SUCCESS STORY

For the past year, case managers have worked with a client who has tried a few different shelters but never found a good fit due to their distance from her hometown and her goal to rebuild connections with friends and family. While City Net has tried to connect the client to shelters that better fit the client's needs, the client's lack of documentation Case managers quickly pivoted to assisting the client with getting a replacement ID and starting her documentation process (continued on page 3).

• (Continued from page 1) In September, the case managers connected with local police officers to search for available beds in nearby Bellflower and were notified that a bed was available. The case managers immediately arranged transportation for the client and saw them off to the shelter after completing their intake appointment. The client expressed her gratitude for all of the assistance from the case managers for the past year, and she hopes that this shelter will be the stable launching point for her permanent housing goals.

• For the past six months, City Net case managers have been working with an elderly couple who were experiencing homelessness for the first time. The couple had originally been referred by the Homeless Solutions Manager of Downey, and the team began working immediately to verify the couple's income and health conditions to focus on a housing placement that would support them. City Net coordinated apartment viewings for the clients and assisted with application fees and appointments Unfortunately, the couple ran into barriers that prevented them from moving into several promising units, including income barriers, properties with units that were not ready for move-in, and high monthly rental prices that would not be sustainable with their fixed income After a long search, case managers finally found a unit for the couple that fit their needs and was within their budget and worked quickly to help the couple submit their application. Before the end of the month, they were approved for move-in and accepted the keys to their new home, thanking City Net for the months of invaluable support.

Positive exits indicate clients moved off the streets or out of shelter into a more stable living situation.

Temporary housing includes rehabilitation programs, institutional care facilities, and all transitional housing placements.

Permanently housed clients rent a private residence where they may receive ongoing subsidies or have moved into a residence with family or friends permanently.
