Leadership Report A MESSAGE FROM JIM CAMPBELL
T
he idea that change is a period of transition between two periods of stability is rapidly disappearing as we realize that periods of stability within our field of Orthotics and Prosthetics and within the Health Care Environment in general are getting shorter. Our key initiatives, or six survival imperatives, are in place to help you, our members, navigate this changing healthcare environment and they are intended to keep AOPA relevant. The development of our Scientific and Evidence Based Research has been one of AOPA’s highest priorities for the last 5 years and we can claim significant progress in supporting and advancing research that demonstrates the value and positive patient outcomes resulting from the care we provide.
Our advocacy agenda seeks to correct many of the ills that have been historic challenges to O&P. We have long sought the separation of O&P from DME and recognition of the O&P professionals’ clinical notes as part of the patients’ medical record. Obligating CMS to adhere to the statutory language of describing off the shelf orthotics eligible for competitive bidding as being only those devices requiring “minimal self-adjustment” to be performed only by the patient is another key legislative goal. In 2016 we came closer than ever before in securing passage of legislation that favorably addressed the O&P advocacy agenda. We worked down to the wire during the “Lame Duck Session” to have provisions of S. 829/HR 1530 incorporated into the 21st Century Cures Bill or the Continuing Resolution to keep the government open. members on December 12th AOPA summarized the effort as follows, “As the saying goes, “close doesn’t count,” but the story of how remarkably close we did come to enacting the bill does both reflect how very different this Congress was from the O&P vantage point, as well as holding some significant promise that we’ll get this done, hopefully early in the next Congress. Former Senator Bob Kerrey has worked closely with AOPA to secure passage and his comment when learning our provisions were not included in either legislative measure underscores our opportunity in the next Congress, -”We are in very good shape to get this done in 2017...Let’s keep pushing!!!”
2016 saw increased momentum for the development of our Patient Registry or Longitudinal Data Base project. This registry of prosthetic patients will house, in a central data base, a clearly defined set of health and demographic data that will be of benefit to our profession, our patients and our industry. As part of our Prosthetics 2020 initiative to further document the value of services we provide, AOPA has engaged the RAND Corporation to help us define, develop and disseminate Cost Effective Analysis Data, the data that measures and demonstrates the true value of prosthetic care and prosthetic technology. AOPA continued its funding of small pilot grants and expanded the number of grants in 2016 to seven totaling $90,000 awarded through the Center on Orthotic and Prosthetic Learning and Evidence-Based Practice (COPL). Three systematic literature reviews in the areas of cost efficacy of transtibial interventions, prosthetic treatment candidacy algorithm and dysvascular foot amputation have been completed and are being prepared for publication. Efforts to further document the cost effectiveness data based on the earlier analysis of Medicare claims data conducted by the

noted healthcare consulting firm, Dobson DaVanzo, were updated with Part D prescription drug claims data in 2016 and expanded to augment the 2007-2010 claims data by securing 2011-2014 Medicare claims data. Much of this work interfaces and complements the RAND Corporation’s efforts. And while we need the documentation that proves timely O&P intervention pays, not costs, equally as important is getting these facts to the media, patients, policy makers and our own profession. That’s why the Mobility Saves website was created, and is constantly updated and expanded to provide the visibility and access for our research developments. A focus group was convened to further explore how Mobility Saves could provide even more value in the future. Making it easier for O&P clinicians to access opportunities for continuing education by offering more than 75 online learning experiences through AOPAversity has resulted in a surge of activity. More and more professionals are turning to online learning to supplement the education they get through AOPA’s coding and billing seminars, National Assembly education programs, and the monthly reimbursement pages in the O&P Almanac.
We still have so many challenges we confront every single day but I hope you will agree that 2016 brought advances on all three key areas of our mission to protect, promote and provide for the O&P community and the patients we serve. AOPA’s loyal membership made our advances this past year a reality. There are signs that the dark days following the issuance of the “Dear Physician’s Letter” which triggered the devastating RAC audits are seeing glimmers of hopeful signs that the worst may be over. Peer-reviewed independent scientific data is needed to prove our treatments are appropriate and cost-effective. In recent years, AOPA has made significant progress in advancing research that demonstrates the true value and positive patient outcomes resulting from the O&P care we provide. n
Prosthetics 2020/Medical Advisory Board
A Medical Advisory Board is identifying the essential endpoints of measuring value in prosthetic care, and consulting on ways to convey our scientific messages to public and private payers. Standards of practice are needed to substantiate care and are the intended outcome.
Cost-effectiveness research: NEW RAND Study and Updated Dobson DaVanzo Study n
AOPA commissioned RAND Corporation to perform a comprehensive, independent validated study quantifying the value of prosthetic services. The study goes beyond the typical health care expenditures to include costs and savings associated with employment, taxpayer status and state and federal social assistance programs. AOPA funded the 2013 Dobson-DaVanzo research study, commissioned by the Amputee Coalition, that proved O&P care saves money and improves quality of

organized orthotics and prosthetics profession in 2017, we remind you that your AOPA membership is the foundation for these successes.
life for patients. AOPA is commissioning an update of this study using more recent health care data and newly obtained prescription drug data. The updated findings will be released in 2017 and will further quantify our message: Mobility Saves Lives & Money.
Systematic Reviews, O&P Patient Registry, and Comparative Effectiveness Studies n
A major factor in turning back the 2015 LCD was its absence of research data to support its flawed proposals. AOPA has committed hundreds of thousands of our membership’s dues dollars to support systematic reviews of the medical literature on prosthetics and orthotic topics, and in support of two major comparative effectiveness studies. We have also supported the evolution of the O&P Patient Registry from concept to reality-pilot data which was collected from prosthetists in 2016, another piece in addition to data being collected from hospitals nationwide with the assistance of the American Joint Replacement Registry (AJRR). Working with Mayo Clinic and others, we have submitted proposals seeking the grant funding to transform all this work to a fully operating registry of patient data and outcomes.

Advocacy Proof of delivery requirements challenged and won!

AOPA challenged CMS on the excessive new proof of delivery policy and the problems it posed for O&P patients and practitioners. As a result, CMS reversed course and now accepts the official L-code descriptors on PODs. Recent audit results indicate that as much as 20% of claim denials are due to an incomplete or missing proof of delivery. This policy change by CMS will lead to a significant decrease in denials due to proof of delivery issues which translates into time and money savings for your business.
Communicating with payers and the public about private payers adopting policies from the draft LCD n
As predicted, ill-conceived policies in the draft 2015 LCD Policy Article are being exploited by private health care insurers to deny coverage to amputees for previously approved medical care and devices. AOPA’s telenews event in April focused on Cigna and United Healthcare denying vacuum pumps using the poorly conceived draft Medicare rule to deny coverage to amputees. AOPA also has been communicating directly with insurers about the unfairness of the policies. Insurers were put on notice that patients, providers, and policy makers were holding them accountable. n
AOPAvotes Campaign
The voice of small business owners matters to Congressional representatives. More than 500 AOPA members used the AOPAvotes.org platform to send over a thousand letters to members of Congress asking for their support of the Medicare O&P Improvement Act.

n Legislation writing session led by former Senator Kerrey at Policy Forum
One of the most memorable highlights of 2016 was the participation of former Senator Kerrey—an amputee himself— in leading a candid discussion on prioritizing our legislative and regulatory needs. More than 150 attendees held 350+ face to face meetings with their legislators, with the one page bill (the O&P Modernization Act) crafted by attendees during the Forum, that seeks to resolve some major issues plaguing O&P. Passage of this legislation will reap an unparalleled benefit for every AOPA member and their patients.

The 99th annual Assembly in Boston represented our most multi-disciplinary line-up, with more physicians, surgeons, and physical therapists presenting their work than ever before with O&P professionals, reinforcing the concept that the best possible care demands a patient-centered and collaborative approach. Boston Strong survivors shared their journeys—from heartbreak to resounding physical and emotional resilience.
