When you are healthy, you spend less on doctors and hospitals, you feel better, and you tend to live longer. The wellness program can help you set and reach your health goals.
The Wellness Portal connects you with the entire wellness program – including a digital library with articles, podcasts, and videos on health topics, and over 30 challenges for conquering stress, sleep, physical activity, and more.

Take the Health Assessment and earn 2,500 points. You will answer questions about diet, physical activity, tobacco use, and emotional health. You can complete it all at once or over time. Log in at www.wellontarget.com and click Start in the Health Assessment box. Based on your answers, you will get a personalized health report and plan.
You can get one-on-one coaching from health experts –including dietitians, nurses, and personal trainers – to help you set and achieve your goals. Message or call a coach from www.wellontarget.com or the AlwaysOn app Coaches can:

3. Once you complete your registration, print or download your membership ID card. You may also request to receive the ID card in the mail.

Routine recommendations for a healthy pregnancy:
z Pre-pregnancy check-up – Visit your health care provider before becoming pregnant.
z First prenatal visit – Review your personal and family history with your provider and discuss any concerns; your provider may examine you and perform some tests.
z Ongoing prenatal visits – See your provider throughout your pregnancy. A normal schedule is every four weeks for the first 28 weeks of pregnancy, then every two weeks until 36 weeks of pregnancy, then weekly until the baby is born.
z After birth visit – Schedule a visit with your obstetrician or provider within the first three weeks of giving birth. Have a comprehensive visit within 12 weeks of birth.
z Age appropriate well child visits
z Vision and hearing screenings

Protected Health Information will only be collected in aggregate form to design programs for the purpose of addressing overall risk(s).
