Community Care LCF

CCLCF is transforming the healthcare experience by reshaping how care is delivered rooted in local communities, guided by clinical expertise, and focused on efficient, high-quality, wholeperson care.

Since 2003, we’ve provided proven, locally based care management across SENC by partnering with prepaid health plans, Accountable Care Organizations, practices and providers to improve health outcomes

We thank our partners for their continued support and are excited to plan new, collaborative work with The Endowment as we strengthen a coordinated Social Care Network for our region.

Through early November, we’ve opened 350 Healthcare Navigation cases with Help Hub clients, surpassing our total for all of 2024! Our 5 year milestone in Help Hub and the growing demand reflects both rising community need and the trust people & families place in our services

Help Hub

For 10 years, neighbors in crisis have called on Help Hub for financial support, navigation to critical resources, or just a kind, listening ear.

Since 2015, residents in our area have known that when a crisis hits whether it’s a sudden rent increase, a medical emergency, a job loss, or a car that breaks down at the worst possible moment they can call on our Help Hub.

We are often the difference between stability and homelessness, between keeping a job or losing it, between living with an unmet healthcare concern or getting important answers and care

What we provide is about so much more than financial assistance. It’s dignity. It’s breathing room. It’s the chance to regroup, rebuild, to believe that your community hasn’t forgotten you

Supporting Help Hub provides direct support to your neighbors. Whether we are paying the landlord, the pharmacy, the car repairman, or the utility company, be assured that your dollars are put to essential and urgent use.

Thankful Hearts Breakfast was a wonderful celebration of a decade of impact at Help Hub. Together, we've invested over $3.6mil into 14,668 homes, providing essential stability for our neighbors. Congrats to our incredible leader, Sonja McFarland for 10 years!

Thankful Hearts Lunch program successfully highlighted how The Harrelson Center advances, connects, and empowers an integrated community of humanitarian nonprofits. We were thrilled to host almost 400 dedicated partners and supporters!

We're $33,025 away from reaching our goal! Let's close this gap and meet the growing needs of our community and our partners!
