Contest Rules and Regulations Heart & Stroke Lottery Survey Sweepstakes 1. HOW TO PARTICIPATE No Purchase Necessary to Enter or Win The sponsor of this Sweepstakes is the Heart and Stroke Foundation of Canada (“Heart & Stroke”). 1.1 Eligibility: The Heart & Stroke Lottery Survey Sweepstakes (the “Contest”) is open to all legal residents of Ontario who have reached 18 years of age (Entrants). No (1) employee, officer, director or member of Heart & Stroke , (ii) employee, officer or director of the advertising or promotional agencies; (iii) immediate family members of the individuals set out in (i) and (ii) above; and (iv) any person residing or domiciled with any individuals set out in (i), (ii), or (iii) above is eligible to enter the Contest. 1.2 Contest Period: Starting at 12:00 a.m., Eastern Time, March 6, 2024 and ending at 11:59pm., Eastern Time, March 13, 2024 (“Contest Period”). All eligible entries must be received within the Contest Period. 1.3 Contest Entry: No purchase necessary to enter or win. Customers can participate in the Contest by completing a Heart & Stroke Lottery online survey. The Entrants must clearly and fully complete the survey with all requested information. Upon completion and submission of the survey, Entrants will be entered into the Contest for one (1) chance to win the Prize (i.e. one entry). Partially completed Heart & Stroke Lottery online surveys will not be accepted. 1.4 Alternatively, individuals may mail their name, mailing address, email address and phone number to 2300 Yonge St., Suite 1200, Box 2414 Toronto Ontario M4P 1E4 to enter. 1.5 Contest Entry Restrictions: Entrants are restricted to one (1) entry per person. Entries indicating the same telephone contact number or e-mail address will be considered, for purposes of the Contest, to belong to the same Entrant and therefore are subject to the above restriction. Any excess entries will be invalid for purposes of the Contest draw. For greater clarity, Entrants cannot use multiple email addresses to enter the contest multiple times. Heart & Stroke will not accept entries by phone, email, fax or any other method not specifically permitted herein. Entries generated by script, macro, robotic, programmed, or any other automated means or by any means which subvert the entry process are prohibited and will be disqualified. 2. PRIZE 2.1 Prize: All Entrants will be entered to win one (1) of five (5) Tim Horton’s $25 digital Gift Cards (“Prize”). Each Prize is valued at $25.00 CAD. Each Prize must be accepted as awarded, with no warranty or guarantee, either express or implied. No substitution or transfers of the Prize will be allowed. Each Prize will be emailed to the winner within four (4) to six (6) weeks of the Draw. The Prize will be e-mailed to the winner and therefore a valid e-mail address must be provided to Heart & Stroke in order to claim the Prize. Failure to provide a valid e-mail address will result in forfeiture of the Prize. The winner is subject to all terms and conditions of the Prize. 2.2 Odds: The odds of being randomly selected to win a Prize depend on the total number of eligible entries received during the Contest Period. 2.3 Prize Draw: The Prizes will be awarded by random computer-generated draws from all eligible Contest entries received during the applicable Contest Period. The draw (“Draw”) will be held on March 20, 2024 at 2p.m., at 2300 Yonge Street, Suite 1200, Toronto, ON M4P 1E4. 2.4 Notification of Selection: The winning Entrants will be contacted by e-mail (using the contact information provided by Entrant at time of entering the Contest) with their Prize. If the selected Entrant cannot be reached using the e-mail provided, another Entrant will be randomly selected, and the initial selected Entrant will be disqualified and will have no recourse towards Heart & Stroke, or anyone involved in the Contest. Entrants will not be individually contacted, except for the potential winner(s). 3. GENERAL RULES 3.1 Each Prize claim is subject to verification by Heart & Stroke and/or its designated representatives. Entries made with multiple addresses, under multiple identities, or through the use of any device or artifice to enter multiple times or entries that are in excess of the permitted
