“Silent heroes.” That’s how Blaze Eppinger, who has had sickle cell disease since birth, describes people who donate blood—and you could be among them. Sickle cell is a lifetime battle and the odds of leading a healthy life depend in part on whether hospitals have adequate inventories of the right blood types.

As a child, Eppinger would get blood transfusions at least monthly. Today, at 28, he still needs nine or 10 transfusions a year—but it’s not always possible. “When blood is not available for me, I have to stay in the hospital longer, basically fighting a sickle cell crisis,” which, he explained, is very painful.

“To my fellow millennials,” he said, “your blood donation can be what’s needed to save a sickle cell patient. Having blood available for everyone who needs it is so important for people like me who fight sickle cell.”

C O M M E N TA R Y EXERCISE, YOUR LIFE DEPENDS ON IT By: Dr. Barry Ramo

M

y patient was 69-year-old man who came to the New Heart New You program at New Heart Fitness and health. He was concerned because his blood pressure had not been well controlled. He had a strong family history of heart disease. He still smoked ½ pack of cigarettes daily. At the New Heart New You program he spent 3 hours a week for 12 weeks meeting weekly with a trainer and nutritionists. A risk assessment indicated that he was high-risk for heart disease and stroke. This program began by focusing on his lack of exercise. His job required to sit all day. In the evening, he watched televsion. As a preventative cardiologist, I always start with exercise. If I can get a patient to begin an exercise program, they will take care of other problems that pose a risk to their heart such as improving their nutrition and weight management and stopping smoking. Exercise has important metabolic effects. The LDL cholesterol or the lousy cholesterol found in the atherosclerotic plaque in the arteries is responsible for strokes and heart attacks It can be reduced with exercise diet and cholesterol-lowering medication. Exercise itself raises HDL cholesterol and lowers LDL cholesterol and triglycerides. Additionally, exercise improves the body’s response to insulin. He was prediabetic. The exercise will help make insulin the makes more effective and reduce the chances of his becoming diabetic. Patients often ask me whether exercise can turn the clock back. In fact, there is data suggesting that people who are regular exercisers do have changes in their DNA that go along with a longer life. There are structures

at the end of the DNA molecules that protect the DNA called telomeres. As we age these telomeres shorten and at that time the DNA that they are to protect begins degrading. Studies have shown that regular exercisers at age 70 can have telomeres that look like they’re 15 years younger. How much should you exercise? The latest guidelines suggest that we should be doing at least 2 ½ hours a week of aerobic exercise and then 20 minutes twice a week of strength training, stretching and balance training. High intensity interval training has become very popular. Carefully designed analysis of the many studies on this subject have been inconclusive in terms of its benefit over continuous exercise. Some studies suggest that it produces more weight loss than continuous exercise and others suggest that one can get a level of fitness in a shorter period from high intensity interval training than from continuous exercise. Exercise improves cognitive function. It improves the ability to focus and improve memory. In part this may be the benefits of exercise on the circulation brain but also may be the result of overall improvement in people’s health. So, with the Senior Games in New Mexico we are seeing the best of older people who recognize the powerful impact that exercise can have on their physical and emotional well-being. ith longer days and warmer nights, entertaining becomes almost inevitable during the summer months. Thanks to the abundance of fresh produce, like California avocados, your backyard barbeques, pool parties and beachside picnics will never have to sacrifice versatility or taste.

1 ripe, Fresh California Avocado, quartered, seeded and peeled 2 Tbsp. sour cream ½ lime, juiced and zested 2 Tbsp. basil leaves, chopped 2 Tbsp. parsley leaves, chopped 2 Tbsp. tarragon leaves, chopped 2½ Tbsp. cilantro leaves, chopped 1 small shallot, peeled and quartered 1 cornichon or small sweet pickle, quartered 2 Tbsp. extra virgin olive oil ½ tsp. sea salt, or to taste

1. Combine all ingredients in a blender and mix until completely smooth. 2. Set aside until ready to serve. Pickled Watermelon Ingredients:

1 cup watermelon rinds or firm honeydew, skinned and sliced thinly 1 clove garlic, peeled and minced ¼ Fresno chile, sliced thinly 1 Tbsp. lime juice Grilled Trout With California Avocado Watermelon Slaw And Green 1 tsp. fish sauce or Worcestershire sauce or soy sauce 1 Tbsp. water Goddess Dressing ½ Tbsp. rice vinegar Recipe created by Chef Brad Cecchi of Canon, East Sacramento for the ½ tsp. sugar California Avocado Commission. Serves: 4 Pickled Watermelon Instructions: Prep time: 55 minutes Cook time: 15 minutes 1. Place watermelon rinds, garlic and chile in a bowl. Total time: 1 hour, 10 minutes 2. Bring lime juice, fish sauce, water, vinegar and sugar to a boil and pour over the watermelon mixture. Ingredients: 3. Let cool and set aside until ready for serving. 4 whole trout, cleaned, de-scaled, deboned and halved Cooking spray (for grilling) California Avocado Watermelon Slaw 2 tsp. sea salt California Avocado Green Goddess Dressing (see make ahead recipe Ingredients: below) California Avocado and Watermelon Slaw (make ahead recipe below) 1 firm-ripe, Fresh California Avocado, seeded, peeled and sliced ½ cup diced watermelon Instructions: Pickled Watermelon 1. Remove head and tail from each fish. 2. Preheat charcoal grill for up to 20 minutes or turn gas grill on high. Instructions: 3. Season each trout with salt and allow to sit for 10 minutes. 4. Spray the grill and each trout with cooking spray to avoid sticking. 5. Place each trout on flesh side down for 3 minutes, then flip and cook for 1. In a large bowl, gently mix the sliced avocado, diced watermelon and Pickled Watermelon. If you would like to loosen the slaw, more pickling an additional 3 minutes. liquid can be added. 6.
