Walking club

Walking is the most popular aerobic activity and it is a wonderful way for adults to be physically active. It’s easy, free, relatively safe and doesn’t require training or equipment. In fact, according to the Centers for Disease Control and Prevention, more people than ever are walking for physical activity. Walking is great exercise and when done briskly over time, it can build endurance—helping older adults walk farther, faster or uphill. It also may make everyday activities such as gardening, shopping or playing a sport easier.

When done regularly, walking at a brisk pace may offer the following benefits:

• Lower the risk of high blood pressure.

• Improve your mood.

• Strengthen bones and muscles.

• Burn more calories.

Membership in a walking club provides motivation to stick with this form of exercise because they know others are counting on their participation. The social connections made in walking clubs can also offer a sense of well-being and a way to avoid a decline in overall health that can occur as we age. In addition, a walking club may also provide a way to stay active for those who are reluctant to walk alone.

Schedule: Monday, Wednesday, Friday, 2 pm; Tuesday, Thursday, Saturday, 10 am You can walk additional times if you want, but please make sure it’s during daytime hours on exterior marked walkways.

Diabetes Prevention Program

The Diabetes Prevention Program (DPP) is an evidence-based lifestyle intervention designed to help people with prediabetes or are at high risk (high blood sugar levels that are not high enough to be diagnosed as type 2 diabetes), reduce their risk of developing type 2 diabetes.

Key features of the DPP:

Evidence based: The DPP is based on research that showed that lifestyle changes, such as weight loss, increased physical activity and healthy eating, can significantly reduce the risk of developing type 2 diabetes.

Lifestyle coaching: Participants receive one-on-one coaching from a trained lifestyle coach who helps them set goals, make healthy changes and stay motivated.

Group support: The program includes group sessions where participants can share experiences, support each other and learn from others.

Curriculum: The DPP curriculum covers topics such as: Weight management, physical activity, healthy eating and stress management.

Duration: The program lasts for one year, with weekly sessions for the first six months and monthly sessions for the second six months.

Eligibility: People with prediabetes, as determined by a blood sugar test, are eligible for the DPP. Other factors that may be considered include age, family history of diabetes and body mass index.

Benefits: Studies have shown that participants in the DPP have a significantly lower risk of developing type 2 diabetes compared to those who do not participate. On average, participants in the DPP lose weight, increase their physical activity and improve their blood sugar control.

Fee: This is a free community program, which is offered in the Spring and Fall. For more information and to register, call (516) 629-2013 or (516) 629-2070

Discussion & Support Groups

BraveHearts & YoungHearts

Founded in 1975, the BraveHearts organization was created by the Franciscan Missionaries of Mary sisters in the St. Francis Hospital convent. Guided by their faith, they decided to form a support group for patients recovering from open-heart surgery. A network was then created, where heart surgery patients could find support and understanding, sharing their first-hand experiences with fellow heart surgery patients. Today, the BraveHearts provide companionship and support to all cardiac patients, sharing information that helps patients to maintain a healthy lifestyle.

YoungHearts

In 2025, the BraveHearts established a new committee called the “YoungHearts” as a way of supporting the cardiac pediatric population at St. Francis Hospital & Heart Center® and in the community. The YoungHearts will have quarterly meetings and will have volunteers offering support to families.

Discussion & Support Groups

Barostim™ Device

Barostim Therapy is an innovative, FDA-approved treatment for certain patients with heart failure. The therapy uses a small implanted pulse generator connected to a lead placed near the carotid artery to gently stimulate the body’s baroreceptors, which are specialized sensors that help regulate blood pressure and heart function. By activating these baroreceptors, Barostim therapy helps rebalance the autonomic nervous system, which can lead to improved heart failure symptoms, increased exercise capacity and enhanced quality of life. Barostim therapy is approved for patients with New York Heart Association (NYHA) Class II or III heart failure, a left ventricular ejection fraction (LVEF) <35% and persistent symptoms despite guideline-directed medical therapy. The device received FDA approval in August 2019.

CardioMEMS Heart Failure Monitoring System

The CardioMEMS Heart Failure (HF) Monitoring System is an FDA-approved implantable device designed to help manage heart failure and reduce hospitalizations. This therapy is available for patients with New York Heart Association (NYHA) Class II or III heart failure, regardless of ejection fraction, who experience frequent hospital admissions for heart failure. The device consists of a small sensor implanted in the pulmonary artery during a minimally invasive cardiac catheterization procedure. Most patients return home the same day. Each morning, patients record their pulmonary artery (PA) pressure readings at home by lying on a special sensor-embedded pillow. The readings are securely transmitted to a CardioMEMS monitoring website, where they are reviewed by the heart failure specialist team. If rising pulmonary artery pressures are detected, indicating early fluid buildup, medications can be adjusted promptly, often preventing hospital readmission. Benefits include continuous, real-time heart failure monitoring, early detection of fluid accumulation and proven reduction in heart-failure-related hospitalizations. St. Francis Hospital & Heart Center® is proud to be among the most active CardioMEMS programs in the nation, offering advanced, patient-centered heart failure care.

Pulmonary Hypertension Program

Pulmonary Hypertension (PH) is a condition in which the blood pressure in the arteries of the lungs is abnormally elevated, placing strain on the heart and potentially leading to serious complications if left untreated. Over time, this increased pressure can damage the pulmonary blood vessels and impact overall heart and lung function. At St. Francis Hospital & Heart Center®, our world-renowned Pulmonary Hypertension experts offer comprehensive, state-of-the-art diagnostic and therapeutic care for all forms and severities of pulmonary hypertension, including advanced medical therapy tailored to the specific cause of Pulmonary Hypertension, expert diagnostic evaluation through right heart catheterization, including exercise right heart catheterization and vasoreactivity testing and ongoing multidisciplinary care integrating cardiology, pulmonology and advanced heart failure management.

Amyloidosis Program

Cardiac Amyloidosis is a progressive heart condition caused by the buildup of an abnormal protein—either AL amyloid or transthyretin (TTR) protein—within the heart muscle. This protein deposition leads to stiffening of the heart walls, resulting in heart failure, arrhythmias and potential involvement of other organs. This condition may occur spontaneously with aging (wild-type ATTR) or be hereditary (variant ATTR) due to a gene mutation. At the Center for Advanced Cardiac Therapies at St. Francis Hospital & Heart Center®, we offer comprehensive evaluation and management for patients with suspected or confirmed cardiac amyloidosis, including advanced diagnostic imaging and biomarker testing, genetic counseling and gene testing for hereditary forms and access to all FDAapproved therapies and emerging treatment options. Our multidisciplinary team is dedicated to providing personalized, leading-edge care to improve outcomes and quality of life for patients living with this complex condition.

Hypertrophic Cardiomyopathy Program

Our Hypertrophic Cardiomyopathy Clinic provides a team-based, multidisciplinary approach to the diagnosis and management of this complex heart condition. Hypertrophic Cardiomyopathy (HCM) is most often caused by genetic mutations that affect the proteins responsible for the structure and function of the heart muscle. Because HCM can present in many different ways, from mild, asymptomatic thickening to advanced heart failure, comprehensive evaluation is essential. Our clinic offers individualized treatment planning through consultation with our experienced non-invasive cardiologists, heart failure cardiologists and cardiothoracic surgeons, and state-of-the-art diagnostic testing, including advanced cardiac imaging reviewed by our expert structural imaging team, and genetic counseling and testing when appropriate. This collaborative approach ensures that every patient receives personalized, leading-edge care tailored to their unique condition and goals.

CPET (Cardiopulmonary Exercise Testing)

CPET is a specialized, non-invasive assessment that measures how the heart, lungs and muscles respond to exercise. At St. Francis Hospital & Heart Center®, we offer CPET to evaluate patients with unexplained shortness of breath (dyspnea) and to provide objective assessment of exercise capacity.

This advanced diagnostic tool also plays a key role in risk stratifying patients with heart failure, helping to determine who may benefit from advanced therapies such as Left Ventricular Assist Device (LVAD) implantation or heart transplantation. Our multidisciplinary team interprets results within the context of each patient’s overall clinical picture, ensuring precise diagnosis and individualized management planning.

Heart Failure Research

The Center for Advanced Cardiac Therapies (CACT) at St. Francis Hospital & Heart Center® is actively involved in clinical research studies focused on advancing the treatment and understanding of heart failure and related cardiac conditions. Through participation in national and international clinical trials, our patients have access to cutting-edge therapies and emerging technologies before they become widely available. A complete and regularly updated list of our ongoing clinical trials can be found on the St. Francis Hospital & Heart Center® website.

Patient safety tip: Breast Health

October is breast cancer awareness month. When was your last mammogram? According to the American Cancer Society:

• Women should start having yearly screening mammograms beginning at age 40.

• Women should have a physical breast exam by a doctor once a year if they are over 40 and once every three years if they are in their 20’s and 30’s.

• Women should also develop self-awareness as to what their breasts normally look and feel like, be able to detect changes in the course of daily life (i.e. while showering or getting dressed) and discuss this with their physician. Contact your doctor immediately if you see or feel any change.
