CEO Report Grace Health was established in June 2008 and is a 501(c)3 non-profit Federally Qualified Health Center (FQHC) serving the southeastern Kentucky counties of Bell, Clay, Knox, Laurel, Leslie, and Whitley. What started as two Community Health Center sites in Gray and Manchester, Kentucky has since grown to include 11 satellite clinics employing over 300 community members, including more than 50 providers, and caring for 30,000+ people each year. Grace Health offers a variety of services, including primary care, women's care, dental care, pediatrics, pharmacy, behavioral health, school-based health, x-rays and labs, and outreach and education. The center offers compassionate and integrated care that is coordinated by primary care practitioners to provide medical, preventative dental, and behavioral health care, as well as 340B discounted pharmacy for all patients regardless of their ability to pay. Grace Health operates 32 school-based health centers in Leslie, Bell, Knox, and northern Whitley counties, providing medical and preventative dental services to students in those areas. The poverty rate in southeastern Kentucky is 33.7%, double the state and national average. Over 50% of the population in southeastern Kentucky is unemployed or no longer seeking employment. The decrease in Kentucky's coal mining industry and the increasing incidence of substance use disorder have fractured many families and negatively affected employability in the service area. OUR HISTORY
As a patient-centered medical home, Grace Health meets the needs of our patients’ physical and mental health. This includes preventative and wellness care, acute care, and chronic disease management.

Hyden Dental Clinic: Adding a dental clinic with six dental operatories to add oral health services for Leslie County. This clinic opened on September 6, 2022. Manchester Dental Clinic: Adding a dental clinic with six dental operatories to add oral health services for Clay County. This clinic is scheduled to open in November 2022. Medical Campus: Renovating/transforming the existing 88,000-square-foot former retail building into a state-of-the-art medical campus covering nearly 2 acres. Pineville Clinic: Purchasing property and constructing a new facility to house integrated medical, behavioral health, dental, and pharmacy services. Will utilize $1M in USDA Emergency Rural Health Care Community Facilities funding.

Estimated Community Impact in First Year $313,400 To expand services, Grace Health will be adding dental clinics to several locations, including the Hyden Clinic in Leslie County, Kentucky. The need for dental providers in Leslie County is overwhelming, with there being only one dentist per 3,210 Leslie County residents compared to the state rate of one per 1,500 residents. Leslie County ranks 108 out of 120 counties in overall health and healthy behaviors, with nearly 26% of overall residents meeting the federal poverty guidelines. The childhood poverty rate in Leslie County is 32%, far exceeding Kentucky's state rate of 19%. Residents of Leslie County have reduced access to dental care, high rates of poverty, experience poor integration of dental services, and are under or uninsured. These factors combine to create critical barriers to accessing care in this area. Grace Health determined it was essential to open the Hyden Dental Clinic to reduce the barriers to care in Leslie County. This clinic is equipped with six, state-of-the-art dental operatories and employs 4.5 full-time employees, including a dentist, a dental hygienist, and several dental assistants. Manchester Clinic Expansion Clay County, Kentucky Grace Health is investing $888,648, including $500,000 from the American Rescue Plan, to alter and renovate the Manchester Clinic, which will add six dental operatories to the current medical, behavioral health, and pharmacy services offered at the Manchester Clinic. The addition of oral health services at the Manchester Clinic will improve access to care for current and future Grace Health patients. The Manchester Dental Clinic will create multiple jobs in an area that has consistently struggled with poverty and limited access to dental care. Construction began in the summer of 2022. Grace Health anticipates providing dental services in the fall of 2022 at the Manchester Clinic.

Medical Campus Corbin, Kentucky
The Grace Health Medical Campus will be completed in the early summer of 2023. In renovating and transforming an existing 88,000-square-foot former retail building, we will bring a state-of-the-art medical campus to rural southeastern Kentucky. This endeavor allows Grace Heath the opportunity to respond to unmet medical, dental, mental health, and substance use treatment needs in Knox and Whitley Counties.
Available space will increase from 46,000 square feet and 53 exam rooms to 88,000 square feet, 85 exam rooms, and 11 dental operatories. Additionally, we expect to see significant growth in patients, visits, and full-time employees.

Pineville Clinic Bell County, Kentucky Grace Health is investing over $4.6M to build a new, comprehensive, 10,000 sq. ft. facility in Pineville, Kentucky. The facility will expand current services in Bell County, bringing patients access to integrated medical, dental, behavioral health, and pharmacy services in one location. Grace Health will use this clinic to broaden access to testing, vaccinations, treatment, and access to integrated healthcare to improve health outcomes in Bell County, KY. In addition to the increased access to healthcare, the new facility will create new jobs in the area, meet the demand for in-house dental services for our underserved population, and help prevent the community spread of preventable viruses, such as COVID-19.

Assistance Programs Grace Health focuses on treating the whole person, regardless of the ability to pay, and offers multiple programs to help patients receive and afford the care they need, including the 340B Pharmacy Program and the Sliding Fee Discount (SFD) Program. The SFD Program offers discounted medical and pharmacy services through a sliding fee schedule to patients who qualify based on household size and gross income. The 340B Pharmacy Program provides prescription discounts to patients who are uninsured, have an income that is at or below 200% of the Federal Poverty Level, or already qualify for the SFD Program.

Grace Health
Outreach In 2021, the Grace Health Outreach Team was blessed to help over 900 people enroll or re-enroll in health insurance during 80 events across the six service area counties. Despite ongoing COVID-19 restrictions and challenges, the team found creative ways to connect, inform, and serve Kentucky during the pandemic. They participated in giving ministries like the Empty Stocking Fund, White Flag Ministry, and the Western Kentucky Tornado Relief Program, as well as large events like the 13th Region Basketball Tournament, NIBROC, the World Chicken Festival, and a multitude of school-based gatherings.

In 2021, Grace Health had 126,137 total patient encounters, a 9.6% increase from 2020. Of all patient encounters in 2021, 37.84% were Medicaid with 10,823 new patient encounters.

Working with 16 different programs from 11 different universities, 23 Grace Health providers instructed 52 students as they completed rotations in 2021. In addition to local universities such as Eastern Kentucky University, University of the Cumberlands, and Lincoln Memorial University, students also came from South College in Knoxville, TN, and Marshall University in Huntington, West Virginia.

Of the providers employed with Grace Health in 2021, almost 50% set aside additional time to work as preceptors for students training to be physicians, nurse practitioners, physician assistants, pharmacists, and more. The Whitley County service area hosted the majority of rotations for 2021 at 25 with Knox County being next at 14 rotations in 2021.

Grace University Grace Health offers two certification programs through Grace University (Grace U) to aid participating employees in increasing their knowledge in patient care, earning credentials, and becoming more versatile employees. Available courses through the National Healthcare Association (NHA) are the Certified Medical Administrative Assistant (CMAA) program and the Certified Clinical Medical Assistant (CCMA) program. To minimize the financial burden on employees, Grace Health fully covers the cost of the programs, pays the employees during their class time, and provides financial incentives for successful certification, including bonuses and pay increases.

In 2021, NHA administered 44,588 CCMA exams with a national pass rate of 77.08% and 10,211 CMAA exams with a national pass rate of 68.74%. In the same year, Grace U had 24 people (nine CCMA and 15 CMAA students) complete certification programs with a 100% pass rate on their exams. Since the inception of Grace U in 2017, Grace Health employees have earned 240 certifications with an average pass rate of 99%. Despite COVID-19 restrictions affecting the number of classes offered in 2020-2021, Grace U had an overall pass rate that is 20% above the national rate.

Building a healthcare team focused on you is more than just a motto. The administrative authority for the operation of Grace Health is provided by a voluntary Board of Directors. A unique feature of Grace Health’s Board of Directors is that a majority of its members are either patients or clients who are Impact Employment current users of FQHC’s services. The remaining members serve as constituent representatives for the different communities in which Grace Health operates a Health $21.97 Million Center. These individuals are selected for their expertise in health care, community affairs, local government, banking and finance, legal affairs, social services, trade unions and other commercial and industrial concerns.
