Should any parts of the gateway readiness not be complete, or answered ‘no,’ then the apprentice is deemed not ready for End-Point Assessment and a period of additional training and preparation must take place.
The apprentice is aware that during the End-Point Assessment some footage may be recorded and stored for quality assurance purposes using either video or audio equipment and gives their consent for this.

The decisions above have been agreed by the following parties.
Declaration: By signing this form, the signatories below confirm that they understand and agree to the following:
1. The apprentice has completed the mandatory on-programme elements of the apprenticeship and is ready for End-Point Assessment
2. That all evidence used within any assessment or presented is the apprentice’s own work and does not infringe any thirdparty rights
3. The apprentice meets all NQual and DfE requirements, including that relating to eligibility to be put forward for End-Point Assessment
4. The apprentice has been on-programme for the minimum duration required by the DfE and assessment plan
5. The apprentice has achieved the minimum prerequisites of maths and English achievement as detailed in this document and on the Assessment Plan
6. The apprentice has achieved the required mandatory qualification and licences during the on-programme training
7. Should the apprentice, employer or training provider be aware of any conflicts-of-interest, they will inform NQual prior to End-Point Assessment taking place.
8. Should the apprentice, employer or training provider wish to appeal the results of the End-Point Assessment they will refer to www.nqual.co.uk for the latest version of NQual Appeals Policy and Procedure.
9. Apprentices, and those involved with End-Point Assessment, agree to be recorded during assessments, for the purpose of ensuring quality End-Point Assessment. All recordings will be kept securely for a maximum of 6 years, then destroyed thereafter.
In addition, it is agreed that, if the apprentice is successful, NQual will apply to the DfE for the apprenticeship certificate on the apprentice’s behalf.
The undersigned also acknowledge and accept that, in the event that any of the above requirements are not met, NQual will be unable to End-Point Assess the apprentice. Furthermore, in such circumstances NQual may draw any defaults to the attention of the DfE or any other relevant authority / organisation.

Suggested EPA Dates / Non-Availability
Please note for this standard, NQual will set the presentation title. The apprentice then has up to 4 weeks to complete and submit their presentation materials. A further 2 weeks is then required for Independent Assessor review. Therefore, the suggested dates below must allow a minimum of 6 weeks from the initial gateway upload to the EPA for the Presentation with Questions. The Professional Discussion may be scheduled a minimum of 2 weeks after Gateway.
If the suggested dates provided aren't available, we will do our best to work as close to them as possible.
Please also stipulate below dates/times or specific days that the apprentice is unable to complete EPA, due to things such as annual leave, work commitments etc. Assessment Method
