The Caring House Skilled Nursing & Rehabilitation

Admissions / Business Office Hours

The Caring House is a skilled nursing facility that provides compassionate and nurturing care for the Community. Services include long-term care, skilled nursing care, respite care, end-of-life or hospice care, specialized rehabilitation therapy, alternative therapies and dialysis.

Dialysis provides hemodialysis services to patients with End Stage Renal Disease. Qualified medical providers oversee dialysis treatment for individualized patient care, to include: monitoring medication regimen, dietary requirements, electrolytes, and other disease management.

Patient-Centered Care Team

Gila River Health Care’s Patient Centered Care Team model of care involves working closely with patients and their families by recognizing their unique needs by respecting their culture and beliefs.

Your Patient-Centered Care Team will provide:

Patient-centered care: We build a relationship with each patient, and their care focuses on the whole person and understanding and respecting each patient’s needs, culture, values, and preferences.

Comprehensive care: The care team works to meet each patient’s acute care and chronic care health needs.

Superb access to care: Patients have access to services with shorter waiting times for urgent needs, enhanced in-person hours, and around-the-clock telephone access to members of their care team.

Our approach to quality and safety: Gila River Health Care uses evidence-based medicine and clinical decision support tools, engages in performance measurement and improvement, measures and responds to patient experiences and satisfaction, and practices population health management.

Coordinated care: Care is coordinated across the broader health care system, including specialty care along with community and support services.

Emergency Medical Services

Gila River EMS (GREMS) was established in 1974 as a community-based emergency medical services department, acting as the main responder for various emergency situations across nearly 600 square miles of the Gila River Indian Community. As part of Gila River Health Care, GREMS is essential to both public safety and healthcare. We work closely with our public safety partners, including Fire, Police, Corrections, Dispatch, and Emergency Management, and we lead out-of-hospital patient care for the Community. We take this responsibility seriously, hiring only skilled individuals dedicated to serving with respect and dignity while delivering the highest quality of care.

Patient Complaint and Grievance Process

1. The complaint is received and assigned to the GRHC Director or his/her designee for investigation.

2. The Director will then review the GRHC employee response and any other pertinent information.

3. The Director will submit a written response to the CCSD Patient Advocate. This response will include complaint details relative to causes, solutions, and any other action taken to prevent reoccurrence.

4. For service recovery, a follow-up telephone call by the Director or designee will be made to the patient regarding the complaint.

5. The Director’s response is expected within 14 days. If there is no response from the Director, the patient complaint will continue up the chain of command.

6. If the patient feels the issue was not resolved, they may elect to formalize the complaint as a written grievance.

The Pharmacy staff is dedicated to provide quality patient care. The Pharmacy Department offers comprehensive clinical pharmacy services for the Gila River Indian Community and other federally recognized tribes. The goal is to ensure that patient’s medications are the most appropriate, most effective, safest possible, and used correctly for the most positive outcome.

Purchased Referred Care

The Purchased Referred Care Department provides appointment scheduling for offsite providers for patients eligible for Contract Health Services (CHS). Federal guidelines and eligibility criteria require that all bill-paying services must be coordinated through the Purchased Referred Care Department.
