ABOUT FAIRCAPE HEALTH TOKAI

Faircape Health Tokai provides restorative care and rehabilitation for those recovering from injury or surgery. Our world-class care aims to bridge the gap between hospital and home. Our Multidisciplinary Team (MDT) of leading healthcare professionals administers care in the most therapeutic environment possible and supports healing outcomes in all aspects. The step down facility is located within 500m of the M3 motorway close to the Blue Route Mall. COVID-19 control and the management of risk is a priority for us. In light of the coronavirus pandemic, we have implemented strict COVID-19 control policies and procedures.

COMPREHENSIVE CARE 24/7 The comprehensive care provided by our Multidisciplinary Team ensures that all healing needs are addressed to support optimal outcomes. Amongst our fields of expertise, we are able to provide individualised care for those recovering from (but not limited to): • •

Neurological conditions (including stroke) Cardiopulmonary conditions (including heart attack)

Long COVID Orthopaedic conditions

COMPASSIONATE CARE SERVICES Faircape Health offers palliative and respite care services. We aim to improve the quality of life of those facing terminal illness, and ensure they are as pain free, dignified and comfortable as possible. Respite care services allow for a short term period of private care (not funded by the medical aid). This may be purely a relief for the caregiver or family as required.

MEDICAL SERVICES Faircape Medical is our aligned medical practice of five doctors. They administer care at the Faircape Health Rehabilition Centre where we have achieved a 98% 5 star rating. On arrival at Tokai Healthcare, the Nursing Services team will admit the patient to the facility. The doctor will immediately assess the patient and refer him or her to the relevant MDT members. Specialised Medical Services include: • • • • •

Dialysis Palliative care Oncology care Transplant care Blood transfusions

NGT and PEG feeding Stoma care IV fluids and antibiotics Wound care X-ray facilities

DOCTORS ROLE IN YOUR CARE WHILST AT TOKAI ESTATE We have a team of very passionate and dedicated Family Physicians and General Practitioners who will look after you during your stay at Faircape Health Tokai. Our role is supportive and we aim to ensure that you are medically stable and able to get maximum benefit from rehabilitation. We focus mainly on acute conditions that immediately affect your rehabilitation progress, and we are in contact with your hospital specialists to ensure continuity of care. All medication given while at our facility will be provided by our pharmacy and claimed from your medical aid hospital benefit. We don’t allow patients to use their own medication while with us. We have X-Ray facilities and are able to give intravenous fluids and antibiotics should you require them. Patients need to follow up with their GP or specialist for chronic medications and conditions after discharge from Subacute. We don’t routinely investigate or treat chronic conditions during Subacute stay.

FREQUENCY OF DOCTOR VISITS You will be assessed by a doctor on the day of your arrival and they will ensure that you receive all necessary chronic and acute medication while at Faircape. Following admission, the doctor will see you daily Monday-Friday and over the weekend if needed.

LENGTH OF DOCTOR VISITS The admission consultation is generally 20 mins. The daily consults that follow are mostly brief and aimed at treating acute problems and ensuring medical stability.

DOCTORS CONTACT WITH FAMILY Our preferred method of contact is via email, and we will generally only call your family on admission and thereafter only if you are medically unstable. We encourage you to stay up to date with specialist followups. Families are responsible for taking you to your follow up appointments as we don’t have transport to / from hospital unless in the case of an emergency. You will also need to follow up with your GP or specialist for chronic medications and conditions after discharge from Subacute.

REHABILITATION SERVICES OCCUPATIONAL THERAPY Tokai OT is our aligned occupational therapy practise. This team promotes participation and independence in meaningful occupations through restoration of ability and environmental adaptations. Patients are seen on a referral basis. We assist patients in the following areas: • • • • •

Self-care training and recommendations Upper limb rehabilitation and splinting Cognitive assessment and therapy Visual perception and sensory rehabilitation Home assessments and adaptations

PHYSIOTHERAPY Tokai Physiotherapy Inc approaches rehabilitation with the same commitment to excellence so intrinsic to Faircape Health. We assist patients in the following areas: • • • •

SPEECH THERAPY Our aligned speech and language therapist helps residents to acquire or reacquire the ability to communicate and swallow to the best of their ability on a referral basis as needed. Our team assists patients in the following areas: • •

Patients with Neurological conditions (ie: stroke, head injury, tumor or dementia) Palliative or end of life patients

We provide treatment of: • • • •

Language disorders following neurological • fallouts • Swallowing disorders • Motor speech disorders Implementation of alternative and augmentative communication where needed

Voice disorders Cognitive linguistic rehabilitation Collaboration with the Dietitian to ensure patients receive the best food in the appropriate consistency in order to meet their needs

Please Note: The number of therapy sessions (such as physiotherapy, occupational therapy, and speech therapy) you receive per day is determined by your medical aid. Please speak to the Patient Assessment Sister or the administrative team to determine what is covered by your specific medical aid.

SUPPORT SERVICES SOCIAL WORKER Our Discharge Planner is a qualified Social Worker. Our Discharge Planner’s role is to facilitate the safe discharge of patients from the sub-acute environment. Where possible, this includes engaging directly with the patient regarding their future care needs and corresponding plan for either returning home with home-based care, or transitioning to a long-term healthcare/residential facility. The Discharge Planner also liaises with the next-of-kin to develop an appropriate care plan as per the recommendations of the treatment team. The Discharge Planner offers emotional support to patients and family members alike, when necessary, and facilitates family meetings to ensure that the patient’s care needs are communicated to and understood by both the patient and their next-of-kin so that optimal recovery is achieved post discharge.

PSYCHOLOGIST Our aligned psychologist, when referred by our MDT, provides therapy using a variety of methodologies to alleviate mood and cognition related symptoms, as well as adapt to changes and strengthen resilience on a referral basis as needed.

DIETITIAN Our Dietitian when referred by our MDT ensures that patients receive optimal nutrition to assist in speedy recovery and lasting health. Patients are seen on a referral basis for input on the following: • Enteral feeding • • •

Oral Nutritional supplementation regimes Meal plan adjustments to accommodate food allergies, intolerances and other feeding challenges Nutritional counselling, guidelines and meal plans www.faircapehealth.co.za

ACCOMMODATION & AMENITIES Rooms: 60 | Medical Beds: 70 Faircape Health Tokai Estate’s accommodation is specifically designed for comfort, and stress-free recovery. Patients will be placed in a shared suite replete with amenities and features to create a healing environment. A co-payment of R600 plus VAT per day will enable you to obtain a private room. From en-suite bathrooms, to assisted and wheelchair accessible showers. Faircape Health combines world-class healthcare provision with the comforts of premium hospitality. We ensure patient safety and security while monitoring carers and nursing staff with CCTV cameras in all rooms and common areas. Please note that the footage is strictly only accessed by our Medical Personnel.

COMMUNICATION POLICY Family communication: On the day of admission a Welcome Email will be sent by our administration department to the patients next of kin/contact person for the multidisciplinary team, as communicated to our Assessment Nurse. This mailer will introduce the healthcare team who will be caring for the patient.

This email will provide the contact details of our reception staff who will be able to contact and relay messages to our rehabilitation team, which includes the Doctor, Physiotherapists, Occupational Therapist, Speech Therapist and Discharge Planner. All patients who are capable of providing feedback on their rehabilitation to their families are requested to do so themselves. This will allow our healthcare team to focus on the patients and provide them their undivided attention. The Doctor will only email or call the families if there is an emergency or the patient is medically unstable. For those patients who are not able to provide feedback to families, the Doctor, Physiotherapists, Occupational Therapist, Speech Therapist and Discharge Planner will communicate to you via email. Our healthcare team takes great pride in offering your loved ones the best care possible and are fully committed to caring for them, and will make contact and provide updates when required. Please note, our discharge times are between 11am and 12pm. If you would like to enquire about a patient in the facility, we ask that you please email the respective rehabilitation team regarding your query.

VISITATION POLICY Family and friends of patients are required to book visiting slots in advance via a visitors booking link which can be found on the Faircape Health website (www.faircapehealth.co.za). Simply navigate to the ‘Book a Visit’ tab and click the Tokai Healthcare ‘Book Now for Sub Acute Residents’ button. • • •

Visiting Hours: 11:00pm - 1:00pm & 5:00pm - 7:00pm (applicable to private and shared rooms) We are allowing 2 visitors per timeslot for a sub-acute patient. Please book visits according to this. Please do not bring any food items or alcohol during your visit. Should you wish to bring alcohol, this needs to be authorised by the patient’s doctor in the facility.

STEP DOWN ADMISSIONS Step down admissions take place on a referral basis from the hospital.

MEDICAL AID PATIENTS 1. At least 48 hours before discharge from Hospital: referring Doctor completes Subacute Authorisation & Patient Referral Form and sends to casemanager@faircape.co.za. 2. Faircape Health Tokai Case Manager submits to medical aid for approval. 3 Once authorisation has been received, F aircape Health Tokai staff liaises with the treating doctor to confirm when the patient will be ready for transfer. 4. Once discharge date is confirmed, the patient is seen for assessment within 48 hours before transfer is accepted. 5. Depending on the medical aid, Faircape Health or the hospital will arrange an ambulance to transfer the patient to Faircape Health Tokai Estate.

PRIVATE PATIENTS 1. At least 48 hours before discharge from Hospital: referring Doctor completes Subacute Authorisation & Patient Referral Form and sends to casemanager@faircape.co.za. 2. Case Manager and Nursing Services Manager confirm bed availability. 3.Patient Assessment Sister from Faircape Health visits patient in the hospital to assess the patient. 4. Quotation is provided by Faircape Health to the patient or next of kin. 5. Once quotation is accepted, referring Doctor discharges from hospital and Faircape Health Tokai can arrange ambulance transfer to Faircape Health. Ambulance transfer is for own account. Please Note: A consumables deposit will be included in the initial quotation which must be settled on admission to cover medical consumables.

WHAT TO BRING Use the following checklist to ensure you bring everything you need for your step down stay at Faircape Health Tokai Estate: Identity Document Medical Aid Card Credit Card Medication Script Toiletries Clothing (Loose-fitting, relaxed wear which will help to keep you as comfortable as possible is suggested.) Swimming costume for hydrotherapy (not for orthopaedic patients or patients with wounds) Clothing appropriate for use in the gym (please note no dressing gowns and pyjamas to be worn in the gym facility) You may also choose to bring along any personal items which will add to your comfort such as a hairdryer, a book, stationary, mobile device and chargers though it’s advisable to leave valuables at home. Double rooms will be provided with earphones and private room residents will need to supply their own earphones. Please note, for safety reasons own medication is not permitted at Faircape Health Tokai. Medication will be issued by our inhouse pharmacy and administered by our staff. Medication is covered by the medical aid and billed under the hospital benefit while admitted to Faircape.
