Geriatric Dental Clinic to Open at Baycrest Health Sciences Following three years of planning and deliberation, the Toronto Chapter of the Alpha Omega Dental Fraternity has unveiled a project to build a geriatric dental clinic at the Baycrest Health Sciences in Toronto. Seniors are the fastest growing patient population and highly vulnerable to oral disease. Many are physically and medically unable to access care. The Alpha Omega Dental Centre for Geriatric Care at Baycrest will fill a void in comprehensive services tailored for this demographic. It will be a state-of-the-art centre for evidence-based geriatric dentistry and a leading training centre. The clinic will include four large private operatories, two wheelchair lifts, lab and sterilization facilities, and comprehensive imaging suite and patient friendly consultation space for family members. The planning process to date has been assisted with generous support from Henry Schein. The clinic will draw patients from within Baycrest and accept referrals from long-term care homes, assisted living residences, hospitals and dentists unable to adequately care for this population. On September 12th, 2016, Toronto Chapter Dr. Steven Millman presented Baycrest Health Sciences with a cheque for $500,000 and that evening raised an additional $100,000 from its membership. A campaign is underway and further contributions are welcomed. Construction has begun and it is anticipated the clinic will be opened in January 2017. Dentalcorp And Sinai Health System Partner To Form The Centre For Advanced Dental Research & Care The Centre for Advanced Dental Research and Care, a clinical and research partnership between Sinai Health System and the University of Toronto Faculty of Dentistry, with the philanthropic and leadership support of dentalcorp, was announced on September 16th. Dedicated to addressing some of the most misunderstood and complex dental, periodontal and orofacial conditions, the Centre will be chaired by Dr. Howard Tenenbaum, Dentist-In-Chief at Sinai Health System. The Centre will bring together clinical care and research, providing the opportunity to facilitate the best new discoveries for patient care. Sinai Health System will draw upon its longstanding partnership with the University of Toronto, Faculty of Dentistry to support the centre’s mission. Dental researchers at the University of Toronto’s Faculty of Dentistry will use advanced diagnostics and discovery science research to identify the underlying causes of periodontal, implant, and oralmaxillofacial conditions and design and disseminate treatment protocols for these conditions. The Centre will also explore the psycho-social implications of such conditions. The generous financial support from dentalcorp will allow the Centre to provide research-based clinical care to patients suffering from numerous complex conditions and will fund research and laboratory operations at University of Toronto, Faculty of Dentistry to support the Centre. The Centre will also support its academic mandate by offering training opportunities to specialty graduate students and dental residents. As planning for the Centre makes headway, dentalcorp will provide strategic leadership and operating expertise to ensure the longterm objectives of the Centre are met.

I have to admit something – I prefer cats to dogs.

F

or my first editorial with the Oral Health Group, my blatant opinion to start this piece off might not be the smartest idea. But I feel that I owe it to our dedicated readers, and to my two feline children at home, Eleanor and Lou, to be honest and open. With that being said, I can without a doubt respect the companionship between owner and dog. Big puppy eyes glazed over in adoration and a tail that wags uncontrollably with excitement is proven to be beneficial for our overall health. And that’s something I can appreciate. Studies have been done for years regarding the physical and mental health effects petting a dog has on humans. Research has shown that petting an animal releases endorphins, otherwise known as feel-good neurotransmitters. This positive effect is becoming widely accepted by both the public and professionals in therapeutic settings, helping to provide a sense of hope. This brings us to the topic of patient anxiety, which is a long-standing challenge for dentists. According to Statistics Canada, 40 percent of Canadians have some fear of going to the dentist, and one in five schoolaged children are afraid of dental appointments. Is pet therapy the solution? I personally would have loved a furry companion at the foot of the dental chair to ease my anxiety while experiencing my first (and hopefully only) root canal last year. Animal-assisted therapy and pet therapy are types of therapy that involve animals, specifically trained to provide comfort and affection as a form of treatment and healing. Animal-assisted therapy is proven to reduce anxiety, pain and depression in people with a vast range of health concerns. It has also been proven beneficial for people with autism, cancer patients, those dealing with post-traumatic stress disorder, substance abuse, and even long-term care facility residents.

Though pet therapy is a growing field using dogs or animals to help people recover from or better cope with targeted health issues, animal-assisted therapy serves a more general purpose. While providing comfort and enjoyment, therapy animals are a welcome distraction, helping reduce stress and anxiety, even in the dental office. Our cover star, Mango, is currently a facility dog at Kids’ Dental Group in Richmond Hill, Ontario. Over the past 20 years, National Service Dogs has placed over 300 Certified Service Dogs, including Mango, across Canada. But what exactly is happening with the legislation in Ontario regarding service dogs? Since June of 2015, NSD has been working with the Ministry of Veterans Affairs and the Canadian General Standards Board (CGSB) to assist in the development of a national service dog standard. Their mission is to “ensure consistency with national standards and to help educate and create accountability across Canada for service dog users and trainers, for general public health and safety, and to strengthen the public trust in the service dog community”. National Service Dogs explains on their website that accessibility laws across Canada differ from province to province, and in many cases are “antiquated, poorly defined, and do not take into account the various needs of individuals with disabilities being supported by service dogs”. Knowing the benefits, NSD is currently working together with Assistance Dogs International, the Canadian Association of Guide and Assistance Dog Schools, and other accredited service dog training schools and clients, to improve laws across the country. After realizing how beneficial these furry companions are to our overall health and healing, I think I’ll reconsider my “cats only” stance. LOOKING TO 2017 AND BEYOND

The live presentation to 2015 award winners at the American Dental Association’s annual session in October marked the end of a transition year for the industry’s most prestigious honor for dental companies. The Cellerant “Best of Class” Technology Award, formerly known as The Pride “Best of Class” Technology Award, recognizes innovative game changing technology offerings, services and devices. In February of 2016, Dr. Lou Shuman launched his new industryside consulting company, Cellerant, which provides a range of services to accelerate the growth of dental manufacturers and solution providers. As founder of the “Best of Class” Awards, it was natural that the award follow Dr Shuman and transition to the Cellerant name. While the name of the award changed in 2016, the “Best of Class” commitment to integrity, based on its rigorous selection remains the same. Since its inauguration eight years ago, the “Best of Class” designation has become a coveted honor for industry. As importantly, it has also become a trusted criterion for dental professionals to make educated, informed product and technology investment decisions for their practices and their patients. The selection process relies on an expert panel of dentists recognized as thought leaders and educators that includes Dr. Paul Feuerstein, Dr. John Flucke, Dr. Marty Jablow, Dr. Parag Kachalia and Dr. Shuman. While the celebration and recognition of this year’s award winners is just winding down, the 2017 selection process has been underway over the course of 2016. In a recent interview Dr Shuman commented, “The committee members have been out there all year (2016) looking at products and technologies. We’re bombarded by companies on a weekly basis who want to make sure we’ve seen their product. Paul, John, Marty, and Parag have been on the show floor at all the meetings, going booth to booth, looking at every technology.” The selection panel will convene with me at the Chicago Dental Society Mid-Winter Meeting to deliberate on and select the leading technologies for 2017. To win, a technology must provide clear benefits for dental providers and their patients and be truly differentiated from the competition. The “Best of Class” selection process and award uncovers, highlights and rewards those companies and products that will dictate and drive the technology discussion in 2017 and beyond.

FINDING THE PERFECT FINANCIAL ADVISOR (FOR YOU) There are over 9000 dentists practicing in Ontario. Each have built a loyal following of patients who think that she/he is the “perfect” dentist. Therefore, it can be a confusing exercise to choose a dentist that is right for you and your family. Finding the perfect financial advisor is much like finding the perfect dentist. It is based partly on quantitative analysis and partly on gut feeling. In order to help with this process, I have compiled some of the important criteria that dentists should look for when choosing a financial advisor.

EDUCATIONAL QUALIFICATIONS

Dr. Wilson Chen, BSc., DDS, CFP, FMA received his DDS from Western University in 1992 and over the past 23 years, has built a patientcentred, family practice in Hamilton. In 2014, he sold his practice to take a more active financial management and business advisory role for his dental clients and their families. Wilson is the only practicing dentist in Ontario to hold the Certified Financial Planner (CFP) and Financial Management Advisor (FMA) designations. Unlike dentistry, the educational background of licensed financial advisors vary widely. Some are financial planners, others are portfolio managers, and still others are licensed to deal only with certain financial products. Although there are many different credentials endorsed by various organizations, start with qualifications that are more widely recognized and are rigorous in terms of educational requirements, work experience, and adherence to a code of ethics. On the financial planning side, look for individuals with the CFP (Certified Financial Planner)* or PFP (Personal Financial Planner) designations. For investment management expertise, both the gold standard CFA (Chartered Financial Analyst) designation and the valuable CIM (Canadian Investment Manager) designation allow advisors to qualify for a Portfolio Manager status. On top of this, many financial advisors come from varied backgrounds and have professional standing in law, accounting and insurance. *Disclosure: The author holds the CFP designation.

EXPERIENCE Experience is a major criteria in selecting a financial advisor. Someone who has dealt with a large number of real life financial situations is in a better position to solve the next problem. Look for advisors who have managed their clients’ portfolios successfully through several market cycles and have had to make decisions through both up and down markets. Of course, experience does not guarantee a better result, but longevity does shed some light on the advisor’s past service.

DENTAL FOCUS Dentists have financial needs that are some-

what unique to the general population. Some common elements include: • large debt loads from student loans and practice purchases • require advice on purchasing and selling a dental practice as well as running a small business • may work within corporate structures such as a Professional Corporation or a Technical Service Corporation • may be eligible to use Individual Pension Plans (IPP) or Retirement Compensation Arrangements (RCA) • enhanced opportunities to tax plan with family members • have a variety of different options to fund retirement A financial advisor who is well versed in these scenarios and have worked with dentists can be a source of valuable support to the client. Familiarity with strategies that have worked well for other dentists can be an asset.

BIG PICTURE Be wary of an advisor that starts the conversation by talking about the great investment products that they can buy for you. More than ever, you should expect a financial advisor to get to know you and to help you with your overall financial situation. Setting savings goals, finding a correct framework for your investments, assessing your risk profile, and building a portfolio that is consistent with your individual circumstances are just some of the “big picture” services that an advisor should provide.

CONSISTENT PHILOSOPHY Look for a financial advisor who has a clear investment philosophy and a consistent process that they follow. Without some conviction to a process, it is easy to be swayed by different market conditions and emotional

reactions. Don’t be shy about asking an advisor what they would do in certain scenarios and to provide some examples of their thought process in real life situations.

FEE TRANSPARENCY There are different ways that financial advisors are compensated for their work. Some are paid by commissions on the transactions they perform, some are paid an annual fee for financial advice and management of your portfolio (usually a percentage of the total assets they manage), while others are paid by third parties such as mutual fund companies. Regardless of how they are compensated, responsible financial advisors will be upfront about the total fees you pay and the total compensation they receive for providing good service. Ask them to explain in plain terms how much you would be paying, both directly and indirectly, as some products have costs that are not readily apparent.

COMMUNICATION Since an advisor-client relationship is built on understanding your needs and your goals, a financial advisor first and foremost should be a good listener. On top of that, a good advisor is able to simplify concepts and articulate options and solutions. Without an easy line of communication, the system breaks down. I have known people that were afraid to call their financial advisor since they did not understand the conversations and were made to feel somehow inadequate. Seek out professionals that you have a good natural connection with. If you have any hesitation in calling your advisor with questions, it may be an indication to look at alternatives.

SERVICE Customer service is an important consideration when deciding on a financial advisor. You should expect to receive prompt answers to your questions and concerns, regular updates on your investments and any changes in the portfolio structure, and scheduled reviews of your overall financial situation. Service is an aspect that is difficult to measure so ask the advisor for client references that are in a similar situation – and follow up with specific questions on the level of service provided. Meeting the advisor at their office and gauging your interaction with their team is also a good indicator of their commitment to service. The process of finding your perfect financial advisor can be time consuming but well worth the effort. Building a long term, supportive relationship can benefit you through every stage of your life. To begin your search, I would recommend the following: • If there is a chance, try to meet a financial advisor in an educational seminar or a social setting. You can get some indication of their knowledge and their personality in these informal sessions. • Ask your like-minded dental colleagues and friends for their recommendations. • Do your research on potential candidates and compile specific questions, then set up face to face meetings. • Ideally meet at least 3 candidates to get differing points of view before making a decision. There are a great many well qualified and diligent financial professionals out there. In many cases, it comes down to the comfort level that you feel with the individual and the team. Like anything else, there is no perfect financial advisor – but likely there will be one that is perfect for you.

Ontario dentists, hygienists, and dental students volunteer again in Jamaica! By Michael Carabash, B.A., LL.B., J.D., M.B.A., C.D.P.M.

On Saturday, September 3, 2016, a group of strangers from across Ontario gathered at the Toronto airport. Their mission: head down to Negril, Jamaica to provide free dental treatment and education to impoverished Jamaicans. This was the second annual trip organized by Great Shape! Inc., the Sandals Foundation, and DMC LLP.1 The volunteer group included dentists, hygienists, dental students, Henry Schein representatives, dental lawyers and non-dental support person– nel. Most of them had never done anything like this before.2 There was little to prepare them for what was about to happen in Jamaica for the 10 days they were there...

I was worried.” Then, when the first patients sat in their chairs and opened their mouths, it was another shock. June Jennings recalls a particular three-and-a-half-year-old boy (a caretaker’s son) who sat in her chair for a cleaning: “I really wanted to help

When the volunteers first approached their temporary makeshift dental clinics at the town of Little London and the settlement of Grange Hill, they were shocked by what they saw and heard: hundreds of people had lined up at the doors from the early morning in the hopes of getting a ticket to see a dentist or hygienist sometime that week. As dental hygienist June Jennings (Thornhill) put it: “I’ve seen lineups like that back home for concerts; not for dental cleanings and treatments!” Dr. Irish Malapitan (Kitchener) definitely felt the pressure: “I

I think they were giving him sugary juices to go to sleep. One of the dentists at our clinic recommended to the boy’s dad that he be taken to the hospital and have all of his primary teeth removed. It was sad”. The final shock the volunteers experienced came from the patients themselves – but in a positive way. Dr. Joseph Fava (Toronto) described it as follows: “Amazingly, the patients were stoic while in the chair. Opening wide. Never closing. Never complaining. Salt of the earth. They really appreciated what we did for them. Such small things we do on a daily basis back home had such dramatic effects on their lives.” Patients often brought vol-

unteers local fruit and dishes - like sugar cane, genips, avocado, june plums, tripe and beans, curried chicken, stew pork, ackee and saltfish, boiled / fried dumplings and boiled bananas. Many volunteers received warm smiles, thumbs up, high fives, and big hugs from their patients. Some patients would come back a few days after being treated to thank the volunteers again. Not all patients were comfortable seeing the dentist, however. Over at Grange Hill, Dr. Christina Bodea (Elmvale) had a young female patient named Tiana who was extremely nervous about the needle; she wouldn’t let Dr. Bodea anesthetize her for a much-needed filling. Then Dr. Bodea and her assistant, husband Stefan Atalick, noticed that Tiana was interested in their hair. Long and straight; perhaps something Tiana wasn’t accustomed to. So Dr. Bodea and Stefan encouraged Tiana to touch their hair. And it did the trick! Tiana got so comfortable that she eventually got the filling without incident. Over at Little London, Dr. Malapitan and her assistant Jason Brown (Henry Schein) had an eight-year-old boy who was very scared to see them. He was crying in the chair. So Dr. Malapitan and Jason tried something different: they started singing Jingle Bells. Soon the whole clinic joined in. The boy’s mood changed. He smiled and relaxed enough to let Dr. Malapitan treat him. Per Jason: “That memory will stay with me always.” Singing Christmas carols or doing a ‘booty-bounce’ dance with dental hygien-

ist Jazz Cohan (Toronto) were some of the things Dr. Malapitan did to make patients laugh and feel more comfortable.

How Did Things Get So Bad? So what was the cause of the poor state of oral hygiene in large parts of Jamaica? Was it lack of dental education? Cultural misconceptions? Dental professionals not being available or too expensive? For Dr. Jacqueline Geroche (Mississauga), it was a combination of all of these factors: The smallest exposure of Dentin – the second layer of the tooth under the Enamel – and the patient would want the tooth pulled instead of getting a filling. They have this misconception that the filling won’t last. To them, if it’s causing pain, just pull it out! Many of the patients I saw had never been to a dentist before. They’re

either afraid, don’t have access or money, or all of the above. People seemed to function with just gums. The end game is dentures. And that’s how they operate. Not to mention that their diet consists of a lot of soft foods to accommodate their lack of functioning teeth. We heard stories from patients in the clinics about a basic cleaning costing USD$100 at a private practice; this was a lot of money, given that the average minimum wage for a Jamaican working full time is about USD$50 per week. Access to dental care is also huge problem: some parts of Jamaica have a 1:100,000 dentist to patient ratio.

Fixing the Problem In the week that we were there, team Little London treated 260 patients and team Grange Hill treated 383 patients. Patients were given toothbrushes and toothpaste and instructed on how to clean their teeth. Some of our volunteers also attended various primary schools and spoke to about 2,000 children about proper oral hygiene and distributed toothbrushes and toothpaste. Great Shape! Inc. runs their program through the Sandals Foundation from late August to November, allowing them to treat about 20,000 patients a year in Jamaica and St. Lucia. They also run sealant projects and have plans to acquire a mobile dental unit. Admittedly, these are all temporary fixes to a more widespread problem. Long-term, Great Shape Inc.! wants

to raise money to establish a free public dental clinic near Montego Bay that runs all year long and which can accommodate Canadian dentists, hygienists and support staff to volunteer a week at a time. There is hope. But equipment, sundries, funds, and volunteers are desperately needed. Great Shape! Inc.’s executive director, Joseph Wright (a.k.a. Papa Joe), is not one to shy away from challenges. He was instrumental in helping to establish Jamaica’s first dental school, University of Technology Dental College (UTech), along with Jamaica’s Chief Dental Officer and now Dean the University of Technology, College of Oral Health Sciences, Dr. Irving McKenzie. That happened by chance over drinks at Margaritaville. And because of that, in just a few years, UTech dental students and graduates have treated thousands of Jamaicans. Let’s give a warm welcome to Papa Joe when he visits Toronto in May 2017.

Group Camaraderie… More Like One Love Teams were assembled based on personality profiles, competencies, rooming, and other factors. I don’t use the word ‘love’ lightly. But I actually believe that team members loved one another from the very beginning. They would eat and drink, swim, do karaoke, take strolls along the beach, dance at the disco, and take a boat ride to Rick’s Café – together. Nicknames naturally developed – like Dr. Potato (Dr. Irish Malapitan), Zee (Marc Zdrojewski), Dr. Propa Touch (UTech Dental Grad Tevin Carter),

Man Child (Jason Brown), Nam (dental hygienist Nina Nguyen), Tetley (dental hygienist Jazz Chohan), La Machina (Dr. Mary Berkmortel), and Candlestick (Stefan Atalick). Unfortunately, there isn’t enough room to get into the (sometimes hilarious) origins of these nicknames. Many volunteers shared intimate stories with the group. Like when dental hygienist Nina Nguyen (Toronto) told team Grange Hill at dinner an unforgettable story about how she had escaped Vietnam as an 8-year-old girl on a tiny boat with little supplies. She eventually made it to a refugee camp in Singapore before her uncle in Canada sponsored her. Nina’s mom spent 2 years in a Vietnamese jail for not telling authorities who had masterminded that escape (it was Nina’s grandmother). There was also a touching moment on Tuesday, September 6th involving dental hygiene student Sandra Busch (Oregon). That day marked the first year anniversary of the tragic death of her sister from lymphoma. Sandra brought some of her sister’s ashes to spread on the beach. When Dr. Geroche found out, she encouraged Sandra to share that story with the Little London group. A circle formed in the ocean as the sun began to set in the background. Sandra shared her story. There were tears, hugs and condolences. And then Sandra, a gifted singer, sang How Great Thou Art to commemorate her sister. “I truly was touched by it all. You all became family to me in such a short amount of time”, said Sandra. These kinds of stories and experiences made us feel human again and brought us closer together. T here were also lots of f u n ny moments. These typically occurred after leaving the clinic and returning to the resort. The official chant for team Little London was “Bar – Patty – Ocean”, which they practiced religiously upon return from their clinic. One day, when they were congregating in the ocean, they egged Jason Brown to join them while he still wearing his scrubs – and he did! Everyone laughed really hard. His reasoning: “Making happy memories!” Apparently, he still has sand in those scrubs. And who could forget the team Grange

Hill’s sleepers? Like Dr. Bodea, who passed out on the ride back one day while still hanging onto the supplies list! Or Marc Zdrojewski, who fell asleep on the ride back numerous times; other volunteers would make funny faces beside his while we snapped photos. The last day at Grange Hill was particularly memorable: it was dental hygienist Melissa Brunette’s (Ottawa) birthday. We were nice about it: we only doused her in beer and made her sit on a towel on the ride home. Her mom, Christine Martel, commented that the van smelled “like a bar”. On that same ride back to the resort, we almost got stuck in a flash flood. A tiny Toyota Corolla ahead of us was brave enough to get through without incident, so we followed suit and made it through too!

Why They Did It They met as strangers at the Toronto airport. Leaving behind comfortable lifestyles, their practices, family and friends to venture into the unknown. When they finally got to the clinics, the working conditions were far from ideal. Per Dr. Millie Calko (Toronto): “Extreme humidity. Low light. Ample mosquitos and other insects. Primitive and sometimes malfunctioning equipment. Low resources. Sore muscles. Scrubs soaked through with sweat.” But she never complained or took a break. In fact, nobody did. They all knew it was going to be hard work. So why did they do it? In Dr. Calko’s own words: We all get caught up in the rat race that is our lives. And at some point, we lose our sense of purpose. But when a group of dental professionals and support personnel work as a team to selflessly treat those in desperate need, we feel personal fulfillment. Love from our patients and support from our teammates creates an abundance of energy that pushes us to keep going throughout the day. We truly felt as though we were one. MOVIN’ ON UP

Three years ago, Dr. Vesna Borovac acquired the practice of Dr. Nicolas Kemp in the updscale Bloor-Bay Street area in Toronto. Having become comfortable in her recently attained space and operation, she was unexpectedly faced with a seemingly daunting task: the existing office was to be expropriated for another tenant, and she would be required to relocate her practice to another space in the building. Fortunately, a 2300 hundred square foot corner unit on the penthouse floor was available, ultimately translating into a spacious 6-operatory office with a view of the city core and lots of natural light. There were some design challenges, however – restricted ceiling heights, structural columns, and an awkward entry. The design solution, achieved by designer Jean Akerman and her team, began with the layout: the operatories were situated along the windows (providing visual dis-

traction for patients), with ancillary spaces in core, and the entry was repositioned to allow a path into the heart of the suite. Interesting elements were conceived to manage encumbrances in the ceiling plane. Customized millwork solutions throughout show regard for a balance of aesthetics and functionality. New dental cabinetry and the stericentre, tailored by MCC to the doctor’s specifications, facilitate implementation of Kois protocols for better patient care. Dr. Borovac desired a clean, almost minimalist environment for the new office, so materials and detailing were meticulously chosen to this effect. Textured wallcoverings, an interesting “curtain” wall behind the water feature, crisp colour palette and warm wood tones, contribute to a welcoming, spa-like ambience enjoyed by patients and staff.

MANGO: PATIENT

A two-year-old is screaming and crying as they’re ushered into a dental office for a simple cleaning. Sound familiar? How about that same child leaving smiling and laughing after their appointment? Maybe that’s not the most common outcome, but it is for Kids’ Dental Group. At Kids’ Dental Group they have Mango, a fouryear-old black Labrador. Mango is a facility dog from National Service Dog and has been with the group for two years. When a nervous child enters the dental chair Mango, wearing her jacket which indicates she’s working, senses their anxiety and comes to the rescue. “If there is a child crying, [Mango] starts walking towards the operatory and just kind of peeks in. She won’t come in unless you call her, because she’s really trained to follow orders, but if she hears something is off she walks and just kind of looks around,” Dr. Edina Heder said. “A dental setting can be very intimidating for a little one. This is just an extra thing that makes it more friendly and also home-like,” Dr. Zhemeng Wang, or Dr. Zee, explained. “Depending on how the child is we actually have Mango sit on the chair with them… Sometimes we just have her in the room and they just like the presence of a dog there.” As the first dental office in Ontario to have a facility dog, there was some adjusting for staff and doctors, but as soon as they took Mango in they felt the stress levels of a busy dental office decrease. For the young patients, distraction seems to be the key

to a stress-free appointment. While patients may have great oral health habits, seeing instruments is a different experience that can be scary for children. Finding something to take their mind off the cleaning has proven to be beneficial for this team. This is known as ‘distraction therapy’ and while it could be done with a number of resources, Mango seems to be quite a successful one. Dr. Heder ensures there are resources for new parents to learn about Mango and her job at the office. Their website features a post explaining the role Mango plays in their child’s visit, as well as some ground rules (no patting, no feeding, etc.). With posters around the office and an educated team there are rare occasions of patients and their parents requesting Mango to not be present during the visit. If this does happen, she stays in her ‘office’ until the appointment is over. “Explanation is the most important thing, right? Just clarifying all the details and letting them know it’s completely elective, too,” Dr. Zee said. When it comes to allergies, the team is cautious but optimistic about the visits. Dr. Zee herself admits to being allergic to dogs, but having lived with Mango for two years now has never experienced an allergic reaction. While admittedly strange, she and her team ensure there is nothing for patients and parents to worry about with Mango sitting in the chair and wandering around the office. “What we understand from National Service Dog is that as long as children don’t touch Mango there won’t

The Life of a 58-Year-Old Dentist There are many key milestones in a dentist’s practice and personal life. One such milestone for the average dentist I know occurs when you turn 58. It is the day you actually start living your retirement plan. That’s because the average dentist I know is out of dentistry by age 62. Therefore, until age 58 the retirement plan is just numbers. At age 58 however, with only four more years to prepare, the realization seems to hit that this retirement thing is actually going to happen – and you better get ready.

Remember, you only get one chance to sell your practice and retire. The decisions you make from age 58 to 62 (for my average dentist) will affect you for the rest of your life. So we need to get them right. To give you an idea of what has to happen over those four years, we collected information about ten of our clients – dentists age 56 to 59 – and created a case study. Our objective is to give you real-life data on your colleagues’ practice and personal finances, as well as on how they approach their practice and personal planning at this milestone – age 58. Note: these figures do not represent the “average” 58-year-old dentist. The sample size is too small. In my experience, the ten people we have selected would rank in the top five percent of all Canadian dentists in terms of financial success. Below is information on the finances of our average 58-year-old dentist – let’s call the couple Andrew and Judy. As you can see, this dentist (who is the average of the ten dentists we used to create the case study) has reached a very high level of financial success. As mentioned earlier, the ten dentists are likely more financially successful than 95% of their colleagues. But even at this level of financial success, they need to follow the same path to selling their practice and retiring as all practice owners do.

THE PATH TO RETIREMENT FOR A PRACTICE OWNER STEP ONE: Quantify where you are today in a financial sense. What resources do you have available to fund your retirement? For the case study, this is summarized in Figure One. STEP TWO: Quantify where you want to go. Up until this point (age 58), most of your retirement goals were likely somewhat abstract: “I plan on retiring in my 60s”. They also likely changed a few times - one kid decides to go do a post-graduate degree which you are paying for, so you decide to retire at 62 instead of 60. Now, four years away from selling your practice, you need to get specific. We need to start putting the pieces in place to finance your monthly cash flow that’s starting in four years. While it may change, you need to decide on the date you are retiring. Are you retiring in January the year you turn 62 or November? There is a big difference. Also, how much money are you going to spend? Are you going to spend more in the initial years because of travelling, and less in your 80s? This is the time you need to get specific about your retirement goals. Andrew and Judy have decided to retire at the beginning of the year they turn 62. For this reason, we now have only three years to prepare since they are already 58. Below is a projection of what they need in order to retire. As mentioned earlier, because Andrew and Judy are in such good financial shape they are in the rare position of being able to finance their retirement lifestyle without eating into their capital. In other words, they can spend $210,000 after-tax, indexed for inflation annually until age 90, at which point they would still have a retirement nest egg of $5.9 million.

STEP THREE: Design strategies to get you from where you are today to where you want to go. These strategies should focus on the sale of the practice, income tax planning, cash flow management, portfolio management, etc. While 95% of dentists will not have the money Andrew and Judy do, many of the strategies that need to be put in place will be the same. For example, in the retirement projection we assume both Andrew and Judy will qualify for the capital gains exemption when they sell the practice. Each person is eligible for an exemption from tax on the sale of the first $813,000 of small business shares. We need to meet certain criteria for them to be eligible. For example, the shares will not qualify for the exemption with the current savings of $1,153,288 inside the corporation. The corporation needs to be “purified” two years prior to the sale. Another example of strategy is to build a retirement portfolio that can finance their lifestyle. Investing changes when you are living off your portfolio. Do not wait until you are retired to adopt the right strategy. What if the stock market crashes after you sell the practice? You need to put safeguards in place. Another strategy might be to obtain a practice valuation. Rules of thumb are no longer accurate. Estimating the practice is worth 100% of gross was fine a few years ago, but now we need to get specific. If your practice estimate is wrong, you do not want to wait until the year you plan to retire to find out. We also encourage dentists at this stage to start what we call a “trial retirement”. The average dentist I know spends just over $10,000 per month in retirement.

While this may seem like a lot of money, it is still a fixed budget which most dentists have not had for decades. We recommend starting to live on that amount a few years prior to actually retiring. It will help identify potential issues while you are still in control of your earnings. STEP FOUR: Monitor and Repeat When the strategies are set, we recommend a four-year timeline be developed to manage all the different aspects of your big picture practice and personal retirement plan. This will allow you to set targets that can be monitored quarterly. Increasing the monitoring of the plan to four times per year allows you to deal with any missed assumptions proactively rather than waiting until it is too late. We also recommend starting each year at step one again and redoing the entire plan. This allows you to take all new information into account as it affects your plan. It keeps your plan relevant.

IN CONCLUSION At age 58, there is a lot that needs to happen in order for you to be confident enough to sell your practice and retire by age 62. One of the reasons the ten dentists we used to create Andrew and Judy are in such good financial shape is because they started much earlier than this. Their plans are more real now because they are so close, but they have been managing towards this point for more than a decade. If you do have a desire to achieve their level of financial success, it is clear to me you need to do the same. Start early and make planning a priority.

STONEY CREEK DENTAL A Breath of Fresh Air This bright and airy practice is situated in the heart of the Stoney Creek community – hence it’s name. When first discussing the project with our client, Dr. Allen Park, it became evident that this new location was his opportunity to reinvent the branding and identity for Stoney Creek Dental, while also allowing to aim for improvement on functionality of everyday operations of an already established practice. Antham Construction Group, along with De Style Design Inc. and Patterson Dental took on this challenge, coordinating efforts in order to achieve the best result while keeping the client objectives and requirements in mind. We started from understanding the way our client conducted his everyday activities and proceeded with analysis of lighting, as well as such functional elements as shaping a large reception desk with multiple payment stations,

ample reception waiting and a welcoming kids area. Thus the plans of the new clinic began to take shape. Everyone on the design and construction team used their skills and expertise to review and amend the emerging design, in order to best reflect the vision of the client. Once the layout plan solidified, the design team proceeded with infusing the space with character. The overall materials palette was designed to facilitate a warm and welcoming environment, while the splash of orange is strategically placed to offer a sense of branding and identity, as well as navigating patients and staff alike. The light and airy feel, created by a multitude of glass panels foster a sense of openness and transparency, another element which puts the patients’ minds at ease, when visiting their neighbourhood dentist.

Marketing your practice well requires the right marketing mix … it takes hard work, innovative, thoughtful ideas and consistent effort. Over the upcoming weeks, I will have many conversations with doctors and office managers wanting to attract more new patients to finish their year off strong and set themselves up for success to begin the new year. Some discussions are with dentists getting started with their first new practice; others are with dentists and office managers of existing offices who would like to see a boost to their business. I took a call recently from the office manager of a new dental practice; they opened their doors only four months ago and had not gotten off to the successful start that they were expecting. They explained to me that their main goal was pure and simple to acquire more new patients. He then went on and asked me a very direct question, “Are community mailers the best way to attract new patients?” After a short pause, my answer to him was, “No, not when they are working on their own and unsupported by other marketing devices.” From there I went on to explain, to consistently be attracting new patients to your practice (the key word here is consistently), you need to have the right marketing mix in place. Having all of your eggs in a one-marketing-initiative basket is a poor approach and in today’s competitive space will, in all likelihood, not provide the desired end result. Taking a look at our own roster of our most successful accounts, the common

link is that they all have the right marketing mix working for them. For example, Dr. R has had a mailer campaign running with us for the past twelve months and with every check in call, the mailer campaign is doing very well. As much as the design, message and quality with this mailer campaign are on point, Dr. R. has a bigger picture marketing mix in place, with the primary purpose of attracting more new patients. So, what makes up an effective marketing mix you ask? While every dental practice is different, with its own range of media and event opportunities available to them, we believe that every marketing mix needs to include some core elements. To get things started, the marketing mix that we advise to our clients employs a 4-plus-1 approach.

1. A Modern & Mobile Website 87% of internet users now have a smartphone. (Source: Global Web Index). I think that we can all agree that most of us have some sort of mobile computer, be it a smartphone, tablet or laptop. No matter how someone hears about your practice, they will use the Internet to learn more about you. First, your website design needs to look modern and clean, as you only have six seconds to engage them to go deeper into your pages. Next, whether on a full monitor or a smartphone, you want to make it easy for people to get the information that they need, including seeing the doctor(s), team and

look of the office. Oh yes, and then it is about leveraging search engine optimization or an adword campaign to get your great mobile friendly website found.

2. An Engaging Facebook Strategy Three million advertisers are now using Facebook to reach their existing customers and attract new ones. (Source: AdWeek). More and more people today are using Facebook as a way of sourcing and learning more about businesses. Your practice should have a Facebook page that is informative and engaging. This is the tool that you will use to stay in front of your patients with events and happenings at your dental office. The more inviting and engaging your page is, the more likely people will share your posts and get new people talking about you. Talking about you in a very professional manner that is. And if you want to take things to a higher-level still, run a contest that will connect with people on a lifestyle level. Facebook is a very exciting marketing opportunity for any business, especially if you’re ready to have some fun with it. As much as you are a professional practice, remember not to take yourself too seriously either.

3. A Proactive Patient Referral System 65% of new business comes from referrals. (Source: New York Times) No matter what your business, having your best clients referring you is a great way of attracting more new business;

however, the truth is many businesses including most dental practices that I meet with have very little in place by way of a formal system to do so. Asking patients for referrals can be very difficult for most practices, as it’s something that does not come naturally. These however are some of your best relationships, and at the end of the day, you are a business. After all, what business isn’t looking for new customers in today’s ultra competitive marketplace? Asking for a referral, introduction or to pass your name along is a very important part of business growth today. By developing a system to do so, you take a lot of pressure and awkwardness out of the equation. Once someone has a sense of what the system is, it is much easier to adapt and have it running continually to deliver consistent, positive results.

4. An Eye-Catching Local Community Presence 67% of unaddressed print advertising are actioned. (Source: Canada Post) Take a look at the media options that are available around you. Select the one that can provide the most value in terms of connecting and getting eye contact with the most people possible in your local area. For my money, professional mailer campaigns remain the best way to get your name out there in a local area. The households around you are living busy lives and really value convenience when then can get it. The Plus-One And the Plus-One is what brings those first four points together. The idea of team and having everyone in your practice working together to make the most of the four points noted above is imperative for immediate and ongoing success: • Everyone in your practice needs to know the domain name of your website. • Together, everyone needs to be participating with ideas to be posting on Facebook and also promoting the page to existing patients, encouraging them to like and follow us. • All team members should understand the structure of the dental office’s patient referral system and be confident in proactively asking patients for referrals. • Everyone should also be aware of the external marketing that is working for the practice in the local community.

These are especially important ideas for the front desk staff, who should be ready to answer the phone to make the most of prospective new patient opportunities. As good as a voice (Gord Downie) might sound on its own, it will sound a whole lot better when accompanied by the right guitars (Paul Langlois & Rob Baker), a strong bassist (Gord Sinclair) and some steady drumming (Johnny Fay). You mix the right instruments together, the right way, and you too will have something that people will take notice of and really want to listen to.
