restaurant along the Imperial Highway. The team learned that the client was a participant at the Hondo Center of Healing in Rancho Los Amigos before she unfortunately suffered a stroke. Due to being hospitalized for three months, she lost her bed at the shelter and had been sleeping in Apollo Park ever since. (continued on page 3).
Outreach teams worked on San Gabriel Riverbed this month, with case managers coordinating with a City Net-run shelter in Baldwin Park to help two clients join the waitlist for their Tiny Homes Program. Case managers also prioritized helping clients obtain vital identifying documents to help them prepare for housing applications.
SUCCESS STORIES •
(Continued from page 1) The case managers immediately submitted a referral to the Housing for Health Interim Housing program, along with a Los Angeles Housing Services Authority county-wide referral to improve her chances for a quick placement. Due to
longer wait times for shelter referrals in August, case managers were proactive in calling first-come, first-served shelters like Safe Landing Triage on a regular basis to inquire about available beds. For two weeks, the team was unsuccessful, but this did not deter the client or her case managers, and their persistence paid off in the middle of the month when the case managers secured a bed for her at Safe Landing Triage. City Net provided direct transportation for the client from Apollo Park to the shelter in Los Angeles. Upon entering the shelter for her intake appointment, the client expressed her gratitude for City Net's assistance in helping her get out of the heat and into a safe shelter environment where she can focus on recuperating before looking for permanent housing solutions. •
While conducting street outreach in late July, case managers observed an individual sitting with their belongings outside of the Downey City Library. The team engaged the individual, who shared that she had lost her apartment in Long Beach earlier that month when her Emergency Housing Voucher ended prematurely due to funding cuts. The client informed the case managers that she had previously lived in Downey and felt safer in the city, so she sleeps outside near the Downey Theatre at night. Quick to enroll the client on the spot, case managers began submitting referrals on the client's behalf for shelters like the Housing for Health Interim Housing Program. While the client waited for shelter placement, she met with her case managers regularly to receive supportive services like referrals to Rio Hondo Mental Health Clinic and America's Job Center of California. City Net also arranged transportation for her to visit the Norwalk Department of Social Services for a replacement EBT card as well as the DMV to order a replacement ID card. When the client lost her phone, case managers arranged a meeting with a California Lifeline Service Provider so that she could obtain a replacement. With her team's attentive support, the client was able to attain specialty medical care from the UCLA Stein Eye Institute.
Late in August, case managers received an update that the client was matched to First to Serve, a transitional housing program that would help bridge the gap between her current circumstances and long-term housing stability.
Positive exits indicate clients moved off the streets or out of shelter into a more stable living situation. Temporary housing includes rehabilitation programs, institutional care facilities, and all transitional housing placements. Permanently housed clients rent a private residence where they may receive ongoing subsidies or have moved into a residence with family or friends permanently.
