WHAT IS THE MI CHOICE WAIVER PROGRAM? The MI Choice Waiver Program is a community-based, in-home services program designed to help seniors and those with disabilities to stay in their own home or transition to an eligible community setting.
WHO IS ELIGIBLE FOR THE MI CHOICE WAIVER PROGRAM? Persons who: Are medically appropriate for a nursing home. Are Medicaid eligible individuals 65 and over in need of nursing home level of care, OR are 18 years old or older and have a disability determination by the Social Security Administration, who require nursing home level of care. Have a monthly gross income and personal asset limit that does not exceed MI Choice eligibility requirements. Reside in Macomb, Oakland, St. Clair, Washtenaw, Monroe, or Livingston counties.
