aking the correct decisions regarding health care isn’t easy. Even defining what “healthy” is can change depending on what study you read or which website you visit. That’s why it’s so important to have access to multiple sources; your family physician is a good start. If your doctor gave you a recommendation that would benefit your health, you’d trust his or her judgment, right? Now imagine if you could ask thousands of doctors for their opinions. That changes the game a bit, although perhaps the thought of sitting in all those waiting rooms doesn’t sound especially appealing. That’s why our ninth annual “Top Doctors” issue of Health Care Quarterly is so useful. Starting in December, we began the process of reaching out to a broad section of medical professionals in Southern Nevada, mailing them directly to ask them to tell us which doctors they recommend to their patients. Are you sure your blood pressure is in line with the current numbers? By Daniel Heller

rowing up, many of us learned that 140 over 90 is the magic number for blood pressure. If you’re over this figure, your blood pressure is high; if you’re under this figure, you’re maintaining healthy levels. What you might not know is that evidence emerged that has completely abolished this standard. In 2015, a study known as the Systolic Blood Pressure Intervention Trial was published in the New England Journal of Medicine and funded by the National Heart, Lung, and Blood Institute. The trial revealed some eye-opening data regarding high blood pressure, also known as hypertension. The main takeaway is that individuals who maintained a blood pressure level of 120 over 80 or below, as opposed to a level of 140 over 90, had a much lower risk of developing a cardiovascular condition such as stroke, heart attack or heart failure. This led the medical community to adopt the new figure of 120 over 80 as the healthy standard for blood pressure. Depending on the margin by which a patient is over this figure, physicians may prescribe several solutions. If a patient’s blood pressure is in the range of 120-129 over a number that is less than 80, that now qualifies as an elevated level. At that point, lifestyle changes are recommended to bring that number down. The recommended lifestyle changes to reduce blood pressure include weight loss for patients who are overweight or obese, adopting a heart-healthy diet, increasing potassium levels (preferably through diet modification), increasing physical activity, limiting of alcoholic drinks and reducing sodium intake. Obviously these recommendations should be taken with a grain of salt (pun intended), and what works best for one person may not be the case for everyone. Adopting an extreme regimen to reduce blood pressure isn’t always a sustainable option and may leave a patient burned out after a short period of time. The main thing to avoid is regressing to previous unhealthy habits. Any step taken to reduce blood pressure is a positive one, and using the marathon approach

of starting with small changes and ramping up to a healthier lifestyle can be just as effective in the long run. Another positive change a patient can make to reduce blood pressure is to stop smoking tobacco if he/she is a smoker. Cessation of tobacco smoking is the No. 1 thing a patient can do to improve cardiovascular health and overall well-being. Quitting is certainly difficult, but it can’t be emphasized enough how beneficial this change can be to one’s physical condition. Once a patient’s blood pressure moves into the range of 130-139 over 80-89, this qualifies as stage 1 hypertension. At this point, physicians are recommended to perform an assessment to determine the patient’s risk of developing a cardiovascular condition in the next 10 years. If this risk is greater than 10%, guidelines recommend the patient receive blood pressure medication and continue to make lifestyle changes. If this risk is less than 10%, no blood pressure medication is required but lifestyle changes should be implemented or continued. Moving into the range of 140 over 90 or greater, no assessment is needed and blood pressure medication plus lifestyle changes are recommended. This figure, now classified as stage 2 hypertension, used to be known as a healthy blood pressure level under the old guidelines. Additionally, 1 in 3 (78 million) adults in the United States now have high blood pressure according to these new guidelines. Not being proactive in reducing high blood pressure puts a person at more risk of stroke, heart attack and heart failure, as well as less obvious conditions like kidney disease. You may think you’re in the clear since high blood pressure usually is symptomless, but it doesn’t hurt to visit your physician to check your current blood pressure level and make sure you’re doing everything you can to lead a healthy, happy life.

ith the weather warming up in Southern Nevada, it is important to focus on actions and activities that maintain a healthy lifestyle. In extreme weather temperatures, safe and healthy habits should be a top priority. There were 45 more heat-related deaths in Clark County in 2017 compared to the previous year, with 40 percent of the incidents happening in July, according to the Southern Nevada Health District. improved record sharing will help enhance care for nevada’s veterans By Michael L. Gagnon

ealtHIE Nevada is making another major stride in connecting all health-care providers in the state to a health information exchange. We are excited to announce that we will be providing that valuable service to our military veterans by connecting to the United States Department of Veterans Affairs to share clinical information between Nevada community providers and VA medical facilities nationwide. HealtHIE Nevada is the statewide health information exchange that provides a secure network to help health care providers share electronic health records, or EHRs. According to the latest U.S. Census Bureau statistics, there are more than 226,000 veterans in Nevada. Many receive care both in the VA system and from non-VA community providers. With HealthHIE’s new connection, providers at the VA will have access to clinical patient information from organizations such as Renown Health, Dignity Health, Saint Mary’s Medical Center, UMC, Valley Health System, Southwest Medical Associates, Steinberg Diagnostics, Desert Radiology, Reno Diagnostic Center and several more. When a veteran seeks treatment from a community provider, those providers will also have access to medical records that are contained in the VA system. This type of connection is essential to ensure that our veterans have equal access to medical care whether at the VA or in the community. Without this type of connection, veterans would have to collect and transport their own paper records to all their providers. The connection allows all VA and community doctors and nurses to view patient events, laboratory results, radiology reports, medications, discharge information and care summaries all in real time. Veterans can expect this new service to be available in July of this year. We’re honored to help our veterans by allowing doctors and other health-care providers access to records in a seamless and secure manner.

A quick HIE history Health information exchanges have been around for more than 20 years. The early health networks usually connected hospitals and laboratories because those were some of the

only health-care organizations that had electronic systems and could share data. The federal government, specifically the Centers for Medicare and Medicaid Services, realized that to improve the quality of care and control costs the U.S. health care system needed to embrace and implement electronic systems. President George Bush kickstarted the effort in 2004 when he created the Office of the National Coordinator for Health Information Technology. In 2009, the Health Information Technology for Economic and Clinical Health (HITECH) Act was enacted. The HITECH Act provided almost $36.5 billion to create a nationwide network of electronic health records and provided incentives for hospitals and providers to install EHRs. The act met its goal of getting health care organizations to install medical records but fell short on creating a nationwide network that would make those EHRs interoperable. Interoperability is when two systems can talk to one another without human intervention. The banking network of automated teller machines, for example, does not work by each bank creating a connection to all other banks. Rather, the bank connects to an inter-bank network such as Cirrus, Plus, MNET or NYCE to provide connectivity to other banks and to other networks. In the case of EHRs interoperability would be, for example, when a patient’s health record from a hospital stay could be sent to the patient’s primary care provider and all the clinical information could be incorporated on that end. HealtHIE Nevada and all HIEs provide the “inter-EHR” network that allows for interoperability between EHRs. This new service is designed to enhance care coordination between VA and local health care facilities. While veterans do not have direct access to the clinical data in the health information exchange, they can make their providers aware of this new service and request that community providers participate.

Weâ€™ve done the research and legwork, mailing surveys to approximately 4,500 doctors, dentists and medical professionals in Southern Nevada. What follows on these pages are the names of specialists who have been recommended by their peers.

FAQ What was the survey? It wouldn’t be out of the ordinary for you to ask your general practitioner for a referral to another doctor. To compile this list, we did that thousands of times. We invited doctors to fill out a confidential survey, asking for their recommendations in the specialties we listed. Our logic: Local doctors are the most qualified to judge other medical professionals in Southern Nevada. It was not necessary for the respondents to offer a name in each specialty. Who got the survey? We got our list of names from state boards for doctors, osteopaths and dentists. Those who have a Southern Nevada mailing address on file with either of those agencies received a survey earlier this year.

So, is this a list of every doctor who was nominated? This is a list of doctors who were nominated multiple times, checked against records with state medical boards, to ensure that all are in good standing. Why isn’t my doctor listed? This list is by no means a complete one; there are many excellent doctors in Southern Nevada, and there are many different ways to define a “top doctor.” If you like your doctors, you should probably continue seeing them — and remind them to keep an eye out for next year’s survey!

I’m a doctor but I didn’t get my survey in the mail. How come? We do the best we can on our end to ensure that the letters arrive at the proper addresses. Using the information we get from state medical boards, we cross reference that with postal records. After that process, however, a number of obstacles could prevent the letter from reaching your hands. For instance, the post office could deem the letter undeliverable (missing suite address or incomplete information).

Can doctors vote more than once? No. Participants are given a unique code that our system tracks. Duplicates are invalidated.

Does that mean this list is nothing but a popularity contest? We encourage the doctors to give some thought to their answers, but we have no way of enforcing it. Doctors will answer with the names of the colleagues they know and trust. Doctors who have

A long-time Nevadan, Michael Daubs, M.D., has worked in medicine for over 20 years. Dr. Daubs is a people person who loves science, and he combines those in his medical practice. When it comes to the potential nutrient deficiencies, special attention needs to be paid to protein, iron, zinc, calcium, vitamin D, fatty acids and vitamin B12, among others. Protein needs vary throughout life but a mild deficiency can result in muscle wasting, an inability to increase lean muscle mass, decreased immunity, bone loss, hair loss, brittle nails, weight gain and inadequate oxygenation to tissues. Adequate protein intake isn’t a mere matter of getting enough; proteins need to be broken down into amino acids before they can be used by the body. That takes certain enzymes but plants contain several compounds that inhibit those enzymes, meaning that a vegan’s body may not be getting all the protein it needs. Sometimes the process is a mere matter of digestibility; soaking or sprouting your nuts, seeds, legumes, and grains can help. Speaking of amino acids, don’t forget about them when planning a vegan diet. Nine are considered essential because they are not produced by the body: histidine, isoleucine, leucine, lysine, methionine, phenylalanine, threonine, tryptophan and valine. Non-essential amino acids (those that are produced by the body) may be needed in increased amounts during times of stress. These

include arginine, cysteine, glutamine, glycine, proline, taurine and tyrosine. Be aware that all plant proteins are low in methionine, leucine, lysine and tryptophan. That can be helped with soy, dry beans, peas, nuts, seeds, whole grains and cruciferous vegetables. All nine essential amino acids can be found in soy, amaranth, buckwheat, quinoa, hemp seeds and chia seeds. To ensure you’re getting enough protein, refer to the chart on the preceding page. It compares serving size, calories, protein, carbohydrate and fat contents. For perspective, some animal proteins have been included. Iron may be an issue for children, teens, women of childbearing age, and pregnant women who follow a vegan diet. Plantbased foods contain non-heme iron, a form that is not as well absorbed as heme iron, the form found in meat and other sources from animal origins. In addition, beans, nuts, seeds and grains contain phytates, compounds that are significant inhibitors of iron absorption. You can improve the absorption of non-heme iron by including fruit juice, broccoli or bell peppers, which are a great source of vitamin C. Zinc, a mineral essential for many body processes, may come up short in vegetarian and vegan diets. As with iron, many plant-based foods contain zinc, but its

bioavailability is less than that of animalbased foods. By eating whole-grain foods, different types of beans and peas, fortified breakfast cereals, seeds and nuts, you can reach zinc goals. Focus on whole-grain bread and pasta because zinc is lost when processing grains to make refined flour. Speaking of bread, phytates can inhibit zinc absorption but get broken down when yeast fermentation is used to make dough rise. Know that supplementing a diet with zinc is potentially harmful as doses can be high and can disrupt the balance of iron and copper. Calcium intake among vegans can vary widely; some low-dose supplementation may be required. Foods that provide a more adequate source of calcium are tofu and tempeh processed with calcium, calcium-fortified soy or rice beverages, broccoli, almonds, soybeans, some greens (kale, collards, mustard greens, turnip greens), okra, bok choy and figs. Vitamin D status depends on sunlight exposure and the intake of vitamin Dfortified foods or supplements. The most common

method of getting vitamin D in the diet is by consuming fortified milk and milk products. For vegans, because dairy is eliminated, another source of vitamin D (which helps absorb calcium) is required. Look at a food’s nutrition facts to see its vitamin D content. Some breakfast cereals; plant-based beverages such as soy, rice, almond, and hemp; cereal bars, and calcium-fortified juices are also fortified with vitamin D — though it is often the inactive vitamin D2 which the body must convert to vitamin D3. Mushrooms that are treated with ultraviolet light can be a significant source of vitamin D. If sunlight exposure and intake of fortified foods is inadequate to meet recommended needs, then a vitamin D3 supplement is recommended, especially for older adults. Essential fatty acids are called such because the body cannot produce them; they are an essential part of the diet. People following a vegan diet may be lacking some fatty acids, although not necessarily and the clinical relevance of the reduction among vegetarians and vegans is

unknown. Good plant sources of omega-3 fatty acids are seeds (flax, chia, camelina, canola and hemp), walnuts and their oils. Vitamin B-12 is abundant in foods of animal origin. Vegans must regularly consume foods fortified with B-12 or take supplements. People with little or no intake of B-12 may feel healthy; however long-term deficiency can lead to stroke, dementia and poor bone health. ••• With research, planning and dedication, a vegan diet can provide the required nutrition one needs. Focus on whole foods and make sure to eat a wide variety of foods on a daily basis to maintain an adequate nutritional intake. If you decide to eliminate animal products and feel that your diet may have deficiencies, seek advice from your doctor or registered dietitian before taking supplements. Adding more plants into our diet is for the best, and as registered dietitians we encourage everyone to explore their diet and see how incorporating more plants will work best for them.

ost people seeking help for drug or alcohol addiction are familiar

with traditional rehab and therapeutic counseling; there are also

many community-based programs. Underscoring the urgency for treatment, a 2018

study from the Centers for Disease Control and Prevention showed that death rates in Nevada from psychostimulants, which include methamphetamines, ecstasy and ADHD drugs such as Ritalin, — hit 7.5 per 100,000 in 2016, up 32 percent from the previous year. The national average for psychostimulant deaths hit 2.4 per 100,000 in 2016. Those battling addiction should consider preference, comfort and compatibility when selecting a program. Free and low-cost addiction recovery resources available in Southern Nevada include:

One of the most well-known rehabilitation programs, Alcoholics Anonymous pairs addicts with the support of “sponsors,” other AA members who serve as mentors and support systems for those who have decided to discontinue addictive behaviors. Established in 1935 in Akron, Ohio, by Bill Wilson and Dr. Robert Smith, this 12-step program offers a safe space for drug and alcohol users to verbally work through their recovery in a judgment-free environment. As with any program, AA has pros and cons. AA is easy to find and is split into different forms, such as Narcotics Anonymous and Emotions Anonymous. However, many struggle with its abstinence-focused approach, rigidity, time requirements and religious undertones. Still, AA has become a household name, so people often choose it over other forms of clinical interventions from which they also might benefit. The AA community itself is also not heavily regulated, which can lead to some unhealthy relationships between those new to recovery and others who may seek to take advantage or are simply not a healthy influence. Studies indicate that around 20 percent of AA members find sobriety within 1-5 years of entering the program. For more information about AA, call the Southern Nevada AA Hotline at 702-598-1888.

Moderation Management Moderation Management focuses on alcohol abuse and addiction with a slightly different approach, teaching moderation over abstinence, but ultimately helping people discover which option benefits them most. Although the program does not “require” abstinence or total restraint from alcohol, it does ask members to stay sober for a month before attempting to practice “safe and responsible drinking habits.” The research does not indicate what is considered to be “safe and responsible” alcohol consumption, according to MM, but the program focuses on teaching people their limits and is governed by four values:

Taking personal responsibility for their own recovery. People helping people is the strength of the organization.

❸ People who help others often discover their own path to recovery.

❹

Members treat each other with respect and dignity.

SMART Recovery Formally known as the Alcohol and Drug Abuse SelfHelp Network, SMART Recovery changed its name in 1992. Founded by Joe Gerstein, it stands for Self-Management And Recovery Training and is a highly recommended alternative to traditional 12-step recovery programs. SMART Recovery focuses on four distinct areas aimed at shifting mental patterns that lead to all types of addictive behaviors, not just alcohol or drugs:

❶ Building motivation and maintaining it over time. ❷ Coping with urges. ❸ Managing feelings through problem-solving. ❹ Learning how to live a balanced life. The nonprofit program includes in-person counseling as well as an online message board that is available 24/7. This combination of face-to-face and online communication is offered free of charge all over the country. The biggest difference between SMART Recovery and AA is that SMART does not carry the religious undertones that AA does and is not limited to specific types of addiction and abuse. Additionally, there is a strong mental health and clinic focus with SMART. One of SMART’s disadvantages is that, unlike AA, it does not offer new members the comfort of a sponsor. Instead, they have the option to gain support online and through meetings, but there is no one person “assigned” to support them. Although AA teaches that addiction is forever, SMART teaches its members that addiction is a disease that can be cured with consistency and determination. In addition to the four points previously mentioned, SMART also proposes six stages of the recovery process. Once a member has evolved through each stage, they have a “graduation” that symbolizes their overcoming an addictive behavior as they enter a new stage in their life’s journey. These six stages are:

❶ Precontemplation: Acknowledging the addictive behavior that lies in the subconscious.

❷ Contemplation: Discovering the advantages/ disadvantages of addictive behaviors.

❸ Determination/Preparation: The decision to pursue

personal changes and start the SMART Recovery plan.

❹ Action: Seeking out new ways to handle/cope with addiction, including self-help and group support.

Secular Organizations for Sobriety Secular Organizations for Sobriety focuses on addictions to drugs, alcohol and food. Similar to AA, S.O.S. preaches abstinence. While many similar programs are rooted in religious values, the group prides itself on the inclusion of all religions and backgrounds and believes the best way to accomplish sobriety is through peer support. S.O.S. has meetings around the world and offers its services free of charge, although it does accept donations. The short film “No God at the Bottom of a Glass” gives an inside look at addiction and how S.O.S. works to help people fight it. For more information about S.O.S., call 323-666-4295.

LifeRing Rather than relying on a higher power or entity, LifeRing teaches its community about the concept of duality, which means that each person is an addict and a non-addict and that the key to sobriety is deciding which energy will win the battle. LifeRing holds individuals responsible for both the good and bad choices they make as it pertains to their recovery and sobriety. The program offers in-person and online treatment, which can be customized to be completely confidential if discretion is desired. When starting the program, members must first find a meeting location and choose a weekly time slot. A handbook allows them to journal about their weekly experiences. In small groups during meetings, they are encouraged to share their issues or concerns pertaining to addiction. And, unlike other programs, members are asked to “be present” and talk about what’s happening to them now rather than what addiction has done to them in the past. The program is free and can be accessed in-person or online. Donations are accepted but not mandatory after meetings. LifeRing requires a bit more independence and responsibility than other programs, which can be good or bad depending on the individual. It does, however, highly encourage members to seek out and encourage the “S” (sobriety) in other members and to connect through positive commonalities rather than negative ones such as drinking and drug use. LifeRing has no formal “graduation” process but the access to online meetings is a big plus for those unable to commit to in-person meetings every week. ❺ Maintenance: Movement away from negative behavior, leading to positive and noticeable change.

❻ Graduation/Exit: Once sustainable change has been noticed, a member can choose to “graduate” from SMART Recovery. This choice gives them the opportunity to be honest with themselves as to whether they are ready to move on. For more information about SMART Recovery, call 866-951-5357.

Local Patient is the first in the world to receive prostate cancer drug By AnaArlene Ramirez

early 10 years ago, Las Vegas resident Anthony Brasich was between jobs, wasn’t feeling well and was incredibly reluctant to see a doctor. When he eventually mustered up the courage to see a physician, the doctor discovered a blood clot in his lungs, his prostate-specific antigen count was more than 5,000ng/mL (whereas the normal count is 4ng/mL) and he was diagnosed with stage 4 advanced metastatic prostate cancer. He was given two months to live and was referred to Dr. Nicholas Vogelzang at Comprehensive Cancer Centers. Flash forward to 2019: Brasich, 71, is still with us, having been given a number of different treatment options including chemotherapy, immunotherapies, radiopharmaceutical drugs, clinical trials and natural remedies. Just a few months ago, when a form of hormone therapy left him feeling low on energy, he was ready to try something new. That was a Phase I clinical trial drug known as ARV-110, which targets and destroys androgen receptor (AR), a hormone receptor protein on cancer cells. Genetic signals from the receptor protein are key contributors to a resistence to treatment. ARV-110 degrades AR, thereby regulating the growth signals to cancer cells and controlling disease progression. In April, Brasich received the first dose of ARV-110, making him the first patient in the world to receive this treatment. Before that hap-

pened and as with any clinical trial, there was a rigorous process for Brasich to qualify — including a deep dive into his medical records, scans, PSA counts and blood tests. Given the nature of a Phase I trial, Brasich must be monitored carefully for any side effects or adverse reactions. During a typical appointment, he takes several blood tests, undergoes EKGs and consults with his physicians before a dose is administered. The drug itself comes in pill form and may be consumed in a matter of seconds. On certain visits, he stays in the clinic for an entire day for continuous monitoring with EKGs and blood collections. Following his first course of ARV-110, Brasich said he’s feeling much better and believes that the drug is working. Where his last drug left him feeling exhausted and in bed for days, he now has the energy and strength to get outside, enjoy some fresh air, and even take his beloved dog on walks. His first PSA counts indicate a drop, which will certainly be monitored over time. There is certainly a long road ahead for Brasich as he continues to battle his stage 4 diagnosis. But he’s off to a great start and continues to carry an optimistic attitude — similar to when he was given just two months to live a decade ago. Since his first treatment, two other patients throughout the U.S. have begun their ARV-110 journey. Together these patients could shape how prostate cancer is treated in the future. And, just to think, the first patient was right here in Las Vegas.

STAGE 4 CANCER SURVIVORS UNITE TO SUPPORT EACH OTHER AND THE COMMUNITY By Dr. Stephani Christensen

riends Jet Mitchell and Kelly Trolia aren’t letting stage 4 cancer slow them down. In fact, they are are bringing a powerful message of love, unity and support to fellow survivors and Southern Nevada. Mitchell was diagnosed in 2015 with stage 3B breast cancer and in 2016 with stage 4 metastatic breast cancer. Trolia was diagnosed in June 2017 with stage 4 metastatic breast cancer that spread to her bones. She recently underwent a double mastectomy, radiation treatment, removal of her ovaries and will have breast reconstruction in time for her 50th birthday. According to BreastCancer.org, approximately 1 in 8 women in the United States will develop breast cancer. The American Cancer Society projects that 2,190 Nevadans will be diagnosed with breast cancer this year. A stage 4 diagnosis is serious, with a five-year survival rate of approximately 22 percent. While breast cancer death rates declined nearly 40 percent between 1989 and 2016, the quest for a cure continues. In addition to the life-saving treatments that exist and are still to be explored, there is no denying the power of human connection. Mitchell and Trolia first connected when Trolia sought out support groups for metastatic breast cancer fighters, or forever fighters, in the area. While Trolia was equipped with the spark of a forever fighter, she needed to connect with someone who fully understood her situation. Mitchell is part of several inspirational community and personal endeavors, including a Metavivors group and the Pink Paddlers Dragon Boat team. That group of breast cancer fighters meets at Lake Las Vegas every other Monday for rowing and support. Since her cancer diagnosis, Mitchell has also run a half-marathon in all 50 states and has a passion for supporting American Cancer Society Cancer Action Network and other advocacy efforts throughout the state, country and world. Trolia found the connection she sought in Mitchell and the Metavivors. The group is comprised of stage 4 metastatic breast cancer fighters and convenes to discuss not only the treatments that individuals are undergoing, side effects, and assistance available, but they also celebrate the positive moments of life. Mitchell and Trolia’s friendship has blossomed, and the dynamic duo has been on the frontlines of several fundraising and awareness events in the community. In May, Mitchell and Trolia were recognized during the Mission Moment of the Coaches vs. Cancer Las Vegas Golf Classic, which raises awareness and funds for cancer and cancer research. In January, they dropped the ceremonial puck at the Ice Vegas Invitational and in December, the friends were honored on-court and alongside other survivors and patients at

T-Mobile Arena during the annual Coaches vs. Cancer basketball tournament. In October 2018, Mitchell and Trolia were part of a moving event where several patients of Comprehensive Cancer Centers helped shave Vegas Golden Knights star Pierre-Edouard Bellemare’s head to kick off Breast Cancer Awareness Month. Mitchell and Trolia are both fun, intelligent and inspiring women. While they are both on a serious path, they are having fun along the way, making the most of each and every minute. They are living proof that stage 4 cancer fighters can continue to lead active lives, sometimes in the most amazing ways possible through building friendships and a sense of community.

Clinical trials are a vital step in improving health care in southern nevada and beyond By Dr. Russell Gollard

he joke is old and tired by now, and as time goes on it becomes more nonsensical: â€œIf you want quality medical care in Las Vegas, the best place to go is McCarran International Airport.â€? The common perception was that major medical centers elsewhere were the place to go for trend-setting treatment and medical advances. While there is much room to grow and advance, with the establishment of the UNLV School of Medicine and the expansion of other medical schools such as Touro University Nevada, Roseman University of Health Sciences College of Medicine and the Nevada State College School of Nursing, significant advances are being made to ensure quality, groundbreaking care. In fact, medical research is taking place here, in the form of clinical trials. Clinical trials, like many medical-related terms, are something many of us have heard but likely have only vague notions about what they really are.

What is a clinical trial? Clinical trials are research studies performed on people with the goal of evaluating a medical treatment. It’s the primary way that researchers find out if a new treatment (e.g., a new drug, medical device or other proposed treatment) is safe and effective. Often, a clinical trial is used to learn if a new treatment is more effective or has fewer side effects than standard treatments.

Phases of clinical trials Clinical trials advance through four phases to test a treatment, find an appropriate dosage and to look for side effects. If researchers find a drug or other treatment to be safe and effective after the first three phases, the U.S. Food and Drug Administration approves it for clinical use and continues to monitor its effects. Phase I trials determine a safe dose of the treatment being studied and monitor how the treatment affects the human body. Phase II trials determine if the new treatment is effective and monitor for possible side effects. Phase III trials gather more information about safety and effectiveness, continue to monitor for side effects and compare the new treatment with the current standard treatment. If the FDA agrees that the trial results are positive, it will approve the experimental drug or device. A Phase IV trial takes place after the FDA approves use and after the drug or treatment has been marketed. Sometimes, the side effects of a drug may not become clear until more people have taken it over a longer period of time.

are and compassion are the hallmarks of registered nurse Johanna Gurr, who has been with MountainView Hospital’s Neonatal Intensive Care Unit for the past 14 years. A shining example of this is her care of premature baby Alita Andaya and her parents, Rosemarie and Joel Andaya. After learning that Alita and Joel are deaf, Gurr learned sign language to help the parents feel more comfortable through their child’s treatment and recovery. Alita’s family was put at ease as Gurr was able to communicate basic needs with the family. (Patients at the hospital can request certified sign-language translators as needed.) Alita is now very healthy. Her mom said, “Johanna really helped me, and gave me confidence to hold my tiny little baby.” “The job of parents is to care for their babies,” said Gurr, an NICU mother herself. “When something goes wrong and plans change, the parents can feel helpless or even feel like a failure. But they are certainly not failures. A big part of my job is to break down that anxiety, explain that their baby will be on a different journey, and they will play a key role in that journey.” At MountainView’s NICU, parents are always viewed as part of the care team. “While we are treating and caring for the babies, we’re also attending to the needs of the parents,” said Gurr. NICU Manager Melissa McDonald said the level of care provided by Gurr is typical of the nearly 40 nurses in the unit, many of whom have worked together for well over a dozen years. MountainView’s NICU was recently upgraded to Level III after having been a Level II since 2003. According to McDonald, Level III means the NICU can care for babies of any gestational age. McDonald said the only exception is cardiac surgical patients, who are taken to Sunrise Hospital and Medical Center. “Opening a Level III NICU at MountainView brings high-level neonatology to our community and allows for babies to be cared for close to home,” said McDonald. The Centers for Disease Control and Prevention estimates that one in 10 babies in the United States are born premature (www.cdc.gov/features/prematurebirth/index.html). To provide better care to premature babies, Moun-

tainView’s 7,200-square-foot, 24-bed unit features new state-of-the-art equipment, including specialized isolation rooms. Additionally, NICU providers received intensive Level III training to ensure babies and parents would get the best care possible. The NICU also offers “perinatal navigation services,” in which the “navigator” works behind the scenes to help things run smoothly for the parents and their baby. “This service starts in the obstetrician’s office when a mom is diagnosed with a high-risk pregnancy or other serious problem,” said McDonald. “The navigator meets with the mom, follows the process into the NICU and after the mom and child go home.” Gurr said, “I have found my calling in life. I work with nurses and doctors who are the most wonderful individuals I’ve ever worked with. They work so hard to make every baby’s experience a positive one, while also catering to the needs of the parents.” “With this latest expansion, we are able to provide the highest level of care for our tiniest patients, in a warm and welcoming environment that purposely seeks to include the entire family,” said MountainView Hospital Chief Executive Officer Jeremy Bradshaw. Bradshaw added that MountainView is the only designated top teaching hospital in Nevada and has earned an “A” Quality Score from the Leapfrog Group.
