Our rehabilitation mission is to drive goals and outcomes. We believe that setting clear goals and measuring outcomes are essential components of rehabilitation. This ensures the best possible care for our patients. Our rehabilitation programmes use the International Classification of Functioning, Disability, and Health (ICF) framework, Goal Attainment Scaling (GAS), and the Modified Barthel Index to measure the outcomes of each patient. On arrival at Tokai Rehabilitation each patient is; •

The ICF Framework: The framework was developed by the World Health Organisation and is a comprehensive model that helps us understand the impact of health conditions on an individual’s functioning and disability. It encompasses three main components:

Body Functions and Structures: We assess which body structures have become impaired and which functions have been affected by the health condition. Activities and Participation: We evaluate how these health conditions affect an individual’s daily life and participation in society. Environmental Factors: We consider the influence of the physical, social, and attitudinal environment on an individual’s functioning.

Goal Attainment Scaling (GAS): Goal Attainment Scaling is a method used to set personalised rehabilitation goals with patients. Instead of relying on generic goals, GAS allows us to work with patients to define specific, achievable objectives that are meaningful to them. Each goal is assigned a numerical score, and we continually monitor progress. This approach ensures that our rehabilitation plans are patient-centred and outcome-driven. Modified Barthel Index: The Modified Barthel Index is a widely recognised tool for measuring an individual’s level of functional independence in performing activities of daily living, such as bathing, dressing, and mobility. This index assists in assessing and tracking patients’ progress. The Modified Barthel Index can be used to quantify changes in a patient’s functional abilities over time.

Orthopaedic Physiotherapists: Our orthopaedic physiotherapists have undergone rigorous training in orthopaedic-related surgeries and injuries. Working in close collaboration with orthopaedic surgeons ensures that patients receive a seamless continuum of care, from surgery to rehabilitation, resulting in a faster and more effective recovery. Cardiorespiratory Physiotherapists: Our cardiorespiratory physiotherapists have enhanced their expertise by completing courses through the American Association of Cardiovascular Pulmonary Rehabilitation. This advanced training equips them to provide specialised care for patients with heart and lung conditions, improving their overall quality of life and well-being. Breathlessness and Respiratory Physiotherapists: The unique skills of our two Bradcliff breathing trained therapists assist individuals in managing breathlessness and respiratory conditions as well as regain control over their breathing, facilitating a better quality of life. Amputee Rehabilitation Physiotherapists: Two of our physiotherapists specialise in amputee rehabilitation. They work closely with Malcolm Freedman and Associates to maximise prosthetic rehabilitation, ensuring that our patients regain their mobility, independence, and confidence. ICU Survivor Rehabilitation (PICS) + Physiotherapists: Our ICU Survivor Rehabilitation team collaborates with key professionals, including Psychologists, Occupational Therapists, and Speech Therapists, to address the cognitive, emotional, and physical impairments that often follow an ICU stay. We believe that true healing goes beyond treatment; it involves personalised care, empathy, and a commitment to our patients’ well-being. Wheelchair seating: An individual who has difficulty in mobilising within the home environment or within the community may require a wheelchair for mobility and occupational engagement needs. A wheelchair is not a one size fits all device. Every individual has different environmental, postural, sizing and occupational needs. Our Occupational Therapy team has 6 therapists trained in basic seating and/or intermediate seating. Cognitive assessments and screenings: We have five certified Occupational Therapists able to administer the MoCA (Montreal Cognitive Assessment or MoCA) Test. The MoCA has been validated as a highly sensitive tool for early detection of mild cognitive impairment (MCI) in numerous peer-reviewed studies for over 20 years. MoCA has been widely adopted in clinical settings and used in academic and non-academic research around the world. Hand therapy: Occupational Therapists have the knowledge and skills in fabricating and fitting splints and orthotic devices to support hand function and healing. These devices can be crucial for immobilisation, protection, and functional improvement. The restoration of hand function is essential for performing everyday tasks and maintaining independence. Several of our Occupational Therapists have performed short courses in hand therapy to improve their knowledge and skills in this field. Driving: One of the occupational therapists, Anne Scott, is currently enrolled in a driving course offered by the Driver Fitness Coalition. Functional Capacity Evaluations: A Functional Capacity Evaluation (FCE) is a comprehensive objective test of an individual’s ability to perform work-related tasks. An FCE is an integral part of the return to work process. Sasha Prinsloo, head of Tokai Occupational Therapy, is a WorkWell certified provider able to conduct FCE’s. An FCE reflects current research and clinical evidence in the fields of functional testing and worker compensation. Assistive devices: Occupational Therapists play a pivotal role in the prescription and utilisation of assistive devices to enhance the independence of individuals in their daily activities. Our approach encompasses assessment, prescription, customisation, training, and ongoing support. We also assist with motivations to funders for specific assistive technologies that may be needed. Palliation: Simone Bezuidenhout one of the Occupational Therapists completed a course in palliation care in 2023. Occupational Therapy plays a vital role in palliative care by helping individuals maintain their quality of life through adaptive strategies and interventions that address their physical, emotional, and functional needs.

Pulmonary Hypertension Symposium Faircape Health, in collaboration with the Jenna Lowe Trust and the Pulmonary Hypertension Association of South Africa (PHA SA) hosted the first ever Patients’ Perspectives Symposium on 13 May 2023 at the Faircape Health in Tokai Estate, Cape Town. While Pulmonary Hypertension is regarded as a rare disease, it is so often misdiagnosed or undiagnosed that the University of Stellenbosch researchers have estimated the prevalence of PH in Africa may range from 10% to 68%. It is estimated that 75 million people worldwide suffer from PH. Dr. Symons of the Jenna Lowe Clinic at Groote Schuur, the country’s first dedicated PH medical unit, provided an overview and insights into the work of the clinic and their learnings. Beyond organ transplants, there is still no cure for pulmonary hypertension; however, early diagnosis and treatment can drastically change the length and quality of a patient’s life.

Breathing Life Back In A Pulmonary Hypertension Success Story With any transplant, the recovery journey requires patience and focused rehabilitation. A rare and often life-threatening disease, pulmonary hypertension affects two of our most vital organs, the heart and lungs. Shortness of breath, lightheadedness, and chest pain are all symptoms of pulmonary hypertension, placing pressure and stress on these organs, straining blood circulation throughout the body. After being diagnosed with pulmonary hypertension shortly after giving birth to her son in 2017, Rafeeqa Lamera’s life drastically changed. Suffering from shortness of breath and lightheadedness, she was repeatedly misdiagnosed with an embolism. After five years of ineffective treatment, in August 2022, she was placed on the transplant list, and in November 2022, she underwent a double lung transplant surgery in the care of Groote Schuur Hospital’s Lung Transplant Programme. Her successful surgery and rehabilitation recovery made her the first pulmonary hypertension transplant survivor. However, after experiencing difficulty in the ICU, the decision was made to transfer her to Faircape Health Tokai Rehabilitation as a pro bono patient. Her rehabilitation team included physiotherapists, occupational therapists, a psychologist, and a team of doctors. The multidisciplinary rehabilitation team’s therapy guaranteed that Rafeeqa’s new organs were being used by a functional body, capable of sustaining her renewed lease on life and returning her to a quality of life that’s meaningful.

Cancer Conqueror An Oncology Success Story

Chad’s journey is an extraordinary testament to the power of the human spirit, unwavering support, and the incredible resilience of a young individual. His inspiring success story has been marked by courage, determination, and the dedication of healthcare professionals. At the tender age of 15, Chad’s life took a sudden and unexpected turn when he began to experience unbearable leg pain in November 2020. Initially attributed to growing pains, the pain grew progressively worse, extending to his legs, back, and neck. After multiple consultations with medical professionals, it became apparent that a deeper medical mystery was unfolding. In March 2021, a new doctor recommended MRI and CT scans, leading to a diagnosis no family ever wants to face: Ewing Sarcoma, a rare form of cancer. Chad embarked on a gruelling journey of chemotherapy and radiation treatments, a journey that would span over a year and be marked by numerous medical complications.

His path to recovery has been nothing short of astonishing. Chad is not only attending school but also navigating life’s challenges with newfound confidence. When needed, he uses a wheelchair with ease, and for shorter distances, he walks independently with crutches. His journey underscores the remarkable progress that can be achieved with courage, perseverance, and a steadfast support network. With the support of his prosthetics, Mr. de Jong achieved remarkable milestones. He was able to traverse five metres with a walking frame, be aided moderately, and stand with minimal assistance while holding the walking frame. A Functional Capacity Evaluation (FCE) is a comprehensive objective test of an individual’s ability to perform work-related tasks, it is also an integral part of the return to work process. WorkWell FCE’s reflect current research and clinical evidence in the field of functional testing and workers compensation. The WorkWell FCE testing includes: •

If there are not any multiple comorbidities, this FCE testing is more effective and can be used for an early functional evaluation (for decisions regarding transitional or alternative work duties). Testing can also be tailored to test specific job requirements. This will enable us to identify limitations or issues in the worker’s ability to return to their job. Our goal is to provide the information needed relating to an individual’s abilities to match their job requirement.
