StraCon provides a comprehensive employee benefits program that offers broad coverage, freedom of choice and tax-saving features. Our program integrates health and wellness, providing tools and programs to help our employees stay healthy. The program is offered to all full-time employees who are regularly scheduled to work an average of 30 hours per week. Coverage becomes effective on the first of the month following date of hire.
MEDICAL AND PRESCRIPTION DRUGS
The medical and prescription drug program helps our employees and their dependents maintain well-being through preventive care and access to an extensive network of providers, as well as affordable prescription medication and telemedicine services through Doctor on Demand. Three plans are available through HPI using the UHC Choice Plus network.
Preferred Provider Organization (Base and
Premier)
Both Preferred Provider Organization (PPO) plans offer services for in-network and out-of-network providers. Benefits are higher when in-network providers are used. In-network office visits, urgent care and prescription drugs are covered with copays. Deductibles and out-of-pocket maximums differ between the two plans.
High Deductible Health Plan
With the High Deductible Health Plan (HDHP), employees have the option of choosing any provider when health care is needed. This plan carries a higher deductible and a 100% general level of coverage after the in-network deductible is met.
Pharmacy Benefits
TrueScripts is your Pharmacy Benefit Manager (PBM), connecting you to expert support, affordable prescriptions, and mail-order convenience. Their inhouse pharmacists, nurses, and clinical advocates deliver personalized service to help you manage your medications and save money.
HEALTH SAVINGS ACCOUNT
Employees may be eligible to open a Health Savings Account (HSA) if enrolled in the HDHP. Contributions can be made through payroll deductions. The funds in an HSA may be used to pay for qualified medical expenses now or in the future. StraCon contributes $25 per pay period for employees enrolled in the HDHP and contributing to an HSA. HealthEquity is the administrator.
WELLNESS PROGRAM
Employees enrolled in one of StraCon’s medical plans receive a $25 monthly medical premium discount by participating in the company’s wellness program and completing required activities. Programs are available at no cost to manage high blood pressure, musculoskeletal pain, weight, risk of diabetes, mental health concerns, pregnancy and tobacco cessation.
DENTAL
Dental insurance with in-network and out-of-network benefits is available for employees and their dependents through Unum . Our DPPO plan provides preventive and basic care as well as major services, including orthodontia for adults and children.
VISION
Our vision plan from Unum/EyeMed provides access to quality vision care and helps pay the cost of routine eye exams, eyeglasses and contact lenses. The plan has a reimbursement and allowances program for out-ofnetwork services.
FLEXIBLE SPENDING ACCOUNTS
A Flexible Spending Account (FSA) allows you to set aside pretax dollars from each paycheck to pay for certain IRSapproved health and dependent care expenses. We offer four different FSAs through Higginbotham: two for health care expenses, one for dependent care expenses, and one for transit and parking expenses.
BASIC LIFE AND AD&D INSURANCE
StraCon pays the full cost of Basic Life and AD&D insurance for eligible employees. Employees are automatically covered at one times your annual earnings for Life insurance and AD&D through Unum
VOLUNTARY LIFE AND AD&D INSURANCE
Voluntary Life and AD&D insurance plans for employees and their dependents may be purchased if more coverage is needed. Evidence of Insurability is required for amounts elected over $150,000 for an employee and over $30,000 for a spouse. The following coverage options are available through Unum :
} Employee: $10,000 to a maximum of $500,000
} Spouse : $5,000 to a maximum of $500,000
} Children: $2,000 to a maximum of $10,000
DISABILITY INSURANCE
Disability coverage from Unum protects employees with a portion of their income in the event of illness or injury. Both Short Term Disability (STD) and Long Term Disability (LTD) are provided by StraCon at no cost to employees.
} STD – Covers 60% of base weekly earnings to a maximum benefit of $1,000 per week up to 12 weeks.
} LTD – Covers 60% of base monthly earnings to a maximum benefit of $6,000 per month.
Benefits may not be paid for conditions treated within the three months prior to the policy effective date until the employee is covered under the plan for 12 months.
MILITARY VETERANS’ BENEFIT
As a complement to TRICARE medical coverage, all retired non-Medicare military veterans can now enroll in a TRICARE Supplement Insurance Plan provided by Selman & Company. A TRICARE Supplement Insurance Plan is designed to supplement TRICARE to help with out-ofpocket health care expenses.
401(K) PLAN
StraCon understands the importance of financial planning and offers a 401(k) plan through Ascensus to help employees reach their retirement goals. StraCon provides a non-elective contribution of 4% of employee compensation. Employees may contribute up to 90% of their earnings, not to exceed current IRS limits. Employees are eligible to participate in the plan upon the attainment of age 21 and three months of service.
PAID TIME OFF AND PAID HOLIDAYS
StraCon provides paid time off (PTO) to help employees strike a healthy balance between their personal and professional obligations. PTO is accrued after your start date or upon transfer into a benefits-eligible position. PTO may be used for a variety of purposes. Employees are also eligible for 11 paid holidays per year.
