IDEAL GOALS EDUCATIONAL SERIES
Introducing the IDEAL Goals Educational Series, crafted specifically to empower clients in their journey to lower cardiovascular disease (CVD) risk! This dynamic program is an immersive learning experience. Participants will discover powerful strategies to take charge of their heart health, enhance their mental well-being, and transform into active participants in their healthcare journey!
With the dedicated support from a team of registered nurses and community health workers, participants will engage in impactful care management activities, all while advocating for treatments that align with the latest guidelines Together, we will tackle CVD risk factors head-on and pave the way for a healthier, more vibrant future! When and Where?
The 8 core sessions are conducted bi-weekly from 11:00 AM - 12:00 noon on Wednesdays in person at the Easterseals MORC Southfield 9 Mile location (22170 W Nine Mile Rd Southfield, MI 48033).
