or the past 115 years, the citizens of Shelby County and surrounding communities have counted on our community hospital to meet their healthcare needs. Regardless of the local hospital’s name, our citizens have benefited from excellent care provided by the physicians, nurses and staff who have been on standby 24/7 ready to serve. In late 2019, the hospital was acquired by a newly formed organization, UofL Health. Since that time, our team has been focused on reinvigorating our hospital. From new leadership to the latest equipment and technology, to new primary and specialty care services, we are committed to making sure residents can get the care they need right here at home. We want to keep healthcare local! r. Valerie Briones-Pryor, “Dr. Val,” was always going to end up in medicine. She wasn’t pressured to be a physician, besides the fact that both her mother and father are doctors, but it seemed to be the only thing she knew. Thanks to growing up going to her dad’s office for free snacks in return for a free filing clerk – medicine was her calling. Raised in Louisville, with a brief stint at Xavier University in Cincinnati for her Bachelor’s, a residency for Internal Medicine at Indiana University after getting her M.D. at the University of Louisville, of course, Dr. Val boomeranged back to Kentucky. Dr. Val became the third person in the state to receive

the COVID-19 vaccine. At the same time, she was in charge of the coronovirus non-ICU unit at UofL Health – Jewish Hospital in Louisville. As she received her shot, she had lost her 27th patient to the virus. “Working on the COVID ward, and watching people die away from home and loved ones, made me realize how important it was to get certain facilities back into the Shelby community,” she said. Currently, Dr. Val is the Hospi Hospitalist at UofL Health - Shelbyville Hospital. But, her first job in medicine was working back at her dad’s practice – now as an adult – with an outpatient focus. After five years, she made the hard decision to shift to inpatient work. While she didn’t like parting from her dad, he had rather serendipitously decided to stay at the office.

he local Shelby County hospital is growing and returning to a vital part of community life since Jewish Healthcare sold the facility to University of Louisville Health in 2019. UofL Health – Shelbyville Hospital has ramped up, most recently returning the Intensive Care Unit to its community assets with Dr. Greg Pfister heading the operation. The ICU hit the ground running, said Pfister, since his June 14 arrival. Several patients have been in the unit per day since the space opened in June. Critical care allows local folks to get intensive care without leaving their own community for medical facilities in Louisville or Lexington. “The hospital is capable of a higher level of care,” said Pfister. “We’re able to treat in Shelby County without transferring patients down-

he University of Louisville took over the local hospital and brought positive change for the community. New and upgraded X-ray and other equipment and the return of the ICU, intensive care unit, are some of the newest additions to UofL Health Shelbyville Hospital.

UofL’s purchase of the Shelbyville hospital is the main reason for upgrades. “The sale almost certainly spurred upgrades,” said James “Andy” Bose, BS, CNMT, LSSBB and director of medical imaging, cardiopulmonary and sleep services. “UofL has stimulated all of this. It’s been a huge help to all of these areas. It was a good move for UofL to extend its footprint.” The hospital wasn’t in a good place, he said, before the takeover. “A lot of services were discontinued like

ICU. That was terrible.”

ICU

The ICU was closed in 2012 due to lack of funds, meaning residents had to go to Louisville or Lexington for inpatient critical care monitoring. However, UofL Health invested $650,000 in bringing the ICU unit back, which opened June 18 followed by ICU patients hours later. “Our main focus right now is the new ICU,” Bose said. The hospital, Bose said, will focus more on cardiac care in the future.

Radiology

Updates in radiology include 3-D mammography, upgraded X-ray equipment from analog to digital, an

upgraded CT scanner, improved cardiac halters and two oxygen therapy devices are on order. The last piece of equipment arrived in late 2020. UofL Shelbyville has updated all X-ray equipment. UofL Shelbyville previously had 2D mammography equipment. The new technology means reduced need for follow-up visits, a quicker image return for patients, may reduce additional imaging and reduce patients from returning and scans can see subtle abnormalities that 2D cannot. “These scans are more defined, which reduces the need for a patient to come back,” Bose said. Bose said more angles of breast tissue are imaged and this technology can see beyond dense areas, unlike the previous method.

Digital imaging

Digital X-rays image faster, are better quality, quicker for the patient and the patient gets a lower dose of radiation with this technology. “You can use enhancement tools to make it sharper,” Bose said. “In standard X-ray, you get what you get. Hospital personnel also can view images more quickly. With 2D, if the image is not good, there is a chance it has to be repeated. “This eliminates that and unnecessary wait time for patients.” CT equipment has the newest upgrade. The

Pfister is a pulmonologist who runs this newly opened ICU. “We have the ability to provide mechanical ventilation for whatever reason a patient can’t maintain their own breathing, the ability to provide ventilation artificially while treating illness, including COVID-19, we treat a wide variety of respiratory issues,” he said. He is building a pulmonary clinic where a wide variety of respiratory issues can be treated, such as asthma and other pulmonary issues, an area of medicine lacking within the community for some time. In addition to running the ICU, Pfister sees patients with lung ailments at UofL Physicians – Pulmonology Associates, located in the Medical Annex building behind the hospital. The practice can diagnose and treat pneumonia, asthma, COPD/emphysema, chronic cough, lung cancer, interstitial lung diseases, cystic fibrosis, pulmonary hypertension, occupational lung disease and pulmonary infections like tuberculosis. Pfister previously provided inpatient pulmonology care in Florida, where he served in several leadership roles, including Section Chief of Pulmonary Medicine and as a pulmonary consultant for several healthcare organizations. PFISTER: Continuing growth is key for the future Virginia Commonwealth University School of Medicine, Pfister completed an internal medicine residency and pulmonary fellowship at the University of Louisville. Returning closer to home, Pfister’s family lives in Louisville, turned out to be a draw for the future. “I wanted to be closer to them,” he said.

Growth for the future

Pfister plans to build a screening program for lung cancer, similar to mammograms for breast cancer or colonoscopies to detect colon cancer. “I’m hoping to do more and more to catch nodules early enough to provide curative treatment,” he said. The pulmonary physician hopes to soon offer pulmonary rehab, exercise done under the supervision of a respiratory therapist and physicians. “That’s shown tremendous benefit to people with long-term illness,” he said. He explained that people with respiratory illness get into a cycle. They are short of breath when they exercise, meaning they get less and less active, lose muscle tone and cardiac fitness, then it takes less activity to get short of breath. “They get into a trap where they go to do something as simple as walking to the mailbox,” he said. “We’re starting to apply rehab to get patients the opportunity to

exercise under controlled conditions while monitoring blood pressure and heart rate to help regain muscle strength and fitness to function in day to day lives again.”

The hospital is building services at the moment. “The goal is to be able to provide the same services as larger institutions,” he said “It’s a work in progress. We are excited to provide care, building the goal to be able to provide whatever that presently ill person would need.” New services are being planned and considered, including working with UofL Health in downtown Louisville to expand specialty services, such as kidney services. “When patients get critically ill, they often need to go temporarily on dialysis,” he said. A new vascular surgeon has been hired and an ENT group will practice in Shelbyville. The local community will benefit from these changes, and residents are reacting positively to the changes UofL is making at the hospital. “It’s obvious that the staff is competent, passionate and caring,” Pfister said. “We have had many favorable reviews.”

EQUIPMENT: New machines increase care options Continued from page 11 upgraded CT scans are faster and the quality increased with a “much lower” dose of radiation. “We started training this week,” Bose said. “What’s nice about the scanner is that it is the exact same one that UofL Health in downtown Louisville has, a Level 1 trauma center.”

Cardiac halters

Bose is getting good feedback from cardiac patients. The hospital brought in new, smaller cardiac halter monitors that are more water resistant and can be used for an extended period. The old halters were larger and only functioned for 24-48 hours. The new halters last up to 14 days. “We get immediate notification, especially if there is a critical finding. “The old ones are bulkier and patients don’t like those quite as much,” Bose said.

Vascular surgery

As a state with a high level of smokers, Vascular Surgery is a necessary service. The study of the vascular system is all about the circulatory system. The disorders of which can include arterial aneurysms, vascular malformations and tumors, vascular disease of intestines and kidneys (visceral disorders), venous and lymphatic disorders. Spider veins and limb swelling can be addressed with vascular surgery as well. “It’s all about the extremities: hands, arms, legs and feet,” said Garofola.

Care close to home

The team at University of Louisville – Shelbyville Hospital has seen at least a 200 percent rise in patients since their ICU reopening. While they are a smaller 70-bed hospital, their med-surg (medical surgery) and outpatient wards can see as many as 50 patients a day. Shelby County’s local hospital has around 25 primary care physicians and specialists. A handful of them, supported by a legion of nursing staff, can be found on the floor at all times. Dr. VAL: Keeping the focus on community care Continued from page 7 “It’s like the show ‘House,’ how he gets his team to think – to put it all together for the right diagnoses. That’s how I think. It’s organized and detailed.” With her family’s blessing, Dr. Val could turn her mind to the organized puzzling she loves so much at the hospital. But being a doctor isn’t about having all the answers, it also means you know when to call in your colleagues. As she says, that’s real community care and it’s what Shelby County deserves. In the 2000s Dr. Val was moonlight moonlighting at Mary and Elizabeth hospital while in school. Then she began working at UoL Jewish hospital and began picking up shifts in Shelbyville. Here – Dr. Val immediately fell in love, crediting it to the pride she felt the community and staff had in their establishment. Fortuitously in 2009, the CEO of Mary & Elizabeth was looking to take their hospitalist program in a new direction – so he asked Dr. Val. This

was the first but not the last time that she would be wearing the dual hats of administration and patient care. Ten years later, when UofL acquired the Shelbyville hospital in November of 2019, a seasoned Dr. Val leapt at the chance to bring her hospitalist specialty to a community she was itching to work with.

Community care

“I love how the community really gets behind the hospital,” said Dr. Val. She was saddened when the deci decision was made to close the Intensive Care Unit in the mid-2010s, sharing that no physician ever wants to turn away a patient. Especially if a doctor has developed a relationship with them, it’s difficult to transfer someone when they’re most vulnerable. But a doctor never wants to put someone at risk, even if that means referring them to another facility farther away. “To say, ‘We can’t take care of you because you’re too sick’ is heartbreak heartbreaking,” said Dr. Val, “Especially to a different environment. It’s scary.” With the 2019 commitment from

UoL to bring “care closer to home,” Shelbyville hospital finally had the funds to bring critical services back. Most specifically the ICU and Pulmonology, since lung diseases and cardiac conditions are a big killer in the Bluegrass State. Dr. Val’s biggest wish for Shelby County is to get the hospital back to that place where the commu community can be proud of it. As a local, she has a lifeline to the community where she can establish dialogue between the medical establishment and the public. “What we [Shelbyville Hospital] offer is because it’s what the communi community said they wanted,” said Dr. Val. She’s humble, crediting the CEO Aaron Garofola and Vice Presi President/ Chief Nursing Officer Shari Kretzschmer alongside the many others who keep the facility running. “They’re really in touch with what the community needs,” she said. UofL - Shelbyville Hospital care goes beyond their walls. They offer Men’s Health Fair and blood drives, and reach out to minority communities to ensure their needs are being met. Dr. Valerie Briones-Pryor will be there every step of the way.
