Coproducing Hospital Discharge Pathways in Stoke-on-Trent presents a collaborative, evidence-based exploration of the challenges faced by people experiencing multiple disadvantage when leaving hospital. Developed by VOICES through partnership working between 2016 and 2022, the report brings together lived experience, frontline insight, and system-wide perspectives to examine persistent gaps in discharge processes. It highlights key issues including late discharge planning, poor information sharing, and limited coordination between hospital and community services (see findings on page 5).
Through a co-production approach, the report identifies practical opportunities for improvement, including earlier identification of housing need, stronger multi-agency working, and greater use of trauma-informed and psychologically informed practice.
