Enhanced Assessments
With enrollment onto the Integrated Health Pathway your family will receive access to the following services:
To ensure your child is receiving the most effective care we will provide monthly assessments. These will be administered by our care coordinator and will be kept confidential. This will help us track your child’s success through the program.
Our Power of Choice group helps teens to learn more about how the substances they are using impact their brain, emotions, and life choices. This is offered regularly in person and virtually. Partner with our prevention specialist to create a substance use safety plan tailored to meet your family’s needs. Your child can receive ongoing support from a trained young adult who has lived experiences with mental health and substance use issues.

Targeted assessments we provide: CRAFFT+N: A screening tool designed to identify potential issues with alcohol, drugs, and nicotine. It assesses risky behaviors such as using substances to relax, forgetting things while using substances, or riding in a car with someone under the influence. Adolescent Drug Involvement Scale: Explores substance use patterns, reasons for using substances, methods of obtaining substances, and the social and emotional consequences of use. This tool allows our treatment team to have a comprehensive picture of an adolescent’s relationship with substance use.
