ideal cup C for the measurement of your breasts; nevertheless, everything depends on the comfort and appearance that a patient wants to have, so the measurements can have variations when it comes to removing the tissue from her bust. OPERATION ON DEMAND
Contrary to what one might think, more and more people are looking for this type of surgical intervention. “It’s amazing, but this is a frequent surgery. I perform on average around two or three on a monthly basis. I believe is an important increase in this type of operation,” he said.

According to the specialist, the reaction of people after having undergone this procedure is of great satisfaction. “Although there is a big scar, the famous anchor or the “T” inverted. To patients this is what matter less to them, because literally they removed a heavy weight off of them,” emphasized. And he acknowledged that, paradoxically, they are cases of people who are seeking the service of a plastic surgeon not so much for aesthetics, but for comfort. “It’s a reconstruction surgery. It is true that we seek that the bust, in addition to reducing it, must be placed in a normal anatomical position, lifting it, but it is also intended that the bust be as beautiful as possible. “That the patient not only feels satisfied because it was lighter but because she had a nice bust,” he said. Technically when a person has sagging
and sagging breasts, this doctor commented that excess skin and breast tissue is removed. “Due to that excess, many patients take a humpback pose,” he added. NATURAL CHANGES OF BREASTFEEDING
Castellanos Muñoz explained that after a pregnancy three scenarios can be presented in a person’s breasts. “That the bust remains in its original size of adulthood, although it is rare. There may also be a loss of volume in some cases, but also gain in size, and that is when they seek this type of surgery because they are not comfortable,” he said. The advantage of this surgery is that, unlike the United States, here in Mexico the cost is very affordable, compared to US prices. “Prices may even vary from surgeon to

surgeon, and although plastic surgery tends to be more accessible each time, the only recommendation I would make to the public is to look for a certified plastic surgeon,” said the clinic director. Silhouette in Reynosa, Tamaulipas. In order for the patient to have that certainty, Castellanos Muñoz suggested consulting the Mexican Association of Plastic Surgery, where a list of all the doctors endorsed to perform these procedures appears. The pain of breast reduction surgery is not much, nor does it compare to the discomfort of carrying excess weight all the time. In addition, another advantage is that the specialist tests and studies in advance to the surgical intervention, such as ultrasound and mammography, which allows a patient to rule out or detect any disease in their breasts.

OPINION
By Juan Carlos Caramés Aranda
Against obesity: advanced endoscopy
I
n our daily life, we are facing a health event very important and negative, which affects the population, it is obesity. Mexico and the United States occupy first and second place respectively. Obesity triggers diseases that cause a very high expenditure on health systems, including hypertension, diabetes, heart attacks, etc. Among the many attempts to combat obesity are diets, physical exercise programs, special devices, reducing creams, medications, food supplements, appetite suppressants, reductive specialized clothes, medical procedures such as the intragastric balloon, until you reach to remove part of the stomach by means of large internal surgeries although of minimal invasion through the skin. Endoscopic gastroplasty in the sleeve or endoscopic gastric sleeve is a new type of procedure to lose weight. This procedure reduces the size of the stomach through a suture device by means of a simple endoscopy. This procedure can be an option if you have considerable obesity (a body mass index of 30 or more), and diet and exercise have not worked. The endoscopic sleeve causes significant weight loss. By limiting the amount of food you can eat. The procedure is minimally invasive, which reduces the risk of surgical complications. Endoscopic sleeve gastroplasty helps reduce the risk of health complications caused by obesity, such as: 4 Hypertension 4 Diabetes 4 Fatty liver 4 High cholesterol and triglycerides 4 Heart disease 4 Sleep apnea 4 Knee and hip disease 4 High risk of heart attacks and/or stroke 4 Some cancers associated with obesity Generally, endoscopic gastroplasty in sleeve and other procedures or surgeries to lose weight are carried out only after trying to lose weight by improving
diet and exercise habits. The procedure is less invasive than other forms of bariatric surgery. In addition, the result in loss of weight is equivalent to that obtained with existing surgical procedures. It is an effective and safe endoscopic procedure. The endoscopic gastric sleeve reduces the size of the stomach without the need for surgery, using a thin and flexible probe to create a smaller stomach. It is reversible and without the need for abdominal surgery, reduces the amount of food that an individual can ingest, decreasing to only 3-4 ounces of the capacity of the stomach. It is impressive how this simple technique allows reducing the size of the stomach without affecting its function since it is essential that the entire stomach intervenes in carrying out digestion for a healthy life. Each area of the stomach performs highly specialized functions. They are local and systemic functions since they are related to several circuits where brain function is linked. The amount of food ingested after the endoscopic gastric sleeve has been performed is much smaller, with completely normal digestion and absorption of nutrients. There is no need to take nutritional supplements or proteins. That is why it will result in an accelerated and healthy weight loss through the gastric sleeve by endoscopy. In conclusion, endoscopic gastroplasty on sleeve is a simple technique, free of surgery, safe, conservative, since the stomach remains intact although reduced. It is a reversible technique, since achieving the desired weight it is possible to remove the sutures and the stomach returns to its original capacity. The results in loss oscillate between 45-60% of its extra weight. With a very short hospital stay and immediate recovery, in a matter of a couple of days, he returns to normal activities. The endoscopic gastric sleeve is projected as the best option for accelerated weight loss.

Contra la obesidad: endoscopía avanzada
E

How to
lose weight
and gain health
With obesity comes diseases and metabolic disorders that lead to problems with glucose, cholesterol, and triglycerides, bypass or gastric sleeve surgery can help solve this issues. By Beatriz Flores Currently, bariatric surgeries in addition to helping in weight loss have multiple benefits and are of great help in the control of other conditions, such as metabolic disorders. As its name says, metabolic disorders are a series of alterations in metabolism, that is, alterations in the processes that the body performs to obtain or produce energy. “It has to do with the digestive tract, food or certain hormones, there are many metabolic alterations, basically they are alterations of glucose, cortisol, insulin, high blood pressure, cholesterol levels and triglycerides, all related to obesity,” the specialist in laparoscopic surgery, Oscar Treviño explained. Although there are many surgical procedures focused on weight loss, the doctor said that not all are focused on correcting metabolic disorders such as gastric bypass and gastric sleeve. “The procedure that is one hundred percent guaranteed for this is the gastric bypass. In it, we make a bridge with a cross of certain organs that produce hormones and this influences the function of certain substances that develop the metabolic syndrome,” he explained. He added that in some cases the gastric sleeve can help, which works restrictively by limiting gastric capacity. To know which procedure is appropriate, an individualized study protocol is made to each patient in which a detailed clinical history is made. It includes age, sex, gender, race, genetic influence, that is if it has a genetic load to develop some type of disease. “Patients who have a diabetic mother or father or with cholesterol problems have a high tendency, over 60 percent of developing some of these conditions,” he said. He added that there are also routine labo-
10
ratory studies and some other specific to detect if there are alterations or malfunctioning of organs and all these factors are taken into account to decide which surgical procedure will be performed. THEY ARE NOT MIRACULOUS
Although these surgeries are designed to eliminate obesity and to be highly effective against metabolic disorders, they do not act solely on their own. The patient should change habits and follow certain recommendations to achieve their goals. “Surgeries are tools, let’s say they have indications of use and if not used correctly it will not give the right results. It’s not that I
TAKE NOTE: 4The No. 1 cause of death worldwide, of people with obesity, are cardiovascular diseases.
operate on you and that’s it; the patient must change actions and have continuous control,” he warned. He explained that with the surgeries the patient will manage to diminish his contribution of medicines to control his illnesses and to lower levels of cholesterol, insulin, among others, but if the patient adheres to the integral management he heals to the one hundred percent.

“It is common for the heart of a hypertensive patient to struggle to distribute all the blood supply to a large body. As your weight decreases, the heart suffers less and the same amount of medication is no longer necessary and the doses are lowered until you stop taking it,” he said. He added that the change in blood pressure is a little slower since it falls slowly and those who are diabetic, a month use very low doses of insulin. UNIQUE SURGERY
He warned that there are patients who, due to some bad evaluation or their own whim, ask for certain surgery and the expected results are not achieved, in what way he said, this type of procedure can not be done twice. “Some people choose a gastric sleeve, whose functionality is two and a half years in terms of weight loss, from then on if the conditions or eating habits remain the same, that stomach believes again, it can fit more food, the patient get stuck and no longer lose weight, on the contrary, will increase,” warned the specialist. It is contraindicated to perform a proce-
dure of the same type when the first did not work; what is indicated is that a restrictive procedure is converted to a gastric bypass. “Then we start talking about conversion surgery and it is done only once. There are different types of gastric bypass and depending on that is if in the future there may be alterations that require some modification of the procedure,” explained the doctor. PREPARED FOR CHANGE
Although anyone can undergo this type of surgery, there are some things that should be taken into account to know if the patient is a candidate for it. He explained that those who have mental illnesses that are not treatable and people who do not control their consumption of alcohol will not be able to submit to them. “Also those who do not understand the procedure or the change that they will suffer and patients older than 65 years or younger than 16 years, mostly because of their maturity, the young patient hardly understands the changes that all this implies. Sometimes they require psychological support or some kind of guidance,” he said. He warned that the metabolic syndrome is the most severe that exists in terms of obesity and pointed out that the number one cause worldwide of death in obese people are cardiovascular diseases. “It is important to bear in mind that a patient with obesity does not have an optimal quality of life and quantity either, as it is estimated that he lives up to 15 years less than a normal patient,” he said.

Even in these times in which information flows and seems to reach everyone, it continues to stigmatize who goes to the psychiatrist, considering him a madman and the doctor, as someone who “drugs” their patients. By Beatriz Flores
DATA SHEET
Many people who are channeled with the psychiatrist refuse to accept it, while those who already attend, hide it for fear of being labeled as crazy. Many decide to go with the psychologist, who has a little more acceptance in society, although it is not adequate to meet certain cases. The psychiatrist is a surgeon with a specialty in psychiatry and they are trained to treat mental and behavioral pathologies, explained Dr. Alejandro Caballero Torres. He acknowledged that although there is some improvement in the perception that people have about a psychiatrist and mental health, it is still a taboo subject due to the misinformation that is sometimes “provided by the doctors themselves”. “They tell you that with the psychiatrist not because he is going to drug you, but now you know that mental attention is as important as physics,” he said. A psychiatrist is not the same as a psychologist, in addition to the number of years of study and training, the first is a doctor and the second a graduate. “Psychiatry is a medical specialty and is responsible for the attention of problems in physiology or brain chemistry; a psychologist is a professional who does a four-year university career, a bachelor’s degree and generally attends through technical skills to modify behaviors,” he said. The psychiatrist also uses these techniques thanks to the amplitude of his training.

Many times the mental problems of children are downplayed, however, their health will depend on the environment in which they live and develop. “Parents who argue, infidelities, insecurity, among other things, alter their mental health, generally also develop behavior problems, learning or anxiety,” he explained. He said that about 30 percent of his patients are children and that most of them are adults. Of them, there are more women who come, because they believe that they can
express their emotions, while men face the machismo that leads them to erroneous behavior. CURRENT DIAGNOSES
Most likely the main cause of consultation is stress, anxiety and generalized anxiety. However, important mental illnesses such as schizophrenia, bipolar disorders, paranoia and delusional disorders must be attended by the psychiatrist who will develop a diagnostic study to provide an adequate treatment that will include pharmacological, psychotherapeutic and perhaps hospitalization support. Doctor Alejandro Caballero Torres said that everything starts in the family and is
related to the formation, values, ethics and citizenship that are encouraged in them. “Unfortunately this values have been lost and it could be due to the fashions that appear, as well as the influence of other countries through music, television, video games and the internet,” he said. He extended the invitation to people who consider that they are living a process that is affecting some area of the mind to consult with the indicated specialist. “I want you to remember that not only is feeling nervous because a patient comes, but there are alterations in many mental functions, such as memory, affection, how to relate to people and how we experience through the senses,” he said.

It's not
cancer it’s fibroids
They can measure from three millimeters to 15 centimeters and the most common symptom is bleeding and although they are benign they must receive medical treatment. By Beatriz Flores The uterine tumors are always caused for concern because patients immediately associate it with some type of cancer, however, myomas or fibroids are benign. Also called uterine myomatosis is one of the most common tumors in the uterine genital tract, said the specialist in Gynecology and Obstetrics, Dr. Teresa Tijerina Ortega. “They are tumors of smooth muscle that appear in the womb or uterus up to 50 percent of women can suffer from them and depend on their location, will be the symptoms that arise, although most of the time it is asymptomatic and can go unnoticed”, she explained. Its size ranges from three millimeters to 10 or 15 centimeters and the cause is due to hormonal effects. “The female hormones, estrogen, and progesterone are what make these tumors grow in patients between 25 and 45 years, which is the fertile age where hormones are most active,” she explained. Having presented menstruation very early, for example, at 9 or 10 years, or the onset of menopause late, at 49 or 50 are factors to develop them and also the use of contraceptives. She explained that fibroids can be classified as submucosal, that is, under the surface of the uterine lining; subcerosos, under the outer covering of the uterus or intramurals, those that grow inside the muscular wall. One of the most frequent symptoms, according to the specialist, is abnormal bleeding, which may occur outside or during menstruation but more abundantly. “It is very common for women who come with bleeding to have anemia due to the excessive amount of blood that is lost, a normal level of hemoglobin is 10, many people arrive with 2, it is not even half of what they should have”, she mentioned. Due to the anemia can be fatigue, poor performance in their activities and headache. She warned that when there are very severe

intercourse and cause pain, and discomfort. She also expressed that if after a year of having sex the patient does not manage to get pregnant, they can be a factor in infertility. “Those tumors that are inside the womb will not allow a pregnancy to be implanted”, she said. DIAGNOSIS AND TREATMENT

hemorrhages they may need a blood transfusion since they are at risk of hypovolemic shock. It is also called hemorrhagic shock and occurs when the blood volume drops to the extent that the heart does not have enough blood to pump the rest of the body and organs fail due to lack of oxygen. If myomas grow too much and outside the womb, they can press on the intestines or the bladder and this compression can cause colitis. The gynecologist said that, although very rarely, compression may occur during sexual
The diagnosis is made through an ultrasound that can be transvaginal or abdominal with which tumors will be detected from three to five millimeters to the largest. “50 percent does not require treatment are asymptomatic should only be monitored that will not grow and treatment depends on age and symptoms,” she said. If the patient presents hemorrhages, medication may be administered or a surgical procedure may be performed. Medical treatment based on hormones can be injections, taken or through the intrauterine device (IUD) and is recommended for young women who have not yet had children or wish to have more. This will end up with the bleeding and decrease the size of fibroids, said the doctor. “The medical treatment is for patients who
do not have symptoms so abundant or who can not undergo surgery if it is close to menopause, hormonal treatment is the solution because if the patient stops menstruating, the problem of myomas ends”, she commented. Regarding surgical treatments, there are two options: myomectomy and hysterectomy. Myomectomy is a surgical procedure through which fibroids or myomas are removed, leaving the uterus intact, explained the doctor. It can be open or laparoscopic surgery, in which small incisions are made to perform it, although they are simple procedures, as with all surgical procedures there are risks, with bleeding being the greatest. The specialist commented that after the surgery they can grow again, but the hormonal treatments will make a block to prevent it. “There are treatments such as gonadotropin analogs that cause a menopause in the patient and will stop growing the fibroids, they are also indicated prior to surgery when they are very large tumors to reduce them and make it easier to remove them,” she explained. Hysterectomy, said Tijerina Ortega, is an operation through which the uterus is removed, the ovaries can be removed or not, according to the doctor’s instructions. The gynecologist said that if the patient no longer wants to have children and has very heavy bleeding, she can be a candidate, so a protocol will be made to do it, a valuation and any type of cancer should be ruled out. “Any woman who hears that the womb will be removed is shocked. Many psychological aspects come into play, but surgery is not indicated in all cases, if I have a young patient without children, the matrix cannot be removed; It will depend on each person and of course that is well convinced of it”, she added. The specialist stressed the importance of going to the gynecologist in the presence of any abnormal bleeding and not downplay it, because treatment on time is the best for the patients health.

The elevation of eyebrows gives patients a younger appearance, however, because of the existence of nerves, blood vessels and ligaments must be performed by a specialist. By José Manuel Meza Beauty is not eternal. Over the years, inevitably, the body begins to show changes that occur naturally as time passes. One of the external areas that most reflect this transformation is the face: wrinkles begin to make their appearance and in many cases cause the eyebrows, and even the eyelids, to fall. However, with advances in medical science, it is possible to improve physical appearance and look younger and healthier. According to doctor, Vidkar Campos Martínez, who is a specialist in Plastic, Aesthetic and Reconstructive Surgery, there is a surgical procedure known as a brow lift. “The first thing we must do is a very adequate evaluation to be able to reach a good diagnosis. If an appropriate diagnosis is made, a correct treatment can be carried out,” he said. When an eyebrow surgery is carried out, as well as with any other facial procedure with aesthetic purposes, the patient is required to be placed in the hands of a specialist, who must have a good preparation and training to be able to execute this type of treatment. operation. The above given that it is necessary to intervene a region of the face that is characterized by the existence of nerves, blood vessels, and ligaments, which must be protected and avoid damaging them. “None of the procedures that involve making approaches to the face and being close to such important structural elements can be done quickly and not carelessly, much less in inexperienced hands. “It requires a precise knowledge not only of anatomy, but of the applicable surgical techniques, because each case is particular and a somewhat different treatment may be necessary, because it seems that we all have the same shape of the muscles, but there are anatomical variants in the facial expression, in the length of the muscles and in the shape of the structure,” said Campos Martínez. Such differences, he said, are those that must be taken into account to decide which is the appropriate technique for each person in order to have a good result, effective and safe. “It is essential to take care not to damage the movement structures of the face, to avoid

that after surgery people cannot move the eyebrow or that an eyelid falls or paralyzes the face,” he said. EXPLANATIONS
In each of the cases, it is necessary to evaluate the level of decline in the eyebrows through an exploration. He mentioned that both men and women can be candidates for this operation if the diagnosis so indicates. “When there is a degree of fall in the eyebrows, the main annoyance is that patients feel heavy eyelids. As if there was some weight on top, although they are two different anatomical zones. “We should not confuse when the skin of the eyelid is loose with the fall of the eyebrows. Many times they go hand in hand and sometimes an eyelid surgery is combined with raising of the eyebrows,” he explained. Campos Martinez said that on average there are more women who request this surgery in a ratio of one to 20 in terms of men and stressed the importance of offering a job that offers lasting effects. “We talk about the person looking good not only for two months, but five or seven years. In some techniques of facial rejuvenation we
can say that results are achieved as good as to last up to 10 to 12 years,” he said. And he acknowledged that the fact that in the case of the eyebrows the area to be operated on is not enough to exempt him as a plastic surgeon to carry out a rigorous protocol. “Never minimize the areas because the scar or anatomical area is small. It is also important to emphasize that being a surgery that can be performed under local anesthesia does not imply that it is easy, simple and that it is no longer complex. “The fact that it is an outpatient intervention does not make it risk-free, so it is necessary that it be carried out by a doctor certified in plastic, aesthetic and reconstructive surgery,” he said. He indicated that after the operation of eyebrows, it is normal that the person may be swollen. “It is an anatomical area that tends to high inflammation and is inherent in the procedure. Fortunately, we can do it in such a way that the process is controlled, that it is not exaggerated and that it does not distort any of the structures. “It is done with such care that it must be a moderate inflammation, although there are people who tend to inflame a little more, it

with this purpose is totally fast in its recovery that the next day the person can rejoin their activities. “You need to maintain some rest and the recovery time is variable from person to person. Normally it goes from 6 to 12 days in which you do not have to be making efforts or carrying heavy objects; Do not walk in the sun or do to much physical activity. The gym has to be suspended also for a period,” emphasized Campos Martínez. He said that the experience of a patient who has already been operated by a trained surgeon is very satisfactory and helps, in addition to improving their appearance, to improve their self-esteem; however, he stressed the need to seek professional help. “All surgery performed on the face will produce a result that can be very favorable, very beneficial that can help and make the person feel very happy and satisfied with the results, but we also have the other side: “A poorly performed surgery, with poor judgment and in inexperienced hands can have a totally catastrophic result with irreversible changes that often cannot be corrected even if they go with an expert in the subject,” he warned. For this reason, he emphasized that not only in the face but any procedure with aesthetic purposes, go to a doctor specialized in plastic, aesthetic and reconstructive surgery; that is duly certified and qualified. “Right now it is very easy to know who is a plastic surgeon and who is not. Information is available at the College of Plastic Surgeons of Mexico and also on the page of the Mexican Association of Plastic, Aesthetic and Reconstructive Surgery. “With the doctors last name the site will take you directly to where the surgeon are and tells you which are the qualified specialists who are close to you,” he illustrated. Regarding eyebrow lift surgery, the interviewee acknowledged that more and more people are interested in this and for patients, it is surprising how small adjustments in the face can have such favorable results.
IN GOOD HANDS 4Dr. Vidkar Campos Martinez said that people in the United States also request plastic surgery procedures. “I always make an invitation to all the people of the Rio Grande Valley and other regions of the United States and abroad to not be afraid to place themselves in the hands of the College of Plastic Surgeons, we are duly qualified. We are constantly preparing to certify ourselves. “The certifications involve a series of exams, conferences, and updates; courses at an international and national level that support current training and updated techniques, safe and with results that are proven to work,” he said. In addition to the great training of these doctors, he specified that patients from the United States and Canada have the advantage that operating in Mexico is more affordable for them. “Also valuing the economic benefit, of course there is a significant difference in terms of the costs of surgeries for aesthetic purposes that are carried out there with those of our country, because of the disparity in the price of the dollar against the Peso, but the preparation we have is really the same and they are in good hands,” he said.

The myth
of
omega 3
Ingesting it does not reduce the risk of heart attacks, strokes or having coronary problems, says a BBC News World publication, which cites scientific studies conducted in the United Kingdom.
The recommendation is familiar: the consumption of omega 3 fatty acid is good for the heart. That is the reason why millions of people from different parts of the world resort to fish oil supplements. Or seek to consume it naturally, in foods rich in omega 3 fatty acids such as spinach or fish such as sardines, tuna, salmon, trout, bonito, swordfish, turbot, mackerel, anchovy, and herring. It serves as a reference that at least 10 percent of Americans take omega 3 pills, according to the newspaper The New York Times. But a recently published study says that taking these supplements does not reduce the risk of heart attacks, strokes or coronary problems. “There is no evidence to support this belief, it has been established over the last few decades, but it was based on studies that were not conducted with sufficient rigor,” said Lee Hooper, who participated in the research. The professor of nutrition at the University of East Anglia, United Kingdom, explained how they came to that conclusion on the website The Conversation, which collects recent academic studies. The finding is based on the analysis of 79 clinical trials conducted previously. The review, coordinated by the Cochrane scientific research center, was carried out at the request of the World Health Organization to establish the effects of Omega 3 supplements on the body. “We evaluated the studies that lasted at least 12 months and in which the follow-up period ranged from one to eight years, and we analyzed the cases of 112,059 patients,” Hooper said. According to scientists, fish supplements have no benefit (or detriment) to the heart. So, where did the belief come from? It all started with a study published in June 1971 in the scientific journal The Lancet. In the 1970s, the habits of a group of 130 Eskimos living on the west coast of Greenland were analyzed by Danish researchers. They performed several tests and found that
24
their cholesterol and triglyceride levels were low, despite regular consumption of high-fat meat. THE DIET OF THE ESKIMOS IS VERY PARTICULAR
Due to the extreme climate of the Arctic, they can not grow fruits, vegetables or grains. They eat what they hunt, mainly in the sea: fish, seals, and whales. So the Danish scientists concluded that it was the diet of the Eskimos that explained the low incidence of heart attacks and the absence of diabetes mellitus in that population. That was how the recommendation came about that fish consumption was good for preventing coronary heart disease. And from that moment began the popularity of fish oil pills, which contains large amounts of Omega 3. Septiembre 2018 | FRONTERA DOCTORS HEALTH MAGAZINE

OPINION By Jesus Vidaurri Leal
What is a macular hole?
T
he macular hole is when a tear or opening in the macula forms. As the hole is formed, things in the central vision will be blurred, wavy or distorted and as it grows in size, a dark or blind spot appears in the central vision. What are the causes of a macular hole? Age is the most common cause of the macular hole. As it ages, the vitreous begins to shrink and become detached from the retina. In general, the vitreous detaches without problems. However, sometimes the vitreous can stick to the retina. This causes the macula to stretch and form a hole. A macular hole may also form when the macula swells due to another eye di-
sease or caused by an eye injury. How is a macular hole treated? Surgery called vitrectomy is the best way to treat a macular hole. The ophthalmologist removes the vitreous that is pulling on the macula, then places a gas bubble inside the eye. This bubble helps to flatten the macular hole and keep it in place wh0ile the eye is healing. The gas bubble disappears by itself slowly. The vision will improve as the macular hole closes. It may take several months until the hole has finished healing. The vision that you recover will depend on the size of the macular hole. It also depends on how long you had the hole before undergoing surgery.

¿Qué es un agujero macular?
E

CONJUNCTIVITIS: Its not always
contagious much is said about the pain, burning and itching that conjunctivitis couses, and in many cases de patient is isolated fron the rest, but it will depend on the type to know if it is contagious or not By Beatriz Flores There are many reasons why a person can develop conjunctivitis, that eye disease characterized by an intense red color, inflammation, tearing, secretions, itching and sensitivity in the eyes. Conjunctivitis is an inflammation of the conjunctiva, said doctor Luis Jian Mo, is the superficial gelatinous layer that covers the white of the eye, called sclera. Contrary to what is mostly thought, conjunctivitis is not always contagious. “Contagious conjunctivitis is divided into acute, which can be bacterial and viral, the most frequent are viral that is like a cold in the eye and in many cases, the patient has a history of having had or has the flu at the same time,” he explained. He said that those who do not get infected are allergic or an autoimmune reaction, that is, an exaggerated reaction of the body’s defenses. In this group are also toxic conjunctivitis medications, which usually occur when the patient self-prescribed, especially when using drops of antibiotics or vasoconstrictors, used regularly for red eyes. He explained that the incubation period of viral infective conjunctivitis is similar to that of a virus that causes the flu, which goes from 48 to 72 hours and the symptoms may be present between one and two weeks. In the case of the bacteria, it can last between seven and 10 days, while those of allergic-type or reaction will depend on the type of allergy and its exposure to the agent that causes it. Ophthalmologist Luis Jian Mo stated that when someone presents this disease, it is better to isolate it from the rest, especially if you work with many people or in closed places. vernalis conjunctivitis, which occurs mostly in spring or autumn. Also, closed or overcrowded places, such as schools or nurseries, favor contagion among minors. However, adults do not escape from it, which can happen when they have contact with a large number of people or places that lack ventilation. Waving hands and then touching your face or eyes, as well as sharing towels, makeup, glasses and even the pillow, are actions that encourage its spread. It is very important not to self-medicate, because not knowing the type of illness that is suffered, the situation could worsen. The doctor warned that if any eye discomfort should appear, you should go to the specialist to prescribe the appropriate treatment. “You should try not to self-medicate and go to your trusted ophthalmologist because your eyes are invaluable,” he said.

Conjuntivitis: No siempre es
contagiosa

‘Cleaning
is vital for
dental health’
According to specialists, oral cleaning is essential to prevent tooth decay and other health problems. By José Manuel Meza Nuevo Progreso, Tam. Visiting the dentist can lengthen the life of your teeth, address wear problems and even help detect diseases such as cancer. Dr. Ana Rubí Reyes Ibarra mentions that although this is a topic that may seem logical, many people do not know exactly the implications of not brushing properly and not go to the dentist either. “There are people who spend four or five years without a dental cleaning and when they have the problem is acute and come with mobility in their teeth. “The importance of dental cleaning is to prevent tooth loss and gum disease,” she said. The main cause of not going to a consultation, added Reyes Ibarra, is that patients feel afraid. “We provoke that fear ourselves by not giving them the confidence. We always have to explain what they will feel, that it will not hurt and that we have a vibrating device, that there will be a lot of water, but that we have another instrument that will be sucking it. “You have to give the patient safety, to raise your hand and warn us if you feel any discomfort,” she said. The doctor, who has her clinic in the first block of Coahuila Street, Nuevo Progreso, Tamaulipas (crossing the international bridge on the left, in room 6), said that there are very constant patients in the cleaning and protection of their teeth. ; however, she noted that the vast majority tend to be neglected. “Many people ask me how often they can go to their dental cleaning, but this depends: if you are a constant person in your daily brushing, and as long as your technique is done well, your visits will be possible twice a year. “On the other hand, if it is a patient that your teeth brushing lasts two minutes and once a day, then to keep your teeth well you have to come every month, otherwise there will be a deterioration,” she said. The interviewee stressed that oral cleaning is not only essential to prevent

tooth decay and dental mobility, but also to detect other pathologies. “There are problems that are cause for not attending our mouth, apart from the loss of teeth, and they are diseases of the heart because as we are generating tartar and bacteria, they end up affecting it. “They also produce digestive discomforts, many people ignore all that, but they are problems that are getting worse and we see them more and more here in the consultation,” she said. SOME SYMPTOMS
Reyes Ibarra makes reference to some manifestations when someone has oral problems. “The gum is also damaged. Many people tell us that they do not come to the dentist because they only have bleeding from time to time, but if that happens it is due to inflammation, which is caused by dental tartar. “What happens when the teeth do not receive the proper cleaning is that they change color, there are spots, mobility and bad breath is generated, apart from a red gum with bleeding,” she illustrated. And she argued that bad breath most of
the time derives from the lack of cleanliness. 99 percent is why she insisted. She said that dental cleaning is a simple procedure that takes 15 to 20 minutes; however, if the patient comes with bleeding gums or periodontal disease (which is the mobility of the pieces); or with an accumulation of tartar from eight to ten years, the application of anesthesia will probably be necessary to do the procedure. WHAT PASTE TO USE?
One of the most frequent questions your patients ask you is which toothpaste and brush work best for you; nevertheless, she specified that the care of the denture does not depend directly on that. “Toothpaste is not the secret, but the technique of brushing. Many people ignore it and buy the brush that is worth 100, 200 pesos and the 100 peso toothpaste, but really the oral health depends on how the teeth are cleaned and obviously also the cleaning that is done with the dentist”, she assured. Another very important aspect that the doctor highlighted is that people who take antibiotics, anticonvulsants; medicines for
mental and diabetic problems, all of them tend to generate excessive gum inflammation and should be going at least every two or three months to be checked. “They are patients who have a lot of bone loss and need to pay close attention to their teeth and not miss their dental visits,” said Reyes Ibarra. She added that cavities and other premature eruptions of wisdom teeth, some mobility, malformation or a cancer problem can be detected from a dental office. “If a person brings a swelling or a ball that is changing color, size on the palate or in the mouth, we see it at the time of the visit. “And we proceed to do a test, send a biopsy with a pathologist and it is diagnosed,” she added. She commented that patients usually associate cancer with other areas of the body, but Dr. Reyes Ibarra said that it can also occur in the mouth. “There is no age for oral carcinoma and there are patients who have never smoked in their lives and have had cancer of this type,” said the dentist graduated from the University of Matamoros.

‘La limpieza es vital para la
salud dental’

The incurable
lupus
It is a disease that especially affects women. A late diagnosis can cause irreversible injuries, but you can learn to live with it. Dr. Luis Javier Cruz Álvarez
By José Manuel Meza
Medical general by the University of the Northeast (UNE) with the specialty in the General Hospital of Mexico.

In simple terms, lupus is a condition of the immune system that recognizes some parts of the human body as foreign and damages them. According to the World Health Organization (WHO) affects primarily people of dark complexion, but also Latinos and although its origin is not fully defined, there are treatments to control it. According to the rheumatologist Luis Javier Cruz Álvarez, this disease is predominant in the female sex and usually appears between the second and fourth decade of life, in addition to the fact that the sun’s rays can stimulate it. “It has a prevalence of approximately four women for every man, but the idea is that people are well informed and evaluated so as not to cause confusion,” he said. And is that the diagnosis of lupus, he said, has always been a dilemma for the doctor of the first contact. In the social networks and some internet sites, information circulates that is not necessarily indicated and instead of helping the patient confuses it. To avoid falling into false news and resolve some doubts - for any suspicion of the disease - recommended consulting the websites of the Mexican Council of Rheumatology and the European College of Rheumatology. “However, the diagnosis of lupus erythematosus should preferably be addressed by the internist or rheumatologist. “Unfortunately from the beginning of the disease and until it is evaluated by a specialist sometimes the time that has passed is considerable and the consequences may be greater, so it is important to attend in a timely manner,” he said. HOW IS IT DIAGNOSED?
According to Dr. Cruz Álvarez, lupus is a disease that has different faces, so it is essential to make a correct assessment of the set of signs and symptoms in order to de-

termine whether a person suffers it or not. “It can present with dermatological alterations, which is the most common, excessive sensitivity to the sun, the presence of lesions on the back of the nose, generically called butterfly wings; oral ulcers, joint problems, reflect an arthritis picture; the presence of neurological abnormalities or compulsive crises. “It also manifests with effusions in serous cavities such as the pericardium, that is, the fibrous membrane that surrounds the heart; or show hematologic signs such as hemolytic anemia or low platelets; but the most serious is the renal disorders, where there is the retention of substances that affect the kidney. “This is in late stages, but there are clinical criteria to make the diagnosis of lupus erythematosus. It is not necessary that all the symptoms are in the same patient to detect the disease, that is why we rely on laboratory studies,” described the interviewee. With the advent of new technological tools, today it is easier to make a quick assessment of the case.
Reumatólogo Luis Javier Cruz Alvarez.
PREVAILS DISAGREEMENT
Until a few years ago, lupus was practically an unknown disease, but there are still many sectors of the population that ignore its existence, even when it has already claimed many lives. “It is often confused with a prognosis similar to a cancerous problem. Until 1950 the mortality of lupus was very high, but
with the appearance of new therapeutic approaches, that figure decreased significantly. “I believe that the public, in general, should be more informed to seek early attention,” Dr. Cruz Álvarez insisted. This doctor said that a lupus patient assisted in a timely manner, for example, can have the same average life of a person suffering from diabetes or hypertension, but everything depends on the stage in which they are and diagnose. “We need to avoid damage to what we call the white organ, such as the lung, brain, heart, liver, etc.,” he added. The rheumatologist explained that the lupus antibodies hurt the organs and tissues, for which reason he emphasized making the patient an integral assessment and giving him reliable information and solid bases so that he can improve his quality of life. “The relationship with the use of stem cells or alternative therapies has not been shown to improve the survival of the patient. “The diagnosis is very important and the integral management also. Patients with lupus can lead a good life. Even to procreate family, as long as the disease is under control, the important thing is to avoid having long-term complications and suffer kidney damage or other,” he said. COST-BENEFIT

On average, some public hospital systems in Latin America pay up to 6,000 Mexican pesos per month for each patient and 40,000 pesos when they are going through a crisis. While in private clinics in the United States the annual cost of the disease amounts to 35 thousand dollars. The WHO estimates that 5 million people suffer from it around the globe, but many people with limited resources can not access medicines and their health worsens more quickly. “Here we have to put on a scale what is cost-benefit. When talking about high-cost medicine, this would have to be viewed from a patient’s point of view of the quality of life. “They are drugs that we call immunosuppressants or those that modify the immune response, including steroids or what the public knows as cortisone, but there is also a wide range of drugs called immunomodulators. “Nowadays these have a variable cost and can be accessible through an institution,” the doctor suggested.
SOME FACTORS
It has been associated that the disease could have hereditary patterns and viral infections, but what is proven is that the recrudescence of lupus is related to prolonged exposure to sunlight and even with the intake of some foods. “Ultraviolet rays have a certain range of action in patients with sun sensitivity or skin changes. It is advisable to avoid them using sunscreen that we call SPF, with a minimum of 50 or up to 100 values to avoid the reactivation of the disease. “In relation to dietary management it has been seen that the intake of some products such as alfalfa can induce a modification of the immune response. “Patients with lupus should not have a special diet, as long as they do not have a kidney condition, because then they should have a diet low in sodium and some proteins,” said Cruz Alvarez. The active participation of the patient and his attachment to the treatment are indispensable to carry out an adequate control.

lupus
