n more or less a year from today, the first hospital in Doral will open its doors. Jackson West Medical Center will be opening in October 2020. It will house an adult and pediatric emergency room, a diagnostic and imaging center, an outpatient clinic and at least 100 beds in the hospital. Jackson West Medical Center is located West of Palmetto on the North side of NW 25th street. It will provide primary and specialized medical service to Doral, Miami Springs, Sweetwater, Westchester, Hialeah, Miami Lakes, West Miami and Virginia Gardens, and it will be open to the general population. The opening of Jackson West Medical Center was announced during a business luncheon organized by the Venezuelan Business Club. Attending this luncheon were Mr. Carlos Migoya, President of the Jackson Health System, the General Manager for the Jackson Medical Center West, Mr. Eddie Borrego; some of the medical team of this prestigious hospital, the Mayor of Doral Mr. Juan Carlos Bermudez, and some other authorities from our city. Eddie Borrego, who will head the management team of the Jackson Medical Center West, informed that this hospital will have one hundred beds and will be named “Jose Milton Memorial Hospital”, this hospital will also include a maternity ward. The Jose Milton Foundation donated $10 million dollars to the Jackson Health Foundation, the fundraising arm of the Jackson Health System. For this reason, it is an important part of the creation of this health and well-being center. Borrego is a young executive, he graduated from Florida State University with a master’s in business administration from the University of Miami. He hopes to conduct this new generation health center with an innovative and dynamic focus and turn this hospital into a health reference in South Florida. ne of the Most American Activities is attending a Super Bowl Party. And the excitement for the Big Game started the festivities early in Doral this past October 9th with the visit and presentation by the Miami Super Bowl LIV Host Committee, at the InterContinental At Doral Miami, thanks to the vision of Doral Family Journal’s Director Ettore Sabatella, who organized this event for the enjoyment of all the community. The bands and cheerleaders from Ronald W. Reagan/Doral Senior High School and Doral Academy Preparatory School performed to receive the more than 500 guests, in anticipation to the arrival of the coveted Lombardi Trophy, which will be awarded on February 2nd, 2020 to the winner of the LIV Super Bowl at the Hard Rock Stadium. It has been ten years since Miami hosted a Super Bowl, and thanks to the investment made on the renovations of the Stadium by Stephen Ross and the Miami Dolphins organization, in addition to the due diligence of the Miami Super Bowl Host Committee (MSBHC) to win the bid with the NFL, South Florida will be able to host the event again in 2020, making it a historic 11th time for Miami, on the NFL’s 100th season. “Between the members of the Board, the County co-chairs and MSBHC team we are a group of about 30 people”, explained Gloria Garcés, Senior Director of Corporate Communications and Public Affairs for the committee. The team was formed in 2015 and serves as “a liaison between the host city and the NFL.” Ms. Garcés was the moderator of the event in Doral, where a panel of members of MSBHC and County Commissioner Pepe Diaz shared their experiences on previous Super Bowls in Miami and their expectations for this upcoming edition. Mr. Rodney Barreto is the Chairman of the Committee, the third time he serves on that position. He was very excited to see the success of the Doral event: “you see the enthusiasm of all the young kids and that is what it is all about, for everyone to be part of it.” On his presentation, Barreto made the announcement that the company Perry Ellis, located in Doral, is going to make all uniforms for the 10,000 volunteers that will serve during Super

Bowl week around the County on the different events that will take place.

“WE ARE SPECIAL, THAT IS WHY WE HAVE THE SUPER BOWL”

The hard work and enthusiasm of County Commissioner José “Pepe” Diaz for the Super Bowl, as well as his longstanding friendship with Mr. Barreto, make him an important support for the MSBHC. “This is the 11th time we are hosting the event in Miami. A record! I love that we are ahead of everyone else” said Diaz, before introducing Mr. Rodney Barreto to the audience. “Miami-Dade County is unique; we have a unique blend of people nowhere else in the State. We are special and that is why we have the Super Bowl here, they all want to come here. It is so important to have these events because of the money and the jobs they bring, and for being acknowledged worldwide as an incredible and awesome community. Everybody is looking for these opportunities and we got it, and we are proud.”

YOUNG DORAL FOOTBALL PLAYERS JOINED THE CELEBRATION

The most special guests at the MSBHC presentation where the young members of the Doral community, who brought their contagious enthusiasm to the audience; young football players, band musicians and cheerleaders from local schools and teams were grateful for this unique chance to stand so close to the real Lombardi Trophy and connect first hand with the people that organize the greatest sport event of the year. The youngest football players were from the Doral Broncos team, who came along

with their coach and shared their impressions of the event: Johnny Chavez, 11, who has been playing since he was 6 years old, was able to hold the trophy “it was really nice! It was worth waking up early coming to this event. I like playing with the Broncos because we are very close friends and we support each other, no matter if we win or lose.” Julian Garcia, 9 years old, looks forward to one day play for the NFL. “It was worth it coming here early to being next to the Trophy.” Ethan Hernandez, 10, felt it was really cool to have the chance to stand right next to the trophy. Samuel Rodriguez, 11, said that he has never “been so close to such a valuable thing and it was wonderful. Y la emoción por el Gran Juego comenzó las festividades en Doral el pasado 9 de octubre con la visita y presentación del Comité Anfitrión LIV del Super Bowl de Miami, en el Hotel InterContinental At Doral Miami, gracias a la visión del Director de Doral Family Journal, Ettore Sabatella, quien organizó este evento para el disfrute de toda la comunidad. Las bandas y porristas de Ronald W. Reagan / Doral Senior High School y Doral Academy Preparatory School se presentaron para recibir a los más de 500 invitados, en anticipación a la llegada del codiciado Trofeo Lombardi, que se otorgará el 2 de febrero de 2020 al ganador del LIV Super Bowl en el Hard Rock Stadium. igh school freshman are trying to decide: Load up on Advance Placement (AP) courses over the next three years or enroll in a Dual Enrollment program? Both of these types of courses are considered college level curriculum that will boost your grade point average (GPA) and possibly earn you college credits while still in high school at little to no cost. So, which do you choose? The best option is a combination that best fits the specific student. However, in many cases, AP courses are better received Dual Enrollment courses. The College Board is a not-for-profit organization that helps “students prepare for a successful transition to college through programs and services in college readiness and college success — including the SAT and the Advanced Placement Program” (https://about.collegeboard.org/overview). During a recent AP training, the College Board explained that the advantage AP has over Dual Enrollment is the standardization. Regardless of which AP course you take or where you take it or when you take it, there is a set curriculum that the instructor must follow for students to pass the national exam. Every May students take a national exam for each and every AP course they have completed throughout the school year. Per course, all students take the same exam and can score between 1 and 5 with colleges typically favoring scores above 3. Through a Dual Enrollment program, high school students can enroll in a college course. Here in Miami, most high schools that offer dual enrollment do so through Miami Dade College or Florida International University. Some Catholic high schools offer dual enrollment through St. Thomas University. Both the high school and the college have flexibility in how to offer the program. Courses might be taught at the high school campus or at the college campus; courses may be taught by the college instructors or the high school instructors; and there are even some options for online courses. The dual enrollment instructor has the freedom to teach course objectives as set by the college and then

administer his own final exam for a grade. There is no national standard in regards to dual enrollment courses as they are managed at a local level. Because AP courses are measured nationally by the same standards, colleges are able to evaluate the value of the course and the grade in a much easier fashion. Colleges and universities recognize the rigor involved in taking these courses while still a high school student. This does not mean that the student will automatically earn college credit. Earning credit for an AP course depends on several factors. The College Board’s website helps with a search engine that will find a school’s policy on accepting AP credits at a particular college. This should not deter you from enrolling in AP courses. Even if you are not granted college credit, the fact that you chose to enroll in a college level course while still in high school tells schools that you are a student willing to put in the work to learn and do well at college. Take the AP course! Dual Enrollment courses will earn you college credit at the college offering you the course. You must inquire as to the college credits transfer policy to understand the criteria and restrictions in using those college credits at other schools. This information can be found by searching the college’s website or contacting the college’s Registrar’s Office. In fact, you should be asking these questions BEFORE you register for the dual enrollment course. Taking a dual enrollment courses at Miami Dade College or FIU and then enrolling there provides you a very high probability that they will be accepted and applied towards your degree at that same school. If you attend a Florida college, particularly one of the state universities, there is a great probability your credits will be accepted because of articulation agreements. Your high school will have a very specific agreement with the college offering dual enrollment courses that will stipulate how your credit will transfer to that college (or possibly others) if you so choose to attend. The challenge lies in the fact that dual enrollment programs are not standardized like an AP courses.

Advanced Placement is a regulated program with one national exam that proves across the line what the student has mastered in the subject. Dual Enrollment courses can be taught by either a college or non-college instructors; each college has varying academic caliber; are taught under differing conditions dictated by the high school and college; and instructors offers a different final exam with different ways of earning a final grade. Therefore, when a college is deciding whether to accept a dual enrollment course, it is difficult to determine if the coursework meets their standards. The accreditation of the colleges involved is a whole other topic! There is also the possibility that students could take enough dual enrollment courses to be able to graduate from high school with an Associate Degree. If you can transfer an Associate Degree to your college, then you have approximately only two years left to earn your Bachelor’s Degree. This type of Dual Enrollment program would allow you to complete two years of college work while in high school. Earning your Associate while still in high school is not easy. It is a heavy course load and you may need to take courses at night or over the summer. It is not for every student, but it can be done. This is similar to the model used by Miami Dade College’s School of Advanced Studies (SAS) High School. So what’s a student to do? Take the AP courses. Pick up some dual enrollment courses. Find a balance so that you can fit both into your schedule and still have time to do extra-curricular activities and get enough sleep. If you are a strong student and willing to sacrifice for the Associate, then go for it! Make sure it’s an Associates of Arts. AP and Dual Enrollment courses give students the opportunity to discover the difficulty of college level work; be challenged in their education; earn college credit; and show colleges the stuff they are made of. However, every student is different and it’s an individual choice as to how much of these advanced courses a student should attempt.

When a person is diagnosed with breast cancer, the support he or she receives is multidisciplinary. They will all work together in the patient’s case. “The oncologist will define what kind of therapy the patient will receive, before or after surgery, (chemotherapy, radiation or hormonal therapy) according to the characteristics of the tumor and what is found in it,” explains Dr. Carlos Guida, who thinks each tumor is unique and must be addressed within what is known as personalized medicine. Among the characteristics that will be analyzed are the type of tumor, as there are different types such as carcinoma in situ, invasive ductal carcinoma or invasive lobular carcinoma; size, hormonal receptors, whether or not the armpit is involved, the cancer stage, and if there is any genetic mutation, among others. As for surgery, according to Dr. Sabates, the surgical options are the same for both men and women with the difference that since men’s breasts are typically not large, they are usually not good candidates for a lumpectomy which purpose is to preserve the breast by removing only the tumor and part of the breast tissue. For this reason, the most common procedure for men is a modified radical mastectomy in which the whole breast is removed, along with the nipples and lymph nodes. When the cancer is widespread and involves the muscles of the chest wall, the option is a radical mastectomy in which the muscles are also removed. People who have positive mutations of any of the BRCA1 and BRCA2 genes may also need to undergo a prophylactic mastectomy in the breast that is cancer-free. But oftentimes, surgery alone is not enough to treat male breast cancer. “As men usually do not have mammograms, they are often diagnosed later when they already have very large tumors that are affecting the lymph nodes. In these cases, they will also need chemotherapy and, sometimes, radiation, ”says Dr. Sabates. Whatever the case is, the chances of success in treatments increase when the tumor is detected on time. Quienes tienen familiares en el primer grado de consanguinidad (padres y hermanos sin distinción de género) que han sido víctimas de esta enfermedad están en un mayor riesgo de padecerla. “Cualquier hombre que tenga antecedentes familiares, tiene el doble de probabilidad de sufrirla comparado con otro que no tenga esta variable. Por eso, en este caso recomiendo que sea evaluado anualmente después de cumplir los 40 años”, dice el doctor Sabates. También hay un riesgo mayor cuando en la familia hay personas con mutaciones de los genes BRCA1 Y BRCA2. Cuando se dan estas condiciones, el paciente debe hacerse un examen para descartar que no tenga dichas mutaciones. “Estas mutaciones se ven principalmente en pacientes judíos y europeos, pero pueden estar presentes en cualquier persona”, comenta el Dr. Guida, quien recomienda este examen a todos aquellos que han sido diagnosticados con cáncer de seno para evaluar si se trata de un problema genético. Si hay mutaciones de estos genes, normalmente el cáncer de seno aparece a una edad más temprana. De lo contrario, en términos generales la enfermedad puede aparecer entre los 60 y 75 años. Por eso, si hay certeza sobre la existencia de estas mutaciones en el grupo familiar más cercano, hay que realizarse la valoración respectiva lo más pronto posible. “Si toda la familia es BRCA positivo, hasta los más pequeños pueden ser evaluados para saber si tienen las mutaciones”, agrega el doctor Guida. Cabe aclarar que aunque la genética suele ser el factor de riesgo número uno del cáncer de seno, hay otros factores que pueden incidir en su aparición según la Sociedad Americana Contra el Cáncer. Estos son: La exposición a químicos o a radiación, el uso de hormonas, la ingesta elevada de alcohol, enfermedades en el hígado, los tratamientos de estrógenos, la obesidad, las condiciones testiculares y el síndrome de Klinefelter.

OPCIONES DE TRATAMIENTO

Cuando una persona es diagnosticada con cáncer de seno, el apoyo que recibe es integral. Esto quiere decir que será evaluado por el médico primario, un cirujano especializado y un oncólogo, quienes trabajarán en conjunto para sacar adelante al paciente. “El oncólogo definirá qué tipo de terapia recibirá, antes o después de la cirugía, (quimioterapia, radiación o terapia hormonal) de acuerdo con las características del tumor y lo que se encuentre en él”, explica el doctor Carlos Guida, quien aclara que cada tumor es único y debe ser abordado dentro de lo que se conoce como medicina personalizada. Entre las características que se analizan está el tipo de tumor, pues existen varios como el carcinoma in situ, el carcinoma ductal invasivo o el carcinoma lobular invasivo; el tamaño, los receptores hormonales, si está o no involucrada la axila, el estadío del cáncer y si existe o no alguna mutación genética, entre otras. En cuanto a la cirugía, de acuerdo con el doctor Sabates las opciones quirúrjicas son las mismas tanto para los hombres como para las mujeres. La diferencia radica en que debido a que los hombres no tienen mamas grandes, normalmente no son buenos candidatos para una lumpectomía que busca conservar la mama removiendo solo el tumor

Por esta razón, la cirugía más común para ellos es la mastectomía radical modificada en la que se extirpa todo el seno, junto con los pezones y los ganglios linfáticos. Cuando el cáncer está muy extendido e involucra los músculos de la pared torácica, la opción es una mastectomía radical en la que también se remueven los músculos. Las personas que tengan mutaciones positivas en alguno de los genes BRCA1 Y BRCA2, posiblemente deberán someterse también a una mastectomía profiláctica en el seno donde no haya cáncer. Pero con frecuencia la cirugía por si sola no es suficiente en el cáncer de seno masculino. “Como normalmente los hombres no se hacen mamografías, a menudo son diagnosticados tardíamente cuando ya tienen tumores muy grandes que están afectando los ganglios linfáticos. En estos casos, necesitarán quimioterapia e, incluso, radiación”, asegura el doctor Sabates. Sea cual sea el caso, las probabilidades de éxito en el tratamiento aumentan cuando el tumor se detecta a tiempo. Cuando es así, también es más factible que el tumor no regrese en el futuro. “La detección temprana es vital para obtener buenos resultados y que haya tasas de curación y supervivencia altas”, concluye el doctor Sabates.

ther than being a mouth full to pronounce, whenever a parent receives a referral for their child to be evaluated by a pediatric neuropsychologist they may be left wondering who, why and how; that is, after they finally practice its pronunciation for a while. For all of you that feel lost with these questions, bellow are basic explanations that may help orient you about its purpose and importance.

WHO?

Pediatric neuropsychologists are professionals trained to evaluate the relationship of a child’s brain and its role in learning, communication, memory, motor functioning, planning, organization and behavior. They are psychologists specifically trained to assess different areas of brain functioning, interpret the results and provide recommendations for home, school and community.

WHY?

As a result of comprehensive evaluation, neuropsychologists may be able to identify functional deficits resulting from a traumatic brain injury or assist in the identification of cognitive strengths and weaknesses that may be insightful for medical teams, parents and teachers. Some of the common reasons children are referred for neuropsychological evaluation include: difficulty learning, developmental delays, medical/neurological disorders affecting the brain (i.e., traumatic brain injury, seizures, chronic illnesses, etc.) or difficulty paying attention. Information obtained from a neuropsychological evaluation will help you and the child’s teachers, therapists and physician tailor treatment and interventions to the child’s unique needs. Additionally, they will provide an array of information that will help care takers teach strategies due to deficits in brain functioning such as: help complete assignments; improve learning; effective ways to cue/direct them when stuck; manage problem behaviors; adapt aids for memory, communication or organization; and recommend alternative

methods of evaluation (i.e., extended time on tests, multiple choice vs. essay answer at school). Academic and environmental accommodations are pivotal in assisting children to function optimally given their unique set of cognitive strengths and weaknesses outlined through neuropsychological testing.

HOW?

Neuropsychologists use a variety of standardized tests that provide information about different parts of the child’s brain function when challenged to reason, analyze, store and recall information. A neuropsychological evaluation typically evaluates all of the following areas: intellectual functioning, academic achievement, language, memory and learning, visual-spatial skills, attention/

concentration, motor coordination, executive functioning and behavior/emotional functioning. While the school system usually assesses to determine if a child qualifies for a special education placement or school therapy, they do not generally diagnose learning or disorders caused by altered brain function or development. Typically, neuropsychological evaluation consists of an initial evaluation to gather background information and assist in decision making for subsequent testing sessions, a couple of 4-6 hour testing sessions and a feedback session where the neuropsychologist discusses results of the evaluation and treatment recommendations as warranted. It is important to note that neuropsychological testing consists generally of paper and pencil tests, there is no poking or pain sustained during evaluations. Please make sure to discuss with your child’s medical professional or school if you think they would benefit from a neuropsychological evaluation for appropriate referral and guidance.
