Pathway

Enhancing your wellbeing, independence and quality of life.

What is the Restorative Care Pathway?

The Restorative Care Pathway provides short-term, government-funded therapy delivered in your home or community. It is designed to help you maintain or regain your independence and improve your ability to carry out everyday tasks.

How does it work?

The Restorative Care Pathway consists of:

y An initial session to discuss your needs and create a personal Goal Plan;

y A tailored program that includes therapy, exercise, and activities to support your health and wellbeing;

y Assistance with normal daily activities to help regain independence;

y One-to-one sessions with nurses and allied health practitioners

y Nursing and allied health practitioners participating in regular case conferences to monitor and adjust the program;

y Connecting you with social or community programs that may help you achieve your goals;

y Help with providing equipment and meeting home set-up needs;

y Support to set long-term strategies to maintain independence after the program

Who delivers Restorative Care?

Restorative Care is delivered by nurses and allied health practitioners including physiotherapists, occupational therapists, dietitians, speech pathologists, exercise physiologists and therapy assistants.

The Restorative Care program can help you:

y Regain your strength and confidence;

y Maintain your independence while living at home;

y Improve your ability to complete everyday tasks;

y Achieve goals you would like to focus on;

y Live your best life, at home.

How do I access a Restorative Care Pathway?

The Restorative Care Pathway is a government funded program. If you are 65 years or older, or otherwise eligible for Commonwealth-funded aged care services, you may be eligible for Restorative Care.

To apply for the Restorative Care Pathway:

Existing BaptistCare Support at Home clients

1. You can be referred for a Restorative Care eligibility assessment through My Aged Care by BaptistCare, yourself, a support person, your GP, or another health professional. To register, call 1800 200 422 or visit www.myagedcare.gov.au.

2. After the referral is made, an Aged Care Assessor will contact you to schedule an assessment and determine whether the Restorative Care Pathway is suitable for your needs.

3. Once the approval is granted, BaptistCare will accept the referral and arrange a visit to develop a personalised Restorative Care Pathway goal plan.

4. The Restorative Care Pathway then formally begins.

New clients

2. After the referral is made, an Aged Care Assessor will contact you to schedule an assessment and determine whether the Restorative Care Pathway is suitable for your needs.

3. Once you have received your approval, you can contact the BaptistCare team on 1300 275 227 or email ask@baptistcare.org.au.

Price Guide 2025/26

Participants will receive Government funding of approximately $6,000 per week, to be used according to the agreed plan of care.
