Prime Time Publishing Company does an excellent job producing this resource for caregivers and the City of Albuquerque Department of Senior Affairs feels that this publication assists many in our community. Mayor Berry and I have been fortunate to serve the citizens in Albuquerque for the last eight years. The topic of caregiving is an important one as many of us will fulfill this role at one point in our life. According to the New Mexico State Plan for Family Caregivers drafted by the NM Aging & Long-Term Services Department and AARP, each year 419,000 New Mexicans provide more than 274,000,000 hours of unpaid family care. This statistic makes it clear that the Family Caregiver Resource Guide is necessary to assist caregivers in our community. The plan also states that trends indicate that New Mexico’s demand for family caregivers will continue to increase. The average age of care recipients was 80 in a 2014 New Mexico survey that was funded by AARP. The 80 and older population in New Mexico is projected to grow 80.5% between 2015 and 2030. As a result, demand for family caregivers is likely to rise. As this Administration transitions to the next, I am confident that the City of Albuquerque Department of Senior Affairs will remain committed to assisting caregivers. Fall 2017 Dear Readers, I’m happy to present to you the 2018 Family Caregiver Resource Guide. Very often, caregivers and those seeking to find a caregiver, feel as if they are on their own, struggling with questions and don’t know where to go to get help.

The Family Caregiver Resource Guide is helping to provide the answers to questions about caregiving and to direct people to qualified caregivers throughout the greater Albuquerque, Rio Rancho and surrounding areas.
This is the 12th year that we have published this guide, which is made possible by our partners - the City of Albuquerque Department of Senior Affairs (DSA) and our many advertisers. We offer the Family Caregiver Resource Guide in November to coincide with National Caregivers Month. It is distributed widely throughout Albuquerque and Rio Rancho. A separate guide is also available for Santa Fe and will be released In January 2018. Communicating With Doctors If you are a caregiver for a senior, you may often find that you are not getting the information you need about the senior’s health from a physician or health care provider. In some cases, the provider may not be aware of your role. But there are things you can do to ensure you are fully informed throughout the process. Here are some tips that can help improve communications:
Identify Yourself as a Caregiver Don’t just say, “I’m her daughter.” Be clear about your role so the provider knows why you are there. Say, “I’m her daughter, and I’m the one who takes care of her at home.” Get written permission for private information to be shared with you.
Many health care professionals will be uncomfortable letting you stay in the room during an exam or talking with you about your elder’s health because of privacy concerns. Have the elder sign HIPAA form(s) giving you the right to this information. The form stays in the elder’s chart so medical staff always see it. It is also prudent to provide a copy of your Power of Attorney for Health Care as well as a “do not resuscitate” form, if you have such documents. Most clinic and doctor’s office visits are short. When you make the appointment, request more time to discuss your concerns. Or ask for a consultation appointment. Finally, make sure you take a written list of questions to ask the doctor.

Family caregivers should ask for assistance with the costs of prescriptions. Assistance can include:
Questions to Ask About a New Diagnosis • What does this mean? What parts of the body might be affected? • Is this related to pre-existing problems? • What treatments are available? • Ask about alternative treatments like massage or acupuncture. • If it may improve, how long might that take? • How might this affect daily function? Other Questions to Ask a Doctor or Clinic Staff • Does the physician have a case manager who can assist the family/caregiver system? • When a patient is discharged from the hospital, is there a discharge planner to assist with follow-up plans at home? (Medicare requires formal discharge planning.) • A home health and safety evaluation consultation might be in order. An assessment can introduce a home health social worker to help broker and make linkages for the caregiver to other resources.

If you live separately from the person you care for and are concerned he/she may not remember to take medications: • Purchase a pill box with large compartments and lettering, and arrange medications weekly. • Call the care recipient when the medications are to be taken, and talk with her/him until they are all taken.
• The medical practitioner prescribing the most affordable alternatives, such as generics that have the same active ingredient(s) as its brand name. • Referral to the NM ALTS MEDBANK for assistance obtaining free or low-cost medications and counseling for Medicare programs such as the discount drug card and Part D prescription drug coverage. • Information about caregiver support group services.

Questions to Ask Doctor and/or Pharmacist About Medications Getting complete information about prescriptions so you are clear what they are for and how to administer them is critical. Some questions to ask are: • Why is this medication necessary? • What is the dose and how should it be taken? • What does it do? • How long will it take to have an effect? • If it doesn’t work, how would one know? • What are potential side effects? What should the response to those be? • Does the medication have contradictions with other meds/food? • Should the medication be taken with food or on an empty stomach? • Ask that prescriptions be written so some or all of them can be refilled at the same time, saving you multiple monthly pharmacy trips.

Manage Your Chronic Disease (MyCD) Chronic Disease Self-Management Education Programs Summary
The Manage Your Chronic Disease (MyCD) program falls under the New Mexico Department of Health’s Paths to Health NM umbrella and is comprised of five evidencebased self-management programs developed by Stanford University’s School of Medicine Patient Education Research Center.

The MyCD program include: the Chronic Disease SelfManagement Program (CDSMP), for adults with any chronic condition and caregivers; the Diabetes Self-Management Program (DSMP), for people with type 2 Diabetes and those who care for them; the Cancer: Thriving and Surviving (CTS) program for cancer survivors and caregivers; the Chronic Pain SelfManagement Program (CPSMP) , for people who suffer with chronic pain; and the Positive Self-Management Program (HIV) for people with HIV.

All MyCD programs are guided workshops delivered two and one-half hours, once a week, for six weeks, in community settings such as senior centers, churches, health care facilities, etc. These workshops are licensed through Stanford and are facilitated by two trained and certified leaders, one or both of whom are nonhealth professionals with chronic diseases themselves.
Subjects covered include: - techniques for dealing with problems such as frustration, fatigue, pain, and isolation; - appropriate exercise for maintaining and improving strength, flexibility, and endurance and preventing falls; - appropriate use of medications; - communicating effectively with family, friends, and health professionals; - nutrition, healthy eating, and weight management; and - how to evaluate new treatments. MyCD workshop sessions are highly participative, where mutual support and success build the participants’ confidence in their ability to manage their health and maintain active and fulfilling lives. MyCD programs do not conflict with an individuals’ existing health improvement program or treatment plan, but rather are designed to enhance regular treatment and diseasespecific education. MyCD workshops are FREE and run throughout the year.

More than 38,000 individuals in New Mexico suffer from Alzheimer’s disease or other dementia. Statistics show that about 60% of these individuals will wander at some time or another. The Albuquerque Police Department and the Department of Senior Affairs are working together through the Silver Alert program to ensure missing persons with Alzheimer’s disease or other dementia are quickly returned to their family or caregiver. Silver Alert is a public notification system to broadcast information about missing persons who suffer from Alzheimer’s disease of other dementia. The system notifies the public of a missing senior using television, radio, electronic billboards, reverse 911 and traffic signs. The public is asked to call a phone number if they see the missing person. Individuals who are 60 or older and suffer from Alzheimer’s disease or other dementia are eligible to register for Silver Alert. The Department of Senior Affairs will place vital information and a photo of the individual on a USB drive and give the drive to the family member/caregiver who accompanied the person to the Silver Alert registration. When the family member/caregiver determines that their loved one is missing, they are asked to call 242-COPS. An officer from the Albuquerque Police Department will come to the residence, or the place from which the person is missing, and initiate an investigation. If the person is not found immediately, the Silver Alert will be issued, notifying all media sources of the importance of finding the individual as quickly as possible. The alert will contain a photograph of the person and state their name, description, age, relevant medical conditions, vehicle type if applicable and their last know whereabouts. If you had previously registered your loved one with Silver Alert, this important information will be on the USB drive in your possession that you already completed with the assistance of the Department of Senior Affairs. At that time, you will turn over the USB drive that contains this vital information to the Albuquerque Police Office working with you. Registration for Silver Alert is on-going. A heart
The Mechanics of a Heart Attack
attack results when a complete occlusion suddenly forms in the arteries that supply blood to your heart – the coronary arteries. Blockages are caused by a disease process throughout the arteries in your body called atherosclerosis, in which plaque – a fatty substance – builds up in the arteries. This plaque narrows the arteries, leaving less room for blood to flow. A plaque can be topped with a thin, fibrous cap that ruptures. With rupture, exposure of atherosclerotic debris to the bloodstream can cause platelets (a component of blood that assists with clotting) and red blood cells to collect at the site of the rupture, cutting off blood flow to the artery. Alternatively, part of the plaque may break off and flow downstream in the blood. This piece of plaque can then lodge in a narrowed portion of the artery and blood will begin to clot around it. This blood clot (thrombosis) can partially or completely cut off blood flow through the artery. When blood flow is cut off, this is called ischemia.

is a muscle. Blood carries vital oxygen and nutrients to the heart muscle, and without blood, the heart muscle begins to die. That is why every second counts when it comes to heart attack treatment. An extensive blockage, especially in a major blood vessel, such as the left anterior descending artery, can cause a large heart attack. Large heart attacks that are not treated early and aggressively can lead to heart failure. The risk of death within five years of being diagnosed with certain types of heart failure can be 50 percent or more, worse than many forms of cancer.

because the consequences of an untreated heart attack are so great. If your symptoms persist for more than 15 minutes, you are at more risk that heart muscle cells will die. It is critical for you and your heart that you receive immediate medical attention. To receive the best care, you have about 90 minutes from the onset of the heart attack for an interventional cardiologist or surgeon to restore the flow of blood to the heart before critical heart tissue dies or is damaged.

inclusive Care for the Elderly (PACE) Generic term for day health centers that include a medical clinic, adult day care, social services, rehabilitation, transportation, in-home services and inpatient care (when needed). In Bernalillo County, this service is available through InnovAge. To qualify for PACE, the senior must be aged 55+ and live within the county or within 30 minutes travel from center. Free for recipients of both Medicare and Medicaid. Private pay also accepted.
POA: Power of Attorney – a legal form designating someone else to act on your behalf if you become unable to chose medical care for yourself.

LIHEAP: Low-Income Home
government health insurance program that is administered by the state where eligibility is determined by the states established criteria. It is for people with low income and limited assets. Medicaid covers medical and personal care options, along with benzodiazepines, behavioral health drugs, not covered by Medicare. With prior approval/ authorization, it may also cover eyeglasses, hearing aids and dental work.
