WHAT DOES THIS EXECUTIVE ORDER DO? This executive order establishes a new “Gold Standard” for childhood vaccine recommendations based on a U.S. Department of Health and Human Services (HHS) assessment comparing U.S. childhood vaccine recommendations with those of peer, developed countries. The order divides childhood vaccines into three categories: vaccines recommended for all children; vaccines recommended for certain highrisk groups or populations; and vaccines based on shared clinical decision-making. Vaccines recommended for all children include MMR, diphtheria, tetanus, pertussis, polio, Hib, pneumococcal disease, HPV, and varicella. Vaccines for certain high-risk populations include RSV monoclonal antibodies, hepatitis A and B, meningococcal vaccines, and dengue. Vaccines subject to shared clinical decision-making include hepatitis A and B, rotavirus, meningococcal disease, influenza, and COVID-19. The order also calls for new options to administer core vaccines, beginning with MMR, as single-disease vaccines rather than combination products, while maintaining the availability of combination vaccines.

WHAT DOES THIS EXECUTIVE ORDER DO? The order directs HHS to review the timing and sequencing of childhood vaccines, including whether to adjust when vaccines are given and administer them separately instead of multiple vaccines at the same visit, research alternatives to aluminum adjuvants, continuously evaluate vaccine risks and benefits, and improve vaccine safety monitoring and transparency. Note: The order does not itself eliminate State vaccination requirements. States currently maintain school vaccination requirements, although requirements and exemptions vary.
HOW WILL THIS EXECUTIVE ORDER BE ENFORCED? HHS, through its Task Force on Safer Childhood Vaccines, must present plans to the President within 90 days addressing vaccine options, timing and sequencing, alternative adjuvants, safety research, and monitoring. Federal executive departments and agencies are directed to review the new recommendations and take appropriate steps to advance them to the fullest extent permitted by law. The order encourages States and territories to review their childhood immunization requirements, including requirements for school enrollment and attendance, and consider whether they should be aligned with the new federal recommendations. HHS, along with the Departments of Justice and Education, are directted to address applicable constitutional and federal statutory obligations involving parental authority, religious freedom, disability accommodations, equal protection, and religious and medical exemptions.

HOW DOES THIS IMPACT BLACK COMMUNITIES? The impact of this executive order on Black communities presents both potential opportunities and significant risks, particularly given existing racial disparities in childhood vaccination coverage and access to preventive care. Recent CDC data show that vaccination coverage for several routine childhood vaccines was lower among Black children than White children, including for Hib, DTaP, pneumococcal, rotavirus, influenza, and the combined seven-vaccine series. A 2026 report found that in all but eight states, Black children were less likely than White children to receive recommended medical and dental preventive care. The report also found that Black or Hispanic children had the highest vaccination rate for the combined seven-vaccine series in only 21 states, indicating that racial disparities in childhood vaccination persist across much of the country. The report emphasizes that preventive care visits provide an important opportunity to administer recommended immunizations. Requiring vaccines to be administered separately rather than in combination, could further burden Black children, particularly those already facing barriers to preventive care. Multiple appointments may require additional time off work or school, transportation, and repeated trips to a health care provider. These added barriers could further reduce vaccination rates in Black communities, which already experience lower vaccination coverage; increase the risk of vaccine-preventable diseases; and weaken community immunity, potentially placing infants, immunocompromised people, and others who cannot be fully vaccinated at greater risk. Changes to federal recommendations and, potentially, State school-entry requirements could therefore have a disproportionate impact if they reduce vaccination rates or create confusion about which vaccines children should receive.

HOW DOES THIS IMPACT BLACK COMMUNITIES? Rather than simply expanding parental choice, the order could create greater uncertainty and confusion around childhood vaccination and worsen existing disparities in access and coverage. For Black families, changes to federal recommendations could increase barriers to care as well as exposure to vaccine-preventable diseases. Without strong protections for affordable vaccine access and clear, evidence-based information, the order could ultimately widen racial disparities in childhood vaccination rates? and public health outcomes.
HOW ARE CBC MEMBERS RESPONDING?
Members of the Congressional Black Caucus (CBC) have an established record of advocating for equitable access to vaccines, evidence-based public health policy, and strong protections for Black communities. In January 2026, CBC Chair Rep. Yvette D. Clarke joined House Energy and Commerce Committee Democrats in a letter to the Acting CDC Director expressing concerns about changes to childhood vaccine recommendations. The lawmakers emphasized the importance of scientific evidence, transparency, and meaningful consultation with public health experts and the Advisory Committee on Immunization Practices (ACIP) before implementing changes that could affect childhood immunization rates and public trust.
