Our primary goal is to help you run your dining program so that it exceeds resident expectations, complies with all regulations, and operates within budget. Regional, Divisional and Corporate Oversight Each community is reviewed monthly by the following Crandall Registered Dietitians: Community RD The community RD completes the scored quality assurance audit quarterly with a monthly follow-up Regional RD The Regional RD furnishes a compliance review of the overall dietary status in the communities they oversee and reports on the performance of the Community RD to the Divisional RD. Divisional RD The Divisional RD provides a summary report of all the communities they oversee to the Crandall CEO and Leadership Team. Crandall Corporate Leadership Communicates with corporate and company leaders regarding any communities with concerns and noting those providing exceptional service.
A 120-bed long term care community in California, with their own in-house dietitian, contacted Crandall with an urgent situation. The state surveyors were in the community and were about to make a determination of immediate jeopardy based upon the apparent lack of follow-through on nutritional assessment and incomplete charting. In less than 12 hours, Crandall sent six registered dietitians to the community. These individuals included the CEO, COO, VP of Clinical Services, the Regional and Divisional RD and a local Crandall RD. Within 24 hours, all residents were re-assessed. Newly implemented interventions were accepted by state surveyors and the threat of the immediate jeopardy deficiency was dropped.

They had a long term care community that had hired a Certified Dietary Manager (CDM), but the individual’s start date was 30 days out. All dietary staff members receive Crandall’s Dietary Orientation Guide, which provides a general overview of their position and the policies and procedures of the community. Trainings are completed both online and in-person by the local RD, when requested by the community, and cover the competencies needed for each position. These are documented in the Crandall Competency Checklists. Continuing education and skill development is provided according to an annual schedule of inservice trainings conducted by Crandall’s Director of Dietary Services and the community’s local dietitian.

Support between Dietitian visits We like to say that with Crandall, you get “the power of a team in one dietitian.” That’s because behind your local Crandall RD is a team you can count on too. If there’s ever a time when your RD is unavailable and you have an urgent need, you have the option of contacting your regional
or divisional dietitian, or you can call and speak to one of our fulltime dietitians in the office. In addition to consulting assistance, you also have 24/7 access to every policy, procedure, and/or form related to resident care, management and survey via the Crandall Web Center.

Survey readiness and support for long-term care communities Crandall promotes always being “survey-ready.” When a community enters their survey window, the local Crandall RD will coordinate a time for Crandall’s Senior VP of Regulatory Compliance to conduct a pre-survey review with the community Administrator, Director of Nursing, Director of Dietary and a corporate leader. Through a conference call or webinar, the team will work with a survey checklist and identify those areas requiring attention.

Action assistance After identifying areas needing attention, a corrective action plan is created to address all issues. Additionally, resources and tools are provided, where necessary, to assist the community and dietary department in being survey ready. Both the local dietitian and Crandall’s VP of Regulatory Compliance will oversee the corrective actions to ensure weekly progress is being made. Progress notes are entered into the pre-survey plan-of-correction found on the Crandall Web Center.

Payson Care Center

During their presurvey review, Payson Care Center, a Life Care Centers of America long-term care community in rural Arizona, identified areas requiring attention. Crandall’s Senior VP of Regulatory Compliance assisted with conducting necessary inservices for the dietary department and helped the Food Services Director correct identified concerns. She assisted the local Crandall Clinical Dietitian and Crandall Registered Diet Technician in reviewing all of the clinical records to ensure compliance. Furthermore, she was on-site during the actual state survey to troubleshoot any concerns. The end result was a deficiency-free survey for the dietary department.

Lewis McCoy, Principal/Founder of Anthem Memory Care, reached out to our CrandallMenus team with concerns about food costs in their buildings and requested a budget review for each community. In preparation, descending dollar reports were provided prior to the individual reviews themselves. Almost immediately, it became apparent that three of the communities had ingredients coded incorrectly, extensive use of pre-made items, and items ordered off-guide. Correcting these findings resulted in a $1200 savings per month for each community. For the remaining communities, Crandall went on to define additional items where changes could be made to reduce costs.
