Across the 32 counties we serve, our team walked alongside thousands of Kentuckians through hospice care and expanded access to compassionate support through programs like Bluegrass PACE Care and our Home Modification Program that provides simple home updates to help older adults continue to live in their homes as they age. This work is only possible because of the dedication of our team, the generosity of our donors, and the selfless service of our volunteers.

In partnership with UK HealthCare, BCN relocated its inpatient hospice unit to a new home within UK HealthCare’s Good Samaritan Hospital to continue to provide high -quality, compassionate end-of-life care.
Hospice Care Earns CHAP Accreditation
Bluegrass Care Navigators earned CHAP accreditation, a nationally recognized mark of excellence that confirms compliance with the highest standards for hospice quality, safety, and patient‑centered care. This achievement reflects the expertise, compassion, and dedication of our interdisciplinary teams across northern, central, and eastern Kentucky.
Nursing Scholarship Opportunities for Team Members
Four Bluegrass Care Navigators team members, (pictured from top left to bottom right) Jennifer Lyon, Susan Howard, Kaitlyn Shepherd, and Deshawn Price, were selected for Kentucky’s first hybrid nursing program at Midway University, supported by full BCN scholarships. The initiative is further strengthened by the Eileen TeKrony Nursing Scholarship Endowed Fund, established by volunteer and donor Dennis TeKrony in memory of his wife to support BCN employees pursuing nursing degrees.

AIC Program Expands Palliative Care for Patients in Central Kentucky
BCN launched an Advanced Illness Care program with Lexington Clinic, expanding palliative services with 24/7 nurse and social work support and medical oversight. The program focuses on comfort, symptom management, and spiritual and emotional support while patients continue curative treatment.
A $50,000 CommonSpirit Community Health Improvement Grant supported grief services and integrative medicine, helping BCN care for more than 1,200 individuals across Lexington, Frankfort, and Cynthiana in 2025.
Bluegrass PACE Care (Program of All-Inclusive Care for the Elderly) enrolled its 100th participant and celebrated its third anniversary that also coincides with National PACE month in September.

Home Modification Program Helps Seniors Stay Safe and Independent
In partnership with Lexington‑Fayette Urban County Government, BCN’s Home Modification Program helps qualifying older adults remain safely and comfortably in their homes. Following an occupational therapy assessment, licensed contractors install modifications that support independence and can reduce the need to move to a nursing home or assisted living facility.
