CHRONIC DISEASE SELF-MANAGEMENT EDUCATION PROGRAMS SUMMARY

The Chronic Disease SelfManagement Education Programs (CDSMEP) include six (6) evidencebased self-management programs developed by Stanford University’s School of Medicine Patient Education Research Center. Each CDSMEP is designed to support individuals in developing skills and coping strategies to manage their symptoms related to chronic health conditions such as diabetes, arthritis, hypertension, lung disease, etc. The Chronic Disease SelfManagement Program (CDSMP) is designed for adults with ANY chronic health condition and caregivers; the Diabetes SelfManagement Program (DSMP) is for people managing Type 2 diabetes and their families / caregivers; the Cancer: Thriving and Surviving (CTS) program is for cancer survivors and caregivers; the Chronic Pain Self-Management Program (CPSMP) is for anyone dealing with chronic pain; the Positive Self-Management Program (PSMP) is for people living with HIV; and the new Workplace Chronic Disease SelfManagement Program (wCDSMP) is for those still working and also managing a chronic health condition. All CDSMEP are guided workshops delivered two and one-half hours, once a week, for six weeks, in community settings such as senior centers, churches, and health care facilities, etc.

These workshops are licensed through Stanford / SelfManagement Resource Center and are facilitated by two certified Workshop Leaders, one or both of whom live with a chronic health condition. Subjects covered include: • Techniques for dealing with problems such as frustration, fatigue, pain, and isolation; • Appropriate physical activity for maintain and improving strength, flexibility, and endurance and preventing falls; • Making decisions • Appropriate use of medications; • Communicating effectively with family, friends, and health professionals; • Healthy eating; and • How to evaluate new treatments. CDSMEP workshops are highly participatory where approximately 10-15 participants focus on building skills to manage their conditions by sharing experiences and providing mutual support. These programs do not conflict with an individual’s existing health improvement program or treatment plan, but rather are designed to enhance regular treatment and disease-specific education and management. More than 38,000 individuals in New Mexico suffer from Alzheimer’s disease or other dementia. Statistics show that about 60% of these individuals will wander at some time or another. The Albuquerque Police Department and the Department of Senior Affairs are working together through the Silver Alert program to ensure missing persons with Alzheimer’s disease or other dementia are quickly returned to their family or caregiver. Silver Alert is a public notification system to broadcast information about missing persons who suffer from Alzheimer’s disease of other dementia. The system notifies the public of a missing senior using television, radio, electronic billboards, reverse 911 and traffic signs. The public is asked to call a phone number if they see the missing person. Any individual who suffers from Alzheimer’s disease or other dementia are eligible to register for Silver Alert. The Department of Senior Affairs will place vital information and a photo of the individual on a USB drive and give the drive to the family member/caregiver who accompanied the person to the Silver Alert registration. When the family member/caregiver determines that their loved one is missing, they are asked to call 242-COPS. An officer from the Albuquerque Police Department will come to the residence, or the place from which the person is missing, and initiate an investigation. If the person is not found immediately, the Silver Alert will be issued, notifying all media sources of the importance of finding the individual as quickly as possible. The alert will contain a photograph of the person and state their name, description, age, relevant medical conditions, vehicle type if applicable and their last know whereabouts. If you had previously registered your loved one with Silver Alert, this important information will be on the USB drive in your possession that you already completed with the assistance of the Department of Senior Affairs. At that time, you will turn over the USB drive that contains this vital information to the Albuquerque Police Office working with you. Registration for Silver Alert is on-going. Keeping Your Loved One Healthy As a caregiver you can play a big role in keeping your loved one healthy. Following these caregiving tips can help to make sure they are feeling their best.

Doctors: Make sure your loved one gets to all his/her doctor appointments. Go to doctor appointments with him/her so you can keep tabs on treatment and be an advocate for them. Have a list of questions and concerns to discuss with the doctor beforehand. If you have noticed any new symptoms? Do any of the medications cause side effects? As the caregiver, you may notice things that your loved one doesn’t realize or forgets to ask about. Medication: Many seniors take a lot of medicines. It’s easy to get confused. You can simplify the process. Get a large, easy-toread weekly pillbox. Help set it up and keep it refilled. Using timers or alarms can be a big help to remind your loved one to take medicine. Activity: Exercise can improve his health, mood, sleep, and strength and lower the risk of falls. Try short walks around the neighborhood, or swimming at the community pools. Encourage your loved one to be active. Mental Health: Keep an eye out for signs of depression and anxiety or changes in mood. Don’t assume that problems will get better on their own. Talk to your loved one’s doctor about getting help.

Maintaining good nutrition can play a big part in staying healthy. When you shop for your loved one, choose nutritious foods that are easy to prepare. Bring over frozen homemade meals that can be reheated in the microwave or oven, clearly labeled and dated. Also you should take a look in the fridge and cabinets to make sure that food has not expired.

The Mechanics of a Heart Attack

attack results when a complete occlusion suddenly forms in the arteries that supply blood to your heart – the coronary arteries. Blockages are caused by a disease process throughout the arteries in your body called atherosclerosis, in which plaque – a fatty substance – builds up in the arteries. This plaque narrows the arteries, leaving less room for blood to flow. A plaque can be topped with a thin, fibrous cap that ruptures. With rupture, exposure of atherosclerotic debris to the bloodstream can cause platelets (a component of blood that assists with clotting) and red blood cells to collect at the site of the rupture, cutting off blood flow to the artery. Alternatively, part of the plaque may break off and flow downstream in the blood. This piece of plaque can then lodge in a narrowed portion of the artery and blood will begin to clot around it. This blood clot (thrombosis) can partially or completely cut off blood flow through the artery. When blood flow is cut off, this is called ischemia.

begins to die. That is why every second counts when it comes to heart attack treatment. An extensive blockage, especially in a major blood vessel, such as the left anterior descending artery, can cause a large heart attack. Large heart attacks that are not treated early and aggressively can lead to heart failure. The risk of death within five years of being diagnosed with certain types of heart failure can be 50 percent or more, worse than many forms of cancer.

Centennial Care: is the 52

name of the New Mexico Medicaid program. Centennial Care is

provided by three managed care organizations (MCOs). These services include physical health, behavioral health, long-term care and community benefits. Most people who are currently enrolled in a New Mexico Medicaid program are eligible for Centennial Care. Eligibility for all Medicaid programs requires that individuals meet certain federal guidelines. These include citizenship, residency and income requirements. To apply for Centennial Care coverage, you can complete an application on-line beginning October 1, 2013 at www.yes.state. nm.us. You may also apply in person at your local ISD office, by fax, or by mail. To download an application, go to ww.hsd.state. nm.us/isd/apply.html.
