SPECIFIC INSTRUCTIONS IN ACCOMPLISHING CAF FORM 4 – QUESTIONNAIRE FOR AQUACULTURE This chapter provides detailed instructions on how you will accomplish CAF Form 4 Questionnaire for Aquaculture. This form will be used for every household with a member who is an operator of aquafarm to record information on his/her aquaculture operation such as characteristics of the aquafarm, type of water environment, different phases of aquafarm, different species cultured, date of stocked/planted and date of harvested/transferred, and aquafarm machineries, equipment, facility, and other tools used. It also intends to collect data on the number of laborers, social, technical, and financial assistance, and challenges encountered during the operation.

CONTENTS OF CAF FORM 4 CAF Form 4 is a seven-page booklet-type questionnaire that contains the following parts:            

Title Panel Interview Records Certification Section A - Geographic Identification Section B - Characteristics of Aquafarm Section C - Aquaculture Operation Section D - Machineries, Equipment, Facilities, and other Tools Section E - Workers/Laborers Section F - Technical and Financial Assistance and Support Section G - Other Economic activity of the Operator Section H - Challenges/Problems Encountered Section J – Use of Physical Card/PhilID Card/Printed E-PhilID

TITLE PANEL This panel is found at the upper part of the front page of CAF Form 4. It contains the particulars about the questionnaire including the following:     

PSA Logo, Name of the Country/Agency, ISO Certification Logo of TüV Rheinland Form Type, PSA Approval Number, and Expiration Date Legal Provisions Name and Logo of the Census, Form Title Letter from the National Statistician and Civil Registrar General.
