EXHIBITION OF MARINE AND COASTAL PAINTINGS THE HAWKER GALLERY: JULY 12TH – 26TH uly 12th sees the opening of an exhibition of marine and coastal paintings at The Hawker Gallery in Amersham Old Town. The gallery can be found oﬀ School Lane at The Maltings, the site of a former brewery. A well-established family business it oﬀers a rich programme of events, including exhibitions on a wide variety of themes and features on particular artists, throughout the year. Details are available on the gallery website www.thehawkergallery.co.uk Representing renowned artists from across Britain and overseas, the gallery holds a large collection of paintings as well as sculpture, ceramics and glass. A selection of work by collectable Scandinavian ceramicists and glass artists, curated by Cultural Collections CC, is always available. At the heart of the gallery’s stock is its “Chiltern Collection”, a wide range of work by nationally renowned artists from the area as well as paintings of local subjects. Over the years, the Chiltern Collection has proved to be popular with everyone who

enjoys living in or visiting the region. From July 12th – 26th the focus will be on marine and coastal paintings in a variety of media. A substantial feature of the exhibition will be work by members of the Royal Society of Marine Artists (RSMA), a well-known national society representing some of Britain’s top painters of maritime subjects. Michael Hawker of the Hawker Gallery commented: “We are extremely proud to be hosting these great artists in our July exhibition. Many now have a long-standing relationship with the gallery and contribute

to various events during the year. We are extremely pleased to be able to show the work of Elizabeth Smith, who was unanimously elected the President of the RSMA in 2013 and who is the first woman to serve in that role since the Society was founded in 1939.” Michael Hawker added: “It would be wrong to assume that this will only be a show of paintings about boats and ships. While the maritime content will be a strong element, a whole range of subjects will be represented, including beach scenes, harbours, figure subjects, classic views of the places we love to visit and the glorious British coastline.” The exhibition is open between 10am and 5pm from Tuesday to Saturday from 12th to 26th July. isitors to this year’s Chilterns Craft & Design Show (22-25 August) will be spoilt for choice with the tempting array of exquisite arts and crafts made by some of the best British craftsmen and women. The show is set once more against the picture perfect background of Stonor Park, Henley-on-Thames over the August Bank Holiday. Over 200 craftspeople will be showcasing their work over the 3 day weekend, making it a fantastic opportunity to buy handmade wares including gifts, ornaments, jewellery, glass, ceramics and accessories for the home and garden. The event will also feature live music, workshops and activities and delicious food and drink making it a wonderful occasion for all the family to enjoy. This year, the Café Eatons Bar will be on hand to oﬀer foodie treats with Italian coﬀees, hand-made cakes, afternoon teas and a full bar, so visitors can sip on a glass of bubbly or two in the sumptuous surroundings – the perfect pick me up after a good browse in the craft stalls. If the delicious eats inspire turning a hand to culinary delights, celebrated professional chef Paul Da Costa Greaves will be sharing his mouth watering recipes in the Live Cookery

Demonstrations, helping keen cooks and enthusiasts to inject plenty of flair into their summer dishes. To complement the Live Cookery Demonstrations, the Chilterns Craft & Design Show will also feature Good Food Live with Specialist Food & Drink Marquees. Packed with plenty of tempting edible goods to try and buy, the marquees will oﬀer a wide selection of farmhouse cheeses, handmade chutneys and curry sauces as well as fine wines, chocolates, fudge, nuts and seeds and much more. Visitors can walk oﬀ an afternoon tea with a stroll through the Show Garden. With truly stunning arrangements, these beautiful gardens will be full of planting ideas to inspire budding or experienced horticulturalists. Plant and gardening pros will be on hand and more than happy to share their expertise, and with a large selection of seasonal plants for sale, visitors will be able to recreate or design their very own show garden. With free entry for under 16’s when accompanied by an adult, the Chilterns Craft & Design

Show is a real family day out. To ensure the children are entertained too, face painting, bouncy castles and a play area are all on hand not to mention live music and craft workshops. Finals for Finlay olly Finlay, aged 20 from Amersham, Buckinghamshire is now a Miss England finalist. After qualifying through various rounds, she competed in the Miss England semi final on Monday the 19th of May in Newark, Nottingham along with 80 other contestants. Holly was chosen by judges decision with another six girls to compete in the final on June 14-16th in Torquay. Holly tells us that she is shocked and totally overwhelmed to be given such an amazing opportunity. Miss England is sponsored by MTPA (Mauritius Tourism Promotion Authority) and Holly will be organising various fundraising events to help promote the sponsor. Fundraising will be easy for Holly as she has already raised 5,500 for Variety GB helping disabled and disadvantaged children across the UK.Â

National Apps for good DR CHALLONER’S GRAMMAR SCHOOL, AMERSHAM ANNOUNCED AS FINALIST IN NATIONAL STUDENT APP AWARDS ear 10 students from Dr Challoner’s Grammar School, Amersham are to be announced as a finalist in the prestigious and competitive national Apps for Good Awards, to be held on the 23 June at the Barbican in London. The national Apps for Good Awards celebrate the next generation of problem solvers and digital creators: students who have created an app that tackles an everyday problem or issue they are passionate about. This year, 18 finalist teams have been selected. Ian Fan, Matt Wood and James O’Brien have been shortlisted for their app, ‘Accumulus’, in the Productivity category, one of six thematic categories for the 2014 Awards. The app is designed to aggregate cloud storage through a single interface. The finalist teams from across the UK will now move forward to the Awards to pitch their apps in front of a panel of expert judges, comprised of industry leaders and high profile figures, such as digital entrepreneur Baroness Martha Lane Fox, Tech City CEO Gerard Grech and TV presenter Davina McCall. Ahead of the judging, the students will spend time in the oﬃces of some of the UK’s leading Tech City companies, acting as ‘accelerators’ to help the teams prepare to face the judges. The finalist teams are also up for the People’s Choice Award, which is selected by an online vote held from 11-23 June on the Apps for Good website (appsforgood.org). The winning school in each category will then spend the following six months having their app professionally produced in association with major sponsors and app development companies, before being launched to the public in January 2015. The boys said, “We’ve loved the chance to take part in the Apps for Good course and to be announced as a finalist is just brilliant! We’ve been able to develop not only our computing skills, but job skills such as teamwork, problem solving and independence.” Dr Mark Fenton, headmaster of Dr Challoner’s Grammar School, also commented, "Apps for Good enables us to bring a creative learning programme to our school that unlocks the confidence and talents of our

ne of the most important dates in the British social and International rowing calendars, the Henley Regatta is the epitome of an English summer day and is steeped in history. The Henley Regatta was first held in 1839 and has been held annually ever since, except during the two World Wars. The first regatta took place on a single afternoon; however its popularity with the oarsmen extended the race to two days. Later in 1886 the event was extended to three days and in 1906 a further day was added, lengthening the regatta to a four-day event. By 1986 it was further extended to five days proving its popularity with the British public during the summer. Originally staged by the Mayor and people of Henley as a public attraction with a fair and other amusements, the emphasis rapidly changed so that competitive amateur rowing became its main purpose. The Regatta occupies a unique position in the world of rowing. It has its own rules and is not subject to the jurisdiction either of the governing body of rowing in the U.K. (British Rowing) or of the International Rowing Federation, but is proud of the distinction of being oﬃcially recognised by both these bodies. The length of the Course is 1 mile 550 yards, which is 112 metres longer than the standard international distance of 2,000 metres. It takes approximately

seven minutes to cover, so there are often two races at once on the course for much of the day. The number of races is, of course, reduced on each successive day, leaving only the Finals to be rowed on the last day. Since 1884 the Henley Regatta has been organised by a self-electing body of Stewards, at present numbering 60, most of whom are well-known and successful rowers and scullers. The Stewards' primary consideration in all their decisions is the best interests of those who are competing in the races. The regatta today is one of the biggest events in the British and International social calendars and sees thousands of spectator’s crowd around the banks of the River Thames every day. The private enclosures are the best way to see all of the sporting action in style and comfort and there is a wide variety on oﬀer from Temple Island to the Stewards Enclosure. One of our favourite private enclosures is the “rhubarb” River Lounge which oﬀers a relaxed, sophisticated and stunning environment. The River Lounge is situated opposite the world famous Temple Island, only yards from the starting line. The famous Temple Island is situated a mile and a half downstream of the picturesque market town of Henley, on one of the most beautiful reaches of the River Thames. Located amidst rolling water meadows and surrounded by wooded hills, it marks the start of the famous

Henley Regatta Course and was a recent feature in the Olympic opening ceremony. The Temple itself is a delightful folly, designed by James Wyatt as a fishing lodge for Fawley Court, the Christopher Wren mansion on the Henley Reach, from which the Temple completed a charming prospect through an avenue of trees and provides a stunning view for the River Lounge and other enclosures. There is no formal dress code in many of the enclosures except the Stewards, however most spectators wear smart clothing with some guests opting for traditional Regatta attire in the form of lounge suits for men and dresses for ladies. Many of the enclosures start the day early including the River Lounge and Temple Island, whereby guests enjoy a delicious champagne and canapé breakfast. The “rhubarb” River Lounge goes further though, by creating beautifully sculptured gardens with alfresco seating and erecting a stunning marquee with a contemporary interior. The lounge serves gourmet cuisine throughout the day, a complimentary bar, live background music, head and neck massages and oﬀers a river cruise on-board the Lilly Anna. River cruises are another great way to see the river and to explore the

Regatta course but make sure you book early to avoid disappointment. Food and drink are as important as the racing during the regatta and there is a wide variety on offer from celebrity chefs to royal caterers across the course. “rhubarb” for example serve a variety of best of British and European menus from canapé and bowl food selections to stunning puddings and mains. Dishes include seasonal produce such as roasted rump of Welsh lamb, boulanger potatoes, green peas “à la Française”, buttered spinach and rosemary jus for a main or a choice of miniature puddings including Tiny Eaton mess, Pimms Royale jelly with summer berries, Mini summer pudding, Scottish raspberries and lemon curd in bitter chocolate pastry tartlet and Sherry trifle. The Regatta is easily accessible from Henley town centre with a quick trip over Henley Bridge towards Maidenhead. Parking is directly to the rear of the enclosures and is well sign posted. Races start from as early as 7.30am on some days and can go onto the early evening. Traffic can be a problem around the Regatta so make sure you leave plenty of time when setting off. The â€œreal Bond carâ€?, an Aston Martin DB 2/4 at the Old Admiralty Building

he car, which will be auctioned at Coysâ€™ traditional Blenheim Palace sale on 12th July, is believed to have been the inspiration for the Aston Martin featured in the original novel Goldfinger. This model is a Vantage specification 1954 DB 2/4 Mk I, bearing the registration JBW 974. For many years it sat idle, collecting dust and rust, until a father and son purchased it for restoration. It was during the restoration that a set of unique special modifications came to light, along with the carâ€™s connections to the British Intelligence Service and the creator of Bond himself, Ian Fleming.

A 1954 Aston Martin DB 2/4, which is understood to have been the inspiration for James Bondâ€™s Aston Martin, was unveiled by international auctioneers Coys at the Old Admiralty Building, where Ian Fleming had his office.

Phillip Ingram Cunliffe-Lister DSO, whose father was Lord Swinton, a close confidant of Winston Churchill and head of MI5 and the Security Executive during WWII. Moreover, the vehicle was regularly at Ian Flemingâ€™s direct next-door neighbour in Kent, and indeed it was the next-door neighbourâ€™s house which was also used as inspiration in the Bond novels, this time as Sir Hugo Draxâ€™s residence in the novel Moonrakerâ€? He added: â€œThis could be one of the most important discoveries of all time, confirming the link between the undoubtedly most famous spy in history and possibly the worldâ€™s most iconic sports car marque.â€?

COLOURS Continuing to set the tone and yet saturated in appearance - that's the colour world for the cool season. New shades of green that become more wintery as the collection progresses are in first place along with intensive blue gradings and deep, expressive tones of red. As the presence of black increases, the looks become calmer, with elegant accents beings added by warm colours such as sherry, chestnut and gold. As well as beige, grey will also be more important, especially in combination with blue or as a melange, and provides an important element for the whole outfit.

FABRICS As the temperature goes down, the proportion of high quality cashmere fine knit qualities will increase in the knitwear segment. Varicose veins are a common problem affecting at least 20% of the adult population in the UK. They occur when small valves inside the veins stop functioning correctly. Rather than blood flowing effortlessly back towards the heart, the blood stagnates within the veins causing aching,

Michael Gaunt tiredness and discomfort. At first, problem veins may not be visible but symptoms occur. Sometimes patches of blue thread veins may be the earliest signs of an underlying problem. Eventually, the abnormal veins may become swollen, enlarged and varicose. This condition can be embarrassing and painful. Varicose veins are usually blue or dark purple in colour and may also be lumpy or twisted in appearance. While any vein can be affected, varicose veins most commonly develop in the legs and feet – predominantly because standing and walking can put pressure on the veins in the lower body. WHY SHOULD I CONSIDER HAVING THEM TREATED? In the developing stages, varicose veins and thread veins may be primarily a cosmetic problem. However, if ignored, the condition can deteriorate and a variety of complications can occur such as leg swelling, pigmentation of the skin around the ankles and ulcers. Occasionally, veins can burst causing bleeding, or the blood within the veins can clot leading to the painful condition of phlebitis or even deep vein thrombosis. People who use air travel for work or holidays may be at increased risk particularly if those flights are long haul. IS THIS TREATMENT JUST FOR WOMEN? Certainly not, it can affect men just as much as it can affect women. Men tend to cover their legs up more, so the problem may be hidden away and ignored, with some men only seeking treatment when their veins become swollen and complications start to occur. It is important to target varicose veins before they become a problem, as without medical treatment some conditions may worsen. WHAT IS INVOLVED IN TREATMENT AND IS IT PAINFUL? Fortunately, modern, minimally invasive treatments can often be 58 3

performed under local anaesthetic as a walk-in, walk-out procedure. This leaves the patient with minimal postoperative pain with practically immediate return to normal activity. Both techniques are associated with considerably less pain, bruising and scarring than traditional stripping techniques. IS IT PERMANENT? Not all visible veins in the legs are varicose veins. Often the appearance of thread veins is due to the effect of daily wear and tear on delicate skin and local treatments are effective. However, sometimes thread veins are an indication of underlying varicose veins which may not be visible on examination. Treatment of the thread veins will not be successful unless the underlying problem - for example the varicose veins - has been corrected first. Therefore, a thorough venous assessment is necessary before initiating any local treatment for thread veins. This could include injection microsclerotherapy or VeinWave therapy. Once underlying varicose veins have been treated or excluded, local treatment of the thread veins is much more likely to be successful. In the shade Sunburn can be painful. It is also a clear sign that your skin has been damaged by too much ultraviolet (UV) radiation. Over time this damage can build up and lead to skin cancer. Spending time in the shade One of the best ways to protect your skin from the sunâ€™s UV rays is to spend time in the shade. The sunâ€™s UV rays are usually strongest between 11am - 3pm, so be careful about protecting your skin during these hours. Trees, umbrellas, canopies or buildings can all provide shade and help you protect your skin. Covering up in the sun Another way to protect your skin is with clothing. At least wear a T shirt, hat and sunglasses. Remember that the sun will reflect from the surface of water. So you can still burn if you are in or near water. Wear a hat with a wide brim for the best protection. Babies and children should always wear wide brimmed hats in the sun. Donâ€™t forget to protect your eyes. Wear good quality, wrap around sunglasses. Wraparounds stop the sun from getting in at the sides. Buy these for children too â€“ toy sunglasses can do more harm than good. Sunscreens Sunscreens can be useful for protecting our skin, but they will not protect us completely on their own. This is why using sunscreens together with shade or clothing can avoid sunburn. You should never use sunscreen in order to spend longer in the sun. What to buy The sun protection factor or SPF tells you the amount of protection the sunscreen gives against UVB radiation. Sunscreens are all tested the same way. So it is the level of protection they give you against the sun that matters more than the price. National guidelines recommend you use at least factor 15 (the higher the better). Choose a sunscreen that has good protection against UVA rays as well as a high SPF. This is because both UVB and UVA rays can cause skin cancer. In the UK, the level of UVA protection might be shown in one of two waysâ€Ś. Star rating You should see a star rating of up to 5 stars on UK sunscreens. The higher the star rating, the better. But the actual UVA protection also depends on the SPF of the sunscreen. For example, an SPF 25 with 3 stars may screen out more UVA overall than an SPF 10 with 4 stars. A symbol A symbol with the letters UVA inside a circle is a European marking. This means that the UVA protection is at least one third of the SPF value. Using sunscreen Donâ€™t assume that because you have put on sunscreen, you can stay out without burning. No sunscreen, no matter how high the factor, can provide 100% protection. And no sunscreen, whether itâ€™s 15 or 50, will provide the protection it claims unless it is applied properly. Therefore it is very important that you put on sunscreen generously and regularly. Research has shown 62

that people apply much less sunscreen than they need to. And worryingly, many people burn more frequently when they use higher factors of sunscreen because they stay out in the sun for longer. There is a concern that higher factor sunscreens may lure people into a false sense of security. Do check the use by date. Most sunscreens have a shelf life of 2-3 years, and will last about 12-18 months after opening. So it is usually fine to use last yearâ€™s, but not a bottle from 5 years ago! You should put your sunscreen on before you go out in the sun so the cream can be absorbed. Then put on another layer to make sure you havenâ€™t missed any bits. If needed you should apply moisturiser, insect repellant or make up afterwards. This is particularly important if you are using an organic (chemical) sunscreen. Vitamin D and the sun We all need some sunshine to make enough vitamin D to build and maintain strong bones. But it is not possible to give a one size fits all recommendation on how much sun is needed as this depends on a number of different factors. You do not need to sunbathe and should not have to redden or burn your skin to make enough vitamin D. Once you have healthy levels of Vitamin D, the body just gets rid of any extra. But some people are more at risk of low vitamin D levels, such as those with darker skin, people who wear clothing that fully covers them, pregnant women and older people. Protecting children Babies and young children have delicate skin. All children, no matter whether they tan easily or not, should be protected from the sun. Children with fair or red hair, pale eyes or freckles are at most risk. Keep babies under 6 months out of direct sunlight, especially around midday. Checking for skin cancers It is important that you make a habit of checking your own skin for any changes. This is especially important if you are at a high risk of getting skin cancer or have had treatment previously. If you find any changes in your skin that happen over a few weeks or months you should visit your GP. If your GP is worried that a skin abnormality may be a skin cancer they will refer you to a skin specialist. Depending on local NHS services you may be referred to a rapid access suspected skin cancer clinic. There are guidelines for GPs so they know when to refer someone with a suspected skin cancer to a specialist. There are private walk in skin clinics in the UK. But before you pay for this type of service, do make sure you know exactly what method of screening they use and whether it is reliable. Check if the people carrying out the screening are medically qualified because sometimes they are not. If you want a skin change checked then it is best to see your GP.

The â€˜orange peelâ€™ effect is caused by fat deposits just below the surface of our skin, causing the skinâ€™s surface to appear dimply and uneven. The gold standard treatment for getting rid of cellulite? Dry body brushing. By sweeping over your skin up towards your heart with a firm body brush on a daily basis, you stimulate the lymphatic system to disperse the fatty pockets built up under the skin. This takes dedication as you will need to repeat your body brushing routine daily but the result is soft legs with less dimples. Resistance training and cardio workouts are great exercises to blast the cellulite if you decide to go down the training route or â€“ if youâ€™re looking for a quick temporary fix â€“ fake tan can dramatically reduce the appearance of cellulite.

What and how? Squats â€“ for hips, glutes, quads and hamstrings Place your feet a little wider than shoulder-width apart with your feet facing forward and extend your arms out in front of you with your palms to the ground. Inhale into your stomach and start moving your hips back while slowly bending your knees. As your bottom sticks out, make sure you keep a straight back with your shoulders upright. Exhale as you reach the ground and then rise up again, using the strength of your legs and core to support you. Leg lifts â€“ for mainly the thighs and bum Lay down on your right side on top of an exercise mat or carpeted surface and rest your right elbow on the ground. Take a deep breath and then exhale as you move your left leg up towards the sky as high as you can. Repeat on your other side and do two sets per day for optimum results. Walking â€“ for inner and outer thighs and fat burning Walking is the perfect leg toning exercise if you favour cardiovascular work over strength training. For maximum results, walk up a hill or another inclining surface â€“ this will boost your metabolism, burn fat and tone your thighs.

Perfect legs Step 2: Exfoliate and de-fuzz Before you start your hair removal regime, remember to exfoliate first. While you take a bath or shower, apply an exfoliating leg scrub (or mix sugar and olive oil together for a natural, purse-friendly option) and scrub your skin in a circular motion. Once youâ€™ve finished showering, pat your legs dry and apply a moisturiser thatâ€™s right for your skin type. By applying your moisturiser when your legs are still slightly damp, itâ€™s easier for the lotion to sink in.

Perfect legs, Step 5: Natural-looking tan Thereâ€™s no doubting that a bronzed glow can make you feel healthier, sexier and thinner and what better way to achieve this look than with a fake tan? Not only are UV rays harmful to your skin, a natural tan from the sun can also be a time-consuming process. Instead, use a fake tan such as the Dove Summer Glow build-up moisturiser. This keeps your legs hydrated and smooth while covering minor imperfections and adding a sun-kissed glow to your pins.

HAY FEVER HELL ay fever, also known as seasonal allergic rhinitis, affects one in five people in the UK. According to John Collard, clinical director of Allergy UK, hay fever is a type of allergy. It happens when your body makes antibodies in response to certain triggers, such as pollen. Itâ€™s most common in children and, particularly, teenagers, but you can develop hay fever at any age. The symptoms usually include sneezing, itchy and watery eyes, and stuffy nose.

What causes hay fever? In Britain, hay fever is mainly caused by grass pollen. Around 95% of hay fever sufferers are allergic to grass pollen. Tree pollen can cause hay fever too. Around a quarter of hay fever sufferers are allergic to tree pollen. Mould spores and weed pollen can also trigger symptoms.The pollens that cause hay fever vary from person to person and region to region. The amount of pollen in the air will affect how bad your hay fever is. Thereâ€™s more likely to be more pollen in the air on hot, dry, windy days than on cool, damp, rainy days. Research shows that pollution, such as cigarette smoke or car exhaust fumes, also makes some allergies worse. When is hay fever worst? The time of year at which you begin to experience hay fever symptoms depends on the types of pollen youâ€™re allergic to. Trees release their pollen in March to early May while grasses release pollen from late May to early August. Weeds and certain shrubs release their pollen in late summer. The hay fever season can therefore last from March to October. And if youâ€™re unlucky enough to be allergic to more than one type of pollen, you may only have just two or three months without symptoms in the winter before the cycle starts again. Tips to relieve hay fever Avoiding exposure to pollen is the best way to reduce the allergic symptoms of hay fever: Keep windows shut at night and first thing in the morning Stay indoors when the pollen count is high (between 50 and 150) Wear wraparound sunglasses. Put some petroleum jelly just inside your nostrils to trap some of the pollen. Donâ€™t mow the grass or sit in fields or large areas of grass. Wash your hands and face regularly. Avoid exposure to other allergens, such as pet fur, or environmental irritants, such as insect sprays or tobacco smoke. Hay fever treatment As with most allergies, the best way to control hay fever is to avoid the triggers. However, itâ€™s difficult to avoid pollen, particularly during the summer. Even straightforward hay fever can be debilitating, causing runny eyes, sleepless nights, bunged-up nose and headaches. A range of over-thecounter products can treat the symptoms of hay fever, including tablets, nasal sprays, eye drops and creams. Antihistamines are the usual treatment for the main symptoms, such as itchy, watery eyes and runny nose, while steroid nasal sprays are the main treatment for a stuffy nose. Hay fever in pregnancy Hay fever during pregnancy can be a particular problem. Hormonal changes make nasal congestion more common during pregnancy, and this often gets worse during hay fever season. Pregnant women are also advised not to take some hay fever medicines. Hay fever and asthma Speak to your GP or pharmacist before you decide on a hay fever treatment. Itâ€™s particularly important to speak to your GP if you have asthma. Hay fever often makes asthma symptoms worse. If this happens, you may need to increase the dosage of your asthma medication. 66

Although ellow most moles are brown, they can vary from flesh coloured through yyello and blue, to black. Moles can be many different shapes and can be single or multiple, on almost any part of the bodyy.. E Evveerybody has moles; some people havee more than others. Howevverr, a small number may change with wiith time and have the potential to become a cancerous mole (malignant melanoma). If a mole changes in size or colourr,, if it itches, is painful or bleeds, or if new nodules devvelop around it or it looks red and inflamed then the mole should be removed for examination. exa M Melanoma elanoma T Tr Treatment reatment It is important that moles are removved properly so they can be thoroughly examined. It is usually recommended that moles are removed with an ellipse of normal skin around them, aiming for a 2mm margin of clearance. Most of the moles that are remo oved turn out to be fine. Pathologists (doctors who examine tissues under the microscope) use different terms for benign moles. They may be junctional, juvenile, blue, compound, dermal or intradermal. All these are descriptions for benign (non cancerous) moles. Some will be what are called dysplastic moles. These are thought to be precancerous moles that have the potential to become malignant melanomas if they are left. Although many of them may not turn cancerous it is impossible to tell and so they should probably be removeed. The next stage after the dysplastic 68

mole is the malignant melanoma in situ. This has just turned cancerous, but does not hav avvee the ability to spread. Like the dysplastic mole, it is cured by removing it properlyy. Malignant melanomas proper have the ability to spread around und the bodyy,, so the earlier they are caught the better. It seems that the most important factor in determining whether a malignant melanoma may have spread or not is its thickness. This is measured in mm and is called the Breslow thickness. Depending on the thickness, a wider removal of the skin around a malignant melanoma is usually recommended. Malignant melanomas that are less than 1mm thick have a greater than 95% chance of causing no further trouble. Another way of measuring the thickness of a malignant melanoma is the Clarkeâ€™s level. This relates to which layyers ers of th the skin the malignant melanoma has penetrated. This is less accurate that the Breslow thickness in predicting the likelihood of further problems.Melanoma Specialists As with other cancers, it is important that people who have had malignant melanomas are treated by a team of specialists. Melanoma specialists include a dermatologist, a surgeon and perhaps a plastic surgeon, an oncologist and a pathologist. Malignant melanoma risk factors Some groups of people should be regularly checked for abnormal moles. These include lude people who havvee had a malignant melanoma before, or there is a family history of it, fair skinned, fair haired or red haired people and those with a large number of moles, as well as those hose who havvee had considerable sun exposure in their lives, particularlyy with episodes of sevver ere sunburn when en they w wer ere young. o

TUBERCULOSIS DESPITE A LARGE REDUCTION IN THE NUMBER OF CASES OF TUBERCULOSIS (TB) IN THE UK, IN RECENT YEARS THERE HAS BEEN AN INCREASE IN INCIDENCE OF REPORTED CASES. LIZ D’ARCY EVANS, SENIOR PRACTICE NURSE AT DOCTORNOW EXPLAINS THE IMPORTANCE OF VACCINATING AGAINST TB. WHAT IS TUBERCULOSIS (TB)? TB is an infectious disease which is mostly caught by a germ called Mycobacterium Tuberculosis. It is a major cause of illness worldwide. The most common site of infection is the lung, but it can also aﬀect other parts of the body including the kidneys, bowel and lymph nodes. HOW DO PEOPLE CATCH TB?

In the UK the majority of cases of TB are caught through close contact with someone who has the infection. Like the common cold it is passed through sneezing and coughing. TB can also be contracted from unpasteurised milk of contact with infected cattle. WHAT ARE THE SYMPTOMS OF TB?

Symptoms of TB vary, if your have any concerns it is advisable to seek medical attention. Symptoms can include a persistent cough of more than three weeks, breathlessness and a lack of appetite and weight loss. 70

CAN TB BE TREATED?

WHO CAN BE IMMUNISED?

TB can be treated with antibiotics, but the treatment is prolonged and occasionally resistant cases are found.

Until 2005, all school children were routinely oﬀered the BCG at about the age of 13, however due to a change in national policy only those defined as being in a ‘high risk’ group are immunised under the Department for Health programme. Children can be vaccinated against TB from birth. The vaccine is not eﬀective in adults over the age of 35 years.

IS TB PREVENTABLE?

A vaccine known as the BCG (Bacille Calmette-Guerin) has been shown to give 70-80% protection against TB. The vaccine is safe and quick to administer. At DOCTORnow I routinely oﬀer the vaccine and we run monthly BCG clinics with our BCG nurse Paula Chamberlain. WHAT IS THE VACCINATION PROCEDURE?

The BCG vaccine cannot be given to someone who has previously had the infection. Before administering the BCG, a Mantoux test is carried out for anyone over the age of 6 years. The site of the Mantoux test is then inspected three days later to determine if the BCG is necessary. If the test is ‘negative’ the BCG vaccine is given. If it is ‘positive’ the vaccine is not given and appropriate advice oﬀered.

ARE THERE ANY ADVERSE EFFECTS FROM THE HAVING THE VACCINE?

Adverse reactions are uncommon, some patients may feel faint or dizzy immediately following the vaccination. Between two and six weeks after the vaccination a small spot will appear which will heal to form a scar, sometimes this forms a shallow ulcer or a raised scar called a keloid. 014 might not have been the perfect spring for sun lovers, but lawns throughout the UK are currently having the time of their lives. “The reasonably clement weather, mixed with a fair degree of rain, is the perfect cocktail for creating a great lawn,” comments Terry Nicholson of Lawnscience, the lawn care specialists. For those of us who are less than pleased with the appearance of our Lawns, he oﬀers three useful seasonal tips.

Again it’s all about the roots. As well as the length of cut aﬀecting the size of your lawn’s root system, the environment in which the roots live, also plays an important role in determining the size and health of the root system. Within the “root zone”, the top twelve inches of your soil, live billions of beneficial bacteria. This bacteria thrives in oxygen rich conditions. Regular lawn aeration, the introduction of holes into the lawn’s surface, encourages fresh oxygen to penetrate down into the root zone, creating a healthier home for your lawn’s roots.

As the days lengthen and the temperature warms up, now is the time of year when you need your lawn to perform. It’s also the time of year when your lawn can easily become stressed and unhappy. But worry not because, with a little knowledge, you can help your lawn to provide the perfect backdrop to those lazy summer garden parties. “All you need to know is the LAW’s of summer lawn care,” comments Terry Nicholson of Lawnscience, the lawn care specialists. For those of us who are less than pleased with the appearance of our Lawns, he oﬀers three useful seasonal tips. LENGTH OF CUT

For your lawn to look it’s best it needs regular access to nutrients and water. Your lawn’s root system extracts nutrients and water from the soil. But in the long warm summer days the water levels in your soil can reduce and, therefore, the root system needs to be well developed in order that it can extract water from a larger area. Basically your lawn needs big roots. The way you cut your lawn aﬀects the size and depth of its root system. Cut the lawn short and the plants photosynthesis is less eﬀective at 84

WATER

A correct watering regime is essential if your lawn is to look green and healthy over summer. Diﬀering soil types require diﬀerent watering regimes. A sandy soil, where the water leaches quickly through the soil, will require more regular watering than a loam or clay based soil, where the water is held within the root zone for longer periods. When you water, ensure that you water deep, with the water penetrating well down into the root zone. Shallow watering encourages shallow rooting leaving the lawn more susceptible to drought. Chilled Tomato Soup with pesto and crisp basil

The English Tomato season runs from June to October and this simple recipe will allow you to make good use of your surplus ripe tomatoes. There are a huge range of tomatoes from the regular round tomato, plum and small sweet cherries to tomatoes of various shades of purple, yellow, orange and stripy thanks to the rediscovery of the Heirloom varieties. The fun part is experimenting with the different flavours and finding one that inspires you. Should the weather change as it so often does this soup is equally as good served warm with toasted bread.

Ingredients

Pesto 1. Blend the pine nuts, garlic and parmesan in a food processor add the basil leaves with a little olive oil till a paste is formed.

(Serves 4)

Soup 1 kilo of ripe tomatoes A pinch of Malden salt 2 tbsp olive oil A pinch of caster sugar Freshly ground black pepper Couple of dashes of tabasco sauce Worcestershire sauce

Dried tomatoes 1. Thinly slice 2 plum tomatoes season with a little salt and pepper, lay on a piece of greaseproof paper, allow to dry in a very low oven for one to two hours Crisp basil 1. Carefully fry oﬀ a few basil leaves in hot oil till they stop crackling in the pan (be very careful to avoid splashes of hot oil) 2. Place your fried leaves on a paper towel until cool dry and crisp

Pesto 1 large bunch basil 2 tbsp lightly toasted pine nuts 1 clove garlic A splash olive oil 2 tbsp grated parmesan cheese

Method

To serve

Soup 1. Roughly chop three quarters of the tomatoes 2. Mix with the salt, sugar and oil and then allow to marinade for 1 hour 3. Roast in a medium oven for 25 minutes then liquidize in a food processor with the remaining raw tomatoes 4. Pass through a fine sieve. Season to taste with salt, pepper, tobasco and worcestershire sauce 5. Chill for 3 hours until nicely chilled The Artichoke is open for lunch and dinner from Tuesday to Saturday.
