Medical Care: Come in for a same-day onsite clinic visit to monitor a condition or engage in preventive care - there is a 24/7 on-call physician.
Therapy: Receive physical, occupational, speech and recreational therapy at home or in the state-of-the-art therapy gym.
Medication: Your over-the-counter medications are covered, and delivery is simple.
Dining: Enjoy meals prepared by a talented chef who balances health and nutrition with flavor and fun.
Enriching Outings: Enjoy community events, movies, shopping and more on outings arranged by the EverTrue PACE team.
Social Activities: Make connections through EverTrue PACE programming, activities, and shared meals. Activities include arts and crafts, gardening, games, live entertainment, group exercise, and more.
Mental Health Counseling and Crisis Intervention: EverTrue PACE social workers provide support in everything from conflict resolution to bereavement and trauma.
Personal Care: At the EverTrue PACE center, you can do everything from take a shower to get help with meal preparation to activities of daily living like shopping and housekeeping.
Case Management: You will have regular assessments to identify if your care plan needs an update due to changes in your health or lifestyle. You can get help accessing benefits and resources like Social Security, housing support, nutrition and more.
Qualifications for Enrollment
Enrolling into EverTrue PACE is voluntary. Enrollment into EverTrue PACE happens on the first day of the month after you are approved.
• Age 55+
• Live in the designated service area
• Certified by the state of Missouri as meeting level of care requirements
• Able to live safely in the community
You can disenroll in EverTrue PACE at any time. Disenrollment becomes effective on the first day of the following month. A team member can help you with this process.
Program Cost
EverTrue PACE accepts Medicare, Medicaid or private payment. If you are eligible for Medicaid, our enrollment team will assist in determining if you will have a monthly premium and explain payment options.
What you pay for EverTrue PACE depends on your eligibility under Medicare and Medicaid.
Once enrolled, our team handles all paperwork and claims.
