ISAIAH 43:19

See, I am doing a new thing! Now it springs up; do you not perceive it?

Dear EverTrue Partner,

In 2025, the story of EverTrue is Evolution

With more than 165 years of history and expertise in aging services, EverTrue embraced an evolution this year, not just in our name, but in our renewed commitment to our faith‑based mission, values and purpose. Our evolution honors a 166 year legacy, remaining “ever true” to those we serve, while embracing the evolution in aging services that is a response to the changing – and growing – needs and expectations of older adults.

Everything we’ve accomplished — and everything we aspire to achieve — begins with our mission: Older Adults Living Life to the Fullest. This mission shaped an evolution in experience, guiding the development of new programs and expanded services that enrich daily life for our residents, clients and team members.

That same mission encouraged an evolution in innovation — launching new ideas, models and approaches — and an evolution in partnerships that support growth, collaboration and mission expansion. Together, these efforts allowed EverTrue to broaden its reach and deepen its impact, affecting more older adults in meaningful ways. As we look ahead, EverTrue’s evolution through growth enables us to serve more people while benefiting from an evolution in philanthropy that celebrates the generosity of those who bring the EverTrue mission to life.

As EverTrue continues its evolution, our purpose remains clear: Centered in Christ, together we create places, services and opportunities for people to age well with purpose and fulfillment. This purpose is lived out through a culture that brings out the best in our team members and in programs and services that adapt with those we serve, all in support of a sustainable financial future.

This evolution is possible thanks to our board members, team members, partners and especially to those we serve. Your trust, passion and collaboration made 2025 a monumental and meaningful year.

We invite you to explore our EverTrue evolution through this Annual Report—and look forward to continuing our mission together.

With gratitude,

OUR MISSION

Older Adults Living Life to the Fullest OUR

PURPOSE

Centered in Christ, together we create places, services and opportunities for people to age well with purpose and fulfillment. OUR VALUES

We receive and give grace in faith.

We give our best. We share ideas. We meet you where you are, and we grow together. We multiply joy.

AN EVOLUTION IN AGING SERVICES

The evolution of aging services supports a reality where people are living longer, more diverse lives; where economics and longevity play a part in choice driven support; and where older adults expect purpose, engagement and independence. Advances in medicine, a rising median age and Baby Boomers as the fastest, and largest, growing age group is creating a long term demographic evolution that shapes health care, housing and aging services.

At EverTrue, we embrace this evolution and have adapted it to create our whole person philosophy – Life According to You. We create housing, services and care options that focus not just on where you live, but how you live. We emphasize wellness for the mind, body and spirit, and as a nonprofit, EverTrue reinvests in our community experiences and home based offerings to continue our legacy while embracing innovation.

Our focus is not only on the individuals we serve, but also our more than 3,700 team members who partner to make EverTrue a trusted, world class leader in the aging services industry.

16,500+ Individuals Served annually through our Life Plan Communities, Affordable Housing and Anywhere Care programs and services annually.

“We are here together, getting to know each other and care for each other. I came here because of the value, but I stay here because of the friendships and comfort they give to me.”

LIFE PLAN COMMUNITIES

Offering 14 Life Plan Communities, EverTrue provides unique opportunities in Independent Living with expanding care models for Memory Support, Personal Care/Assisted Living, Long term Care and Short term Rehabilitation. “As a 91-year-old widow who has lived alone for 40 years, EverTrue has given me safety, comfort, activities, new friends, outpatient therapy, walking paths and more,” said Janet Rathgeber, EverTrue Laclede Groves resident “Thank you EverTrue – You have provided security, companionship and fun.”

AFFORDABLE HOUSING

Our 16 EverTrue Affordable Housing locations offer individuals on a fixed income the option to stay in a community that is not only focused on wellness and security, but also offers companionship and opportunities. “I moved here when my house started to feel more like a burden than a comfort,” said Ellen Brownlee, a 15 year resident of EverTrue Rose Hill House. “The people are friendly, the environment is welcoming and everything I need is close by, from stores to my doctor’s office. I love it here as it gives me both independence and peace of mind.”

ANYWHERE CARE

EverTrue Anywhere Care expanded to eight programs and services, including Answers, Home Health, Home Help, At Home+, GUIDE, Palliative Care, Hospice and PACE. For EverTrue team member Dia Finder, who also benefitted from an Anywhere Care program as her mother’s caregiver, being a part of a team who reach people where they are, when they need it, is more than a job, it’s a calling. “It’s so much more of a relationship than a job,” said Dia. “What I experienced was so meaningful and to know that I can give that back to my clients, it’s a win-win. It makes my days have purpose.”

AN EVOLUTION IN EXPERIENCE

The EverTrue Experience — or how we show up through our mission, purpose and values — encourages us to remain “ever true” to our legacy while continuing to expand our impact. Our faith based Lutheran legacy and its continuation in our organization, creates an EverTrue experience that reinforces our mission, centered on Christ’s example.

We look to our residents, family members, team members and the public to partner with us in this evolution, providing feedback, insights and requests so that together we can enhance and expand the EverTrue experience for all.

14% Employee Growth

shows EverTrue’s dedication to the team members who serve our residents, clients and families. EverTrue was honored as Healthiest Employers in Illinois, Pennsylvania and St. Louis, recognized for our benefits, committed career growth and joyful atmosphere.

“When I joined EverTrue five years ago, I loved that my role was more of a ministry. I build relationships – with my fellow team members, with community volunteers and benefactors and with the residents here. We become family, someone you can depend on and rely on without fail. That continues to inspire me daily.”

FAITH EXPERIENCE

As a faith based nonprofit, EverTrue regularly enhances chaplain services in support of those we serve. “Our 21 chaplains are known deeply within each of our communities and services, working with residents, clients, team members, even family members in support of every faith-based request,” said Travis Scholl, Corporate Director of Mission Integration. “Our chaplains are constantly reviewing how they can bring ministry to their communities, while also providing specific, individualized guidance, comfort and care.”

TECHNOLOGY EXPERIENCE

EverTrue has launched its Modernized Network, a turnkey digital experience delivering fast, reliable internet, in home entertainment and seamless communication for residents, visitors and team members in all our communities — expertly supported by EverTrue’s technical team. “This investment elevates everyday living through intuitive, connected technology,” said Josh King, Vice President of Information Technology. “The Modernized Network brings innovation to life with purpose, performance and measurable value.”

FUTURE EXPERIENCE

Using our existing footprint and adding feedback from residents, family members and caregivers, our EverTrue Master Plan ensures that the programs and services supporting our mission continue to reflect the desires and needs of those we serve. Our infrastructure emphasizes lifestyle, care, flexibility and resident centered design. EverTrue’s efforts in reinvestment were acknowledged by LeadingAge Ziegler 200 List, which recognized us as the 11th Largest Nonprofit Aging Services organization in the country.

AN EVOLUTION IN INNOVATION

EverTrue’s evolution in innovation reflects a deliberate commitment to shaping the future of aging services with purpose and possibility. As the needs, expectations and realities of older adults change faster than traditional models can keep up, EverTrue works to innovate not only in technology, but in our programs and services to use resources wisely while remaining sustainable in a rapidly shifting environment.

In 2025, we saw technology innovations that enhanced connection, safety and operational insight. We worked on evolving our care models toward more personalized and responsive care plans by trademarking our StillYou® care model; and we invested in construction that prioritizes sustainability and flexibility. By focusing on innovation, EverTrue continues to reimagine what aging services can be for those we serve.

102 Residents of Affordable Housing benefiting from Project Wellness, a monthly nurse visit at the community that provides one on one consultations and care.

“I love that EverTrue is looking at preventative health. Older adults come with different abilities, needs and goals, and Project Wellness meets them where they are—literally and figuratively because we are right where older adults are living.”

LIFE PLAN COMMUNITIES: INNOVATION IN CARE

Our Life Plan Communities nurture wellness through learning and community engagement. Our trademarked StillYou™ care model, a customized care philosophy created by our own team members, provides residents and clients with a diagnosis of dementia with intentional, person centered care. We also became certified as experts in the treatment of heart failure patients by the American Heart Association.

AFFORDABLE HOUSING: ACTIVITY INNOVATIONS

Our Affordable Housing team broadened its wellness programming through grants and foundation funding, launching three new programs and services designed to meet the needs of our residents – Vitality in Motion dance classes, Missouri Botanical Garden’s flower arranging and Uber Health transportation opportunities. The team also helped to advance Project Wellness, with more than 250 residents receiving health and wellness monthly check ups from a qualified caregiver.

ANYWHERE CARE: INNOVATIVE OUTREACH

EverTrue Anywhere Care’s evolution included refining services it already provided and adding customized care offerings. We added GUIDE, a dementia program for patients and caregivers, and were accredited by the National Institute of Jewish Hospice, providing care during times of need and respite to our Jewish community. We also expanded PACE in St. Louis, supporting older adults like Richard, who want to remain at home but need help. “EverTrue PACE had therapists who helped me get back to myself. This is a blessed place.”

AN EVOLUTION IN PARTNERSHIPS

EverTrue’s evolution has been guided by a simple belief: we are stronger together. As the needs of older adults grow more complex, partnership has become essential to advancing our mission and ensuring long term sustainability.

Through strategic and community partnerships, EverTrue has grown thoughtfully — extending our reach while remaining rooted in nonprofit stewardship. We look for partners who share our values and embrace community centered care.

Each partnership brings local expertise, advancing trusted relationships by providing deep understanding of the communities we serve. Partnership also allows us to reinvest more deeply in what matters most — our people, our places and the future of aging services. As we look ahead, collaboration will remain a cornerstone of our work, enabling us to honor our EverTrue legacy, while responding to the evolving needs of the aging services industry in our efforts to embrace our mission: Older Adults Living Life to the Fullest.

Team Members

have completed the AI training modules, helping both Daugherty Foundation and EverTrue learn more about caregiving for individuals with dementia.

“The AI training made me more comfortable with my clients because each situation in the training was different – just like the people we serve. Now the answers are part of me, making me a better caregiver.”

PPH PARTNERSHIP EMBRACES EXPERTISE

In 2025, EverTrue launched a management partnership with Philadelphia Protestant Home (PPH), a Life Plan Community in Philadelphia, with a goal of sharing talent and expertise. According to PPH Board Chair Tim Hinchcliff, “We knew our board made the right decision with EverTrue when we could see our values align to empower our staff, bringing forward hidden talents that have made our residents happy and our staff happy, offering the best quality of life for both.”

THREE OAKS PARTNERSHIP LAUNCHES

Three Oaks Senior Dining, a joint venture of EverTrue, Presbyterian Senior Living (PSL) and hospitality expert Culinesse, was launched in 2025. This partnership creates efficiencies and best practices to meet the needs of communities while advancing dining services. Tyler Milner, dining director for EverTrue Meramec Bluffs, is now Regional Director of Operations with Three Oaks, “My experiences at EverTrue shaped my belief that dining is more than a meal, it’s a daily opportunity to create connection and joy. Three Oaks will take our dining a step higher.”

EVERTRUE AND AI TRAIN DEMENTIA CAREGIVERS

EverTrue has partnered with Daugherty Foundation on interactive, real time Artificial Intelligence (AI) training for caregivers who interact with individuals with dementia. Offering multiple scenarios and real time responses, this AI “resident” adds to EverTrue’s team training. “Going through the AI training opened my eyes to what it must feel like living with Alzheimer’s,” said Lauren Jones, Administrator of Care Center and Assisted Living, EverTrue Mason Pointe. “The real-time feedback changed the way I communicate — making me aware of how meaningful small adjustments can be.”

AN EVOLUTION IN GROWTH

At EverTrue, growth is a deliberate act of stewardship — rooted in mission and guided by purpose. Through growth, EverTrue continues to evolve into an organization positioned to respond to changing demographics, workforce challenges and the growing demand for high quality, values based aging services.

EverTrue’s thoughtful affiliations are guided by a shared commitment to mission, people and long term sustainability. We have expanded by welcoming organizations and communities that share common values, including faith based service and a believe that aging deserves respect. We have also embraced innovation thanks to philanthropic partnerships, providing resources to our communities, programs and services that support an increasingly complex aging services environment.

This growth ensures that EverTrue can continue to honor its mission today while building a sustainable future for generations to come.

Growth

for EverTrue Anywhere Care

“I was impressed by the longevity of EverTrue and its emphasis on people first,” said Mary. “It is a real comfort to know that they are focused on improving care and being thoughtful to the changes that might bring the most impact for the most people.”

NEW CONSTRUCTION ADDS COMMUNITY

EverTrue Crossroads at Bevo Mill opened providing affordable housing to older adults in a historic St. Louis neighborhood. Designed to meet the city’s growing need for stable, affordable housing for older adults, the new community features 64 apartments. Built in partnership with Tower Grove Community Development Corporation, the community brings new housing options to an area where longtime residents increasingly face affordability and accessibility challenges.

AFFILIATION EXPANDS EVERTRUE

The EverTrue affiliation with St. Andrew’s Resources for Seniors System included two Life Plan Communities, six Affordable Housing communities, a specialized 55+ community, in home services and philanthropic programs. We welcomed more than 1,500 new residents, hundreds of new home care clients, and 425 new team members, adapting to evolving aging services through growth while maintaining financial and operational strength. “The change was in the best interest of the residents,” said Ann Prenatt and Mary Calcatera, both residents of The Willows of EverTrue Brooking Park. Ann continued, “EverTrue brings with it more resources and human capital to support us and to support the staff we already have.”

EXPANSION REACHES MORE OLDER ADULTS

For individuals who prefer to age at home, EverTrue Anywhere Care launched At Home+, a private pay membership program for older adults looking for expert support. EverTrue also partnered with CMS to pilot the GUIDE program, designed to improve the quality of life for people living with dementia as well as their caregivers.

AN EVOLUTION IN PHILANTHROPY

Acts of generosity advance our mission, while we utilize our values to sustain our purpose in service to older adults. For EverTrue, giving ensures security, upholds dignity and provides hope by fueling the programs that enhance care and expand access with the goal of enriching the lives of older adults and those who serve them.

Philanthropy is more than support for EverTrue—it is a partnership in purpose. Every gift, whether large or small, helps create stability, replaces uncertainty and says “You are seen” to older adults and team members alike. Together, through generosity grounded in shared values, we can create lasting impact and carry EverTrue’s nonprofit purpose forward with security, dignity and hope.

3,043 Donors gifted to EverTrue’s programs and services through direct philanthropy.

$14.7 Million raised through Philanthropic Giving in 2025.

“I am a huge proponent of music and the arts. Through my reading and research, I have learned the importance of offering sensory opportunities to our memory care residents. I am very excited to be involved in helping EverTrue offer innovative ways to offer therapeutic intervention and build meaningful connections for our residents.”

GALA REACHES GOAL OF $300,000

In 2025, EverTrue relaunched an annual gala event, with a theme of Keys to Life that celebrated the joy in aging. The evening featured awards for two valued contributors – the Duesenberg Family Foundation and BSI Constructors – as well as a live auction and fund a need. The celebration was a great success, with almost $300,000 raised in support of our EverTrue mission, programs and services.

AFFILIATION EXPANDS GRANT-FUNDED PROGRAMMING

EverTrue also expanded its grant funded programs and services through the St. Andrew’s affiliation, adding five dedicated programs that included diverse programming, such as Circle of Care and Healthy Foods and securing more than $355,000 in grant funding. With a larger goal for 2026, EverTrue continues to grow its community based giving through dedicated campaigns, major gifts and events.

GENEROSITY INTERNALLY HELPS EXTERNALLY

Acts of generosity were not limited to residents, with $159,000 in Christ Care made available to team members in need. This unique program saw an increase due to changes in the Federal SNAP Benefits program, with an additional $16,000 in giving accomplished in under two months to support necessities for team members. Christ Care program is there to provide funds for team members who experience financial emergencies and is an important element in our workforce retention success.

2025 FINANCIALS

Through continued reinvestment in our communities, team members and advancements in our services, care and technology, EverTrue is actively evolving to meet new expectations and opportunities. This intentional evolution positions us to expand our reach, strengthen our impact and carry our mission forward: Older Adults Living Life to the Fullest.

As a nonprofit organization, we align revenue and expenses, ensuring resources are managed responsibly while sustaining the services older adults rely on. This disciplined approach creates the stability needed to support ongoing evolution. Through the generosity of donors, strong investment performance, increased assets and continued strategic expansion, we are well positioned to serve while thoughtfully investing in the future of aging services.
