Coinsurance
The fixed dollar amount you must pay directly to a medical provider at the time certain covered services are received, such as office visits or prescription drugs.
Sharing, between yourself and the medical plan, of the cost of covered services after you have paid the deductible. Out-of-Pocket Maximum
Deductible
The maximum dollar amount you have to pay under the terms of the insurance company’s contract in a coverage year for covered services. Note that you would be responsible for anything exceeding the Plan’s Allowable per Benefit Maximum or Coverage Year Limit should you reach that limit within a Plan Year.

Getting Yourprescriptions Prescriptions Getting your Optum® Rx is your plan’s pharmacy benefits manager and works to offer safe, easy and cost-effective ways for you to get the medication you need. Show your member ID card at retail pharmacies in the U.S. to limit your outof-pocket expenses. Optum Rx also offers the convenience of receiving prescription medications delivered to your U.S. address. You can order a three-month supply, often with a reduced copayment compared to retail pharmacies. U.S. federal regulations prohibit shipment of prescription medications outside the U.S., Puerto Rico and Guam.
What if the medication name is different? Medication names and strengths can vary from country to country. Visit myuhc.com to see drug name translations and get detailed information on medications. Call Customer Care for help in understanding medication differences and your benefits.
Where to find pharmacies In the U.S., Puerto Rico and Guam, you and covered family members can fill prescriptions at thousands of in-network retail pharmacies. Locate pharmacies at myuhc.com or call Customer Care for help. A few things to note: • Your plan covers prescription medication only. Pharmacy benefits will not apply if your medication is available over-the-counter in the U.S. • If you can’t get a specific medication in another country, there may be a comparable option. Discuss this with your doctor ahead of time so you are prepared. New prescriptions should arrive within ten business days from the date the completed order is received by the Mail Service Pharmacy. Completed refill orders should arrive in about seven business days. Optum Rx will contact you if there will be an extended delay in the delivery of your medications.

Life Insurance BASIC LIFE INSURANCE AND AD&D International Alliance Group offers Group Term Life Insurance Plan at no cost to the Member.
Life insurance provides protection for those who depend on you financially. Your need varies greatly due to age, number of dependents, dependent ages and your financial situation. Accidental Death and Dismemberment (AD&D) benefits provide a benefit to you or your beneficiary if you are seriously injured or die in an accident.
Coverage for each full-time benefit eligible member is $25,000.
Zero Monthly Cost International Alliance Group enrolls members in health insurance at no cost to the member, unless the member chooses District coverage. Members enrolled in IAG’s health benefits are only responsible for any out of pocket expenses they may incur.
