Develop an intervention (your capstone project), as a solution to the patient, family, or population problem you've defined. Submit the proposed intervention to the faculty for review and approval. This solution needs to be implemented (shared) with your patient, family, or group. You are not to share your intervention with your patient, family, or group or move on to Assessment 5 before your faculty reviews/approves the solution you submit in Assessment 4. In a separate written deliverable, write a 5-7 page analysis of your intervention.

In your first three assessments, you applied new knowledge and insight gleaned from the literature, from organizational data, and from direct consultation with the patient, family, or group (and perhaps with subject matter and industry experts) to your assessment of the problem. You’ve examined the problem from the perspectives of leadership, collaboration, communication, change management, policy, quality of care, patient safety, costs to the system and individual, technology, care coordination, and community resources. Now it’s time to turn your attention to proposing an intervention (your capstone project), as a solution to the problem.

Your task is to develop a well-supported intervention as a solution to a defined health problem affecting a patient, family, or population. This involves designing an evidence-based, feasible strategy that addresses the core issues identified in earlier assessments. The intervention should incorporate multiple considerations: leadership, collaboration, communication, change management, policy, quality of care, patient safety, costs, technology, care coordination, and community resources. Following development, you must seek faculty approval before implementation and then analyze the intervention's impact through a comprehensive 5-7 page paper addressing key components outlined below.

The paper should begin with a clear presentation of the patient, family, or population problem, including the reasons this issue is significant to your practice and its relevance to patient outcomes. You must then explain how leadership and change management strategies influenced your intervention development, demonstrating your understanding of professional nursing standards and ethics. The role of organizational policies and state licensing standards in guiding your intervention should be articulated, supported by current literature showing their effectiveness.

Communication and collaboration strategies are vital—identify the patient, family, or group, their input,

and how their engagement enhances care. Incorporate evidence-based best practices from recent literature to support your approach. The intervention’s potential to improve care quality, patient safety, and cost-efficiency should be substantiated with scholarly sources. Additionally, discuss the integration of technology, care coordination, and community resources, supported by evidence, in tackling the identified problem.

Describe the tangible outcomes expected from your intervention, such as increased patient satisfaction, improved safety, and reduced healthcare costs. Analyze how your intervention aligns with organizational goals and health policies, including the impact of health policy on planning and implementation. Reflect on whether outcomes meet initial projections and discuss the potential for your intervention to be adopted as best practice or generalized beyond the specific setting.

The paper must be formatted according to APA style, including a title page, full references, appropriate section headings, and adherence to length requirements (5-7 pages excluding references). At least five scholarly or professional sources published within the last five years should support your discussion. Proper proofreading is essential to ensure clarity and professionalism.

In addition, document your practicum hours and include feedback from the patient, family, or group regarding the intervention’s practicality and effectiveness, whether through a form or a reflective video. The latter should be approximately 10–15 minutes long, discussing your experience, the development process, and your personal growth throughout the RN-to-BSN program.
